A Cross-Sectional Study on the Association of Fiber Intake and Large Bowel Function (대장기능과 섬유질 섭취와의 상관성에 대한 단면적 조사연구 -서울지역 성인을 대상으로-)

  • Recently, as korean eating habits have been westernized, the incidence of large intestinal disease, which has been recognized as western diseases until now, has been increasing. This study was conducted to find the association of fiber intake and large bowel function. This was a cross-sectional survey, the data were obtained by self-administering questionnaire and study population was adults aged more than 18 years old in Seoul(N=1200). The response rate was 81.8%. The results were summarized as followed: 1) The factors associated with bowel function was sex, habits of defecation such as regularity and frequencies of defecation, fecal form, gastrointestinal diseases, psychological scores, self-assessment of physical status, exercise and alcohol drinking. 2) The association between the amount of fiber intake and bowel function was significant. This association was still significant, when the confounders such as disease of upper digestive system and alcohol drinking were controlled.

Functional Status of Stroke Patients among Different Long-Term Care Settings (장기요양서비스 유형별 뇌졸중 환자의 기능수준 비교분석)

  • Purpose: The purpose of this study was to measure the functional status of stroke patients cared for in different long-term care settings. Method: We assessed all stroke patients in two home health care agencies, four nursing homes and one geriatric hospital in Korea (n=171) using the Resident Assessment Instrument (RAI), which comprises Activity of Daily Living (ADL), urine incontinence, bowel incontinence, a Cognitive Performance Scale (CPS),and being understood and understanding others. Data was collected by face-to-face surveys with patients. Results: The mean ADL score, urine incontinence score, bowel incontinence score, CPS, and being understood score and understanding others score were lowest for the patients receiving home health care, and highest for the patients in nursing homes. Low scores described poor and high scores good functional status. The results showed significant differences in physical and cognitive function scores between the three groups of patients. Conclusion: This study suggests that there may be large differences between the patients in these three types of long-term care settings. These findings can be used to help develop and implement efficient long-term care programs.

Effect of Rice Bran Dietary Fiber Level on Serum Lipid Concentration, Bowel Function, and Mineral Absorption in Rats (미강 식이섬유 첨가량이 흰쥐의 혈청지질농도, 장 기능 및 무기질 흡수율에 미치는 영향)

  • This study was performed to investigate the effect of rice bran dietary fiber powder on serum lipid levels, bowel function, and mineral absorption in rats. Four weeks old male Sprague Dawley rats(SD rat) were divided into four groups : control group fed 5% cellulose as a fiber source, RB10 fed 5% of cellulose and 10% of rice bran dietary fiber powder, RB20 and RB30. The animals were fed the experimental diets for 4 weeks. Serum lipid levels were not significantly different among the groups. But, fecal total cholesterol(TC), triglycerides(TG), and high density lipoprotein cholesterol(HDL-c) excretion increased in the RB30 group. Fecal weight and fecal water content were higher in the rice bran added groups than those in the control group. Transit time was significantly shorter in the rice bran fiber-added groups than that in the control. Weight of the stomach and large intestine in the RB20 and RB30 groups were significantly greater than those in the other groups. Absorption rates of Ca, Mg, P, and Zn decreased significantly in the RB30 group compared to those in the other groups. A high amount of rice bran increased fecal lipids, including TC, TG and HDL-c. Rice bran increased fecal weight and fecal water content and shortened gastrointestinal transit time. However, a high level of rice bran diet decreased mineral absorption rates.

Effect of Resistant Starch on the Large Bowel Environment and Plasma Lipid in Rats with Loperamide-Induced Constipation (저항전분 투여가 Loperamide 유도 변비 쥐의 대장환경과 혈청지질에 미치는 효과)

  • The present study was conducted to evaluate the effect of resistant starch (RS) on the large bowel function and plasma lipids in rats with constipation induced by Loperamide. Animals were divided into six groups: normal control-5% cellulose, constipation-5% cellulose, constipation-5% pectin, constipation-5% RS-type 2 (RS2), constipation-8% RS2 and constipation-5% RS type 3 (RS3) groups, and fed experimental diets for five weeks. The results from RS groups were compared with those from other dietary fiber groups. The groups supplemented with RS3 or high level of RS2 showed significantly increased counts of bifidobacteria in the cecum than the other groups. The production of total short chain fatty acids in the cecal contents was significantly high in pectin, RS3 and high RS2 groups. The pH in the cecal contents of the RS supplemented groups was significantly decreased compared with the cellulose supplemented groups. The production of prostaglandin E2 in the colon mucus of the RS groups was higher than the normal group; however, it was significantly decreased compared to the cellulose or pectin supplemented constipated groups. The thickness of the mucus layer and the production of mucus from epithelial cells were significantly increased in RS3 group compared to the constipated cellulose group. Supplementation of resistant starch significantly elevated the ratio of HDL-cholesterol to total cholesterol and significantly lowered plasma atherogenic index compared with cellulose or pectin supplementation in constipated rats. The results of the present study demonstrated that resistant starch supplementation may help in improving the large bowel environment by stimulation of bifidobacterial proliferation, reduction of pH and inflammation factor and by increased production of mucus. It has also been found that an additional health benefit is improvement in lipid levels of serum.

Effect of Insoluble Dietary Fiber Extracted from Salicornia herbacea L. on Large Intestinal Function in Rats (불용성 함초 식이섬유의 섭취가 흰쥐 장기능에 미치는 효과)

  • This study investigated the effect of insoluble dietary fibers extracted from Salicornia herbacea L. (S. herbacea) on the improvement of intestinal function in rats. Sprague-Dawley rats were fed on diet containing 5% and 10% S. herbacea dietary fiber (SHDF) for four weeks. Rats receiving the SHDF diet showed a significant decrease in their triglyceride levels and an increase in HDL-cholesterol levels. In addition, compared with the control group, the SHDF group showed a significant increase in the total quantity of the feces and its moisture content. The intestinal transit time of the feces was also shorter in this group. The pH of the feces decreased in all the other experimental groups. Particularly, the bile acid content of the feces and the thicknesses of the mucus layers showed significant recovery on SHDF intake. These results suggest that dietary fiber isolated from S. herbacea has a marked effect on the improvement of bowel function in rats with loperamide (2 mg/kg)-induced constipation.

Effects of Alginic Acid, Cellulose and Pectin Level on Bowel Function in Rats (알긴산과 셀룰로오스 및 펙틴 수준이 흰쥐의 대장기능에 미치는 영향)

  • The purpose of this article is to know the effects on bowel function of the kind of fiber and the amount of fiber in SD-rats. To do this experiment, we select of $\alpha$-cellulose as n insoluble cellulose source and alginic acid and pectin as soluble cellulose source. The rats diets contained callolose camcentrations of 1.0%, 3.6%, 6.0% and 10.0%. After that, we raised the SD-rats for 4weeks and measured the amount of food intake, body weight, the food effciency ratio, the length of liver and stomach the weight of the intestines, the transit time through the intestines, pH in feces, and the amount of bile acid and Ca, Mg, pp. 1) The amount of food intake was 15.75-31.00g/day. It was highest in the 10.0% cellulose group and the lowest in the 3.6% and 6.0% alginic acid group (p<0.05). The body weights of rats were 277.50-349.809. It was highest in the 1.0% pectin group and lowest in the 3.6% alginic acid group, 6.0% cellulose group, and 10.0% pectin group. It had differences according to the content fiber and the kind of dietary(p<0.01). The food efficiency ratio was (p<0.01). The higher the content of dietary fiber, the lower the calory and the food efficiency ratio. 2) Transit time was 446.0-775.0 minutes and it showed signidicant ifferences according to the content and kind of dietary fiber(p<0.01). It was long in the 1.0% cellulose group and 1.0% pectin group but short in the 10.0% alginic acid group. As the content of dietary fiber increased, the transit time through the intestines was shortened. The length of small intestine was 101.03-120.40cm and there were no difference cegardloss of the content and kind of fiber. The length of the large intestine was 20.92-25.42cm and there were significant differences according to the content and kind of the fiber. High-fiber diets resulted in increases in the length of the large intestine. 3) The weight of the liver was 8.68-10.96g and there were no differences according to the content and kind of fiber. The weight of stomach was 1.28-1.74g and there were no differences resulting from the kind of dietary fiber, but it was highest in the 10.0% alginic acid group. The weight of the small intestine was 5.52-8.04g with no difference resulting from to the kind of fiber. It was highest in the 10.0% the alginic acid group and lowest in the 1.0% alginic acid group(p<0.05). The weight of large intestine was 2.50-3.30g with no differences related to the kind of dietary fiber. It was heaviest in the 6.0% and 10.0% alginic acid groups and in the 10.0% pectin group with differences related to the content of fiber(p<0.05). 4) The pH of the feces was 5.82-6.86 according to the kind of dietary fiber, alginic acid group was high at 6.66, the cellulose group was 6.26. but the pectin group was low at 6.30. There were difference according to the content of fiber, but no consistency. The content of bile acid was 6.25-34.77umol per 1g of dry feces. According to the kind of dietary fiber, the alginic acid group was low at 12.91umol, cellulose group was 18.64umol and, the pectin group was the highest at 27.78umol(p<0.001). Based on the content of dietary fiber, alginic acid group was low at 1.0%, but high at 3.6% pectin group(p<0.001). 5) The amount of feces was 1.00-5.10g/day. The weight of rat feces was 2.23g/day in the alginic acid goup, 2.75g/day in the cellulose group, and 1.82g/day in the pectin group. According to the content of fiber, cellulose group was high at 10.0% but alginic acid group was 1.0%, and there were significant difference according to the dietary fiber. The more the content of fiber, the more increase the content of feces in alginic acid, cellulose and pectin group. The content of Ca in the feces was 80.10-207.82mg/1g of dry feces. In the dietary fiber, alginic acid group was 193.08mg, cellulose group was 87.5mg, pectin group was 138.16mg. In the content of fiber, alginic acid group was high at 1.0% and 3.6% but low at 10.0% of Pectin group. The content of Mg was 19.15-44.72mg/1g of dry feces. According to the kind of dietary fiber, alginic acid group was 35.33mg, cellulose group was 23.60mg, and pectin was 36.93mg. According to the content of fiber, pectin group was high at 1.0% and low at 10.0% of cellulose group. The content of P was 1.65-4.65mg/1g of dry feces. According to the kind of dietary fiber, alginic acid group 2.23mg/g dry feces, cellulose group was 2.29mg/g, pectin group wa 4.08mg/g dry feces. In the content of fiber, pectin group was high at 6.0% and low at 6.0% alginic acid group, but there were significant difference among the analysis value. The conetnt of Ca and MG was higher in soluble alginic acid group and pectin group than in insoluble cellulose group. The high the content of the dietary fiber, the lower the food efficiency ratio and the short the transit time through intestine with the increase of the length of large intestin as well as the higher level of the stomach, the small intestine and the large intestine. According to the content of the dietary fiber, the amount of the feces, Ca, Mg and P was increased but the length the small intestin, the weight of liver, pH of the feces and the amount of bile acid showed no differences and consistency.

Giant Right Atrial Thrombi Formation Associated with Hemodialysis Catheter Placement in a Dog

  • A Cocker spaniel (7-year-old, female) was presented with one week of anorexia, halitosis, oral ulcer, intermittent vomiting, acute weight loss and 3-day history of oliguria. The patient was diagnosed with acute on chronic kidney disease and pancreatitis. Hemodialysis was continued three times a week (total 7 sessions) with improved clinical signs and kidney panel, but presented with another episode of abdominal distention, respiratory distress, and large bowel diarrhea. Echocardiography revealed nearly totally occlusive thrombus in the cranial vena cava, as well as a right atrial mass of approximately $2cm{\times}1cm$. The patient was treated with catheter removal, thrombolysis and anticoagulation therapy with recombinant tissue plasminogen activator. During anticoagulation therapy, size of atrial thrombus was not changed and heart function was not improved after treatment. Since clinical signs were deteriorated, the patient was euthanized by owner's request. Catheter-related giant thrombus in right atrium is a rare complication and treatment guideline for atrial thrombus does not exist. This case is first report of hemodialysis catheter-induced thrombus in Korea.

Effects of Indigestible Dextrin on Bowel Function and Serum Lipid in Rats (난소화성 덱스트린이 횐쥐의 장기능 및 혈청 지질에 미치는 영향)

  • The present study was undertaken to evaluate the effect of indigestible dextrin on bowel function and serum lipid. Recently developed indigestible dextrin is water soluble dietary fiber obtained by heating and enzyme-treatment of potato starch with low viscosity. SD male rats weighing 80~85g were divided into three groups; control diet(IC), control +5% pectin(IP), control +5% indigestible dextin(ID). Experimental rats were fed one of the experimental diets ad libitum for six weeks. Weight gain, food intake, and food efficiency ratio were not significantly different among other groups. Those of ID tended to be higher than in IP and IC. Weights of liver, spleen and kidney were not significantly different among groups. Epididymal fat pads weight in W and ID tended to be lower than in IC. The weight of cecum was sinificantly higher in m than in other groups(p<0.05). The weight and length of large intestine tended to be higher in IP and ID than in IC. Total feces excretion of ID and If tended to be more than that of IC. Transit time was shorter in IP than in other groups. Serum total lipid, total cholesterol and TG levels were higher in IC group than other groups. The fecal excretion of lipid was significantly higher in IP than in other groups(p<0.05). That of ID tended to be more than that of IC, so fat apparent absorption of ID was lower than that of IC. Ca absorption were decreased more in IP than in other groups(p<0.05).

A Study of the Evaluation of the Spinal Cord Injuries (척수 손상 환자에 관한 실태 조사)

  • The incidence of spinal cord injury increase due to traffic accident, industrial accident and leisure sports. Spinal cord injury damages motor and sensory function below the injury level, also affects autonomic functions associated with voiding and defecation. Sexual dysfunction and psychosocial, vocational maladaptations are also some of the unwanted consequences of injury. The purpose of this study is look for means to prevent and to manage complications in spinal cord injury through investigation and analysis. The subjects of this study in spinal cord injured patients were admitted to the department of physical therapy, Kwangju christian Hospital, Nam Kwang Hospital, Chun Nam university Hospital and Cho Sun university Hospital, from April, 1, 1995 to March, 31, 1996. The results are as follows: 1. The subjects comprised 96 cases of spinal cord injury, ranging from 17 to 85(mean-40.8 yrs) and included 72 males and 24 females. Among these patients, 58 were cervical injury, 20 were thoracic injury and 18 were lumbar injury. 2. As for a major causative of spinal cord injury were traffic accident(59.4%), fall down (27.1 %), and motocycle(4.2%).. 3. The bladder control were taken by indwelling cathetar(41.7%), Crede maneuver(37.5%) and self voiding(16.7%). The bowel control were taken by all aid(61.5%), assitance(32.3%) and self defecation(6.2%). 4. Possible of sexual function were 35 cases (47.9%). 5. The device of transfer used wheel chair(69.8%) and bed(16.7%). 6. The patients with higher cord lesion got more serious pain than lower cord lesion. Also the patients with higher cord lesion got a serious spasticity. 7. The incidence of decubitus ulcer among 96 patients were in case 46(47.9%). The largest group of the pressure sore sites were sacral portion(82.0%), less than 1 month of onset occured a large numbers(50%). Incidence of pressure sore by spasticity occured many patients in case of mild or moderate. Incidence of pressure sore by pain occured many patients in case of severe pain.

FREE JEJUNAL FLAP FOR INTRAORAL RECONSTRUCTION CASES REPORT & LITERATURE REVIEW (유리 소장이식을 이용한 구강내 결손부위의 수복 증례보고 및 문헌 고찰)

  • Large oral defects following tumor resection pose formidable challenge for the reconstructive surgeon. Ideally, wound closure should utilize like tissue in providing expedient, single-stage closure, returning maximum function while minimizing deformity. Recent methods have reported and utilize variable mucocutaneous flaps. However, the ideal reconstruction has yet to defined. The small bowel serves as a readily available donor site for satisfying reconstructive needs in oropharyngeoesophageal defects. Segments of jejunum may be opened along the antimesenteric border and transferred to oral defect as free tissue transfers. Some of the benefits of this technique have included a one-stage procedure, abundant donor tissue with characteristics similar to oral mucosa, near normal facial appearance, preservation of maximum tongue function and relief of annoying xerostomia by jejunal mucous secretion. Three cases re presented in which two cases show successful use of this flap. The other one patient developed total necrosis of this flap. We report cases of reconstruction using free jejunal flap transfer in oral reconstruction.
