Post-traumatic Atlantoaxial Rotatory Dislocation in an Adult Treated by Open Reduction and C1-C2 Transpedicular Screw Fixation

  • Atlantoaxial rotatory dislocation [AARD] is an uncommon disorder of childhood in which clinical diagnosis is generally difficult and often made late. It is very rare in adults because of the unique biomechanical features of the atlantoaxial articulation. We report a case of post-traumatic AARD in an adult. Reduction was difficult to obtain by skull traction and gentle manipulation. Therefore, the patient was treated surgically by an open reduction, transpedicular screw fixation, and posterior C1-2 wiring with graft. The normal atlantoaxial relation was restored with disappearance of torticollis. Postoperatively, the patient remains neurologically intact and has radiographic documentation of fusion. Atlantoaxial transpedicular screw fixation can be one of the treatment options for the AARD.

Age, education, melancholia, estrogen, woman, smoking, thyroid gland disease, aluminum etc. are danger factor of an Alzheimer dementia. Familyish factor was proved in some degree by gene. Medicine in early patient's case imbecility some measure progress late in degree develop. However, ceilinged thing is true in treatment of Alzheimer dementia up to now, and must help so that their quality of life may can rise and laws of physical therapist must help to keep function in everyday life.

  • In other advanced countries, an ABC-type distribution was employed few decades ago. It was, however, introduced, in this country, in late 1999. In order to apply it to the sites, several things have to be improved to comply with Korean distribution condition. It is very important to investigate the prospect and the problems in applying this type of distribution at site. To effectively employ it at site ASAP, follows have to be considered: easy installation of a transformer the development of branch gear, easy connections, the preventive diagnosis technique of insulating materials for ABC applications, etc. This paper describes the problems and the advanced plans of ABC-type distribution, and in construction.

  • The tetralogy of Fallot is one of the most frequent and serious congenital cardiac malformation accompanied cyanosis. For relief of cyanosis, the Waterston operation is a successful, palliative procedure in infant & young child under age of five with obstructive lesions of the right: side of the heart who require a systemic-pulmonary arterial shunt for survival. A patient, aged 3 and weighing 13 kg., who had been cyanotic since one month after birth,. was admitted to the University of Severance Hospital under a diagnosis of tetralogy of Fallot, The side to side anastomosis between the right pulmonary artery & the ascending aorta was performed in March 1976. The anastomotic channel was made only 4 ram. in diameter, thereafter massive unilateral pulmonary congestion on the side of the anastomosis developed shortly after operation. And the. patient died of congestive heart failure within a hour. And so the purpose of this report is describe the immediate & late effect of systemic-pulmonary shunt for T.O.F. with review of literatures.

Surgical Treatment of Intracardiac Myxoma (심장 점액종의 외과적 치료)

  • Since November 1979, 20 patients have undergone excision of an intracardiac myxoma, which was located in the left atrium in 18[90%], in the right atrium in 1[5%], and in the right ventricle in 1[5%], There were 17 female and 3 male patients with a mean age of 43.5 years [range 23 to 68 years]. Only one patient was asymptomatic, the others were seen mostly exertional dyspnea, palpitation, sings of systemic illness. Diagnosis was confirmed by echocardiography in all cases and angiography in two cases, preoperatively, The myxomas were successfully removed in all patients, either by shaving them from the atrial septum or by excising a portion of normal atrial septum with tumor. All heart chambers were carefully explored for presence of multi-centric myxoma or tumor debris. We conclude that excision of intracardiac myxoma is curative and long term survival is excellent and long term clinical and echocardiographic follow-up are recommended since late recurrence, although rare, has been reported.

Late development of a mandibular second premolar

  • In this report, we present the case of a girl with delayed odontogenesis of a lower second premolar for which she was followed up for 8.5 years. Congenital absence of permanent mandibular second premolars was observed at the initial radiographic examination at 8 years and 1 month. One year later, during the treatment period, an unexpected odontogenesis of a right second premolar was diagnosed on follow-up radiography. The original treatment plan was revised and a new plan was successfully implemented. This unusual case showed that the orthodontist's clinical philosophy must be flexible because unexpected situations can arise, especially when treating growing patients.

Management of Mediastinitis after Open Heart Surgery (개심술후 발생한 종격동염의 치료)

  • Between January 1984 and January 1994, 13 patients developed mediastinitis after cardiac operations. There were 7 women and 6 men with a mean age of 24.2 years[range 0.7 - 61 . Initial operation included 7 valve replacements and 6 congenital cardiac repairs. The duration between initial operation and diagnosis was 13.5 days[range 4-57 . Organisms cultured from debridement material included S.aurus[n=9 , S.epidermidis[n=3 and Enterobacter[n=1 . Fever was the most frequent complaint and purulent drainage was noticed in 9 patients[69% . Seven patients were treated with radical debridement followed by closed irrigation. In other 6 patients, the wounds were managed by debridement, open granulation and delayed wound closure. Two hospital deaths [15.4% following open granulation method, resulted from sepsis. The 11 survivors were followed-up for 1-120 months, averaging 42 months and had healed wounds. One late death occurred due to massive hemorrhage.

  • Dentinogenesis imperfecta is one of the most common autosomal dominant traits experienced in humans. It generally affects both the deciduous and permanent dentitions. There are 3 forms of dentinogenesis imperfecta that have been classified into type I(osteogenesis imperfecta associated), type II(hereditary opalescent dentin), and type III(Brandywine isolate opalescent dentin).1,2 The prevalence for all types of dentinogenesis imperfecta is approximately 1 per 8000 subjects. Early diagnosis and treatment are therefore, fundamental, aiming at obtaining a favourable prognosis since late intervention makes treatment more complex. This clinical report describes a treatment solution to the problems encountered by a dentinogenesis imperfecta patient with minimally invasive techniques.

  • When the fault occurred at distribution system, the restoration was late. There are 2 reasons The one is the error of fault location the other is multiple possible candidates of fault location. This paper presents two of new techniques for diagnosing fault regions. The proposed diagnosis scheme is capable of accurately identifying the location of fault upon its occurrence. based on the integration of information available from protective devices and measured load current change at the substation. In this paper expert system for real fault region is presented using these facts. Testing of the developed system using EMTP Simulation Model has demonstrated.

  • Malignant fibrous histiocytoma is the most common soft tissue sarcoma of late adult life, but it is rare in the pleura. We recently experienced a case of cytology of malignant fibrous histiocytoma in plural fluid. The smear revealed histiocytic malignant cells, spindle malignant cells and inflammatory cells. The histiocytic cells showed abundant, pale cytoplasm and ovoid, irregular eccentrically-placed nuclei. The spindle cells showed elongated nuclei. Intercellular collagen was also present.
