Three-dimensional finite element analysis of unilateral mastication in malocclusion cases using cone-beam computed tomography and a motion capture system

  • Purpose: Stress distribution and mandible distortion during lateral movements are known to be closely linked to bruxism, dental implant placement, and temporomandibular joint disorder. The present study was performed to determine stress distribution and distortion patterns of the mandible during lateral movements in Class I, II, and III relationships. Methods: Five Korean volunteers (one normal, two Class II, and two Class III occlusion cases) were selected. Finite element (FE) modeling was performed using information from cone-beam computed tomographic (CBCT) scans of the subjects' skulls, scanned images of dental casts, and incisor movement captured by an optical motion-capture system. Results: In the Class I and II cases, maximum stress load occurred at the condyle of the balancing side, but, in the Class III cases, the maximum stress was loaded on the condyle of the working side. Maximum distortion was observed on the menton at the midline in every case, regardless of loading force. The distortion was greatest in Class III cases and smallest in Class II cases. Conclusions: The stress distribution along and accompanying distortion of a mandible seems to be affected by the anteroposterior position of the mandible. Additionally, 3-D modeling of the craniofacial skeleton using CBCT and an optical laser scanner and reproduction of mandibular movement by way of the optical motion-capture technique used in this study are reliable techniques for investigating the masticatory system.

Effects of NaOCl on Neuronal Excitability and Intracellular Calcium Concentration in Rat Spinal Substantia Gelatinosa Neurons

  • Recent studies indicate that reactive oxygen species (ROS) can act as modulators of neuronal activity, and are critically involved in persistent pain primarily through spinal mechanisms. In this study, we investigated the effects of NaOCl, a ROS donor, on neuronal excitability and the intracellular calcium concentration ($[Ca^{2+}]_i$) in spinal substantia gelatinosa (SG) neurons. In current clamp conditions, the application of NaOCl caused a membrane depolarization, which was inhibited by pretreatment with phenyl-N-tert-buthylnitrone (PBN), a ROS scavenger. The NaOCl-induced depolarization was not blocked however by pretreatment with dithiothreitol, a sulfhydryl-reducing agent. Confocal scanning laser microscopy was used to confirm whether NaOCl increases the intracellular ROS level. ROS-induced fluorescence intensity was found to be increased during perfusion of NaOCl after the loading of 2',7'-dichlorofluorescin diacetate ($H_2DCF$-DA). NaOCl-induced depolarization was not blocked by pretreatment with external $Ca^{2+}$ free solution or by the addition of nifedifine. However, when slices were pretreated with the $Ca^{2+}$ ATPase inhibitor thapsigargin, NaOCl failed to induce membrane depolarization. In a calcium imaging technique using the $Ca^{2+}$-sensitive fluorescence dye fura-2, the $[Ca^{2+}]_i$ was found to be increased by NaOCl. These results indicate that NaOCl activates the excitability of SG neurons via the modulation of the intracellular calcium concentration, and suggest that ROS induces nociception through a central sensitization.

Inhibitory Effect of Lactococcus lactis HY 449 on Cariogenic Biofilm

  • Dental caries is caused by cariogenic biofilm, an oral biofilm including Streptococcus mutans. Recently, the prevention of dental caries using various probiotics has been attempted. Lactococcus lactis HY 449 is a probiotic bacterium. The aim of this study was to investigate the effect of L. lactis HY 449 on cariogenic biofilm and to analyze its inhibitory mechanisms. Cariogenic biofilm was formed in the presence or absence of L. lactis HY 449 and L. lactis ATCC 19435, and analyzed with a confocal laser microscope. The formation of cariogenic biofilm was reduced in cultures spiked with both L. lactis strains, and L. lactis HY 449 exhibited more inhibitory effects than L. lactis ATCC 19435. In order to analyze and to compare the inhibitory mechanisms, the antibacterial activity of the spent culture medium from both L. lactis strains against S. mutans was investigated, and the expression of glucosyltransferases (gtfs) of S. mutans was then analyzed by real-time RT-PCR. In addition, the sucrose fermentation ability of both L. lactis strains was examined. Both L. lactis strains showed antibacterial activity and inhibited the expression of gtfs, a nd t he d ifference b etween both strains did not show. In the case of sucrose-fermenting ability, L. lactis HY 449 fermented sucrose but L. lactis ATCC 19435 did not. L. lactis HY 449 inhibited the uptake of sucrose and the gtfs expression of S. mutans, whereby the development of cariogenic biofilm may be inhibited. In conclusion, L. lactis HY 449 may be a useful probiotic for the prevention of dental caries.

Effects of Reactive Oxygen Species and Nitrogen Species on the Excitability of Spinal Substantia Gelatinosa Neurons

  • Reactive oxygen species (ROS) and nitrogen species (RNS) are both important signaling molecules involved in pain transmission in the dorsal horn of the spinal cord. Xanthine oxidase (XO) is a well-known enzyme for the generation of superoxide anions ($O_2^{\bullet-}$), while S-nitroso-N-acetyl-DL-penicillamine (SNAP) is a representative nitric oxide (NO) donor. In this study, we used patch clamp recording in spinal slices of rats to investigate the effects of $O_2^{\bullet-}$ and NO on the excitability of substantia gelatinosa (SG) neurons. We also used confocal scanning laser microscopy to measure XO- and SNAP-induced ROS and RNS production in live slices. We observed that the ROS level increased during the perfusion of xanthine and xanthine oxidase (X/XO) compound and SNAP after the loading of 2',7'-dichlorofluorescin diacetate ($H_2DCF-DA$), which is an indicator of intracellular ROS and RNS. Application of ROS donors such as X/XO, ${\beta}-nicotinamide$ adenine dinucleotide phosphate (NADPH), and 3-morpholinosydnomimine (SIN-1) induced a membrane depolarization and inward currents. SNAP, an RNS donor, also induced membrane depolarization and inward currents. X/XO-induced inward currents were significantly decreased by pretreatment with phenyl N-tert-butylnitrone (PBN; nonspecific ROS and RNS scavenger) and manganese(III) tetrakis(4-benzoic acid) porphyrin (MnTBAP; superoxide dismutase mimetics). Nitro-L-arginine methyl ester (NAME; NO scavenger) also slightly decreased X/XO-induced inward currents, suggesting that X/XO-induced responses can be involved in the generation of peroxynitrite ($ONOO^-$). Our data suggest that elevated ROS, especially $O_2^{\bullet-}$, NO and $ONOO^-$, in the spinal cord can increase the excitability of the SG neurons related to pain transmission.

Therapeutic Effect of Low Level Laser Therapy on the Trigger Points (발통점에 대한 저출력 레이저의 치료효과)

  • 구강안면통증환자에서 저출력 레이저의 임상 효과는 많은 임상가에 의해 연구되었으나 그 결과에 대해서는 아직도 논란중이다. 이에 본 연구는 두 가지 방법으로 시행하여 구강안면부위의 발통점에 대한 저출력 레이저의 효과를 평가하였다. 첫 번째 방법은 저작근 중 교근, 측두근과 경부근육 중 승모근에 발통점을 가진 부산대학교 치과대학생 69명중 37명은 레이저 조사군, 32명은 레이저 모의 조사군으로 무작위로 분류하여 저출력 레이저의 치료효과를 평가하였다. 둘째 방법으로 동일 근육에 발통점이 있는 19명의 환자와 발통점이 전혀 없는 20명의 정상인을 무작위로 환자군과 정상 대조군으로 분류하고, 각 군을 다시 레이저 조사군과 모의 조사군으로 나누어 실제 저출력 레이저의 치료효과와 위약효과를 평가하였다. 50mW, 820nm의 GaAlAs 반도체 레이저를 이용하여 4주 동안 첫 주는 2회, 이후 3주 동안 각 1회씩 총 5회를 조사하였고, 레이저 모의 조사군에서도 동일한 방법으로 시행하였다. 치료반응은 전자통각계를 이용하여 압력통각역치를 치료 전, 치료 2주 및 4주에 측정하여 이를 비교한 바 다음과 같은 결과를 얻었다. 1. 레이저 조사군의 각 근육에서 측정한 압력통각역치는 치료 2주 후부터 유의하게 높아졌으며(P<0.05), 모의 조사군과의 차이는 이후 점점 더 증가하였다(P<0.001). 모의 조사군에서는 압력통각역치의 유의한 변화가 없었다. 2. 레이저 조사-환자군에서 측정한 압력통각역치는 레이저 모의조사-환자군의 압력통각역치보다 그 증가폭이 더 크게 나타났다(P<0.05). 정상 대조군은 레이저 조사와 관계없이 압력통각역치에 유의성이 없었다. 3. 약간의 위약 반응이 레이저 모의조사 환자군과 정상 대조군에서 관찰되나, 레이저 조사 환자군의 실제 레이저 치료효과가 위약 반응보다 우세하였다.

Evaluation of marginal adaptation in three-unit frameworks fabricated with conventional and powder-free digital impression techniques

  • PURPOSE. The purpose of this in vitro study was to evaluate the marginal misfits of three-unit frameworks fabricated with conventional and digital impressions techniques. MATERIALS AND METHODS. Thirty brass canine and second premolar abutment preparations were fabricated by using a computer numerical control machine and were randomly divided into 3 groups (n=10) as follows: conventional impression group (Group Ci), Cerec Omnicam (Group Cdi), and 3shape TRIOS-3 (Group Tdi) digital impression groups. The laser-sintered metal frameworks were designed and fabricated with conventional and digital impressions. The marginal adaptation was assessed with a stereomicroscope at ${\times}30$ magnification. The data were analyzed with 1-way analysis of variances (ANOVAs) and the independent simple t tests. RESULTS. A statistically significant difference was found between the frameworks fabricated by conventional methods and those fabricated by digital impression methods. Multiple comparison results revealed that the frameworks in Group Ci (average, $98.8{\pm}16.43{\mu}m$; canine, $93.59{\pm}16.82{\mu}m$; premolar, $104.10{\pm}15.02{\mu}m$) had larger marginal misfit values than those in Group Cdi (average, $63.78{\pm}14.05{\mu}m$; canine, $62.73{\pm}13.71{\mu}m$; premolar, $64.84{\pm}15.06{\mu}m$) and Group Tdi (average, $65.14{\pm}18.05{\mu}m$; canine, $70.64{\pm}19.02{\mu}m$; premolar, $59.64{\pm}16.10{\mu}m$) (P=.000 for average; P=.001 for canine; P<.001 for premolar). No statistical difference was found between the marginal misfits of canine and premolar abutment teeth within the same groups (P>.05). CONCLUSION. The three-unit frameworks fabricated with digital impression techniques showed better marginal fit compared to conventional impression techniques. All marginal misfit values were clinically acceptable.

Micro-shear bond strengths of resin-matrix ceramics subjected to different surface conditioning strategies with or without coupling agent application

  • Purpose. This study aimed to assess the influence of various micromechanical surface conditioning (MSC) strategies with or without coupling agent (silane) application on the micro-shear bond strength (µSBS) of resin- matrix ceramics (RMCs). Materials and Methods. GC Cerasmart (GC), Lava Ultimate (LU), Vita Enamic (VE), Voco Grandio (VG), and Brilliant Crios (BC) were cut into 1.0-mm-thick slices (n = 32 per RMC) and separated into four groups according to the MSC strategy applied: control-no conditioning (C), air-borne particle abrasion with aluminum oxide particles (APA), 2W- and 3W-Er,Cr:YSGG group coding is missing. The specimens in each group were further separated into silane-applied and silane-free subgroups. Each specimen received two resin cement microtubules (n = 8 per subgroup). A shear force was applied to the adhesive interface through a universal test machine and µSBS values were measured. Data were statistically analyzed by using 3-way ANOVA and Tukey HSD test. Failure patterns were scrutinized under stereomicroscope. Results. RMC material type, MSC strategy, and silanization influenced the µSBS values (P<.05). In comparison to the control group, µSBS values increased after all other MSC strategies (P<.05) while the differences among these strategies were insignificant (P>.05). For control and APA, there were insignificant differences between RMCs (P>.05). The silanization decreased µSBS values of RMCs except for VE. Considerable declines were observed in GC and BC (P<.05). Conclusion. MSC strategies can enhance bond strength values at the RMC - cement interface. However, the choice of MSC strategy is dependent on RMC material type and each RMC can require a dedicated way of conditioning.

Histologic Effects of a Contact Diode Laser on Intraoral Soft Tissue (접촉식 다이오드레이저조사가 구강연조직에 미치는 조직학적 영향)

  • 본 연구는 두 가지 관점, 즉 다이오드레이저를 구강연조직에 접촉식으로 조사하여 조직을 절제할 경우 첫째 심부조직에서의 온도 상승 정도에 대한 분석 평가, 둘째 절제에 따른 조직의 열적 응고에 대한 조직학적 분석 평가를 위해 시도되었다. 연구를 위해 도축된 소의 혀 배면으로부터 5.0mm깊이에 T형 열전대(type T thermocouple)를 설치하고 조사조건에 따른 레이저절제 도주의 온도변화를 측정하고, 또한 해당조사부위의 조직을 대상으로 통법에 의한 H-E염색 후 조직학적 측면에서 열적 응고상을 검색하여 Nd:YAG레이저의 경우에서와 비교하였다. 즉 파장815nm의 다이오드레이저를 0-10W의 출력, 50Hz와 연속파의 주파수조건에서 접촉식으로 조사하였으며, 또한 파장1064nm의 Nd:YAG레이저를 0-10W의 출력, 50Hz와 100Hz의 주파수 조건에서 접촉식으로 조사하였다. 레이저의 접촉조사시 레이저광섬유 첨단부에서 10(10gm의 일정한 압력과 25mm/sec의 이동속도로 조직을 절제하였다. 레이저가 접촉조사된 조직부위를 통법에 의해 조직학적으로 H-E염색을 시행하였으며, 계측현미경을 이용하여 10배율 아래에서 조직절제의 폭과 깊이, 측방 및 하방 쪽으로 열적 응고의 폭을 계측하였다. 계측치에 대한 통계학적 처리결과 조직절제의 폭과 측방쪽으로 열적응고의 폭은 다이오드레이저에와 Nd:YAG레이저에서 같은 정도를 나타냈다. 조직절제의 깊이는 Nd:YAG레이저에서에 비해 다이오드레이저에서 더 깊었다. 하방쪽으로의 열적응고의 폭은 Nd:YAG레이저를 10Hz의 조건에서 조사한 경우에서 가장 넓었으며, 다이오드레이저를 50Hz와 연속파의 조건에서 조사한 경우에서 가장 좁았다. 레이저절제 도중 심부조직에서의 온도변화는 다이오드 레이저에서와 Nd:YAG레이저에서 모두 출력이 증가함에 따라 상승되었으며, 다이오드레이저에서 보다 Nd:YAG레이저에서 더 높이 상승되었다. 결론적으로 본 연구에서 시도된 조사조건 범위 이내에서는 구강연조직 절제시 다이오드레이저가 펄스형 광섬유 전달식 Nd:YAG레이저 보다 심부조직에서의 낮은 온도상승과 하방쪽으로의 좁은 열적 응고의 폭을 보이면서 우수한 조직절제효과를 나타냈다.

An experimental study of cutting efficiency of air-driven diamond burs on human tooth (수종 air-turbine 다이아몬드 버의 절삭 효과에 관한 실험적 연구)

  • Purpose: The purpose of this study was to investigate the cutting efficiency of coarse grit diamond burs with air-turbine handpiece on natural tooth. Materials and methods: Four groups of coarse grit diamond bur were selected: Komet (A), Shofu (B), Premier (C), and Mani (D). The extracted maxillary central incisors were used, and ten cuts were made on each specimen, using the rotary diamond burs. The surface of each bur was measured at the upper, middle, and bottom of the bur with confocal laser scanning microscope and imaged with SEM. The data were analyzed with one-way ANOVA and t-test at the significance level of 0.05. Results: The surface roughness was measured. At the A diamond bur, the Sa values were $52.93\;{\mu}m$, $48.32\;{\mu}m$, $46.79\;{\mu}m$, $45.06\;{\mu}m$, and $43.43\;{\mu}m$ for control, test 1, 2, 3, and 4 respectively. The Sa values were $50.68\;{\mu}m$, $45.62\;{\mu}m$, $44.41\;{\mu}m$, $44.10\;{\mu}m$, and $42.46\;{\mu}m$ for B diamond bur, $58.02\;{\mu}m$, $55.53\;{\mu}m$, $52.22\;{\mu}m$, $48.26\;{\mu}m$, and $45.36\;{\mu}m$ for C diamond bur, and $50.11\;{\mu}m$, $46.73\;{\mu}m$, $45.46\;{\mu}m$, $42.58\;{\mu}m$, and $41.80\;{\mu}m$ for D diamond bur. Surface roughness after each bur use showed significant changes, but no significant difference was found in surface roughness change between bur systems. Conclusions: Surface roughness in the same bur system showed significant differences after each tooth preparation. However no statistically significant differences were found in surface roughness between bur systems. The SEM images between control and test 4 showed the abraded particles.

The effect of using laser for ceramic bracket bonding of porcelain surfaces (세라믹 브라켓 부착 시 레이저를 이용한 포세린 표면처리 효과)

  • Objective: The purpose of this study was to investigate the effect of using laser for ceramic bracket bonding of porcelain surfaces and to compare it with conventional treatment of porcelain surfaces. Methods: Ninety feldspathic porcelain specimens were divided into 9 groups of 10, with each group having different surface treatments performed. Surface treatment groups were orthophosphoric acid, orthophosphoric acid with silane, hydrofluoric acid, hydrofluoric acid with silane, sandblasted, sandblasted with silane, laser etched, laser etched with silane, and glazed surface served as a control group. In the laser etched groups, the specimens were irradiated with 2-watt superpulse carbon dioxide ($CO_2$) laser for 20 seconds. Ceramic brackets were bonded with light-cure composite resin and all specimens were stored in water at $37^{\circ}C$ for 24 hours. Shear bond strength was determined in megapascals (MPa) by shear test at 1 mm/minute crosshead speed and the failure pattern was assessed. For statistical analysis, one-way ANOVA and tukey test were used. Results: Statistical analysis showed significant differences between the groups. The HFA + S group showed the highest mean shear bond strength ($13.92{\pm}1.94\;MPa$). This was followed by SB + S ($10.16\;{\pm}\;1.27\;MPa$), HFA ($10.09\;{\pm}\;1.07\;MPa$), L + S ($8.25\;{\pm}\;1.24\;MPa$), L ($7.86\;{\pm}\;0.96\;MPa$), OFA + S ($7.22\;{\pm}\;1.09\;MPa$), SB ($3.41\;{\pm}\;0.37\;MPa$), OFA ($2.81\;{\pm}\;0.37\;MPa$), G ($2.46\;{\pm}\;1.36\;MPa$), Bond failure patterns of HFA and silane groups, except L + S, were cohesive modes in porcelain while adhesive failure was observed in the control group and the rest of the groups. Conclusions : A 2-watt superpulse $CO_2$ laser etching of porcelain surfaces can provide a satisfactory result for porcelain surface treatment for ceramic bracket bonding. Laser irradiation may be an alternative conditioning method for the treatment of porcelain surfaces.
