Background: Minimally invasive surgery of pectus excavatum by Dr. Nuss is a new technique that allows the repair of this deformity without any cartilage resection or sternal osteotomy We describe the early experiences with Nuss procedure. Material and Method: From December 1999 to January 2001, twenty patients with pectus excavatum underwent repair by Nuss procedure There were 14 males and 6 females whose mean age was 10.1$\pm$7.7 years, ranging from 1 to 33 years. Most patients(N=19) were below 20 years, except 33 years old female patient(N=1). Result: The severity of depression was assessed by computed topography(CT). CT index was mean 4.9$\pm$5.7(ranged from 3.3 to 8). The average operating time was 85.8$\pm$23.7 minutes. The used metal bars were ranged in length from 8 inches to 16 inches(average 11.8$\pm$14.4 inches). Early postoperative complications were pneumothorax in three patients, paralytic ileus in one, and postoperative chest pain requiring analgesics in all patients. Epidural analgesia was used in one adult patient for control of postoperative pain. In our experiences, there were no serious complications posteoperatively. Conclusion: There were good early results with the Nuss procedure that we performed for repairing of pectus excavatum. However, we believe the procedure needs to be observed for the long term results for it to be broadly accepted.

Background : Ventricular assist devices(VADs) are being used for patients in postcvardiotomy cardiogenic shock status bridge to cardiac transplant settings and in post-myocardial infarction cardiogenic shock. The VAD which was developed at the Deparment of medical engineering in Dankook University College of Medicine was a pneumatically driven device and can maintain pulsatile flow. The goal of this study is to develop animal experimental models using the VAD and to clarify the reliability and hemodynamic property adequacy of end organ perfusion durability and severity of thrombotic-hemolytic tendency of the device. Material and Method : The pneumatic VAD was applied to 8 adult female lambs, We examined some hemodynamic parameters such as arterial blood pressure pulmonary capillary wedge pressure(pcwp) pulmonary artery pressure(PAP) left atrial pressure hour urine output cardiac index VAD flow EKG to determine the reliability of the VAD and hemodynamic compatibility of the experimental animals within 24 hours of experiment. We also observed the end organ perfusion durability of the VAD and thrombotic-hemolytic property of the VAD after 24 hours of VAD insertion. Result: We could monitor all hemodynamic parameters including pcwp PAP cardiac index EKG, adn hour urine as true clinical settings. We observed that the reliability of the VAD was excellent and the hemodynamic property of the experimental animal and end organ perfusion were adequate within 24 hours of experiment. In four lambs surviving 24 hours after insertion the reliability of the VAD and end organ perfusion were excellent and no thrombotic-hemolytic tendency was noted. However after 15 days of experiment the diaphragm of the VAD was torn and it was recommende that the durability of the VAD should be extended. Conclusion : e conclude that the pneumatic VAD developed at Dankook University Biomedical Engineering has good hemodynamic property and low thromboembolic tendency and presents adequate end organ perfusion but we noted that the durability of the device should be expanded further. It will be possible to do more reliable experiment in the future according to the animal experimental method developed in this study especially with the heart failure models.
From October, 1987 to December, 1995, nine patients underwent total correction of tetralogy of Fallot in adults over 20 years of age. There were 5 male and 4 female patients aged from 22 to 42 years(mean, 29.6 years). Three patients were in New York Heart Association(NYHA) functional class II, and 6 patients in class III. The hemoglobin values ranged from 10.8 to 20.7 gm/㎗ (average, l5.6gm/㎗). The preoperative clinical features were as follows: cyanosis, 8 patients; dyspnea on exertion, 6 patients; clubbing of fingers, 5 patients; frequent upper respiratory infection, 3 patients. At the operation both infundibular and valvular stenosis were present in all patients. Reconstruction of right ventricular outflow tract(RVOT) using Goretex was required in 7 patients, and transannular patching with Goretex in 2 pateints. Left pulmonary angioplasty with pericardium was done in 2 patients. No hospital deaths occurred. Four of 9 patients(44.4%) had postoperative low cardiac output syndrome, and postoperative bleeding in 5. One patient required reoperation due to residual ventricular septal defect and tricuspid regurgitation 3 months after the first operation. The mean follow-up period was 25 months, range 11 to 77 months. All was asymptomatic and in NYHA class I. We suggest that advanced age is not contraindication to surgery in tetralogy of Fallot, and tetralogy of Fallot in adults could be operated on due to low mortality.
This study investigate the epidemiological feature of Metagonimus infection in Kangwon-do (province). The average Infection rate of the surveyed inhabitants was 7.8% (83 positives out of 1, 067 examinees) by stool examination; male, 11.4% and female, 3.2%, respectively. The egg positive rate in residents in the Som river area was 7.3%, that of the Chuchon river area 6.3%, the Pyongchang river area 12.8%, the Tong river area 3.8%, the Hongchon river area 9.8%, and the Ohsip stream area of Samchok 8.0%,respectively. The average metacercarial infection rate of genus Metagonimus in the fish was 81.0% (256 positives out of 318 fish). The infected fleshes were Zacco platypus. Zacco teminki, Opswiichthys biens, Squdidis sp., Corqssius carassius, etc. in western Kangwon-do Meanwhile, in the Ohsip stream area of Samchok-gun, eastern costal Kangwon-do, the infected fish were Plecoglossus altivelis and Tribolodon hokonensis. The rats and dogs are infected with the metcercanae obtained from Zacco platypus and Opsariichthys biens, adult worms collected were Miyata type of Metagonimus with some M. takahashii. When infected with metacercariae from Plecoglossus ltivelis, Metagonimus yokogowai was only found. M. yoogawai and Metagonimus Mlyata type were fecund together in Tribolodon hakonenis in Ohsip stream area of Samchok, in the eastern Kangwon-do. The intestinal flukes of genus Metogonimus in western Kangwon-do were Miyata type of MetQnonimuT and M. takahashii transmitting mainly by Zacco platypus and Opsariichtys bidens as a source of infection. In the eastern part of Kangwon province (Ohsip stream area of Samchok), M. yokogowai was mainly distributed by P. altivelis and T. hakonesis, but some T. hakonensis harbored the metacercariae of Miyata type of Metagonimus with those of M. yokogawai.
Background: Despite the favorable geo-climatic potential of Cameroon, the national production of tilapia remains low due to poor tilapia growth reported by fish farmers. One of the underlying reasons is the early female maturation at a very small size and precocious breeding in earthen ponds, resulting in overpopulation which leads to stunted growth and therefore to the production of unmarketable fish size. Studies have shown that dietary supplementation of G. kola enhanced growth in young Clarias gariepinus and Oreochromis niloticus. It was also reported that G. kola inhibited spawning in Tilapia adult females. Therefore, this study sought to assess the effects of Garcinia kola as growth promoter and inhibitor of gonadal development in young Oreochromis niloticus. Methods: A total of 108 juveniles weighing $13.32{\pm}0.62g$ were randomly distributed in 9 hapas of 12 fishes each (9 females and 3 males) and fed for 70 days with three isonitrogenous diets, 40% crude protein with increasing Garcinia kola supplementation levels of 0 (normal diet), 6% and 10% (experimental diets). Physico-chemical parameters of the water (temperature, dissolved oxygen, pH, nitrate, nitrite, ammonia, and transparency) were measured twice a week. Every 14 days, fish were harvested, counted, and weighed. At the end of the experiment, three fish of each sex per replicate were sacrificed and their gonad and liver collected and weighed. Data were statistically analyzed using one-way analysis of variance repeated measure followed by Newman-Keuls multiple tests. Results: The results showed that all physico-chemical parameters of the water were within the recommended values for Tilapia culture. Tilapia fed 6% Garcinia kola supplemented diet displayed higher final body weight in males ($38.60{\pm}3.50g$) and females ($36.77{\pm}3.62g$) compared to those receiving normal diet ($36.23{\pm}1.36g$ and $25.87{\pm}3.32g$; respectively to the final body weight in males and females). The gonadosomatic index and hepatosomatic index indicated no significant variation in males while in females, these were significantly low in the experimental fish compared to control fish. Conclusion: The results of this study demonstrated that supplementation of G. kola seeds in diets of young Tilapia improved growth performance and impaired gonadal development in females.
We investigated to elucidate the early life history of siberian stone loach, Barbatula nuda (Pisces: Nemacheilidae). Adult fish were collected using net in the Sangcheoncheon Stream, Cheongpyeong-myeon, Gapyeong-gun, Gyeonggi-do, Korea, 26th April 2020. HCG 10 IU/g was injected to female and male fish and fertilized by dry method in the laboratory. The number of eggs of B. nuda was 1,308±293 (TL: 70.64~79.24 mm), and the egg color was yellowish grey. Fertilized eggs measured 1.13±0.01 mm (n=10) in diameter and showed adhesion. The fertilized eggs started hatching after 104 hours. At the time of egg development, the 15℃ group showed an average hatching rate of 80.1±1.2% and the 20℃ group showed 61.5±6.8%, so the 15℃ group was suitable (p<0.05). Newly hatched larva an average were 3.12±0.06 mm (n=10) in total length. At 4 days after hatching, the total length was 4.89±0.09 mm (n=10), and feeding began with the mouth and anus opened, and entered to preflexion larvae stage. At 21 days after hatching, the total length was 10.98±0.40 mm (n=10), and the distal part of the notochord was bent to 45°, and entered to postflexion larvae stage At 26 days after hatching, the total length was 13.47±0.37 mm (n=10), and entered the juvenile stage.

Background: Extracorporeal membrane oxygenation (ECMO) has been widely used in patients with cardiorespiratory failure. The serum albumin level is an important prognostic marker in critically ill patients. We evaluated the efficacy of using pre-ECMO serum albumin levels to predict 30-day mortality in patients with cardiogenic shock (CS) who underwent venoarterial (VA) ECMO. Methods: We reviewed the medical records of 114 adult patients who underwent VA-ECMO between March 2021 and September 2022. The patients were divided into survivors and non-survivors. Clinical data before and during ECMO were compared. Results: Patients' mean age was 67.8±13.6 years, and 36 (31.6%) were female. The proportion of survival to discharge was 48.6% (n=56). Cox regression analysis showed that the pre-ECMO albumin level independently predicted 30-day mortality (hazard ratio, 0.25; 95% confidence interval [CI], 0.11-0.59; p=0.002). The area under the receiver operating characteristic curve of albumin levels (pre-ECMO) was 0.73 (standard error [SE], 0.05; 95% CI, 0.63-0.81; p<0.001; cut-off value=3.4 g/dL). Kaplan-Meier survival analysis showed that the cumulative 30-day mortality was significantly higher in patients with a pre-ECMO albumin level ≤3.4 g/dL than in those with a level >3.4 g/dL (68.9% vs. 23.8%, p<0.001). As the adjusted amount of albumin infused increased, the possibility of 30-day mortality also increased (coefficient=0.140; SE, 0.037; p<0.001). Conclusion: Hypoalbuminemia during ECMO was associated with higher mortality, even with higher amounts of albumin replacement, in patients with CS who underwent VA-ECMO. Further studies are needed to predict the timing of albumin replacement during ECMO.
The purpose of this study was to compare the health status, functional status, and home environment hazards by the fear of falling and explore the factors that contribute to the fear of falling. This was a descriptive study. Data were collected for two years with 973 older adults who enrolled in a health center at A city. Trained nurses visited subjects' home and collected data using Resident Assessment Instrument MDS-HC. The average age of the subjects was 76.8 (±7.6) and female was 74.8%. 20.7% of subjects reported fear of falling. In terms of the fear of falling, two groups were compared. There were significant differences in vision, urinary incontinence, pain that interrupts daily activity, unstable gait, comorbidities, frequency of depressive or anxious sign. The greater the age, IADL and ADL dependency, comorbidities, unstabled gait, the more fear of falling. In conclusion, this study reassured the importance of assessing the major factors related to fear of falling such as age, IADL and ADL, comorbities, gait among older adults. This study will contribute to establish strategies for preventing fall. Reducing the fear of falling through the comprehensive nursing intervention will ultimately improve the quality of life in the elderly.
The purpose of this study was to understand socio-demographic factors related to older adults' participation patterns in lifelong education. For the purpose, this study used the raw data of 2017 Survey of the Living Conditions of the Elderly (SLCE) conducted by The Korea Institute for Health and Social Affairs. From the data of 10,073 older adults, their lifelong education participation, participating program types, participating organizations, and participating frequency were analyzed by their sex, age, educational level, household income, the longest job status, and health status. This study found that female, age of 70-74 and 75-79, educational levels of high school and higher, the longest job status of regular employees and unpaid family workers, and decent health status of older adults more participated in lifelong education. According to lifelong education program types, significant differences were found between education groups of middle school/lower and groups of high school/higher and between 1, 2 quintile income groups and 3, 4, 5 quintile income groups. In relation to the participating organizations, groups of 70 years and older, middle school and higher education level, under 3 quintile income, and poor health tended to participate in lifelong education at the elderly welfare center, senior citizens, and elderly classrooms. In terms of participation frequency, high school and college/higher than 0 year of school education, and regular workers than unpaid family workers were more frequently participated in lifelong education. This study showed the inequality in lifelong education participation according to older adults' demographic characteristics; finally, this study suggested necessary policies and academic discussions for future older adults' lifelong education.

Purpose: This study was designed to investigate the fitness of adjustable dental impression trays on the Caucasian and African American. Material and methods: The size and shape of these trays were designed from the results of the dental arch size of Korean adults. Tray samples were made by CAD-CAM working. Sixty Caucasian (male:30, female:30) and sixty African American (male:30, female:30) were selected for taking irreversible hydrocolloid impression using these trays. The author measured the width and length of impression material on the several measuring points. Results: 1. Uniform impression material width was achieved by controling the width of the tray using stops and beveled guides. 2. In the maxillary tray on the Caucasian, the impression material thickness was measured to be rather great showing thickness of the midpalatal part 13.0 mm. 3. In the maxillary tray on the African American, the impression material thickness was measured to be rather great showing thickness of the midpalatal part 12.0 mm, posterior palatal part 11.0 mm and the labial frenum width was 11.0 mm. 4. In the maxillary tray on the African American, the impression material width of posterior border (0.8 mm) was measured to be small. 5. In the mandibular tray on the Caucasian, the impression material width was measured (2.7-6.7 mm) and posterior border width (2.1 mm) was measured small. The impression material length was measured (2.8-6.7 mm). 6. In the mandibular tray on the African American, the impression material width was measured to be rather great showing width of the labial frenum 9.2 mm and the width of posterior border was measured too small (0.3 mm). Conclusion: This adjustable dental tray shows good accuracy to Korean because it was designed by the analysis of the dental arch size of Korean adult model. With this result, it can be applied to Caucasian and African American, we can take more easy and accurate dental impressions.
