물리치료 중재가 뇌졸중 환자의 신체조성에 미치는 영향 (The Effects of Physical Therapy Intervention on Body Composition in Stroke Patients)

  • 뇌졸중의 발생빈도가 증가하는 추세에 따라 뇌출혈과 뇌경색, 마비부위 그리고 성별을 중심으로 신체조성 변화의 차이를 조사해 뇌졸중 유형에 미치는 영향을 분석하여 뇌졸중 환자의 발병 원인과 특성에 알맞는 중재를 실시하기 위한 기초 자료를 제공하는데 연구의 의의가 있으며, 물리치료를 실시하여 뇌졸중 환자의 체성분 변화를 검증한 결과 물리치료 실시 후 남자 환자 집단과 좌측 부위 마비 환자 집단에서 근육량의 변화를 보여, 뇌졸중 환자의 신체조성의 변화를 위해서는 물리치료를 적용하고, 앞으로의 연구에 기초자료로 활용되기를 바라며, 지속적인 물리치료 재교육 프로그램을 실시하는 것이 중요하다.

여성에서 절식이 체지방과 혈청의 Leptin 및 지질조성에 미치는 영향 (Effect of Complete Fasting on Body Fatness, Serum Leptin and Lipid Profile in Women)

  • The study was designed to observe the effect of complete listing for 10 days on body fatness, serum leptin level and lipid profile in thirty women who were admitted to fasting Clinical Program in Kyung-Hee Oriental Medical Center. Most of the subjects had food habits of over-eating and preferred oily foods which was correlated to the increase of body weight and BMI during realimentation. Average loss of body weight was 7.98kg and waist-hip ratio was significantly decreased during fasting. Body weight showed significant positive correlation with body fat(r=0.8182), lean body mass(r=0.7797) and total body water(r=0.7563). Body fat was gradually reduced after 9 days fasting, but lean body mass was not diminished any more after 7 days luring. Serum leptin level was significantly decreased after luring and re-feeding. The leptin showed significant positive correlation with body weight(r=0.9324), body mass index(r=0.8279) and body fat(r=0.8376). The levels of serum total cholesterol and LDL-C were significantly increased but HDL-C level was significantly decreased after fasting which increased could be harmful for persons with hypercholesterolemia. VLDL-C and triglyceride levels were not significantly decreased after fasting. Serum total free fatty acid level was significantly increased after fasting. These data suggest that the best way of maintaining the reduced body weight was to carry out the modification of food habits and behavior in daily lift. It could not be recommended to repeat often complete luting for reduction of body weight. (Korean J Nutrition 33(1) : 42-48, 2000)

국민학교 학동의 혈압에 관련된 요인분석 (Risk Factors in Relation to Blood Pressure in Schoolchildren)

  • 대구시내 1개 국민학교 4학년 284명(남아 144명, 여아 140명)을 대상으로, 1992년 5월에 일반적인 특성을 조사하였고, 체격, 체지방 및 혈압을 측정하였으며, 혈액의 생화학적 검사를 실시하여 혈압과 관련된 요인을 분석하였다. 대상자의 평균 수축기 혈압은 103.5mmHg였으며, 남아가 104.1mmHg, 여아가 102.9mmHg로 남아가 약간 높게 나타났으며, 평균 확장기 혈압은 67.1mmHg이고, 남아가 66.6mmHg, 여아가 67.5mmHg로 여아가 약간 높았다. 수축기 및 확장기 혈압을 종속변수로 총콜레스테롤, 고밀도 지단백 콜레스테롤, 저밀도 지단백 콜레스테롤, 중성지방, 총단백질, 요산, 혈당, R$\ddot{o}$hrer지수, 체지방 비율, 체지방 무게 및 제지방체중을 독립변수로 하여 상관관계와 중회귀분석을 실시하였다. 수축기 및 확장기혈압과 독립변수들과의 상관관계를 성별에 따라 분석한 결과 남아의 수축기혈압은 신장, 체중, R$\ddot{o}$hrer지수, 제지방 체중, 체지방 무게 및 중성지방과 유의한 상관관계가 있었으며(p<0.01). 여아의 수축기혈압은 신장, 체중, 제지방체중 및 요산과 유의한 상관관계가 있었다(p<0.01). 남아의 확장기혈압은 체중, R$\ddot{o}$hrer지수 및 제지방체중과 유의한 상관관계가 있었으며(p<0.01). 여아의 확장기혈압은 체중, 제지방체중(p<0.01), 신장, 체지방무게 및 총단백질(p<0.05)과 유의한 상관관계가 있었다. 수축기 혈압을 종속변수로 한 중회귀분석에서 남아는 R$\ddot{o}$hrer지수와 sodium이 유의한 변수였고, 여아는 요산, R$\ddot{o}$hrer지수 및 체지방 비율이 유의한 변수였다. 확장기 혈압에서 남아는 R$\ddot{o}$hrer지수가 유의한 변수였고, 여아는 총단백질이 유의한 변수였다. 이상의 결과로 보아 소아의 혈압에 영향을 미치는 인자로는 R$\ddot{o}$hrer지수, sodium, 요산, 총단백질 및 체지방 비율 이었고, 그 중 R$\ddot{o}$hrer지수가 가장 중요한 요인으로 인정되었다. 따라서 고혈압 발생을 예방하기 위해서는 비만, 식이 및 이와 관련된 생활양식의 개선과 이에 대한 보건교육이 요망된다.

정상체중 성인과 과체중 성인의 에너지 섭취량, 휴식대사량, 활동대사량 비교연구 (A Comparison of Energy Intake and Energy Expenditure in Normal-Weight and Over-Weight Korean Adults)

  • The purpose of this study was to compare the energy expenditure and energy intake of normal-weight and overweight Korean adults. We recruited 242 adults to determine resting energy expenditure, physical activity and energy intake. Resting energy expenditure was measured by indirect calorimetry. Energy intake for consecutive two days was assessed by 24 hour recall method. Daily activity pattern for 24 hour was collected from each subject. Body weight, lean body mass and percentage body fat were measured by INBODY 3.0. The subject were divided into normal ($20\leqBMI$ < 25) and overweight ($BMI \geq 25$) groups by BMI. There was no significant difference in intake of energy between two groups. Energy intake of each group was lower than the 7th Korean RDA of energy. Overweight subjects showed significantly lower REE/kg body weight. However, REE/kg lean body mass (LBM) did not differ between the two groups. Total activity energy was significantly higher in the overweight group compared to the normal group. Daily activity coefficient of overweight group in male was lower than that of normal group. Daily activity coefficient was almost same in two female groups. LBM was highly correlated with REE and total energy expenditure. We concluded that the overweight group consumed more energy than the normal group due to the heavier body weight.

내과 환자의 입원후 영양상태 변화 연구 (Changes in Nutritional Status of General Medical Patients During Hospitalization)

  • The purpose of this study was to determine the incidence of malnutrition among patients on admission to hospital, to monitor changes in their nutritional status during hospitalization, and to determine the factors which might affect changes in nutritional status. The subjects for the study were patients who were admitted to general medicine for more than one week. Patients suffering from cardiovascular. renal disease, or dehydration were excluded. Nutritional assessment of the patients was performed on admission and nutritional status was reassessed one week and two weeks after admission. The nutritional assessment tool consisted of subjective history taking and anthropometric measurements. Biochemical measurements were performed only on admission. For anthropometric assessment : patients' body weight, subcutaneous skinfolds thickness, % of body fat, body mass index, and lean body mass were measured using caliper or Bio impedance Analyzer. Factors which might influence current nutritional status, like dietary intake, anorexia, nausea, vomiting, diarrhea, sleep disturbance, and number of days of NPO for diagnostic examinations were analyzed. The results are as follows : 1. Of the 59 patients who were studied, 61% were male and 39% female. The nutritional status of all of the 59 subjects was reassessed one week after admission, but it was only done for 22 subjects at two weeks. 2. The anthropometric measurements. including weight body mass index, lean body mass, body fat. and skin fold thickness. were all significantly decreased at one week after admission compared to the values at admission. On the other hand, two weeks after admission, only body weight and abdominal skinfolds thickness were decreased. 3. The subjects reported anorexia for an average of two days, sleep disturbance for two days, and no food intake due to diagnostic test for one day. In the second week of hospitalization, almost none of the patients complained of gastrointestinal symptoms or sleep disturbance except anorexia. Food consumption which was measured based on rice intake was 60% of the food served during the first week of hospitalization, and 66% during the second week of hospitalization. 4. There was no correlation between the subjective nutritional assessment and anthropometric assessment. 5. There was no statistical significance in anthropometric measurements among the patients with various diseases whereas sleep disturbance and no food intake due to various diagnostic test was prominent in patients with gastrointestinal diseases.

Peroxisome Proliferator-Activated $Receptor-{\gamma}$ 2 $(PPAR{\gamma}2)$ Pro12Ala (P12A) 유전자 다형성이 한국여성의 체지방분포에 미치는 영향 (Effects of Peroxisome Proliferator-Activated $Receptor-{\gamma}2$ Pro12Ala Polymorphism on Body Fat Distribution in Female Korean Subjects)

  • Objectives: The effects of peroxisome proliferator-activated receptor ${\gamma}2\;(PPAR{\gamma}2)$ Pro12Ala (P12A) polymorphism on body mass index (BMI) and type 2 diabetes are well documented; however, until now, only a few studies have evaluated the effects of this polymorphism on body fat distribution. This study was conducted to elucidate the effects of this polymorphism on computed tomography (CT)-measured body fat distribution and other obesity-related parameters in Korean female subjects. Methods & Results: The frequencies of $PPAR{\gamma}2$ genotypes were: PP type, 93.0%; PA type, 6.8%; and AA type, 0.2%. The frequency of the A allele was 0.035. Body weight (P .012), BMI (P .012), and waist-to-hip ratio (WHR) (P .001) were significantly higher in subjects with PA/AA compared with subjects with PP. When body composition was analyzed by bioimpedance analysis, lean body mass and body water content were similar between the 2 groups. However, body fat mass (P .003) and body fat percent (P .025) were significantly higher in subjects with PA/AA compared with subjects with PP. Among overweight subjects with BMI of greater than 25, PA/AA was associated with significantly higher abdominal subcutaneous fat (P .000), abdominal visceral fat (P .031), and subcutaneous upper and lower thigh adipose tissue (P .010 and .013). However, among lean subjects with BMI of less than 25, no significant differences associated with $PPAR{\gamma}2$ genotype were found, suggesting that the fat-accumulating effects of the PA/AA genotype were evident only among overweight subjects, but not among lean subjects. When serum lipid profiles, glucose, and liver function indicators were compared among overweight subjects, no significant difference associated with $PPAR{\gamma}2$ genotype was found. Changes in body weight, BMI, WHR, and body fat mass were measured among overweight subjects who finished a 1-month weight lose program of a hypocaloric diet and exercise; no significant differences associated with $PPAR{\gamma}2$ genotype were found. Conclusions: The results of this study suggest that the $PPAR{\gamma}2$ PA/AA genotype is associated with increased subcutaneous and visceral fat areas in overweight Korean female subjects, but does not significantly affect serum biochemical parameters and outcomes of weight loss programs.

  • It is very important that the ratio of Soft Lean Mass and Body Fat Mass in body composition. Generally we make an effort not only weight reduction but body fat mass reduction, and finally percent body fat is reduced. And we make an effort to reduce of partial obesity. I have one case that improvement of imbalance on the upper and lower part of the body by Oriental Medicine. We have the result that the upper part of the body composition is increased and the lower part of the body composition is decreased.

Association of waist circumference with muscle and fat mass in adults with a normal body mass index

  • BACKGROUND/OBJECTIVES: We aimed to investigate the association of waist circumference (WC) with body composition among individuals with a normal body mass index (BMI) to distinguish muscle and fat mass, as both affect health differently. SUBJECTS/METHODS: We analyzed dual-energy X-ray absorptiometry data (derived from the Korean National Health and Nutrition Survey, which includes information on fat and lean mass) of 7,493 adults with a normal BMI. Subjects were categorized into four groups of increasing WC. The fourth group was defined as being centrally obese. Each number of subjects are as follows: 1,870, 695, 231, and 39 among men and 3,054, 1,100, 406, and 98 among women. We conducted a sex-stratified linear regression analysis of body composition according to WC group after adjustments for covariates. RESULTS: We observed a positive association of body fat with increasing WC in both men and women (all P for trend: < 0.001). The adjusted mean values for percent body fat with 95% confidence intervals (CIs) according to the four WC groups in ascending order were 17.8 (17.5-18.3), 21.0 (20.6-21.5), 22.1 (21.5-22.8), and 25.1 (24.2-26.1) in men and 29.7 (29.4-30.0), 32.0 (31.6-32.3), 32.9 (32.4-33.4), and 34.7 (33.2-36.1) in women. However, there was an inverted J-shaped association between muscle mass and WC. The fourth group had a higher percent body fat and lower muscle mass than other groups. The adjusted mean values for appendicular skeletal muscle mass index (kg/m2) with 95% CIs according to the four WC groups in ascending order were 7.55 (7.51-7.59), 7.62 (7.56-7.68), 7.65 (7.56-7.74), and 7.22 (7.04-7.41) in men and 5.83 (5.80-5.85), 5.96 (5.92-6.00), 6.03 (5.96-6.10), and 5.88 (5.73-6.03). CONCLUSIONS: There was a positive association between body fat and WC among individuals with normal BMI; conversely there was an inverted J-shaped association between lean body mass and WC. Our findings support the WC measurement should be included in obesity evaluations for adults with a normal BMI.

A Study of the Anthropometric Indices and Eating Habits of Female College Students

  • A study was conducted to investigate the anthropometric indices, fat distribution, eating habits and levels of daily physical activity based on the body mass index (BMI ; kg/㎡) of 180 female college students. This stady was based on survey 2001 using a questionnaire. The results are as follow ; The mean age, height, weight and BMI were 20.41 ${\pm}$ 1.82 years, 161.86 ${\pm}$ 4.51cm, 53.49 ${\pm}$ 7.02kg, and 20.43 ${\pm}$ 2.65kg/㎡, respectively. The group with BMI of under 20 (the lean group) comprised 48.9% : the group with BMI of 20 to 25 (the normal group) comprised 44.4% and the group with BMI of over 25 (the obese group) comprised 6.7% of the subjects. Even though the lean and the normal groups were determined according to BMI. 12.5% of the lean group and 31.3% of the normal group had more than 25% body fat. Of the students, 11.5% of the lean group, 25.0% of the normal group and 58.3% of the obese group had of more than 0.8 waist/Hip Ratio (WHR). The medical problems suffered by the subjects were constipation (24.4%), gastroenteritis (20.6%), anemia(15.0%) and edema(13.3%), which was a result of their eating habits. Out of all the subjects, 27.2% ingested their meals irregularly, 74.4% skipped breakfast, and 92.4% of the lean group, 89.2% of the normal group and 81.8% of the obese group ingested snacks on a daily basis. Their favorite types of snacks were cookies breads and cakes (33.7%), spicy sliced rice cakes pork sausage and boiled fish cakes (18.5%), fruits (15.2%), juices and sodas (13.5%) and hamburgers and pizzas (3.9%). Their consumption of high calorie and processed foods was high, Of the subjects, 38.9% ate out daily. 45.1% had tried weight loss. Those with large upper bodies comprised 11.5%, large intermediate bodies 18.5% and large lower bodies 69.0%. Most of the students were dissatisfied with their body snaps. Of the subjects, 95.1% exercised irregularly. Daily physical activity tended to increase significantly with an increase in BMI : the lean group's average energy consumption was 1509.40㎉ the normal group's was 1792.21㎉ and the obese group's was 2334.63㎉, which were below the recommended value. This study suggests that the maintenance of an ideal weight, the improvement of eating habits and adequate exercise are needed for female college students to improve their health and nutritional status. 비만 아동에서 8주간의 학교 체중 조절 프로그램 실시 후 비만도, 체형 만족도 및 체형 존중감 변화에 대한 예비연구 (A Pilot Study of the Effects of an 8-week School-Based Weight Loss Program on Obesity, Shape Satisfaction, and Shape Esteem in Obese Children)

  • This study was conducted to examine the effects of an 8-week weight loss program, on body composition, body shape satisfaction, body shape esteem in 15 obese children. This program included nutritional education and regular physical fitness. After completion of this program, height and body weight increased slightly but body compositions changed. BMI, degree of obesity, and body fat levels dropped slightly, however these differences were not significant. After the 8-week program body weight increased from $63.61{\pm}8.80\;kg$ to $64.07{\pm}9.11\;kg$, degree of obesity and BMI decreased from $151.73{\pm}13.62%$, and $29.08{\pm}2.35$ to $146.60{\pm}12.92%$, and $28.57{\pm}2.26$, respectively, and the percentage of body fat decreased slightly from $42.70{\pm}4.87%$ to $41.46{\pm}4.88%$. There were slight changes in weight of body muscle and lean body mass from $19.62{\pm}3.48\;kg$ and $36.17{\pm}5.78\;kg$ to $20.30{\pm}3.64\;kg$ and $37.16{\pm}6.18\;kg$, respectively. LBM and amount of muscle were higher after this weight loss program. The body shape esteem score increased from $7.60{\pm}4.01$ to $9.00{\pm}6.20$. There was no significant difference in body shape satisfaction before and after the program, and the participants also wanted to be leaner. Finally, body weight, BMI, and body fat showed negative correlations to body shape esteem.
