A Study on Safety, Performance and Clinical effectiveness Test Guideline of Versatile Ophthalmic Laser System (다목적 안과용 레이저 시스템 안전성 성능평가 및 임상적 유효성평가 가이드라인 수립을 위한 연구)

  • Ophthalmic Laser System is widely used in Selective Laser Trabeculoplasty of Open Angle Glaucoma and Ocular Hypertension. Versatile ophthalmic laser system is a medical device with technology that checks the condition of the treatment area by irradiating a continuous laser pulse on the treatment area, and monitoring the microbubble reaction caused by the temperature increase of the melanosome through the ultrasonic signal and the optical signal sensor. It performs selective laser treatment without damaging the photoreceptor by controlling the wavelength of the laser when microbubbles are detected. This study aims to suggest a guideline for evaluating safety, performance and clinical effectiveness of Versatile Ophthalmic Laser System in accordance with the growing technology. International Standards, Regulations, and Clinical Trial Protocols were investigated and analyzed for this study. As a result of this study, the safety, performance and clinical effectiveness test guideline for Versatile Ophthalmic Laser System were proposed. This guideline will ensure the safety and efficacy of Medical device, and furthermore it is expected to be able to promote the development of technology development by supporting a clinical trial plan.

  • A 355 nm (3.5 eV) neodymium:yttrium aluminum gamet laser, produced by a harmonic generator, was used to create silver metallic particles as seeds for nucleation in photosensitive glass containing Ag+ and Ce3+ ions. The pulse width and frequency of the laser were 8 ns and 10 Hz, respectively. Heat treatment was conducted at 570 C for 1 h, following laser irradiation, to produce crystalline growth, after which a LiAlSi3O8 crystal phase appeared in the laser-irradiated Li2O A1203 SiO2 glass. For the Present study, we compared the effect of laser-induced crystallization on glass crystallization with that of spontaneous crystallization by heat treatment.

  • The fabrication of micro-size patterning on alumina substrate is generated by laser direct writing, which has high precision and selectivity of various laser beam energies. The depth and width of patterns is affected by laser parameter such as laser power, scan rate. Through the chemical and mechanical polishing Pd seeds was effectively got rid of alumina substrate for selectivity electroless Ni plating. Thermal treatment is good method for changing electrical property of conductor line, because the treatment can control of the grain size.

Transformation Hardening of High Power Laser (고출력 레이저에 의한 표면 경화)

  • Heat flow equation and FEM have been used to calculate the hardening section of material in laser transformation hardening. SCM440 used as the diesel engine piston of vessel has been hardened by a $CO_2$ laser with the wavelength of $10.6{\mu}m$. The specimens were inclined from 0 to 70 degree to investigate the characteristics of laser hardening. The geometrical factor of heat flow equation affects the size of hardening area. The case width decreased with increasing travel speed and the case width increased with increasing inclined angle. Maximum case depth was achieved about 1.0mm and maximum hardness of laser hardened area was of 2.8 times than that of base metal. Experimental data show good agreement with the theoretical calculations for given laser hardening conditions.

  • Partial unilateral lentiginosis (PUL) is an unusual pigmentary disorder characterized by numerous lentigines on the skin, with onset usually during early childhood. It is characterized by unilateral segmental distribution with sharp margins in one or more dermatomes. Conventional laser treatments result in several adverse effects, such as mottled pigmentary changes (hyper or hypopigmentation), especially in people of Asian descent. A 57-year-old man with PUL on the neck was treated with a high-fluence 1,064-nm Q-switched (QS) neodymium-doped yttrium-aluminum-garnet (Nd:YAG) laser. After 20 treatment sessions, the lesions markedly improved without adverse effects or recurrence. We suggest that high-fluence 1,064-nm QS Nd:YAG laser treatment is an effective and safe modality for PUL.

  • The carbon dioxide (CO2) laser is one of the treatment options used for xanthelasma palpebrarum. However, even if the full dermal layer is removed, the lesion can recur due to the residual lipid deposits. A 44-year-old male patient with xanthelasma on both upper eyelids was treated with a pulsed dye CO2 laser. On the right upper eyelid, we carried out a CO2 laser treatment until the yellowish plaque was almost invisible and the full thickness of the dermal layer was removed. On the left upper eyelid, the dermal layer was partially removed and an additional squeezing out of yellowish particles was done. The lesion treated by the squeezing out of lipid droplets showed better long-term results than the lesion treated up to the deeper dermal layer.

  • Pyriform sinus cyst is a rare disease. This disease can be found by chance or appear as lymph nodes with cancer, infections, shortness of breath, or airway compromise, and can be an unusual cause of dysphonia. This paper presents a case of a pyriform sinus cyst in the pharyngeal wall of the left pyriform sinus, which presented as neck discomfort in a 63-year-old female. The plan was to remove this cyst with a transoral CO2 laser because a CO2 laser is a useful tool for removing cysts without bleeding and consuming time. The excision was performed successfully. Transoral laser pyriform sinus cyst excision is a simple, safe, and effective method for the treatment of pyriform sinus cysts. This procedure is likely to be used as a selective treatment when managing this condition.

  • A 50-year-old Korean female with oral erythro-leukoplakia was treated using a 532-nm Diode laser at 6 Watts with a pulsed width of 25 milliseconds. After two months following the laser treatment, the resected region was well-healed without any significant scar contracture. We suggest that the use of the 532-nm Diode laser can be a safe and effective treatment modality for patients suffering from oral leukoplakia.
