Eastern and Western Treatments for Improving Wrinkles and Treatment of Fine Wrinkles with Subcision (주름 개선을 위한 한.양방의 치료 동향 및 절개침을 사용한 잔주름 치료법 소개)

  • The purpose of this article is to review all the wrinkle treatments reported so far, and to introduce a method of dermal subcision, especially localized fine wrinkles. In order to remove wrinkles, laser, ulthera and drug therapy are commonly used in western medicine while Miso facial rejuvenation acupuncture, $Jung-An$ acupuncture and needle-embedding therapy are used in oriental medicine. However, as researches on fine wrinkles have been insufficiently conducted until now. Dermal subcision stated in this study is considered to be a safe and effective way to ameliorate fine linear-shaped wrinkles around or below eyes and mouth by increasing the circulation of qi and blood. also, reproducing dermal layer. More and further related cases and researches are expected in the future.

  • In this paper, an novel optical measuring system based on the electro-optic effect has been proposed and realized using Pockels cell with a view to detecting partial discharge taking place at the needle plane electrode. This system has the following advantages ; nonmetallic probe sensor, immune to external EMI noise and broad band response of the Pockels cell from DC to GHz. And also it is constructed by He-Ne laser, optical fiber, $LiNbO_3$ Pockels cell, photo detector, grin lens, oscilloscope and PC. The characteristics of the developed prototype sensor are investigated under AC and corona discharges.

  • In this paper, new Partial Discharge (PD) detection technique using Pockels-cell was proposed. For this purpose, PD was generated from needle-plane electrode in air and detected by optical measuring system using Pockets cell, based on Mach-Zehnder interferometer, consisting of He-Ne laser, single mode optical fiber, 50/50 beam splitter and photo detector. We show the characteristic of the proto-type sensor for the corona discharge.

  • A giant thyroid mass can be associated with severe compressive symptoms, requiring surgery for treatment. This paper reports the case of an 82-year-old man who presented with a giant thyroid mass that was suspected of being a metastatic papillary thyroid carcinoma on fine-needle aspiration cytology. A harmonic scalpel was used to excise the giant thyroid mass. A pectoralis major myocutaneous flap was used for reconstruction. The histopathology assessment confirmed the metastases in the lymph node and papillary thyroid carcinoma in the resected gland.

Experimental Study of Dadun(LR1)·Shaofu(HT8) Reduction in Acupuncture on the Improvement of Cerebral Hemodynamics (대돈(大敦)·소부(少府) 사법자침(瀉法刺鍼)이 뇌혈류역학(腦血流力學) 변동(變動) 개선(改善)에 미치는 영향)

  • Objective: The aim of this study was to investigate anti-ischemic effect of LR1 HT8 Reduction in Acupuncture Methods: I designed to investigate whether LR1 HT8 Reduction in Acupuncture affects cerebral hemodynamics [regional cerebral blood flow(rCBF), pial arterial diameter(PAD), mean arterial blood pressure(MABP)] in normal rats and to make manifest whether LR1 HT8 Reduction in Acupuncture is mediated by cyclooxygenase or guanylate cyclase. The changes of rCBF and MABP were determinated by laser-doppler flowmetry(LDF), and the change of PAD was de terminated by video microscope and width analyzer. Results: The results were as follows; 1. LR1 Reduction in Acupuncture was increased rCBF and PAD, but decreased MABP. 2. HT8 Reduction in Acupuncture was significantly increased rCBF, but decreased MABP, and increased PAD. 3. LR1 HT8 Reduction in Acupuncture was significantly increased rCBF, PAD, but decreased MABP after withdrawing of the needle. This results suggest that LR1 HT8 Reduction in Acupuncture increased significantly rCBF by dilating PAD. 4. Pretreatment with indomethacin(1mg/kg, i,v.) was significantly inhibited LR1 HT8 Reduction in Acupuncture induced increase of rCBF and PAD, but increased LR1 HT8 Reduction in Acupuncture induced decrease of MABP after withdrawing of the needle. 5. Pretreatment with methylene blue($10{\mu}g/kg$, i,v.) was decreased LR1 HT8 Reduction in Acupuncture induced increase of rCBF and PAD, but accelerated LR1 HT8 Reduction in Acupuncture induced decrease of MABP. Conclusions: I suggest that LR1 HT8 Reduction in Acupuncture has an anti-ischemic effect through the improvement of cerebral hemodynamics, and the mechanism is mediated by cyclooxygenase.

Angiogenic Responce to Transmyocardial Mechanical Reveascularization(TMMR) with Polymer Myocardial Stent (고분자 중합체 심근 스템트를 이용한 기계적 경심근 혈류재건술의 혈관생성 반응)

  • Background: Transmyocardial laser revascularization(TMLR) for revascularizing ischemic myocardium in patients was originally based on the assumption that laser channels remain their patency much longer. But recent studies show that laser channels did not remain open and that TMLR could achieve treatment benefits without long-term channel patency. The angiongencesis is currently thought to be induced by non-specific inflammatory response to mechanical tissue injury. This study is to evaluate hypothesis that various transmyocaridal mechanical revascularization(TMMR) may induce the angiogenic responses similar to that seen with TMLR, and transmyocaridal polymer stent revascularization(TMSR), the polymer stent in the myocardial tissue is hydrolyzed in 2 weeks, may enhance the non-specific inflammatory reaction resulting angiogenesis. Furthermore, polymer myocaridal stent channels remain long-term patency. Material and Method: Eight domestic pigs underwent ligation of the proximal circumflex artery, and 2 weeks later they were randomized to undergo transmycardial acupunctural revascularization (TMPR, Group I) of the left lateral wall with 18-G needle(n=2), to undergo transmyocardial (TMDR, Group II) with industrial 2mm steel drill(n=2), to undergo transmyocardial polymer stent revascularization (TMSR, Group III) after drilling the infarcted myocardium(n=2), the stent is poly(lactic acid-co-glycolic acid), which is self-degradated in the myocardium, and to a control group the ischemic zone was unterated(n=2). All the pigs were sacrificed after 4 weeks TMMR. Sections from the ischemic zone were submitted for vascular endothelial growth factor (VEGF) ELISA and histology. Result: There were makedly increase in the VEGF immunoassay in the ischemic zone of the TMMR group compared to the ischemic zone of the control group(control: each 30.85 and 43.15pg/mg protein, TMPR: each 44.14 and 68.61 pg/mg protein, TMDR: each 65.92 and 78.65 pg/mg protein, TMSR: each 177.39 and 168.87 pg/mg protein). TMSR channels caused greatest VEGF expression than channels made by other group and the polymer stent channels remained vacuole after 4 weeks. Conclusion: Transmyocardial polymer stent revascularization promoted the most angiogenci response by the VEGF immunoassay, although our study did not show the statistical significancy. The channels remained but the flow patency was not verified. Transmyocardial polymer stent revascularization (TMSR) is desirable in future experimental trials and in view of the significant cost implications comparable to that of laser.

Analysis of rutile single crystals grown by skull melting method (Skull melting법에 의해 성장된 rutile 단결정 분석)

  • Rutile single crystals grown by skull melting method were cut parallel and perpendicular to growth axis, and both sides of the cut wafers (${\phi}5.5mmx1.0mm$) were then polished to be mirror surfaces. The black wafers were changed into pale yellow color by annealing in air at 1200 and $1300^{\circ}C$ for $3{\sim}15\;and\;10{\sim}50$ hours, respectively. After annealing, structural and optical properties were examined by specific gravity (S.G), SEM-electron backscattered pattern (SEM-EBSP), X-ray diffraction (XRD), FT-IR transmittance spectra, laser Raman spectroscopy (LRS), photoluminescence (PL) and X-ray photoelectron spectroscopy (XPS). These results are analyzed increase of weight in air, decrease of weight in water and specific gravity, shown secondary phase of needle shape, diffusion of oxygen ion and increase of $Ti^{3+}$. From the above results, we suggest that the skull melting method grown rutile single crystals contain defect centers such as $O_v,\;Ti^{3+},\;O_v-Ti^{3+}$ interstitials and $F^+-H^+$.

Research Trends on the Acupuncture Treatment of Shoulder Impingement Syndrome (어깨 충돌증후군의 침치료에 관한 연구 동향)

  • The purpose of this study is to investigate reasearch trend of acupuncture treatment on shoulder impingement syndrome. This study investigated the recent studies about acupuncture treatment for shoulder impingement syndrome via searching Korean and foreign electronic databases(Research Information Services, National Digital Science Library, Oriental Medicine Advanced Searching Integated Services, PubMed, China National Knowledge Infrastructure). The search term was searched by combining 'shoulder impingement sydrome', 'subacrominal pain', 'acupuncture', 'needle therapy', and was limited to the articles published from 2005 to 2017. 14 studies were found to be analyzed according to the type of study, the number of cases, the type of treatment, the instruments for assessment and published year. 14 studies were published since 2005, 6 case reports, 8 randomized controlled trials. In the treatment of shoulder impingement syndrome, treatments such as acupuncture, acupotomy, pharmacopuncture, laser acupuncture, combined treatment of exercise and rehabilitation were performed. Visual analogue scale(VAS), range of motion(ROM) and various questionnaires were used as primary assessments. In each study, acupuncture treatment was reported to have a significant effect in the pain, range of motion and disability of shoulder impingement syndrome. In order to ensure objective evidence on acupuncture treatment, large scale case reports and randomized controlled trials should be continued.

Acupuncture Clinical Trial Study for Prevention of Postoperative Nausea and Vomiting in Children (소아의 수술 후 오심 및 구토에 대한 침술치료 임상연구 고찰)

  • Background and objective : Postoperative nausea and vomiting(PONV) are frequent complications of general anesthesia in children. High cost and associated side effects of antiemetic drugs, have increased interest in nonpharmacological methods for treatment of PONV. The aim of this study was to report the effect of acupuncture for prevention of PONV in children. Material and Method : Randomized clinical trials retrieved by a search of articles indexed on the Medline and NDSL database. Key words to search were 'PONV', 'POV', 'acupuncture', 'children', 'pediatric'. Results : Eleven articles were selected and six of them showed significant effectiveness of acupuncture in preventing PONV in children. Acupuncture methods were needle acupuncture acupressure, electro-acupuncture, laser acupuncture, acupuncture point injection. The main acupuncture point was P6(Neiguan). Conclusion : The effectiveness of acupuncture for prevention of PONV in children has been shown in some, but not all trials. Further studies are needed.

  • The effects of the internal flow in a diesel injection nozzle on the atomization of the spray has been investigated experimentally. Flow visualization was made using a transparent acrylic model nozzle. And also, measurement of the sac chamber pressure was made for clartfying the effect of pressure fluctuation in the sac chamber on the wpray behaviors. The geometry of the model nozzle was scaled up 10 times of the actual nozzle and the injection pressure for the model nozzle was adjusted so as to achieve a Reynolds number at the discharge hole which was the same as the actual nozzle. Polystyrene tracers, a laser sheet light and a still/high speed video camera were used to visualize the flow pattern in the sac chamber. When the needle lift was small, the high turbulence in the sac chamber generated by the high velocity seat flow made the spread angle of the spray large. Cavitation which arose in the sky chamber induced the pressure fluctuation and then affects the spread angle of the spray.
