  • Both pelvic pain associated with cancer and chronic benign conditions may be alleviated by blocking the superior hypogastric plexus. Traditional bilateral placement of needles is the appropriate technique for the patient with pelvic pain due to the bilateral distribution of the disease as well as allowing for individual difference of pelvic cavity. However the technique is a very difficult procedure and painful for the paient. However we have performed transdiscal superior hypogastric plexus neurolysis free of complication.

  • The pain clinic at the Chonbuk National University Hospital opened in 1983. We analyzed 3,212 patients who visited the clinic during the 10 years from 1983 to 1993. The patients visited total of 11,628 times. The most frequent age group was in the fifties. About an half of the patients complained of low back pain, and the epidural block was most frequently given as a treatment. We need more personnels involving in pain management and more equipments in diagnosing and treating the patients with chronic pain. Understanding and cooperation among departments under the basis of closer interdepartmental relationship would be essential for the further development of the pain clinc.

  • The number of clinics for Oriental Clinic is increasing sharply in Korea after the inception of National Health Insurance System in 1987. However there have been many troubles in making efficient plans for oriental clinic facilities because of the lack of proper guidelines related to the design of them. This research was conducted as part of the planning project for the "G" Oriental Clinic. The purpose of this project Is to place a optimal number of beds in a small oriental clinic and medical-environmental friendly design After arranging necessary spaces, such as the Director's Office, Herbal Medicine Preparation Room, Multipurpose Room, Medicine Cabinet, and Waiting Room in the clinic, partitions are used to differentiate the spaces and the storage area.

Medical Accessibility and Its Effects on Medical Care Utilization -Experiences from Yonsei Health Insurance Cooperatives- (의료근접도 및 용이성이 의료이용에 미치는 영향 -연세건강공제회원의 외래의료이용을 중심으로-)

  • Accessibility to medical facilities and personnels has been known as one of important determinants of medical care utilization. This study attempted to identify the effects of medical accessibility in terms of geographical distance and occupational opportunity to the medical utilizations. Two-year-experiences of Yonsei University Health Insurance Cooperatives were used as the sources of data. Out patient utilization patterns of 713 members sampled from 4,352 members of Health Insurance Cooperatives were analyzed in order to identify the effects of medical accessibilities. Findings: 1 Average clinic visit rate of Yonsei Health Insurance is 1.66 per person per year. 2. The utilization rates of geographically more accessible group were 33% higher than that of less accessible group. 3. No marked difference in clinic visit rate were observed between medical and non-medical personnel and their family members. 4. Clinic visit rates among occupationally accessible group were slightly higher than those of less accessible. The utilization rate was more sensitively changed by the insurance policy changes in occupationally accessible group.

Length of Stay, Health Care Cost, Postpartum Discomfort, and Satisfaction with Medical Service in Puerperas Giving Birth in Midwifery Clinic and Hospitals (조산원과 병원 분만 산모의 재원일 수, 의료비용, 산후불편감과 의료서비스 만족도 비교)

  • Purpose: To determine traits related to pregnancy and delivery, length of stay, health care cost, postpartum discomfort, and satisfaction with medical service of puerperas giving birth in midwifery clinic and hospitals. Methods: This study used a comparative survey design. Data were collected from a total of 140 postpartum mothers composed of 70 mothers who gave births in two hospitals and another 70 mothers who delivered in one midwifery clinic. Results: Delivery in midwifery clinic had higher Apgar score at 1 minute and 5 minutes after birth than hospital. Those who delivered in midwifery clinic had shorter stay in the clinic, fewer health care cost, less postpartum discomfort in physical, environmental, social, and cultural areas, higher satisfaction with medical services than those who delivered in hospitals. Conclusion: Results of this study can be used as a basis for studies on giving birth in midwifery clinic and hospitals. They might increase the autonomy of women in giving birth with positive effect on the delivery experience of the mother and her spouse.

  • The aim of this study was to show the clinical aspects in outpatients who visited Oriental Neuropsychiatry Clinic. Method : We studied 419 patients who visited Oriental Neuropsychiatry Clinic of Dong-Sea Medical Center from march, 2007 to August, 2007. Results & Conclusion : 1. The ratio of male to female patients was 1:2 and age distribution was most frequent in 50s. 2. From onset, 32% of patients visited Oriental Neuropsychiatry Clinic in a week, 58% of patients visited in a month. 3. Oriental Neuropsychiatry diseases were 80%, Non-Oriental Neuropsychiatry diseases were 20%. The most common Oriental Neuropsychiatry diseases were Somatoform Disorders. 4. The most common diagnosises of the Oriental Neuropsychiatry diseases were Psychosomatic diseases. 5. The most common cheif corn1ains were Headache. 6. The most commonly used herbal medicine was Gamiondam-tang and Guibi-tang

Distribution of Private Medical Practitioners' Income from Medical Insurance and its determinants (의원의 의료보험진료비 수입분포와 그 결정요인)

  • This study was conducted to investigate the distribution of private medical practitioners' income from the medical insurance and its determinants. Total amount of the medical service fee paid by the medical insurance to 1,268 private clinics(767 in Taegu and 510 in Kyungpook that had been in practice at least for one year) in 1993 was compared by the characteristics of practitioner, clinic, patient and population. The practitioners in 40-49 years of age and 6-10 years inpractice had the highest income. Total income of a clinic was increased with the number of physicians, employees and equipments. The largest income differentials were observed among obstetrics and gynecology clinics and the least differentials were among pediatrics clinics. The characteristics of practitioner, clinic and population accounted for 41.7% of the total variance of income. The important determinants of income were specialty of the clinic, age of the practitioner and number of the employee and equipments. The large income differentials among clinics imply a skewed distribution of patients and thus long waiting time, inefficient utilization of manpower and inadequate quality of care. Effective measures to reduce the income differentials need to be developed.

A Peculiar Case of Ischemic Fasciitis Appeared on a Pressure Ulcer after 10 Days of Negative Pressure Wound Therapy

  • In this article, we reported a single case of ischemic fasciitis in a young woman with a progressive immobilization due to a multifocal demyelinating disease of central nervous system, which appeared on an extensive pressure ulcer of the sacral region treated with 10 days of negative-pressure wound therapy (NPWT). Wound examination revealed a significant nontender brown neoformation (9 cm in length × 10 cm in width × 7 cm in height), fixed to the sacrum, presenting hard consistency, and grown in the central portion of the sacral pressure sore. The histologic examination showed central fibrinoid necrosis, and vascular and atypical fibroblastic proliferations, and a diagnosis of ischemic fasciitis was made. Ischemic fasciitis is a rare benign proliferation of atypical fibroblasts that occurs in physically weak patients with reduced mobility. In the literature, the relationship between the use of NPWT on pressure ulcers and the development of ischemic fasciitis is, to the best of our knowledge, not described yet.
