성장기 환자에서 하악의 차등적 소구치 발치 (MANDIBULAR DIFFERENTIAL PREMOLAR EXTRACTI0N IN GROWING PATIENTS)

  • 성장기 환자에서 치아의 배열이나 입술의 외형을 개선하거나 경미한 악골 부조화에 의한 부정교합을 절충(camouflage)하기 위하여 발치가 적용될 수 있는데, 하악 치열궁에 총생의 양이 미약하거나 전치부 분절을 과도하게 후방견인 시킬 필요가 없는 경우 하악 제 2소구치 발치를 계획할 수 있다. 그러나, 성장기 환자에서 이러한 제 2소구치 발치의 기준과 치료에 의한 상, 하악 치아의 이동양상에 대한 연구는 부족한 실정이다. 본 연구는 상, 하악 소구치 발치를 통한 포괄적 교정치료를 시행한 성장기 환자를 대상으로 상악 제 1소구치와 하악 제 1 소구치를 발치한 군(4/4발치군, 14명)과 상악 제 1 소구치, 하악 제 2 소구치를 발치한 군(4/5발치군, 15명)의 치료 전, 후의 측모두부규격방사선사진을 비교하여 하악 제 2소구치 발치의 적응증에 대한 진단기준을 평가하고, 안정골 구조물을 참고한 중첩법을 이용하여 치아이동에 어떠한 차이가 있는지를 평가하여 다음과 같은 결론을 얻었다. 1. 4/4발치군과 4/5발치군은 치료 전 계측치에서 교합평면에 대한 상악전치의 치축각도, 구치부 교합관계, IMPA 그리고 interincisal angle에서 차이를 나타내었다. 2. 하악 제 1 대구치의 전방 이동량은 4/5 발치군에서 더 전방이동 되는 경향을 나타내었다(p<0.05). 3. 치료 전, 후의 상악 치아의 이동양상은 4/4 발치군과 4/5 발치군 사이에 차이가 없었다(p<0.05).

Functionally generated path technique 및 구내 디지털 스캔을 이용한 상악 구치부의 고정성 보철 수복 증례 (Fixed prosthodontic rehabilitation of maxillary posterior teeth using functionally generated path technique and intraoral digital scan: Case report)

  • 환자의 교합에 맞춰 적절한 고정성 보철물을 성공적으로 제작 및 수복하기 위해서 필요한 다양한 고려사항이 있다. 그 중 환자가 갖고 있는 교합의 형태를 파악하여 고정성 보철물에 부여하는 것은 필수적이라 할 수 있다. 견치 유도 교합, 군 기능 교합 등 여러가지 교합 형태를 파악하는데 있어서 하악의 운동을 기록하는 것이 중요한데, 1933년 Meyers에 의해 소개된 Functionally generated path (FGP) technique은 구내에서 왁스 등의 재료를 이용하여 교두가 지나가는 하악 운동로를 기록한 후 보철물 제작에 적용하는 치료 술식이다. 본 증례에서는 두 명의 환자에 있어서 상악 구치부에 이차 우식 혹은 치주염으로부터 기원한 치수 유래 병소가 있어, 근관 치료 및 치주 치료 완료 후 고정성 보철물 수복이 필요하였다. 장기적인 예후가 좋지 않은 치아이기에 측방 간섭을 최소화하기 위해 FGP technique을 이용하여 하악 운동을 기록하였다. 통상적인 석고 인상 방법이 아닌 구내 디지털 스캔을 시행함으로써 더 효율적으로 보철물을 제작하였다. 치료 후 환자와 술자 모두 기능적으로 만족할 만한 결과를 얻었기에 이를 보고하는 바이다.

상악골 전방견인 장치의 효과와 안정성에 대한 두부방사선 계측학적 연구 (Clinical Effects and Stability of the Maxillary Protraction Using the Lateral Cephalogram in Korean)

  • Skeletal Class III malocclusion is one of the most difficult type to treat and stabilize. For a child with developing skeletal Class III malocclusion, the treatment objective would be to stimulate maxillary growth, particulary one who has markedly deficient maxilla, and to restrain excessive mandibular growth. In order to stimulate the maxillary growth, maxillary protraction appliance is the one of the effective orthopedic appliances in skeletal Class III. The purposes of this study were as follows ; evaluation of the skeletal and dental changes of the maxillary protraction in children with Class III Maxillary deficiency , comparison of the clinical effects between the group with RPE and labiolingual intraoral appliances , comparison of the clinical effects and stability related to the ages of the patients : stability of the maxillary protraction about 1 year after retention. The subjects consisted of 60 children between the ages of 8 and 13.4 who were diagnosed as Class III with maxillary deficiency and were treated with Face Mask (Delaire Type) from the Dept. of Orthodontics Yong Dong Severance Hospital, Yonsei University. 48 children wore the RPE and 12 children wore Labiolingual Appliance. Lateral Cephalograms were taken for each patient at before and after correction of anterior cross-bite in 60 children, and after an observation period of 10 to 14 months in 19 children. X and Y coordinate of 10 landmarks were analyzed using a horizontal line through sella and rotated $6^{\circ}$ down anteriorly as the horizontal reference axis, and a perpendicular verticual line through sella as the vertical reference axis. Each of the 31 measurents (10 verticals, 10 horizontals, 2 angles and 9 others) was statistically analyzed using SPSS/PC statistics. The results are as follows; 1. After maxillary protraction the maxilla and maxillary teeth moved downward and forward, while the mandible and mandibular incisor rotated downward and backward. 2. Maxillary protraction with rapid palatal expansion appliance was more effective than with labiolingual appliance. 3. More downward movement of the posterior palatal plane obserbed with maxillary protraction doing the midpalatal suture opening than with protraction after finishing the palatal expansion 4. The clinical effects of protraction and changes of the retention periods were not statistically significant among the age groups. 5. During the retention period, maxilla and maxillary teeth, and mandible and mandibular teeth moved downward and forward, however the mandibular changes were larger than the maxillary changes.

Open versus closed reduction of mandibular condyle fractures : A systematic review of comparative studies

  • Objective : The objective of this review was to provide reliable comparative results regarding the effectiveness of any interventions either open or closed that can be used in the management of fractured mandibular condyle Patients and Methods : Research of studies from MEDLINE and Cochrane since 1990 was done. Controlled vocabulary terms were used. MeSH Terms were "Mandibular condyle" AND "Fractures, bone". Only comparative study were considered in this review using the "limit" function. According to the criteria, two review authors independently assessed the abstracts of studies resulting from the searches. The studies were divided according to some criteria, and following were measured: Ramus height, condyle sagittal displacement, condyle Towns's image displacement, Maximum open length, Protrusion & Lateral excursion, TMJ pain, Malocclusion, and TMJ disorder. Results : Many studies were analyzed to review the post-operative result of the two methods of treatment. Ramus height decreased more in when treated by closed reduction as opposed to open reduction. Sagittal condyle displacement was shown to be greater in closed reduction. Condyle Town's image condyle displacement had greater values in closed reduction. Maximum open length showed lower values in closed reduction. In protrusive and lateral movement, closed reduction was less than ORIF. Closed reduction showed greater occurrence of malocclusion than ORIF. However, post-operative pain and discomfort was greater in ORIF. Conclusion : In almost all categories, ORIF showed better results than CRIF. However, the use of the open reduction method should be considered due to the potential surgical morbidity and increased hospitalization time and cost. To these days, Endoscopic surgical techniques for ORIF (EORIF) are now in their infancy with the specific aims of eliminating concern for damage to the facial nerve and of reducing or eliminating facial scars. Before performing any types of treatment, patients must be understood of both of the treatment methods, and the best treatment method should be taken on permission.

하악 우각부 골절 시 2개의 miniplate를 이용한 관혈적 정복술에 대한 전향적 예비 임상연구 (The prospective preliminary clinical study of open reduction and internal fixation of mandibular angle fractures using 2 miniplates)

  • Introduction: The placement of a single miniplate is not sufficient to achieve rigid fixation in mandibular angle fractures. It often causes difficulties in reducing the intermaxillary fixation (IMF) period. Consequently, the placement of 2 miniplates is preferable. The intraoral approach in an open reduction and internal fixation (ORIF) of a mandibular angle fracture with 2 miniplates is often challenging. Accordingly, an alternative of transbuccal approach is performed. However, this method leaves a scar on the face and can result in facial nerve injury. This clinical study suggests a protocol that can maintain rigid fixation without a transbuccal approach in mandibular angle fractures. Materials and Methods: The subjects were 7 patients who sustained fractures of the mandibular angle and treated at Department of Oral and maxillofacial surgery, Sacred Heart Hospital, Hallym University. ORIF under general anesthesia was done using the intraoral approach. One miniplate was inserted on external oblique ridge of the mandible, and the other was placed on lateral surface of the mandibular body with contra-angle drill and driver. A radiographic assessment and occlusal contact point examination was carried out before surgery, and 2, 4 and 6 weeks after surgery. Results: The mean operation time was 80 minutes. Regarding the occlusion state, the number of contact points increased after surgery. Paresthesia and infection were reported to be complications before surgery. Conclusion: The placement of 2 miniplates using contra-angle drill for ORIF of mandibular angle fractures allows early movement of the mandible without IMF. We propose this approach to reduce the patients’discomfort and simplify the surgical procedure.

Three-dimensional finite element analysis of unilateral mastication in malocclusion cases using cone-beam computed tomography and a motion capture system

  • Purpose: Stress distribution and mandible distortion during lateral movements are known to be closely linked to bruxism, dental implant placement, and temporomandibular joint disorder. The present study was performed to determine stress distribution and distortion patterns of the mandible during lateral movements in Class I, II, and III relationships. Methods: Five Korean volunteers (one normal, two Class II, and two Class III occlusion cases) were selected. Finite element (FE) modeling was performed using information from cone-beam computed tomographic (CBCT) scans of the subjects' skulls, scanned images of dental casts, and incisor movement captured by an optical motion-capture system. Results: In the Class I and II cases, maximum stress load occurred at the condyle of the balancing side, but, in the Class III cases, the maximum stress was loaded on the condyle of the working side. Maximum distortion was observed on the menton at the midline in every case, regardless of loading force. The distortion was greatest in Class III cases and smallest in Class II cases. Conclusions: The stress distribution along and accompanying distortion of a mandible seems to be affected by the anteroposterior position of the mandible. Additionally, 3-D modeling of the craniofacial skeleton using CBCT and an optical laser scanner and reproduction of mandibular movement by way of the optical motion-capture technique used in this study are reliable techniques for investigating the masticatory system.

성장기 반대교합자의 FR III 효과에 관한 임상적 고찰 (A CLINICAL CONSIDERATION ON THE EFFECT OF FR III IN GROWING PATIENTS WITH ANTERIOR CROSS-BITE)

  • FR III에 의해 전치부 반대교합이 개선된 성장기 반대교합자 7명을 대상으로 치료전과 전치부 반대교합개선 즉시의 측모두부방사선 규격사진 및 모형을 분석 검토한 결과를 다음과 같이 요약할 수 있으나, 악골형태, 성장형태, 부정교합상태등에 따라 아주 다양한 결과를 나타내어 FR III에 의한 악안면의 변화를 한마디로 요약하는 것은 상당히 힘든 것으로 사료된다. 1. 상악골은 미약한 전상방으로의 회전 경향과 전방성장을 보였다. 2. 하악골은 개체간의 차이는 있으나 미약한 수직적 위치변화와 함께 하안면고가 증가되었다. 3. 상악전치는 치체이동 또는 순측경사되었다. 4. 모든 증례에서 하악전치는 설측경사를 보였다. 5. 상악치열궁 폭경은 증가되었으며, 하악치열궁 폭경은 개체간의 차이는 있으나 대체적으로 거의 변화되지 않았다.

골격성 3급 부정교합자의 악교정 수술 후 설골 위치와 상기도 크기의 변화 (CHANGES OF THE HYOID BONE POSITION AND THE UPPER AIRWAY DIMENSION AFTER ORTHOGNATHIC SURGERY IN SKELETAL CLASS III PATIENTS)

  • After orthognathic surgery in skeletal class III patients, the hyoid bone position and the upper airway dimension could be changed due to mandibular setback. There has been many studies about airway dimension of the patients with skeletal class II malocclusion or obstructive sleep apnea. but not with skeletal class III. The purpose of this study was to examine the change of position of the hyoid bone and the consequent change of airway space as the result of retrusion of mandible after orthognathic surgery in skeletal Cl III malocclusion patients. It is also to apply this results in predicting, diagnosing and treating the subsequent obstructive sleep apnea. Forty patients who were diagnosed as skeletal Cl III maloccusion, received orthoganthic surgery of both jaws including mandibular setback, and were followed up post-operatively for more than 6 months were selected. There were 10 male patients 30 female patients. The preoperative and postoperative lateral cephalograms were traced and the distances and angles were measured. The nasopharyngeal space increased postoperatively while the oropharyngeal space decreased. Except for the change of oroparyngeal space, the changes in male patients were greater than female patients. The hyoid bone moved in the posterior-inferior direction, and the change was greater in males than in females. If the postoperative mandibular setback is great, then a significant decrease of airway space and posterior and inferior movement of the hyoid bone were observed. This can result in symptoms related to obstructive sleep apnea. This result should be considered in the diagnosis and treatment planning of orthognathic surgery patients.

Pantographic Reproducibility Index(PRI)를 이용한 하악운동의 재현성에 관한 연구 (A Study on Reproducibility of Mandibular Movements Using Pantographic Reproducibility Index (PRI))

  • In order to determine if a relationship exists between the clinical symptoms of TMJ dysfunction and the reproducibility of mandibular movements, twenty one subjects were chosen. The control group consisted of 5 subjects who were determined to be free from signs and symptoms of dysfunction . The sixteen experimental subjects were selected on the basis of their having dysfunctional symptoms. The author obtained two sets of pantographic tracings. Each set consisted of tracings from three both lateral and one protrusive movements. A second set of tracings were recorded immediately on the new recording papers using the same procedure as the first tracing. The tracings were scored by Pantographic reproducibility index (PRI). The obtained results were as follows. 1. Mean PRI scores of groups increased as the degree of dysfunction were increased. 2. For the groups of no or slight dysfunctional symptoms the PRI scores of the second tracing were smaller than the first one, wherea the scores of the second tracing from $D_2,\;D_3$ group were larger than the first tracing. 3. Differences between the mean PRI scores of control group and those of experimental group were statistically significant. 4. The second sets of tracings were more reliable statistically than those of first ones. 5. PRI can be used as a meaningful aid for the evaluation of the diagnostic and therapeutic results of treatment modalities for the TMJ dysfunction. 6. At 3east two sets of tracings should be recorded when the PRI is to be used to detect the incoordinated movements of TMJ dysfunction patient. 7. PRI scores of control group ($D_0$) was 137.7, thus, mandibular movement was reproducible, whereas PRI scores of experimental groups ($D_1,\;D_2,\;D_3$) were 22.5, 27.7, 30.45 respectively, thus were nonreproducible.

Evaluation of stability after pre-orthodontic orthognathic surgery using cone-beam computed tomography: A comparison with conventional treatment

  • Objective: The aim of this study was to evaluate the skeletal and dental changes after intraoral vertical ramus osteotomy (IVRO) with and without presurgical orthodontics by using cone-beam computed tomography (CBCT). Methods: This retrospective cohort study included 24 patients (mean age, 22.1 years) with skeletal Class III malocclusion who underwent bimaxillary surgery with IVRO. The patients were divided into the preorthodontic orthognathic surgery (POGS) group (n = 12) and conventional surgery (CS) group (n = 12). CBCT images acquired preoperatively, 1 month after surgery, and 1 year after surgery were analyzed to compare the intergroup differences in postoperative three-dimensional movements of the maxillary and mandibular landmarks and the changes in lateral cephalometric variables. Results: Baseline demographics (sex and age) were similar between the two groups (6 men and 6 women in each group). During the postsurgical period, the POGS group showed more significant upward movement of the mandible (p < 0.05) than did the CS group. Neither group showed significant transverse movement of any of the skeletal landmarks. Moreover, none of the dental and skeletal variables showed significant intergroup differences 1 year after surgery. Conclusions: Compared with CS, POGS with IVRO resulted in significantly different postsurgical skeletal movement in the mandible. Although both groups showed similar skeletal and dental outcomes at 1 year after surgery, upward movement of the mandible during the postsurgical period should be considered to ensure a more reliable outcome after POGS.
