  • The pelvic floor is a muscular structure, pierced by urologic, genital, and distal intestinal tract. Also pelvic floor is not a frozen but a functional unit. The pelvic floor dysfunction has 1) laxity of soft tissue and muscle 2)rupture of pelvic floor, 3)increased the tension. The purpose of this study is to give information about the pelvic floor dysfunction and pelvic exercise. This investigate the pelvic floor structure and function, pelvic floor dysfunction, pelvic floor exercise, and recent research trends. The pelvic floor exercise is one of important exercise in physical therapy, this exercise program will be improved patients with pelvic floor dysfunction.

  • Patient with facial paralysis may develop opthalmic complications. Poor eyelid closure, ectopion and lagophthalmos place the patinet at increased risk for development of corneal problems such as epitheilail defects, stromal thinning, bacterial infection, and even perforation. Inilital treatment should be conservative and include the use of ocular lubricants and taping of the lower eyelid into the proper position. Surgical intervention may be required in patients who have failed medical therapy or in whom the facial paralysis is not expected to improve. Gold weight implantation in upper eyelid and lower lid tightening in lower eyelid has become a popular procedure to correct upper eyelid retraction and lower eyelid laxity and to improve corneal coverage. We describe technique for placement of a gold weight in the upper lid, with attention of the maintenance of symmetric eyelid crease and susture canthopexy to correct malpositioned lower eyelid.

Splenic Infarction due to Torsion of Wandering Spleen - A Case Report - (유주 비장의 염전에 의한 비장 경색 - 1예 보고 -)

  • Wandering spleen is very rare condition in children characterized by migration of the spleen from its normal position due to laxity or absence of the supporting splenic ligaments. We experienced a case of splenic infarction due to torsion of a wandering spleen in a 6-year-old boy who presented with fever, vomiting, and abdominal pain of 2 day's duration. On physical examination, there was severe tenderness in the left upper quadrant of the abdomen. The plain abdominal radiograph showed marked colonic gaseous distension. Contrast-enhanced abdominal computed tomography scan showed decreased density of spleen in the normal position, consistent with infarction. At emergency laparotomy, a wandering spleen twisted $360^{\circ}$ on its pedicle was found. Despite splenic detorsion, blood flow could not be restored. Splenectomy was therefore performed. The child was discharged 7 days after surgery without any complications.

A Heuristic Scheduling Algorithm for Reducing the Total Error of an Imprecise Multiprocessor System with 0/1 Constraint

  • The scheduling problem of satisfying both 0/1 constraint and the timing constraint while minimizing the total error is NP-complete when the optional parts have arbitrary processing times. In this paper, we present a heuristic scheduling algorithm for 0/1 constraint imprecise systems which consist of communicating tasks running on multiple processors. The algorithm is based on the program graph which is similar to the one presented in[4]. To check the schedulability, we apply Lawler and Moore's theorem. To analyze the performance of the proposed algorithm, intensive simulation is done. The results of the simulation shows that the longest processing first selection strategy outperforms random or minimal laxity policies.

  • Purpose: The reverse abdominoplasty has been reported infrequently as a procedure to improve the upper abdominal wall contour. Especially, there have been rare cases on the surgical techniques with augmentation mammoplasty using implant. It is known to maintain the result. Methods: This is a retrospective review of the senior surgeon's patients who underwent reverse abdominoplasty with augmentation mammoplasty. A 63-year-old female was dissatisfied with her contracted breast and upper abdominal contour after previous abdominal wall liposuction. We performed reverse abdominoplasty with augmentation mammoplasty through same inframammary incision. Results: There was a significant improvement of the upper abdominal wall and breast contour. There was no perioperative complication. The patients was satisfied with the results and retained a good shape during the 3 months follow-up periods. Conclusion: Reverse abdominoplasty with augmentation mammoplasty is an acceptable technique that provides good results and should be considered in cases of upper abdominal laxity with capsular contracture on both breasts.

Cutaneous asthenia (Ehlers-Danlos syndrome) in a Korean short-haired cat

  • A 1-year-old Korean domestic short-haired cat presented with skin hyperextensibility and a severely macerated wound on the skin of the dorsal part of the neck. Diagnostic studies including histopathology and skin extensibility index revealed congenital cutaneous asthenia (Ehlers-Danlos syndrome). In this cat, the skin wounds and defects were successfully managed with standard wound management and cosmetic surgery. Although skin hyperextensibility is persistent, the cat has lived well without other complications to date. To the best of our knowledge, this is the first report of cutaneous asthenia in a cat in Korea.

  • A variety of theories have been reported as causes of shoulder pain in overhead throwing athletes. Recently, an explanation with microinstability of the shoulder and internal impingement has been proposed. The concept of the microinstability is that pathologic laxity of the anterior capsule caused by repeated abduction and external rotation of the shoulder leads to abnormal glenohumeral biomechanics and causes internal impingement of the shoulder. Based on the understanding of the pathology, it is recommended to identify the causes of shoulder pain in the overhead throwing athletes and perform appropriate rehabilitation or surgical treatment.

Clinical Outcomes of Minimum 12-Month Follow-Up of Anatomical Double Bundle ACL Reconstruction with a Longitudinally Split Tibialis Anterior Allograft (종분할된 전경골 동종건을 이용한 해부학적 전방 십자 인대 이중다발 재건술의 최소 12개월 임상 결과)

  • Purpose: The purpose of this study was to investigate the clinical results after a anatomical double bundle ACL reconstruction using a longitudinally split tibialis anterior allograft. Materials and Methods: We evaluated 24 patients with a minimum follow-up of 12 months who had undergone anatomical double bundle ACL reconstructions. The grafts utilized in all cases were tibialis anterior allografts which were longitudinally split into two strands. A standard rehabilitation protocol was applied in all patients. The pre- and post-operative data including Lysholm scores, International Knee Documentation Committee (IKDC) scores, Lachman test, pivot shift test and the side-to-side differences of anterior laxity measured by KT-2000 arthrometer were analyzed by use of a statistical method Results: The mean side-to-side instrumented laxity measured by the KT-2000 arthrometer significantly improved to a mean of $1.04{\pm}0.80\;mm$ (P < 0.001). The Lysholm knee scores also improved from $58.34{\pm}15.32$ to $86.25{\pm}6.48$ after surgery (P < 0.001). The patients exhibited improved IKDC scores (A: 15 cases, B; 9 cases) at the final follow-up, compared to preoperative scores (B: 5, C: 10, D: 9). Conclusion: Our data demonstrated that clinical results of anatomical double bundle ACL reconstruction with a split tibialis anterior allograft are encouraging with excellent side-to side laxity, significantly improved Lysholm knee score, IKDC score, Lachman and pivot shift data.

Clinical Results of Anterior Cruciate Ligament Reconstruction Using Hamstring Tendon (슬괵건을 이용한 전방십자인대 재건술의 임상적 결과)

  • Purpose: To evaluate the clinical results after anterior cruciate ligament (ACL) reconstruction with hamstring tendon and Ligament Anchor (LA) screw, which is newly designed for fixation of graft into femur. Materials and Methods: Fifty eight patients who were followed up at least more than 2 years after ACL reconstruction with four strands of Hamstring tendon and LA screw were included in this study. The graft was fixed with LA screw at femoral tunnel and with only bioabsorbable interference screw at tibial tunnel. The mean follow-up period was 28 months. The clinical results were evaluated by physical examination and Lysholm knee score. Widening of bony tunnel and anterior laxity difference compared with normal side by instrumented anterior laxity test with Telos(R) (Telos stress device; Austin & Associates, Inc., Polston, US) were evaluated. Results: The Lysholm knee score improved from 60.0 points preoperatively to 94.0 points at last follow up. On the Lachman test, there were mild (+) instability in 16 cases, moderate (++) in 24,severe (+++) in 18 preoperatively. 50 cases were converted to negative and 8 to mild instability at postoperative follow up. On instrumented anterior laxity test with Telos(R), difference between normal and affected knee on 20 lb was 12.9 mm in average preoperatively, and was decreased to 3.1mm at last follow-up. The femoral tunnel was widened from 10.6 mm postoperatively to 12.7 mm (21.1$\%$) at follow up on antero-posterior plane and from 10.7 mm to 12.4 mm (16.5$\%$) on lateral plane. Tibial tunnels was also widened from 9.8mm to 11.8mm (20.7$\%$) on antero-posterior plane and from 9.9mm to 11.7 mm ($18.9\%$) on lateral plane. Complications were: anterior knee crepitus in 17 case, quadriceps muscle atrophy(>3 cm) in 6, penetration of screw over the lateral femoral cortex in 5, saphenous nerve paresthesia in 2.Conclusions: ACL reconstruction with hamstring tendon and LA screw was one of the choice of grafts and fixation devices in restoring knee stability and in improving clinical results with little complications such as excessive widening of bony tunnel and anterior knee pain

Medial Anterior-inferior Capsular Shift in Multidirectional Shoulder Instability (내측 전하방 관절낭 이동술을 이용한 다방향 견관절 불안정성의 치료)

  • Purpose: The purpose of this study was to prove the effectiveness of the open medial (glenoid-based) anterior-inferior capsular shift in patients with multidirectional shoulder instability. Materials and Methods: We reviewed 19 patients treated by medial anterior-inferior capsular shift for multidirectional shoulder instability from March, 1998 to December, 2003. 15 patients of them have experienced recurrent dislocation. 8 patients(42%) showed bilateral laxity and 11 patients(58%) generalized ligamentous laxity, and 2 patients(10.5%) voluntary subluxation. An average follow-up was 24 months (range : $9{\sim}32$ months). Results: Pain improved in 18 patients of all. There was an average loss of 10 degree of external rotation, but no limitation of activity of daily living. There was no redislocation and subluxation, but two patients had some apprehension in sports activity. With Rowe score, the result was excellent or good in all patients. There were hematoma and local skin problem in 1 patient, but all had healed up. Conclusion: Medial anterior-inferior capsular shift in multidirectional shoulder instability provided satisfactory results in pain relief, patient's satisfactions and stability of glenohumeral joint. Though some of them have anterior gleniod deformities and large Hill-Sachs lesions, we could get good stabilities.
