Liver is an oragan which has various functions. For this functions in liver disease change variously, many methods of examination of them are invented. Each liver disease impairs these functions diversly rather than uniformly, thus it is possible to differentiate each disease by using various methods of testing it. Liver has high reproductive capacity and despite partial impairment of its function localized disease may not show any abnormality in liver function test due to compensation of remaining intact tissue of the liver. Thus the result of liver function test should be interpreted under adequate clinical investigation with other test, not depending on the liver function test only. This thesis deals with the composition, distribution in living body and clinical significance of liberating enzyme, GOT, GPT, and LDH which are included in serum enzyme that show abnormality in liver disease.
The reduction in the amount of asialoglycoprotein (ASGP) receptor, which resides exclusively on the plasma membrane of functioning mammalian hepatocytes, as a consequence of hepato-cellular damage has been demonstrated in various pathologic conditions of the liver. Galac-tosylated human serum albumin (GSA) is a newly developed receptor-binding agent, specific for the ASGP receptor. Tc-99m GSA binds quantitatively to liver ASGP receptors and the rate of accumulation in the liver is dependent on hepatic function represented as the amount of receptor, as well as the amount of ligand injected, its affinity to the receptor and the hepatic blood flow. The findings of Tc-99m GSA scintigraphy were reported to reflect the hepatic function of the patients with large hepatic tumors, obstructive jaundice, acute and chronic liver disease. Tc-99m GSA scintigraphy is an easy and reliable test and has the clinical potentials to evaluate the liver function in the patients with hepatic disorders.

Isoflurane and enflurane are currently used on orthognathic surgery in Korea. Since starting to use enflurane and isoflurane in orthognathic surgery, we have questioned their effect on liver function. Many studies have reported liver function after enflurane and isoflurane anesthesia. Although both enflurane and isoflurane are less hepatotoxic than halothane, some cases of liver dysfunction have been reported after enflurane and isoflurane anesthesia. And, we know that isoflurane is less hepatotoxic than its predecessors, enflurane. But, fulminant liver failure and necrosis were also reported after isoflurane anesthesia. The purpose of this study was to compare immediate liver function in healthy orthognathic surgical patients receiving enflurane or isoflurane anesthesia. To assess the effect of enflurane and isoflurane on liver function, we measured pre-and post-operative serum concentrations of aspartate aminotransferase(AST), and alanine aminotransferase(ALT), alkaline phosphatase(ALP), total bilirubin(Tbil).
Objectives: The aim of this study was to investigate the safety and the efficacy of Korean herbal, western and combination medicine use in patients with abnormal liver function tests. Methods: We investigated nerve disease patients with abnormal liver function tests who were treated with Korean herbal, western and combination medicine at Dong-Eui University Oriental Hospital from January 2011 to August 2011. We compared aspartic aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP) and total bilirubin (T-bil) levels before and after taking medicine and excluded patients who had liver-related disease when admitted. Results: AST and ALT were decreased significantly in patients who had taken herbal, western medicine. AST, ALT and ALP were decreased significantly in patients who had taken combination medicine. Compare to herbal medicine, AST, ALT and ALP were decreased significantly in patients who had taken western medicine, and ALT and ALP were decreased significantly in patients who had taken combination medicine. There were no significant differences between western and combination medicine. Conclusions: This study suggests that prescribed Korean herbal medicine, at least, does not injure liver function for patients', moreover, it was shown to be effective in patients with abnormal liver function tests.

Objective: The purpose of this study was to investigate the level of safety on liver functions when Korean herbal medicine was taken internally. Method: 101 inpatients who took Korean herbal medicine were enrolled and liver function test (aspartic aminotransferase, alanine aminotransferase, alkaline phosphatase, total bilirubin, gamma glutamyltranspeptidase, albumin, lactate dehydrogenase) was performed on admission and 1 month later. Results: In 101 patients, alkaline phosphatase and gamma glutamyltranspeptidase decreased significantly compared with the value taken on admission (p<0.05) but aspartic aminotransferase, alanine aminotransferase, total bilirubin, albumin, and lactate dehydrogenase were not significantly changed (p>0.05). In the patients who took Scutellaria baicalensis (n=34), alkaline phosphatase decreased and albumin increased significantly (p<0.05). Among the patients who took Atractylodes macrocephala (n=29), alkaline phosphatase decreased significantly (p<0.05). In the patients who took Glycyrrhiza uralensis or Paeonia lactiflora, liver function parameters were not significantly changed (p>0.05). On admission 11 patients had abnormal liver function and 2 patients had liver injury while 7 patients had abnormal liver function and 2 patients showed liver injury 1 month later. Conclusions: This study suggests that prescribed Korean herbal medicine does not injure liver function.
Many obesity patients were affected by depressed liver function including hepatosteatosis. The aim of this study was to investigate the clinical effect of the obesity therapy with Korean herbal medicine (KHM) on obese patient with abnormal liver function. This is a retrospective observational study performed on obesity patients with abnormal liver function. All subjects have taken serum liver function tests (LFTs) before treatment. Ten patients showed abnormal LFTs. They were treated with KHM at least 15 days. During this treatment, LFTs were evaluated afterwards. Body weight and body mass index of the patients were decreased during treatments. Five of 10 subjects recovered normal levels of LFTs. While other subjects remained at abnormal level but their levels were decreased. This study shows that the obesity therapy including KHM could be effective to improve liver function on obese patient. Additional large scale studies are necessary for more evidences.
In the view of oriental medicine, the liver is the general of the army in its function of protecting against the enemy. So this concept is very closely associated to the immunological function. Its relations with immunological function are as follows. 1. The liver produces most of the proteins and converts them with hepatocytes, composes 80% in total reticuloendothelial system with Kuffer cells & endothelial cells and has typical structure of sinusoidal vessels closely related with the blood system. 2. The liver plays an important role in innate immunity with Kuffer cells as well as with the molecules that the liver produces, related to complementary systems. 3. In the embryonic period, the liver is associated with immune associated cell growth and their maturation. After birth, it is associated with removing old red blood cells and with systematically modulating immune system through hormone metabolism. 4. The liver controls the autoimmune disease resulting from immune complex by removing molecules like immune complex. 5. In the processing of blood 19A from the digestive system, the liver has an important role in protecting the body from unnecessary immune responses. 6. In the oriental medical view, liver plays a major role in the immune function by storing blood and dispersing stagnated hepatoqi with the help of the kidneys and spleen.
Despite improvements in operative techniques and perioperative care, post-hepatectomy liver failure (PHLF) remains the most serious cause of morbidity and mortality after surgery, and several risk factors have been identified to predict PHLF. Although volumetric assessment using imaging contributes to surgical simulation by estimating the function of future liver remnants in predicting PHLF, liver function is assumed to be homogeneous throughout the liver. The combination of volumetric and functional analyses may be more useful for an accurate evaluation of liver function and prediction of PHLF than only volumetric analysis. Gadoxetic acid is a hepatocyte-specific magnetic resonance (MR) contrast agent that is taken up by hepatocytes via the OATP1 transporter after intravenous administration. Gadoxetic acid-enhanced MR imaging (MRI) offers information regarding both global and regional functions, leading to a more precise evaluation even in cases with heterogeneous liver function. Various indices, including signal intensity-based methods and MR relaxometry, have been proposed for the estimation of liver function and prediction of PHLF using gadoxetic acid-enhanced MRI. Recent developments in MR techniques, including high-resolution hepatobiliary phase images using deep learning image reconstruction and whole-liver T1 map acquisition, have enabled a more detailed and accurate estimation of liver function in gadoxetic acid-enhanced MRI.
Objectives : The purpose of this paper is to study the metaphor that the liver is the admiral from which strategic planning is derived in 『Suwen Linglanmidianlun』 in relation to Liver function. Methods : Annotations to the verse along with descriptions of strategic planning in classical texts and their relation were examined. Results : Previous annotators described the Admiral organ and Strategic Planning separately, in which it is difficult to fully explain the totality the two. They cannot be separated from each other, and only when they are in dependence of each other can they become complete. Liver function as described in classical texts and contemporary text books such as detoxification and sterilization all embody this meaning of strategic planning of an admiral. Conclusions : Reinterpretation of the Liver function based on an integrated understanding of the Liver as described in 『Suwen Linglanmidianlun』 is required. This will allow for better understanding of what the metaphor means, and to better connect this description to Liver physiology for an integrated understanding of the Liver itself in Korean Medicine.

The purpose of this study was carried out to investigate the effect of scoparone(6, 7-dimethoxyco-umarin) on liver function. Sprague-Dawley rats were treated with scoparone at a dose of 20mg/kg body weight for 5 days. Hepatic bile flow, liver weight, BSP(bromosulfophthalein) biliary excretion, alanine aminotransferase(ALT) and aspartate aminotransferase(AST) activities, malondialdehyde production and lactate dehydrogenase(LDH) release were assayed. Among them, ALT and AST activities, malondialdehyde production and LDH release were assayed by using primary hepatocyte cultures at a concentration of 0.1mg/ml. Scoparone treatment had no effect on liver weight and hepatic bile flow. Scoparone treatment not only increased BSP biliary excretion, but also recovered the decreased BSP biliary excretion by CCl4, Also scoparone significantly decreased with the increases of ALT and AST activities, malondialdehyde production and LDH release induced by CCl4. These results suggested that scoparone could protect the liver damage by chemicals via promoting the liver excretory function.
