Effects of bodily retraction of mandibular incisors versus mandibular setback surgery on pharyngeal airway space: A comparative study

  • Objective: The purpose of this study was to compare the changes induced in the pharyngeal airway space by orthodontic treatment with bodily retraction of the mandibular incisors and mandibular setback surgery without extraction. Methods: This retrospective study included 63 adult patients (32 men and 31 women). Thirty-three patients who had been treated via four-bicuspid extraction and bodily retraction of the mandibular incisors (incisor retraction, IR group) were compared with 30 patients who had been treated via mandibular setback surgery (MS group) without extraction. Lateral cephalograms were acquired and analyzed before (T1) and after treatment (T2). Results: The superior pharyngeal airway space did not change significantly in either group during treatment. The middle pharyngeal airway space decreased by $1.15{\pm}1.17mm$ and $1.25{\pm}1.35mm$ after treatment in the IR and MS groups, respectively, and the decrease was comparable between the two groups. In the MS group, the inferior pharyngeal airway space (E-IPW) decreased by $0.88{\pm}1.67mm$ after treatment (p < 0.01). The E-IPW was larger in the MS group than in IR group at T1, but it did not differ significantly between the two groups at T2. No significant correlation was observed between changes in the pharyngeal airway space and the skeletal and dental variables in each group. Conclusions: The middle pharyngeal airway space decreased because of the posterior displacement of the mandibular incisors and/or the mandibular body. The E-IPW decreased only in the MS group because of the posterior displacement of only the mandibular body.

구순구개열 환자의 Le Fort I 골절단술 후 상악골의 위치적 안정성에 관한 연구 ; 예비보고 (Skeletal Stability after Le Fort I Osteotomy in the Cleft Patients; Preliminary Report)

  • It is well known that the postoperative skeletal instability after Le Fort I osteotomy for advancement of maxilla in the cleft patients is one of the major surgical problems. So we had tried to compare the amount of relapse after Le Fort I advancement surgery in the horizontal and vertical positional change, angular change of reference points between cleft patients and non-cleft patients. Longitudinal records of 10 consecutive cleft patients (test group) and 20 non-cleft patients (control group) were analyzed. Lateral cephalograms were taken preoperatively, immediately postoperatively, and 2, 6, 12 months postoperatively. We measured horizontal and vertical changes (ANS, PNS, AI) and angular change (SNA) of the reference points and lines. In the test group, horizontal relapse of ANS, PNS, AI point are 36.4%, 37.5%, 32.0% respectively at 12 months postoperatively. The vertical relapse of ANS, PNS, AI are 25.3%, 32.3%, 39.1% respectively at 12 months postoperatively. The angular change of SNA is 33.6% at 12 months postoperatively. In the control group, horizontal relapse of ANS, PNS, AI point are 23.8%, 30.2%, 21.7% respectively at 12 months postoperatively. The vertical relapse of ANS, PNS, AI are 22.7%, 27.3%, 25.1% respectively at 12 months postoperatively. The angular change of SNA is 22.2% at 12 months postoperatively. The cleft patients have a larger tendency of skeletal and dental relapse compared with non-cleft patients after Le Fort I surgery. So the oral and maxillofacial surgeons must keep in mind these facts in order to minimize the relapse phenomenon from the beginning of surgical planning to postoperative care.

상악전돌에 관한 방사선 두부계측학적 연구 (A ROENTGENOCEPHALOMETRIC STUDY ON MAXILLARY PROTRUSION)

  • This study was undertaken to compare the craniofacial morphology of Class II, Division 1 malocclusion with that of normal occlusion in children, and to investigate the incidence of various Class II, Division 1 craniofacial skeletal patterns. The subjects consist of thirty seven boys and fifty three girls with Class II, Division 1 malocclusion, and forty six boys and eighty one girls 10-15 years with normal occlusion. Measurements were recorded, tabulated and analyzed on the lateral cephalograms by the degree of SNA, SNB and ANB. The following characteristics of the Class II, Division 1 skeletal pattern were observed. 1. The anteroposterior relationship of the maxilla to the cranium in the Class II, Division 1 was very similar to that of normal occlusion. 2, Mandible of the Class II, Division 1 malocclusion was in the posterior position in relation to the cranial anatomy when compared to normal. 3. The chin point as measured by SN Pog and NS Gn showed distal positioning in relation to normal occlusion. 4. SN to mandibular plane angle was large in Class II, Division 1 malocclusion. 5. Mandibular incisor inclination was not significantly different between Class II, Division 1 malocclusion and normal occlusion, but maxillary incisors inclined and positioned labially and consequently overjet was large in Class II, Division 1 malocclusion. 6. Class II, Division 1 malocclusion was divided into four types of craniofacial skeletal pattern. The most common Class II, Division 1 pattern was found to be type C in which SN-Mand. Pl. was above mean range of normal occlusion. The next frequent pattern was found to be type A in which maxilla and mandible were within normal range of protrusion while upper incisors were severly labially inclined.

하악 전방분절 절골술을 이용한 경미한 하악전돌증의 교정 (The Correction of Mild Mandibular Prognathism Using Mandibular Anterior Segmental Osteotomy)

  • Purpose: For a minor degree of mandibular prognathism, mandibular anterior segmental osteotomy (ASO), usually extracting the bilateral premolars, has been performed frequently to correct malocclusion of the anterior teeth. Preoperative planning using cephalometry and a dental model is very important for such a orthognathic surgery. Depending on the specific preoperative mock surgery with the dental model, ASO, with ipsilateral unitooth extraction, is defined to be feasible and performed for ten patients. The comparisons of its preoperative and postoperative analysis of clinical photographs, dental casts, and lateral cephalograms, for soft tissue profiles, skeletal and dental relationships are described in the following, and its clinical applications are noted. Methods: From March 1, 2004, to March 31, 2006, We performed 10 mandibular ASO by extraction of ipsilateral unitooth to improve their lower facial profiles and the lip relationships. Patient age ranged from 19 to 33 years, with a mean age of 25.6 years. Two were males and eight were females. Results: All patients were satisfied with aesthetic and occlusal changes postoperatively. Significant and persistent decrease in the SNB and interincisal angle were observed in the postoperative cephalometries. The soft tissue profiles also were improved and near Ricketts's esthetic line. Other combined procedures include nine genioplasties, two rhinoplasties, and one blepharoplasty. One patient complained of transient unilateral inferior mental nerve paresthesia. There were no other significant complications or relapses throughout the follow-up period(6-20 months). Conclusion: Mandibular ASO, extracting the ipsilateral unitooth, was performed for ten patients to correct mild mandibular prognathism. The amount of setback of the mandibular anterior portion was 2 to 3 mm, and satisfactory results were obtained combined with genioplasties.

전치부 개교합 환자의 하악지시상분할골절단술 후 수직적 안정성에 관한 연구 (THE STUDY ON VERTICAL STABILITY OF ANTERIOR OPEN BITE PATIENTS AFTER BSSRO)

  • Purpose : The purpose of this study was to investigate the vertical stability after BSSRO surgery in skeletal class III malocclusion patients with mild anterior open bite and to present a method to increase the stability. Materials and methods : 36 patients, 11 male and 25 female, who received BSSRO surgery with the diagnosis of skeletal class III with anterior open bite at the Department of Oral and Maxillofacial Surgery in Samsung Medical Center, from January 2002 to August 2003, were selected for this study. The patients were between 18 to 45 years of age. Preoperative and postoperative (immediate, 6 months, and 1 year after operation) lateral cephalograms were compared to evaluate the vertical stability by measuring the distance of nasion-menton, mandibular plane angle, and overbite. Results : The nasion-menton distance decreased by 1.65mm immediately after the operation in comparison to the preoperative value. This distance further decreased by 0.60 mm at 6 months and 1.06mm at 1 year after the operation. The mandibular plane angle increased after the operation and further increased at 6 months and 1 year. The amount of overbite increased by the operation was 2.34mm and an additional increase of 0.70mm at 6 months and 0.94mm at 1 year were shown. Conclusion : Clinically, none of the patients showed relapse of anterior open bite and the vertical stability is highly influenced by orthodontic treatment after the operation. In this study, BSSRO surgery is considered to be a rather reliable procedure that restores stability to skeletal class III malocclusion patients with slight anterior open bite.

하악전돌증 환자의 악교정 수술후 시간경과에 따른 안모 연조직 변화에 관한 연구 (A STUDY ON CHANGE OF THE SOFT TISSUE FACIAL PROFILE AFTER ORTHOGNATHIC SURGERY IN PATIENTS WITH THE MANDIBULAR PROGNATHISM)

  • This study was performed to evaluate the change of the soft tissue facial profile after mandibular set back surgery during time intervals. For this study, 33 patient, 8 males and 25 females, were selected and their lateral cephalograms were taken and analyzed periodically. Hard and soft tissue changes during postoperative time intervals, correlation between surgical skeletal changes and postoperative soft tissue changes, and prediction for long-term soft tissue changes were established through varying statistical methods. The results were as follow : 1. There were meaningful changes of anteroposterior skeletal position at 6 months and 2 years after mandibular set back by mandibular ramus osteotomy. Two years postoperatively, there was 30%, 32%, 29% relapse on B point, pogonion, menton each. 2. Two years after the mandibular ramus osteotomy, the relative changes of the soft tissue to their osseous counterparts showed 76% on the lower lip and 91% on the pogonion. 3. The movements of the mandibular landmarks in correlation to anteroposterior position of the lower lip and soft tissue of the chin showed to be effective on a long-term basis. 4. Using surgical changes of pogonion, prediction of changes in soft and hard tissue pogonion was useful and the coefficient of determination was 0.46 each and their reliability decreased 2 years postoperatively. 5. The upper lip position after the mandibular set back surgery was somewhat anterior 2 years postoperatively, but that has no statistical meanings.

CHANGES IN HYOID BONE AND TONGUE POSITIONS, AND ORAL CAVITY VOLUME AFTER MANDIBULAR SETBACK BY SAGITTAL SPLIT RAMUS OSTEOTOMY

  • Purpose: This retrospective study evaluated the changes in hyoid bone and tongue positions as well as oral cavity volume after mandibular setback by BSSRO. Materials and Methods: 18 Koreans who underwent BSSRO to correct mandibular prognathism were studied. Lateral cephalograms were taken and traced preoperatively (T0), immediately (T1) and approximately 6 months postoperatively (T2). Submentovertex radiograghs were taken and traced before surgery (T0) and about 6 months after surgery (T2). The area and volume of oral cavity, the vertical and horizontal dimensions of the hyoid bone and tongue dorsum were measured. Results: Mandibular setback surgery resulted in a significant reduction of lower oral cavity volume. The hyoid bone displaced posteroinferiorly immediately after surgery, and it tended to return to its original vertical position at 6 month after mandibular setback by BSSRO. The retropalatal space around tongue was maintained and the retrolingual space around tongue was reduced immediately postoperatively. The readaptation of tongue was not evident for that the follow up period was not long enough. No significant statistical correlations between the amounts of mandibular setback and the changes of oral cavity volume were observed. Conclusion: Mandibular setback surgery resulted in a significant reduction of lower oral cavity volume, which was most likely attributable to the posterior movement of the mandible. More subjects and long-term observations should be performed to assess the changes of oropharyngeal configuration following mandibular setback surgery.

성장기 반대교합자의 FR III 효과에 관한 임상적 고찰 (A CLINICAL CONSIDERATION ON THE EFFECT OF FR III IN GROWING PATIENTS WITH ANTERIOR CROSS-BITE)

  • FR III에 의해 전치부 반대교합이 개선된 성장기 반대교합자 7명을 대상으로 치료전과 전치부 반대교합개선 즉시의 측모두부방사선 규격사진 및 모형을 분석 검토한 결과를 다음과 같이 요약할 수 있으나, 악골형태, 성장형태, 부정교합상태등에 따라 아주 다양한 결과를 나타내어 FR III에 의한 악안면의 변화를 한마디로 요약하는 것은 상당히 힘든 것으로 사료된다. 1. 상악골은 미약한 전상방으로의 회전 경향과 전방성장을 보였다. 2. 하악골은 개체간의 차이는 있으나 미약한 수직적 위치변화와 함께 하안면고가 증가되었다. 3. 상악전치는 치체이동 또는 순측경사되었다. 4. 모든 증례에서 하악전치는 설측경사를 보였다. 5. 상악치열궁 폭경은 증가되었으며, 하악치열궁 폭경은 개체간의 차이는 있으나 대체적으로 거의 변화되지 않았다.

발치 및 비발치 치료증례에서의 치료전후 치열궁형태의 변화에 관한 연구 (A STUDY OF THE VARIANCES IN PRE- AND POST-TREATMENT DENTAL ARCH SHAPES IN EXTRACTION AND NON-EXTRACTION CASES)

  • This study was carried out in order to findout the amount of tooth movement, the changes arch size and the changes in arch morphology following orthodontic treatment and to provide a guideline for to predict post-treatment arch morphology. The sample group for this study consists of 15 males and 22 females, totalling in 37 persons, who received orthodontic treatment at Orthodontic Department of Dankook Univ. Dental Hospital. They are classified into Extraction Class I treatment group (E I), Non-extraction Class I treatment group (N I), and Non-extraction Class III treatment group (N III), according to their pre-treatment malocclusion state and methods of treatment. Following conclusions and averaged dental arch form for each group were obtained by cephalometric linear measurements and dental arch measurements using pre- and post-treatment lateral cephalograms and plaster study models. 1. Intercanine width were reduced in max. of both EI and NI during the period of treatment, 2. Intermolar width were reduced in max. of EI and increased in max. of NI. Therefore although there was no difference between these two groups before the treatment, intermolar width of the max, of NI was wider than that of E1 after the treatment. 3. PMV-incisor distance and PMV-canine distance were decreased in both max. and mand. of EI and that of NI, during the period of treatment. PMV-molar distance was decreased in both max. and mand. of NI and in mand. of NIII. 4. Items that showed stability during the treatment were: max. & mand. PMV-molar distance, mand. intercanine and intermolar width in EI; mand. intercanine and intermolar width in NI; mand. & max. PMV-incisor distance, PMV-canine distance, max. PMV-molar distance and max. & mand. intercanine and intermolar width in NIII. 5. The differences in averaged canine and molar variances to post-treatment dental arch form were present only in EI and in NI. There was no variance between maxilla and mandible in each group.

치조성 양악 전돌 환자에서 컴퓨터를 이용한 치료 결과 예측의 정확성 (Comparison of computer-based treatment prediction with true results in bimaxillary protrusion cases)

  • 치조성 양악 전돌 증례에서 컴퓨터 시뮬레이션을 이용한 연조직 측모의 치료 결과 예측의 객관적 정확성을 상, 하악 제 1소구치 발거를 통해 교정치료를 받은 21명의 성인 여성을 대상으로 $Quick\;Ceph\;Image\;Pro^{TM}\;&\;Quick Eeph\;2000^{TM}$ 을 이용하여 조사, 평가하였다. 치료 전 측모 두부방사선 규격사진 투사도에서 경조직 이동량을 계측하고 Quick Ceph system 상에서 경조직을 동일한 양 이동시켜 컴퓨터시뮬레이 션을 위한 기준으로 삼았다. 이후 프로그램을 통해 연조직 변화를 시뮬레이션하고, 이 자료와 실제 치료 결과로 얻어진 연조직 계측 값을 측정하여 비교해 봄으로써 치료 결과 예측에 대한 정확도를 확인하고 임상에의 적용시 고려할 사항을 알아보았다. 시뮬레이션 결과 계측 항목의 수평 성분은 실제 치료 결과보다 작게, 수직 성분은 치료 결과보다 크게 예측하는 것으로 나타났으며 치료시 의 후방 견인은 실제보다 확대되어 나타나는 경 향을 보였다. 상악에서는 항목간 예측 오차의 방향이 일정하여 예측치가 실제보다 더 후방 이동되어 나타났으나, 하악에서는 증례간 차이가 컸다 Upper Lip Length는 짧게, Upper Vermilion Length는 길게 예측하여 시뮬레이션시 상순이 말려 보이는(rolling)효과를 확인할 수 있었으며 하순에서도 유사한 성향이 나타났으나 상순에 비해서 약하게 나타났다.
