A Comparison of coincidence between the Light field & the Radiation field using film and BIS (필름과 BIS 영상장치를 이용한 광/방사선조사야 일치성 비교평가)

  • Purpose : Film has been the primary tool in coincidence testing between the light field and the radiation field, which constitutes the quality assurance list of a linear accelerator. But there is a great chance of errors being different among the observer when using film. Thus this study set out to use the BIS(Beam Image System) in addition to film in comparing and evaluating coincidence results between the two fields and in searching for the improvement measures. Materials & Methods : Photon beam of 6 and 15MV was exposed to film and the BIS using a linear accelerator. The light and radiation fields were each $50{\times}50,\;100{\times}100,\;and\;200{\times}200mm^2$. The gantry angle was $0^{\circ}$ when using film and $0^{\circ}\;and\;270^{\circ}$ when using the BIS. The devices adopted to test coincidence between the two fields were a ruler and film scanner when using film. With the BIS, the width of the scanned light and radiation fields was measured for errors with setting the X and Y axis. Results : The visual measurements of the observer with film resulted that the radiation field was bigger than the light field and that their maximum error was 1.9mm. The results were the same with the measurements using the film scanner except for the average error, which was less than 1.9mm. On the contrary, the measurements using the BIS showed that the light field was bigger than the radiation field at the gantry angle of $0^{\circ}\;and\;270^{\circ}$. The maximum error was 0.96mm, and the error range was $<{\pm}2mm$ both in the X and Y axis. The average error of ${\Delta}X$, Y was the smallest in the order of the visual film measurements, film scanner measurements, and BIS measurements Conclusion . This requires a careful measurement for accurate quality assurance since errors are much different according to each observer that tests coincidence between visual fields with film. And an observer needs to use another image device or develop a measuring device of his own if it seems necessary for accurate measurements.

  • In this study, we have fabricated a fiber-optic dosimeter for a proton beam therapy dosimetry. We have measured scintillating lights with the various kinds of organic scintillators and selected the BCF-12 as a sensor-tip material due to its highest light output and peak/plateau ratio. To determine the optimum diameter of BCF-12, we have measured scintillating lights according to the energy losses of proton beams in a water phantom. Also, we determined the adequate length of organic scintillator by measuring scintillating lights according to the incident angles of proton beam. Using an optimized fiber-optic dosimeter, we have measured scintillating lights according to the dose rates and monitor units of proton accelerator.

Antimicrobial Effects of Photodynamic Therapy using Photofrin Against Staphylococcus aureus and Staphylococcus epidermidis (포토프린을 이용한 황색포도알균과 표피포도알균에 대한 광역학 치료의 항균효과)

  • Photodynamic therapy(PDT) has been recommended as an alternative therapy for various diseases including microbial infection. The aim of the present study is to evaluate the antimicrobial effect of PDT using a photofrin and home made 630 nm Light emitting diode(LED) against Staphylococci. To examine the antimicrobial effect of photofrin-mediated PDT against Staphylococcus aureus and Staphylococcus epidermidis colony forming units(CFU) quantification, and bacterial viability using flow cytometry were formed. The CFU quantification results of S. aureus and S. epidermidis were 1 cfu/ml and 16 cfu/ml of average, respectively, after PDT application with photofrin of $50{\mu}g/m{\ell}$ and 630 nm LED and energy density of $18J/cm^2$. In addition, S. aureus and S. epidermidis isolates yielded forward-scatter (FSC) and fluorescence intensity (FI) differences on flow cytometry (FCM) after PDT. S. aureus and S. epidermidis cell size(FSC) increased 8.96% and 5.55% respectively, after PDT. Also the numbers of dead cell of S. aureus and S. epidermidis were a 39% and 61% incerased. These results suggest that photofrin-mediated PDT can be an effective alternative treatment for antibacterial therapy.

  • Objective : The objective of this study was to research depression with acupuncture & moxibustion treatment. Methods : We searched Chinese Medical Literature(published from 1985~2002) related to depression and acupuncture. Results : 1) PC6(內關), HT7(神門), ST36(足三里), GV20(百會), LR3(太衝), SP6(三陰交) are much used for the treatment depression. 2. LR3(太衝), CV17(전중), GB34(陽陵泉), ST40(豊隆), PC6(內關) are much used for the excess type of depression and PC6(內關), SP6(三陰交), HT7(神門), BL15(心兪) are much used for deficiency type of depression. 3. ST36(足三里), SP6(三稜交), HT7(神門), PC6(內關), HT5(通里), KI1(通泉) are used for the treatment depression in electroacupuncture therapy. BL15(心兪), BL20(脾兪),ST36(足三里) are used for the treatment depression in acupoint injection therapy. 4. In dermal needle therapy, it is used to tap slightly until the skin becomes congested in the region of Bladder meridian and nape, back of Governor Vessel. 5. It seems that psychotherapy, music therapy and light therapy will be applied for the treatment of depression. Conclusion : We expect that acupuncture treatment of depression will be applied practically in clinical medicine due to further study on depression with acupuncture.

The effect of Light Emitting Diode electric toothbrush on gingivitis: a randomized controlled trial (발광 다이오드(Light Emitting Diode) 전동칫솔의 치은염에 대한 효과: 무작위 배정 임상시험)

  • Purpose: The aim of the present study was to compare clinical antiplaque and antigingivitis effect between Light Emitting Diode (LED) electronic toothbrush and electronic toothbrush without LED for gingivitis and mild periodontitis patients. Materials and Methods: 30 patients included in this study. 15 patients in experimental group used LED electronic tooth brush which has red and white LED within its head, and other 15 patients in control group used same product which specially modified that function without LED. Clinical parameters ($L{\ddot{o}}e-Silness$ gingival index (GI), Quigley-Hein plaque index (PI)) were measured at the baseline, 2 weeks and 4 weeks later. Wilcoxon signed rank test and Mann-Whitney test were used for statistical analysis. Results: Compare of GI change between experimental and control group with time, both groups showed that reduced GI, but lower GI values detected at 2 weeks and 4 weeks later in experimental group than control group. And lower PI values detected at 4 weeks later in experimental group than control group, but not statistically significant. Conclusion: Based on these results and within the limits of this study, the electronic toothbrush with LED could reducing gingivitis in a short period and infer that decreasing plaque accumulation in a long period.

Study on Human Physiological Responses to Emotional Lighting System using LED Flat Lighting (LED 면조명을 이용한 감성조명시스템의 인체 생리학적 반응에 관한 연구)

  • The purpose of this study was to verify human physiological responses to emotional lighting system using LED (light emitting diode) flat lighting. Subjects were ten males in their twenties without medical history to eyes. Colors of LED lighting are red, orange, yellow, green, blue, purple and colorless (white). They were stimulated by LED lighting for 5 minutes. We measured body temperature, heart rate variability (HRV) and electroencephalogram (EEG) before and after color stimulus. In case of EEG analysis, relative power ${\alpha}$ wave ratio decreased in the groups of colorless, red and orange color light. Also, sympathetic nerve was more activated than parasympathetic nerve and the body temperature was increased in the groups of colorless, red, orange, yellow color light. On the other hand, relative power ${\alpha}$ wave ratio increased and parasympathetic nerve was more activated than sympathetic nerve and the body temperature was decreased in the groups of green, blue and purple color light. The results imply that the LED color lighting system in the realistic experiment environment. In the future, studies with compounded both colors and modes according to situation or auditory as nature sound or olfactory as aroma will be required.

Radiation Adaptive Response Induced by I-131 Therapy in Patients with Differentiated Thyroid Cancer (분화 갑상선암 환자에서 I-131 치료에 의해 유도되는 방사선적응반응)

  • Purpose: The purpose of this study was to ascertain whether radiation adaptive response could be induced by high dose I-131 therapy in patients with differentiated thyroid cancer. Materials and Methods: Lymphocytes from 21 patients (7 males, 14 females, mean age $55{\pm}12$ years) were collected before and after administration of 5,550 MBq (150 mCi) I-131. They were exposed to a challenge dose of 1 Gy gamma rays using a Cs-137 cell irradiator. The number of ring-form (R) and dicentric (D) chromosomes was counted under the light microscope, and used to calculate the frequency of chromosomal aberration. Ydr, which was defined as the sum of R and D divided by the total number of counted lymphocytes. Results: Ydr in patients before I-131 therapy ($0.09{\pm}0.01$) was not different from that of controls ($0.08{\pm}0.01$). Ydr was significantly increased to $0.13{\pm}0.02$ (p<0.0001) after I-131 therapy. Increase of Ydr after the challenge irradiation of 1 Gy was significantly lower in patients after I-131 therapy than before I-131 therapy ($0.17{\pm}0.03\;vs\;0.21{\pm}0.02$, p<0.0001). Cycloheximide (CHM), an inhibitor of protein synthesis, abolished this effect. Ydr after CHM ($0.20{\pm}0.01$) was significantly higher than Ydr after I-131 therapy ($0.17{\pm}0.03$, p<0.0001), but was not different from Ydr before I-131 therapy ($0.21{\pm}0.02$).Conclusion: High dose I-131 therapy induces an adaptive response in peripheral lymphocytes of patients with well-differentiated thyroid cancer, which is associated with protein synthesis.

Photodynamic Therapy induced Cell Death using ALA and 632nm Diode Laser in A549 Lung Cancer Cells (A549 폐암세포주에서 ALA와 632nm Diode Laser를 이용한 광역학치료 유도성 세포사)

  • Background : Photodynamic therapy (PDT) is a new therapeutic method aimed at the selective destruction of cancer cells. The outcome is death of cancer cells through apoptosis or necrosis. The aim of this study was to investigate the characterization of PDT induced cell death in A549 lung cancer cells. Materials and methods : A549 cells were used as the lung cancer cell. 5 aminolevulinic acid (ALA) was used as the photosensitizer and a 632nm diode laser (Biolitec, Germany) as the light source. Cells were incubated with various concentrations of ALA. The 632nm diode laser was then administered for various laser irradiation times. The treated cells were incubated with 24, 48 and 72 hours. The cell viabilities were measured using the crystal violet assay and light microscopy. To observe the cell death mechanism after PDT, cells were observed under fluorescence microscopy after double staining with Hoechst 33342 and propium iodide after PDT. Results : In the crystal violet assay at 24 hours after PDT with a $3.2J/cm^2$ laser irradiation power, the cell viabilities were $89.56{\pm}4.11$, $87.67{\pm}5.48$, and $69.37{\pm}8.84$ with ALA concentrations of 10, 100, and $1mg/m{\ell}$, respectively. In crystal violet assay at 24 hours after PDT with $1mg/m{\ell}$ of ALA, the cell viabilities were $74{\pm}19.85$, $55{\pm}6.1$, and $49.06{\pm}16.64%$ with 1.6, 3.2 and $6.4J/cm^2$ laser irradiation powers, respectively. However, increasing the interval time after PDT did not change the cell viabilities. In the apoptosis assay, photodynamic therapy was inducing the apoptotic cell death. Conclusions : This study shows the apoptotic anticancer effect of photodynamic therapy in A549 lung cancer cells. However, further evaluations with other cancer cells and photosensitizers are necessary.

Clinical Efficacy and Possible Applications of Genomics in Lung Cancer

  • The heterogeneous nature of lung cancer has become increasingly apparent since introduction of molecular classification. In general, advanced lung cancer is an aggressive malignancy with a poor prognosis. Activating alterations in several potential driver oncogenic genes have been identified, including EGFR, ROS1 and ALK and understanding of their molecular mechanisms underlying development, progression, and survival of lung cancer has led to the design of personalized treatments that have produced superior clinical outcomes in tumours harbouring these mutations. In light of the tsunami of new biomarkers and targeted agents, next generation sequencing testing strategies will be more appropriate in identifying the patients for each therapy and enabling personalized patients care. The challenge now is how best to interpret the results of these genomic tests, in the context of other clinical data, to optimize treatment choices. In genomic era of cancer treatment, the traditional one-size-fits-all paradigm is being replaced with more effective, personalized oncologic care. This review provides an overview of lung cancer genomics and personalized treatment.

The Difference of Hand Functions in Dominant Hand Types (우세손의 유형에 따른 손 기능 차이에 관한 연구)

  • Purpose. This study was conducted to prove the difference of dominant hand and non dominant hand in hand functions. Methods. We study difference of dominant hand and non dominant hand in hand functions that 40 university students participated in the study and performed a correlation between Jebsen-Taylor Hand Function test, O'conner Finger Dexterity test and Purdue Pegboard test. Results. In left dominant hand are functional of small common object, simulated feeding and large light object in Jebsen-Taylor Hand Function Test. Also O'conner Finger dexterity test are functional in left dominant hand and same result in Purdue pegboard test. Conclusion. The results of this study was left dominant hand is more functional than right dominant hand. So, we suggests that both hand using are improving of hand function in right dominant hand.
