A Comparative Study of Reported Nursing Needs and Nursing Care During Labor and Delivery (산부의 간호요구와 간호원의 간호활동과의 비교 연구)

  • Contemporary nursing is concerned with meeting patients physical psychological and secio-economical needs and with helping persons to sore their own problems. At the time of confinement and delivery, women often have physical and psychological disco- mort related to fear, tension or anxiety. Pain related to these causes varies with the individual woman. Effective nursing care to meet individual needs during labor and delivery is important if nurses are to understand and relieve woman's physical and psychological pain. A questionnaire prepared for the purpose by the investigator was administered to 162 women in two general hospitals, one in Seoul and one in TaeGu, during August 1 and September 20, 1975 and to the 13 delivery-room nurses who cared for them. It was hoped that the study would contribute to comprehensive nursing care during labor and delivery and to improve maternal and child health. 1. The sample of mothers ranged in age from 18 to 39; the majority were from 25 to 29 years old (54.3%). Most of them were housewives (87.6%). Sixty-three percent had high school or higher education. They had a range of one to seven deliveries. Sixty-one percent were primiparas Most had some prenatal care (87.6%). 2. The age of the group of nurses ranged from 22 to 39. Ten were 22 or 23. Five of the 13 had from six to ten months experience in the de]ivory room. Twelve were single and ten had a religious affiliation. 3. Both primiparas and multiparas thought equally that physical and psychological care, nursing skills and attitudes were important during labor and delivery, but did not relate basic nursing care directly to normal delivery. Need for nursing care was rated more highly by primiparas (2.83-3.48) than by multiparas (2.51-3.17) (p〈0.05). 4. There was no difference in need for nursing care according to the educational level of the women nor according to whether they had a religious affiliation or not (p〉0.05). 5. There was no difference in the reported nursing care given regarding of the educational preparation of the nurses (p〉 0.05). 6. There the reported nursing needs of the women and the reported nursing care given were compared, physical and psychological nursing care directed toward protecting the mother and fetus and a safe do]ivory were considered important by both groups. Neither group related simple nursing care directly to protection of the mother and fetus or to a safe delivery. The women rated highly their needs for nourishment (3.05) and having a relative with them (2.90) for emotional support but there was little evidence (2.39) that the nurses provided care to meet these needs (p〈0.05). In conclusion, the nursing needs of the women during the three stages of labor and the nursing care given were generally similar. The women had more psychological needs than physical needs but in comparison, nurses gave a little more physical than psychological care. The results point up the need to make adjustments in labor and delivery room nursing care the nurse should Prepared to pay more attention and bigger consideration in psychological comfort care than physical care.

An Analysis on Predictors of Move Intention to Silver Town in Jeju Island (실버타운 입주의도 예측요인에 대한 분석)

  • The purpose of this study was to examine predictors of move intention to silver town in Jeju Island. Participants were 449 middle age and elderly who live in Seoul, Gyungido, Inchun Metropolitan and Jeju island. Participants were completed a questionnaire and/or structured interview that included measures of socioeconomic variables, motivation to move, determinants of move selection, conditions of location, and preparation for old age. Results of descriptive analysis revealed that cognition level of sliver industry and silver town was relatively high and medical service was the best necessary field, followed by field of leisure activity, house, life and finance. Logistic regression analyses on the effects of socioeconomic variables on move intention indicated that region, occupation and house ownership had significant effects on move intention to silver town. Result of motivational effect on move intention revealed that solution of offspring's burden was the most important factor, followed by convenient life of the aged, enjoyment of freedom for the elderly life. Result on the influence of determinants of move selection indicated that medical service facilities, resort facilities and leisure, cultural service facilities had significant effect on move intention. Among conditions of location for Jeju island, mild climate, comfort environment and low-price were significant predictors on move intention. Overall, result on relative influence of individual factors indicated that low price factor was the most important predictor, followed by resort facilities, cognition on necessity of silver town, enjoyment of freedom for the elderly life, leisure cultural service facilities, and medical service facilities.

An Exploratory Study about the Importance of Selected Nursing Activities during the Puerperal Period, as Viewed by Women in the Puerperal Period and by Nurses Caring for Them (산모와 간호원이 본 선택된 산욕기 간호활동의 중요도에 관한 탐색적 연구)

  • The desire to maintain health is increasing, consequently the role of nursing which has as one chief aim the solving of man′s basic problems is more and more important. Today, in spite of a growing concern about the nursing activities which nurses provide for individual human having specific needs, clinically in fact, it is questionable that individual′s expectation of nursing activities agrees with nurse′s performance of nursing activities. In this study the importance and agreement of the importance of the nursing activities during the hospitalized puerperal period as viewed by women in the puerperal period and by nurses caring for them, were assessed. The present study was undertaken in an attempt to furnish the basic data for expediting the progress of research activities in this area and further to be helpful in planning maternity nursing practice. The study population defined and selected was nurses (13) caring for women in the puerperal period and doing duty on obstetric & gynecologic ward at Y. hospital, and the women in puerperal period (39) as sum of 3 women selected by each nurse during the period of May 13th-June 4th 1976. The study data was collected by the direct interview method based on the questionnaire which the investigator made out. The study result was analyzed by percentage, t - test. The findings can be summarized as follows: 1. General characteristics of nurses doing duty on puerperal ward: a. Nurses′average age was 24.8 years old. b. 84.6% had educational background of 4 years of college. c. 69.2% had a religion. d. 53.8% were married. e. 53.8% had clinical experience of 1 year -3 years. f, 61.5% did duty on puerperal ward during 1 year -3 years. g. 46.2% desired to do duty on obstetric ? gynecologic ward. 2. General characteristics of the women who were studied during their puerperal period: a. Women′s average age was 26.4 years old. b. 79.5% had educational background above high school. c. 56.4% had a religion. d. 84.6% had living standard above medium. e. 89.7% had no occupation. f, 53,8% had previous hospitalization experience. g. 56.4% had previous delivery experience. 3. Examining the importance of 39 nursing activities during puerperal period selected by investigator, studied group of women considered that the most important nursing activity was "Record precisely about condition, medical treatment and nursing activity results etc". Nurses considered that the most important nursing activity was "Notice whether having pain and care for that". Both groups considered that the least important nursing activity was "Talk with her about topics such as news, hobbies, other interests". 4. Examining the importance of nursing activities in 4 specific categories, studied group of women considered that the most important nursing activity in physical nursing category was "Be sure of safety measure to prevent accidents, injuries", and nurses considered that the most important nursing activity was "Make her sleep and rest sufficiently". Studied group of women considered that the most important nursing activity in psychological category was "Explain about medical treatment and nursing activity ahead of time so she knows what to expect" , and nurses considered that the most important nursing activity was "Explain about puerperal period so she understands". Studied group of women considered that the most important nursing activity in relation to medical care was "Record precisely about condition, medical treatment and nursing activity results etc.", and nurses considered that the most important nursing activity was "Observing, cleaning and protecting the perineum" Studied group of women considered that the most important nursing activity in nursing category in preparation for discharge was "Instruct about personnel hygiene during puerperal period", and nurses considered that the most important nursing activity was "Instruct self-care to protect the perineum". 5. The analysis of this study showed a significant amount of disagreement computed by subtracting the nurse′s score from the patient′s score. Studied group of women put greater importance on physical nursing category, psychological nursing category, nursing in relation to medical care, than the nurses. These results were statistically significant at 0.01 level.

A Study of the Health Service Computerization State and the Occupational Nurses's Satisfaction Level on Computerization (산업간호현장의 보건업무 전산화시스템 활용현황과 산업간호사의 전산화 직무만족도 연구)

  • This study aims to investigate the use state of the health service computerization system in the occupational nursing field and the occupational nursers' satisfaction level, and provide basic data to promote the development of the health service computerization system for the nursing field. For this study, a questionnaire was provided to 118 occupational nurses who belong to Busan and Gyeongnam branches of KAOHN(Korean Association of Occupational Health Nurses) for 2 months (from Dec. 1, 2002 to Jan. 31, 2003). A tool of Choi Yong-Heui(2000) was used to investigate the satisfaction level of using the health service computerization system. The collected materials were analyzed in real number and percentage, average and standard deviation, t-test and ANOVA by using the SPSS WIN 10.0 program. This study is summarized as follows: 1. The average age was $31.99{\pm}5.58$ old in this study. The married were 54.2%. Participants who graduated from a junior college was 76.9%. The average service period was $4.48{\pm}4.68$ years. In service types, 79.7% of participants served in a health care center. The average service period was $3.22{\pm}2.89$ years. The service place which had 1000 workers or more was 35.6%. 2. Only 20.3% of participants in this study had a computer use education. 3. The field who participants used mostly was communication/internet, $3.29{\pm}.85$ hours in average. 4. 97.1% of occupational fields had computers and peripheral devices: 71.4% in pentium computer, 42.8% in the hard disk capacity of 20-29GB, 60.0% in 15 inch monitors, 86.2% in printers, 18.1% in digital cameras, 12.4% in LAN, and 9.5% in scanners. 80.1% of the occupational fields which were objects of study could use communication. 5. The occupational fields which did not introduced the health service computerization system were 62.8%. The main cause was attributable to entrepreneurs' insufficient recognition 66.6%. 51.5% of the entrepreneurs did not have an introduction plan. 37.2% of participating companies had the health service computerization system. 56.4% of them introduced it since the year 2000. 81.6% of the introduction motivation aimed to the efficiency of health service. The most issue upon introduction was insufficient understanding of a person in charge - 25.6%. The in-house development of the system covered 56.4%. 61.5% of the participants accepted their demands from the first stage of development. The direct effect of computerization showed the increase of 25.9% in the quickness and continuity of service treatment, and 25.9% in the serviceability of statistical treatment. 6. 22.0% of the participants had a computerization system use education. 69.2% of them had a in-house education. An educational method by nurses who used the computerization system was 76.9%. 92.3% of the education was helpful for practical duties. 7. An analysis of the computer use by health service fields showed that the medicine management in a health management field was 15.9%. the work environment measuring management in a work environment filed was 32.9%. the employment. general and special examination management in a heal th management field was 61.1 %. the various reports management in an administrative field was 64%. the health education data preparation management in an educational field was 58.0%. and the medicine and expendables management in an equipment management field was 51.6%. An analysis of the computerization system use showed that the various statistical data manage in a health management field was 13.0%. the work environment measuring management in a health management field was 34.8%. the personal disease management in a health management field was 51.9%. the heal education data preparation management in an educational field was 54.5%. and the equipment management of health care centers in an equipment management field was 52.6%. 8. 31.6% of the participants wanted that health service computerization system would include the generals of health services. 42.4% of the participants thought that first of all. the aggressive interest and investment of employers were required to build the health service computerization system. 9. The participants' satisfaction level on the computerization system use was $3.51{\pm}.57$ points. An analysis by each factor showed $3.62{\pm}.68$ points in a service change factor. $3.15{\pm}.63$ points in a computer program use factor, and $3.45{\pm}.71$ points in a continuous computerization use factor. 10. An analysis of the computerization system use by general characteristics of participants showed that the married (p = .022) had the satisfaction level higher than the unmarried. 11. The satisfaction level of the computerization system use by participants' computer use ability tended to be higher in proportion to the increase of computer use abilities in spreadsheet (F=2.606. p=.048). presentation (F=3.62. p=.012) and communication/internet(F=2.885. p=.0321. Based on the study results mentioned above. I will suggest as follows : The nationwide enlargement and repetition study is required for occupational nurses who serve in occupational nursing fields. The computerization system in a health service field is inferior comparing with other fields. The computerization system standard by business types and characteristics should be prepared through employers's aggressive participation and national support. Therefore various statistical data which occurs in occupational fields will be managed systematically and efficiently. A regular and systematic computer education plan for occupational nurses in charge of health services in the filed is urgently required to efficiently manage and improve the health of on-site workers.
