Objectives This study was aimed to evaluate the effects of Daeyoungjeon (hereinafter referred to DYJ) treatment on the injury of articular cartilage induced by monosodium iodoacetate in rats. Methods Twenty-four male rats were divided into normal, osteoarthritic control and DYJ group. Rats of normal group were injected with 0.1 ml physiological saline, rats of control and DYJ groups were injected with 0.1 ml monosodium iodoacetate (3 mg/ml) into each left and right knee joint cavities. Rats of DYJ group were administrated extracts of DYJ during 60 days per orally. At 60 days after treatment, gross lesions, area and proteoglycan contents of articular cartilage, histopathological lesions, immunohistochemistry on matrix metalloproteinases (MMP-2, MMP-3, MMP-7) were evaluated. Results Grossly, degenerative changes of articular cartilages were observed weak in DYJ group. The areas of articular cartilages were broader significantly in DYJ group. The proteoglycan contents in articular cartilages were lesser significantly in DYJ group. Histopathologically, the chondrocyte score was lesser significantly in DYJ group. MMP-3 expression in articular cartilages was observed weak in DYJ group. Conclusions From above results, DYJ treatment has inhibitory effects on the injuries of articular cartilage induced by monosodium iodoacetate in rats, and it's effects may be related with down regulation of MMP-3.
This investigation has been conducted based on the medical chart of 344 patients who have been diagnosed of fibromyalgia syndrome during Oct. 9, 1996 through Nov. 20 at the Rheumatism Hospital of H. University located in Seoul. 280 which have been included in the analysis. 1. Only one patient was male in 280 patients, age distribution was from 28 to 76, in which the average age was 52.4. 2. Percentage of treatment duration was 6 months in 46.1%, 3 years in 22.1% and 2 years in 12.5%. Considering these results, it can be predicted that the number of patients might be increased and accumulated in the future. 3. Percentage of patients having primary fibromyalgia syndrome was 39.3%, having combination with osteoarthritis was 36.7% and the rest case have combination with rheumatoid arthritis at the same time. 4. The percentage of cases having patients 10-12 tender points was 37.1%, while the most of cases have pain at 12-19 tender points. The common locations of the tender point were at lateral epicondyle of elbow in 92.0%, at midpoint of upper border Trapezius in 84.8%, at upper part of scapula Supraspinatus in 82.9%, at medial fat pad proximal to the joint line knee in 81.85%, at intertransverse of $C_{5-7}$ Low cervical in 73.4% and at 2nd distal costochondral Junction 2nd rib in 72.0%. And most of the patients had joint functional disability at all in 47.1% with average 2.41 joint functional disability. 5. Age was not a variable influencing the number of tender points and the number of joint functional disability.
Objectives The aim of this study is to observe the effectiveness of complex Korean medicine treatment applied to the patients with degenerative meniscal tear and the correlation among clinical effectiveness, body mass index (BMI) and Kellgren-Lawrence grade (KL-grade). Methods The study participants were 38 patients who had been diagnosed with degenerative meniscal tear. Participants were classified by BMI, KL-grade and treated with acupuncture, electroacupuncture and pharmacopuncture. Clinical outcomes were assessed using Numeric Rating Scale (NRS), Western Ontario and McMaster Universities Arthritis Index (WOMAC Index) and EuroQol-5 Dimension Index (EQ-5D Index). Results Both NRS and WOMAC scores were significantly reduced after treatment (p<0.001). The EQ-5D for assessing quality of life showed further improvement (p<0.05). A statistically significant correlation was observed between the BMI and NRS, EQ-5D. KL-grade was correlated with WOMAC. Conclusions These results show that complex Korean medicine treatment to the patient with degenerative meniscal tear may be effective as a conservative therapy. Further research is required to confirm the effectiveness of Korean medicine treatment.

Background: The chondrogenic differentiation of mesenchymal stem cells (MSCs) is regulated by many factors, including oxygen tensions, growth factors, and cytokines. Evidences have suggested that low oxygen tension seems to be an important regulatory factor in the proliferation and chondrogenic differentiation in various MSCs. Recent studies report that synovium-derived mesenchymal stem cells (SDSCs) are a potential source of stem cells for the repair of articular cartilage defects. But, the effect of low oxygen tension on the proliferation and chondrogenic differentiation in SDSCs has not characterized. In this study, we investigated the effects of hypoxia on proliferation and chondrogenesis in SDSCs. Method: SDSCs were isolated from patients with osteoarthritis at total knee replacement. To determine the effect of oxygen tension on proliferation and colony-forming characteristics of SDSCs, A colony-forming unit (CFU) assay and cell counting-based proliferation assay were performed under normoxic (21% oxygen) or hypoxic (5% oxygen). For in vitro chondrogenic differentiation, SDSCs were concentrated to form pellets and subjected to conditions appropriate for chondrogenic differentiation under normoxia and hypoxia, followed by the analysis for the expression of genes and proteins of chondrogenesis. qRT-PCR, histological assay, and glycosoaminoglycan assays were determined to assess chondrogenesis. Results: Low oxygen condition significantly increased proliferation and colony-forming characteristics of SDSCs compared to that of SDSCs under normoxic culture. Similar pellet size and weight were found for chondrogensis period under hypoxia and normoxia condition. The mRNA expression of types II collagen, aggrecan, and the transcription factor SOX9 was increased under hypoxia condition. Histological sections stained with Safranin-O demonstrated that hypoxic conditions had increased proteoglycan synthesis. Immunohistochemistry for types II collagen demonstrated that hypoxic culture of SDSCs increased type II collagen expression. In addition, GAG deposition was significantly higher in hypoxia compared with normoxia at 21 days of differentiation. Conclusion: These findings show that hypoxia condition has an important role in regulating the synthesis ECM matrix by SDSCs as they undergo chondrogenesis. This has important implications for cartilage tissue engineering applications of SDSCs.
Objective: The aim of this study was to investigate the differences in spatiotemporal gait performance, function, and pain of lower-extremity according to foot morphological characteristics. Method: This case-control study recruited 42 adults and they were classified into 3 groups according to foot morphology using navicular-drop test: pronated (≥ 10 mm), normal (5~9 mm), and supinated (≤ 4 mm) feet. Spatiotemporal gait analysis and questionnaires including Foot and Ankle Ability Measure activities of daily living / Sports, Western Ontario and McMasters Universities Osteoarthritis Index, Lower Extremity Functional Scale, International Physical Activity Questionnaire, and Tegner activity score were conducted. One-way analysis of variance was used for statistical analysis. Results: The pronated feet group showed longer loading response and double limb support in both feet and increased pre-swing phase in non-dominant feet. The supinated feet group demonstrated a longer swing phase in non-dominant feet and single limb support in dominant feet. However, there was no significant group difference in function and pain of knee joint and lower-extremity between groups. Conclusion: Our results indicated that abnormal spatiotemporal gait performance according to foot morphology. Although there was no difference in lower extremity dysfunction and pain according to the difference in foot morphology, they have the possibility of symptom occurs as a result of continuous participation in activities of daily living and sports. Therefore, individuals with pronated or supinated foot should be supplemented by utilizing an orthosis or training to restore normal gait performance.
목적: 퇴행성 슬관절염 환자에서 관절경을 이용한 미세 천공술 후 재생된 연골의 임상적, 방사선학적 그리고 조직병리학적 결과를 분석하고자 한다. 대상 및 방법: 1997년 10월부터 1998년 12월까지 관절경을 이용한 미세 천공술로 치료한 퇴행성 슬관절염 환자 46명, 48례를 대상으로 하였으며, 평균 연령은 56세이었고 평균 추시기간은 1년이었다. 이차관절경술은 수술 후 6개월에 22명, 24례에서 시행하였다. 임상적 평가는 최종추시 시에 Baumgaertner의 슬관절 기능 평가 방법에 의해 시행하였다. 이차관절경술을 시행한 24례에서 일반 조직학적 검사를 시행하였고 제2형 교원질의 존재를 확인하기 위하여 18례에서는 면역 조직화학적 검사를 시행하였으며 12례에서는 Western blotting test를 시행하였다. 정량화된 제2형 교원질의 양에 따라 세군으로 나누고 임상적, 방사선학적, 이차관절경 소견, 환자의 나이 그리고 체중과의 상관관계를 분석하였다. 결과: 임상적 결과는 43례(90%)에서 우수, 5례(10%)에서 양호의 결과를 보였다. 이차관절경술을 시행한 24례중 21례에서 연골결손 부위의 80%이상이 재생된 연골로 덮혀 있는 것을 확인할 수 있었으며, 재생된 연골은 조직학적으로 초자연골과 섬유연골로 이루어진 혼합형 연골이었다. 면역 조직화학적 검사 및 Western blotting test결과상 정도의 차이는 있었지만 전례에서 제2형 교원질의 존재를 확인할 수 있었다. 정량화된 제2형 교원질의 양이 많을수록 수술 전 내반변형의 정도는 적었고 이차관절경소견상 재생된 연골의 범위가 넓었으며 이는 통계학적으로 유의한 차이를 보였다. 요약 및 결론: 퇴행성 슬관절염 환자에서 관절경을 이용한 미세 천공술은 연골 결손 치료에 있어 유용한 치료방법으로 사료되며 이러한 재생된 연골이 지속적인 체중 부하에 의해서 생역학적인 변화를 일으키지 않고 얼마나 유지 할 수 있는지는 보다 장기적인 추시 관찰이 필요할 것으로 사료된다.
목적: 슬관절의 연골 결손에 대한 미세골절술 후 병변을 덮는 생체막을 이용한 환자들의 치료 결과를 비교, 분석하고자 한다. 대상 및 방법: 2008년 1월부터 2010년 1월까지 관절경 검사로 확인된 슬관절 연골의 국소적 전층 결손이 있으며, 골관절염을 보이지 않는 53명(총 59례)의 환자를 대상으로 하였다. 무작위로 선정된 36명(42례)은 미세골절술 후 생체막으로 병변을 덮어주었고(실험군), 나머지 17명(17례)은 고식적인 미세골절술만을 시행받았다(대조군). 모든 환자는 수술 후 6개월째에 임상적 결과를 IKDC 설문지와 통증, 만족도에 대한 VAS 점수를 이용하여 비교하였고, 자기공명영상 촬영을 통해 골연골 병변의 회복 상태를 비교하였다. 결과: 임상 결과(IKDC, 통증 VAS, 만족도 VAS)에서 실험군이 대체로 더 우수하였으며 IKDC에서는 두 군 간의 통계적으로 유의한 차이를 보였다. (IKDC, p=0.047, 통증 VAS, p=0.074; 만족도 VAS, p=0.001). MRI 추시결과에서 실험군의 33명(78.6%)에서 우수한 결손 치유(67~100%)를 보이고, 5명(11.9%)만이 불량한 치유 소견을 보인 반면, 대조군에서는 4명(23.5%)의 환자에서 양호한 결손 치유가 관찰되고 9명(52.9%)의 환자들에서 결손 치유가 불량하였다(p=0.001). 실험군에서 35명(83.3%), 대조군 중 6명(35.3%)에서 변연부 결합부에서 간격이 관찰되지 않았다(p=0.001). 생체막 사용과 관련하여 심각한 부작용은 발견되지 않았다. 결론: 슬관절의 연골 결손 병변에 대해 미세골절술 후 생체막으로 도포한 군에서 단기 추시 임상적 결과 및 MRI를 이용한 영상학적 결과에서 결손부의 연골재생이 잘 된 것을 확인하였고, 미세골절술 후 ArtiFilm$^{TM}$을 함께 사용했던 군에서 연골 치유가 더 잘되는 경향이 있음을 시사한다.
목 적 : 급성 화농성 관절염 및 골수염은 성인에 비해 소아에서 비교적 흔히 볼 수 있는 질환으로 진단 및 치료가 지연되거나 부적절할 경우 만성적인 후유증을 초래할 수 있다. 본 연구에서는 최근 6년간 우리나라 2개 대학병원에서 경험한 급성 관절염 및 골수염의 원인균과 임상 양상을 분석하였다. 방 법: 2003년 6월부터 2009년 7월까지 지역사회에서 발생한 급성 화농성 골관절염으로 서울대학교 어린이 병원과 분당 서울대학교병원에 입원한 15세 이하 환아들 중 혈액이나 관절액, 조직에서 시행한 세균 배양 검사상 원인균이 증명된 63례를 대상으로 하여 의무 기록을 후향적으로 분석하였다. 결 과:총 63명의 중앙 연령은 60개월이고, 남아는 35명, 여아는 28명으로 약 1.25:1의 비율을 보였다. 침범 부위 중 관절은 고관절을 침범한 것이 15례(37.5%) 로 다수를 차지하였고, 슬관절 7례(17.5%), 견관절과 주관절이 각각 4례(10%)에서 침범되었고, 골수 침범 부위는 대퇴골이 20례(33.9%), 경골이 13례(22%), 요골과 상완골이 각각 7례(11.9%)씩 차지하였다. 원인균 중 S. aureus가 49례로 77.8%를 차지하였고, 이 49례 중 MSSA가 41례(83.7%), MRSA는 8례(16.3%)를 나타내었다. GBS, Salmonella spp.는 각각 3례를 차지하였다. 한편 생후 1개월 미만의 신생아 4례중 3례에서 GBS가 동정되었다. 초기 항생제로는 1세대 cephalosporin 또는 nafcillin 단독 요법을 선택한 경우가 27례(42.9%)로 가장 흔했고 vancomycin을 포함한 경우는 총 9례 (14.3%)이었다. 총 63례 중 54례(85.7%)에서 합병증 없이 호전되었고, 만성 골수염, 성장판 손상, 관절 신전 장애, 재감염 및 재수술 등 합병증을 동반한 호전을 보인 경우가 9례(14.3%)이었고 사망례는 없었다. 결 론 : 지역사회에서 발생한 소아청소년 관절염 및 골수염의 가장 흔한 원인균은 S.aureus이고 이 중 MRSA 는 16.3%를 차지하므로 신중한 항생제 선택을 필요로 한다.
다발성 골단이형성증은 임상적 및 유전학적으로 이질적인 연골이형성증으로, 골화 중심의 발달 지연, 골단만을 침범하는 사지의 변형 및 경한 저신장을 보이는 질환이다. 원인으로는 COMP 유전자의 돌연변이가 가장 흔히 발견되고 있으며, 돌연변이의 대부분은 칼모듈린 유사 반복(calmodulin-like repeats)부분과 C-말단에 위치한다. 저자들은 4대에 걸쳐 12명의 가족구성원에서 다발성 골단이형성증을 보인 한 가계에서 COMP 유전자 분석으로 새로운 돌연변이를 발견하여 보고하는 바이다. 이 가계는 젊은 나이에 퇴행성 관절염이 발생하고, 추후 관절치환술이 요구되는 상염색체 우성 유전 방식을 보이는 질환을 가지고 있었다. 방사선학적 검사상 양측 무릎 관절의 내측 대퇴돌기에 골연골 결손을 보였으며, 양측 무릎과 엉덩이 관절은 다양한 정도의 퇴행성 변화가 관찰되었다. 계보발단자 및 5명의 다른 이환된 가족들에서 시행한 COMP 유전자 분석에서, 12번째 엑손에서 현재까지 보고되지 않은 새로운 돌연변이인 c.1280G>C (p.Gly427Ala)을 확인하였다. 이 돌연변이는 다른 3명의 이환되지 않은 가족들에서는 발견되지 않았다. 상염색체 우성 방식을 보이는 다발성 골단이형성증 환자들에서 COMP 유전자의 직접 염기서열 분석법은 질병의 원인이 되는 돌연변이를 찾는데 도움이 되며, 고위험 가족들에서 보인자를 찾아낼 수 있고, 질병의 합병증이 발생하기 이전에 조기 진단 및 중재를 가능케 하는 유전 상담의 기회를 제공해 줄 수 있다.
This study was carried out to investigate the effects of GCP treatment on the expression of NOS, c-fos, serotonin and substance P in central nerve system of monosodium iodoacetate(MIA)-induced osteoarthritic pain model. Arthritis was induced by injection of MIA(0.5 mg) into knee joint cavities of rats. Arthritic rats were divided into control(n=8) and treated(n=8) group. Control group was taken distilled water for 20 days. Treated group was taken extracts of GCP by oraly for same duration. Normal group(n=8) was infected with normal saline and was taken distilled water for 20 days. The numbers of NADPH-d positive cells in superficial dorsal horn of spinal cord of treated group($21{\pm}5$) was significantly (p<0.01) decreased compared with control($33{\pm}5$). The numbers of NADPH-d positive cell in dorsolateral periaqueductal gray matter of treated group($111{\pm}16$) was significantly(p<0.01) decreased compared with control($143{\pm}14$). The numbers of c-fos positive cells in dorsal periaqueductal gray matter of treated group($57{\pm}16$) was significantly(p<0.01) decreased compared with control($78{\pm}13$). The numbers of c-fos positive cells in paraventricular thalamic nucleus of treated group($60{\pm}15$) was significantly decreased compared with control($88{\pm}27$). The numbers of serotonin positive cells in median raphe nucleus of treated group($171{\pm}31$) was significantly(p<0.05) decreased compared with control($217{\pm}48$). On the basis of these results, we concluded that GCP treatment has inhibiting effects on the pain transmission in monosodium iodoacetate-induced osteoarthritic pain model in rat.

일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
