Effects of Lower Rib Expansion Limitation on Maximal Respiratory Pressure and Abdominal Muscle Activity During Maximal Breathing in Healthy Subjects

  • Purpose: The aim of this study was to determine the effects of lower rib cage lateral expansion limitation on the maximal inspiratory and expiratory pressures and on abdominal muscle activity during maximal respiratory breathing in healthy subjects. Methods: Fifteen healthy male subjects voluntarily participated in this cross-sectional study. During maximal breathing, maximal inspiratory and expiratory pressures were measured, and abdominal muscle activity was determined with using surface electromyography. Also, the measurement was repeated with using a non-elastic belt to the lower rib cage for limiting of lateral expansion. A Wilcoxon signed-rank test was performed for obtaining the statistical difference with a significance level of 0.05. Results: The findings of this study are as follows: 1) There were no significant differences in maximal inspiratory and expiratory pressure with and without lower rib cage lateral expansion (p>0.05), 2) There was no significant difference in abdominal muscle activity during the maximal inspiratory phase (p>0.05). However, right external oblique muscle activity decreased significantly during maximum exhalation with lower rib expansion limitation (p<0.05). Conclusion: The results of the current study indicate that a non-elastic belt was effective in decreasing right external oblique muscle activity during forced expiratory breathing in healthy subjects.

A Case Report of Ptosis and Eye Movement Limitation Caused by Idiopathic Oculomotor Nerve Palsy (특발성 동안신경마비의 안검하수와 안구운동장애 치험 1례)

  • Oculomotor nerve palsy causes ptosis, limitation of eye movement, diplopia, and facial pain. Despite imaging investigation, the cause of the palsy cannot be established in most cases. We treated a patient with idiopathic oculomotor nerve palsy with acupuncture, electroacupuncture, pharmacopuncture, and Korean medical physical therapy. In this case, the symptoms remarkably improved after 28 days of treatment. This case report suggests that traditional Korean medicine is effective in treating ptosis and limitation of eye movement diagnosed as idiopathic oculomotor nerve palsy.

  • When a loss of coolant accident (LOCA) occurs in a nuclear power plant, accident scenarios which can prevent core damage are defined based on break size. Current probabilistic safety assessment evaluates that core damage can be prevented under small-break LOCA (SBLOCA) and steam generator tube rupture (SGTR) with rapid cool down (RCD) strategy when all safety injection systems are unavailable. However, previous research has pointed out a limitation of RCD in terms of initiation time. Therefore, RCD success criteria estimation based on allowable coping time under a SBLOCA or SGTR when all safety injection systems are unavailable was performed based on time-line and thermal-hydraulic analyses. The time line analysis assumed a single emergency operating procedure flow, and the thermal hydraulic analysis utilized MARS-KS code with variables of break size, cooling rate, and operator allowable time. Results show while RCD is possible under SGTR, it is impossible under SBLOCA at the APR1400's current cooling rate limitation of 55 K/hr. A success criteria map for RCD under SBLOCA is suggested without cooling rate limitation.

A Case Report of Oculomotor Nerve Palsy Treated with Acupuncture and Moxibustion (침구 치료로 호전된 동안신경마비 치험 1례)

  • Objectives : The aim of this study is to report the effect of acupuncture and moxibustion treatment for patient with oculomotor nerve palsy. Methods : We treated a patient who had suffered from right. ptosis, diplopia, ophthalmoplegia and diagnosed with oculomotor nerve palsy with acupuncture(including pharmacopuncture, electroacupuncture), moxibustion, cupping therapy. Photographs of eye movement were used to evaluate the changes in limitation of eye movement. We also evaluated symptoms such as diploma, ptosis. Results : After treatment, patient's symptoms such as limitation of eye movement, diplopia, ptosis were improved. There are no adverse effects and no relapse after treatments. Conclusions : This case report suggests that acupuncture and moxibustion treatment can be effective for patient with limitation of eye movement, diplopia, ptosis in oculomotor nerve palsy.

Effect of Space Limitation of Rhizosphere on Morphology and Development of Root System in Tobacco Seedlings (담배 육묘시 근권의 공간 제한이 근계의 형태와 발달에 미치는 영향)

  • This study was carried out to acquire the basic information of root growth under different pot size, imposing different space limitation on rhizosphere. Different size of pots that had same surface area but different depth, 5cm(Iength)$\times$5cm(width)$\times$30, 15, 5cm(depth), were used during the seedling stage of tobacco plant. Space limitation on rhizosphere affected not only the aerial growth, stem height, leaf area and shoot dry weight, but also root growth and root architecture. Aerial growth was highly related to growth of underground part, so space limitation on rhizosphere decreased aerial growth. Limitation on pot volume by reducing pot depth induced new rooting on crown. Root number and relative multiplication rate were higher in small pot that had 5cm depth than large pot, but total root length and mean extension rate showed reverse patterns. Root numbers of 1st order and 2nd order were increased as pot depth was increased, but the root number of 3rd order was increased in small pot. Root system of seedling grown in large pot distributed more horizontally than that in small pot at 20 days after temporary planting (DAT), but the root architecture of seedling was reversed at 25 DAT.

  • The definition of 'critical cancer' in critical insurance(CI) is more insurance meanings than medical meanings. The difference between critical cancer of insurance and critical cancer of medical cancer is made difficult problem to the underwriting of insurer, contractor and medical doctor.   • This study was performed to search for correlation between activity limitation level and participation restriction of children with cerebral palsy by investigating the activity limitation level through the use of the GMFCS E&R and the MACS and by figuring out participation restriction level through questionnaire survey. This study was performed, from May 1, 2010 to August 31, 2010, on 152 children with cerebral palsy ranging from 4 to 12 years, who are receiving rehabilitation therapy in the hospitals and community clinics in Busan and Gyeongnam province. The levels of activity limitation were assessed by using GMFCS E&R and MACS, and a questionnaire survey was conducted for participation restriction of the participating children. Spearman rank correlation was used for correlation analysis with the statistical software, SPSS 12.0. Majority of the children scored level 5 in both GMFCS E&R and MACS which is high frequency of activity limitation levels. Children in these levels also showed severe or complete participation restriction for mobility, education, and social relations. The analysis showed a significantly positive correlation between activity limitation level and participation restriction. Therefore this study may be useful in assessing the functional movement component of participation in children with cerebral palsy and developing the intervention plan for participation.

Debridement Arthroplasty using Posteromedial Approach in Elbow Joint with Pain and Bony Limitation of Motion (동통과 골성 운동 제한이 있는 주관절에서 후내측 도달법을 이용한 변연 절제 관절 성형술)

  • Purpose: This study evaluated the clinical outcomes of debridement arthroplasty using the posteromedial approach in elbow joints with pain and bony limitation of motion. Materials and Methods: This study involved 16 elbows in 16 patients with pain and bony limitation of motion, which were treated by debridement arthroplasty using the posteromedial approach from March 2005 to March 2008. The mean follow up period was 27.6 (13~52) months. The clinical outcomes were analyzed using the Visual Analogue Scale(VAS) for pain scale, the preoperative and postoperative range of motion and the Mayo Elbow Performance Scores(MEPS). Results: The VAS was decreased significantly from a preoperative mean of 4.5 to a postoperative mean 1.1 (p<0.001). The average arc of motion improved significantly from $61.6 (0~90)^{\circ}$ preoperatively to $109.4 (80-120)^{\circ}$ postoperatively (p<0.001). The MEPS also improved significantly from 59.4 to 85.6 postoperatively (p<0.001). There were no complications, such as hematoma and elbow instability. Conclusion: Debridement arthroplasty using the posteromedial approach is a useful surgical procedure in the elbow joint with pain and bony limitation of motion, where all compartments can be debrided, the ulnar nerve can be manipulated easily and damage to the medial collateral ligament can be minimized.

Interrelationship between the Shipowner's Limitation of Liability and the Coverage of Liability Insurance: Focus on the Judgment of the Supreme Court of Canada in the Realice Case (선주의 책임제한과 책임보험의 보상 간의 상호관계: Realice호 사건에서 캐나다 대법원 판결을 중심으로)

  • In Paracomon Inc. v. Telus Communication, Realice's anchor became entangled with a working fiber-optic submarine cable during its voyage and are presentative of the shipowner(the captain) cut the cable. The owner of the cable brought a claim for the repair cost against the shipowner. The shipowner then advanced a third party claim against a liability insurance underwriter. The Supreme Court of Canada (SCC) held that the shipowner was entitled to limit its liability under the 1976 Convention on the Limitation of Liability for Maritime Claims. The SCC also ruled that even though the misdeed of the shipowner was insufficient to break its right to limitation of liability, its wrongdoing constituted willful misconduct under the 1993 Canada Marine Insurance Act, allowing the underwriter to deny coverage for the incident. Thecasewasthefirsttoaddresstheinterrelationship between the shipowner's right to limit liability under the international convention regime and the availability of liability insurance with respect to such limited liability. This study analyzes the reasoning behind the SCC's judgment and evaluates the appropriateness of this court's decision based on the current maritime industry as well as prevailing maritime law. It concludes that the SCC's decision to declare that the shipowner retained the right to limit its liability is appropriate under the Limitation Convention (1976). However, its declaration that the liability insurer was discharged from liability is not correct in due consideration of the common recognition in the maritime industry, the intended purpose of a third party's right against the liability insurer, and the adoption process of the conduct barring limitation. Based on the SCC's decision, this study finally reviews the issue of the shipowner's right to limit and the coverage of the liability insurer in the Sewol case (2014).
