Malignant Inflammatory Fibrous Histiocytoma in a Pointer Dog

  • Malignant fibrous histiocytomas (MFHs) is the most common type of soft tissue sarcoma in the old animal with a aggressiveness, a high local recurrence rate and significant metastatic rate, which associated with a poor prognosis. In most histologic and immunohistological studies, the tumor cells raised from a fibroblastic and/or myofibroblastic phenotype, presumably from undifferentiated mesenchymal cell origin. MFHs are usually firm and invasive, arising in the subcutis; metastasis depends on tumor grade (many are grade 3) [1,2]. The primary tumor cells are pleomorphic, varying in appearance from fusiform to round. Often nucleoli are prominent and irregular [5]. Extracellular amorphous eosinophilic material may be prominent and likely represents reactive collagen production by the tumor [5]. (omitted)

소아 흉벽에 생긴 Askin`s tumor 1례 (Askin Tumor[Peripheral Neuroectodermal Tumor of the Chest Wall] - A Case Report -)

  • Askin tumor is rare malignant small round cell tumor that orgins from interconstal nerve of chest in children It was not until 1979 that Askin first reported that tumor. Although few sporadic reports had been reported, its incidence were too low to analize its clinical featurs. That tumors prognosis is so grave that no therapy would success to cure, but early diagnosis and enbloc excision with following combind chemotherapy and radiotherapy will prolong their survival. Other small round cell tumors of chest wall that must differentiate are Ewing`s sarcoma, rhabdomyosarcoma, lymphoma, neuroblastoma and pulmonary bla-stoma. The most prominant histologic charactersistics of this tumor is neuron specific eno-lase which is detected with immunohistochemistry technique, and neurosecretary electron dense granules within cytoplasm. We expirienced a case of Askin tumor occuring 12-year-old female who has huge right lower chest mass with dull chest pain. She have been underwent excision and postoperative radiotherapy. We are following her up for months and there is no evidence of local recurrence.

폐암 환자에서의 기관지 성형술 -5례 보고- (Sleeve Resection of Lung Cancer - A report of 5 cases-)

  • Sleeve resection is safe, effective, and appropriate treatment for a wide range of endo-bronchial lesions including neoplasms of low grade malignant potential and selected cases of bronchogenic carcinoma. Five cases of bronchoplastic procedures were performed for primary bronchogenic carcinoma patients at Department of Thoracic and Cardiovascular Surgery, Chonbuk National University Hospital from Aug. 1983 to Oct. 1987. Of the 5 patients, four were male and one patient was female and ages ranged from 51 years to 66 years old. Histopathologically, 4 cases were squamous cell carcinoma and one case small cell carcinoma. Operative procedures of the 5 patients were as follows: Right upper sleeve lobectomy, 1 case; Left upper sleeve lobectomy, 1 case; left lower sleeve lobectomy, 1 case; Left lower lobe and lingular segment sleeve resection, 2 cases. The early and late postoperative complications of the above operations were pneumonia, atelectasis, bronchopleural fistula, empyema, brain metastasis, and local recurrence of primary tumor.

위 식도 경계 부위의 선암에 대한 임상적 고찰 (Adenocarcinoma Involving Esophagogastric Junction)

  • Adenocarcinoma involving esophago-gastric junction[EGJ is usually originated from the gastric cardia and it presents unique clinical manifestations, requires special surgical care, and bears a much poor prognosis. We analyse the clinical data of 109 adenocarcinoma involving EGJ operated between August, 1987 and March, 1994. Curative resection of primary tumor including esophagus and lymph node dissection was possible on 102 cases[93.5% . Among these cases, 89 cases were advanced state over the stage III. The operative mortality was 1.8% and postoperative morbidity was 16.5%. The overall 3 year and 5 year survival rate was 48.5%, 34.1% each, and median survival was 27.5 month in the curative resected cases. The treatment failure was mainly distant metastsis including lymph node, except one local recurrence.Among many factors influencing long term results of resected adenocarcinoma involving EGJ, the only effort a surgeon can make is to attain completeness of tumor removal by dissecting all involved lymph node and ensuring adequate tumor free margins of both esophageal and cardiac side.

  • Chondrosarcoma is a rare entity of malignant tumor which arises from cartilaginous tissue, and the literatures on this disease are scarce. The first-line of treatment for cardiac chondrosarcoma is surgery. Due to early local recurrence and distant metastasis, the prognosis is poor even after complete surgical excision. We present a case of chondrosarcoma in the left atrium causing functional mitral stenosis which required urgent surgical intervention, and the successful treatment outcome.

유방암 진단에서 유방스캔의 역할 (The Role of Scintimammography in the Diagnosis of Breast Cancer)

  • Breast cancer accounts for the higher proportion of cancer related deaths among women, and the incidence of this malignant disease is still increasing. The commonly used screening method is mammography. However, mammography has the drawback of low specificity in differentiating malignant and benign breast diseases. To overcome this low specificity of mammography, scintimammography using various radiopharmaceuticals such as Thallium-201, Tc-99m MIBI, Tc-99m tetrofosmin and Tc-99m MDP was introduced and now actively under the investigation. Several studios have shown that high sensitivity and specificity in detection of primary breast cancer and axillary lymph node metastases using these radiopharmaceuticals. Scintimammography may play important roles in the diagnosis of primary breast cancer, evaluation of the patients with high risk, determining axillary lymph node metastasis, evaluation of the response to chemotherapy, determining the extent of disease, and surveillance of local recurrence. In the future, we should investigate the prognostic role of scintimammography after treatment of breast cancer patients and cost-effectiveness of scintimammography in the detection of primary breast cancer.

슬관절부위의 골종양에서 시행한 하지 분절 절제 및 회전 재접합술 (Segmental Resection and Rotationplasty for Bone Tumors about the Knee)

  • A segmental resection and rotationplasty was performed in 23 patients with malignant and aggressive boning tumor of the distal femur or proximal tibia between February 1988 and September 1992 at the Severance Hospital. There were 13 male and 10 female patients. The yongest was 5 years old and the oldest was 37 years old. Mean age at operation was 22 years. Of the 23 cases 14 were osteosarcoma, 7 were giant cell tumor and 2 were synovial sarcoma. After an average observation period of 32.5months, there was no evidence of local recurrence but 6 distant metastasis developed. According to Shriner's rating scale, the functional result were excellent or good in all cases. All patient were convinced that they chosen the appropriate operative procedure and the ability to achieve high level of activity was considered as the main benefit.

  • Aggressive angiomyxoma is a rare neoplasm that arise from soft tissue of the perineum and usually affects young woman. It has a high risk of local recurrence due to infiltrative lesion. This tumor should be distinguished from benign and malignant tumor or tumor like lesion of the perineum. We experienced a case of an aggressive angiomyxoma on soft tissue of the vaginal wall in 43-year-old woman and reported it with a brief review of literature.

  • The fibromatoses are defined as a group of non encapsulated, non metastasizing, fibrous tumors that have a tendency for infiltrative growth and local recurrence following surgical excision and can be divided into fascial and musculoaponeurotic fibromatoses. Infantile fibromatosis is childhood counterpart of musculoaponeurotic fibromatosis and is more common in the head and neck, the shoulder and upper arm and the thigh. Treatment includes excision, radiation, steroid and chemotheraphy, etc. Treatment of choice is complete excision but postoperative deformity and sequelae should be considered. Recently, we have experienced a case of infantile fibromatosis originating from infratemporal fossa and report this case with a review of literatures.

개에서 절개성 겸상인대 허니아 발생례 (A case of incisional falciform ligament hernia in a bitch)

  • 5 years Poodle was referred to the Veterinary Teaching Hospital, Chonnam National University in January 1997, with progressive swelling on the rear xiphoid process after cesarean section last year in local-vet clinic. Physical, radiographic, ultrasonographic and intraoperative findings that were obtained from this case were as follow; Physical findings were known as no pain, no fever in the hernia] sal at palpation. Radiographic findings revealed the decreased density of swelling contents more than peritoneum and did not identify the abdominal wall defect line. Ultrasonographic findings showed abdominal wall defect line of 3mm and intra-abdominal originated fat contents with mixed echo(hypoechoic). During the operation, falciform ligament into the hernial sac was observed and not excised, manipulated back into the cavity. The dog was given an herniorraphy with no recurrence and infection.
