Effects of Keonbodan (健步丹) on MIA-Induced Osteoarthritis in Rat (건보단(健步丹)이 MIA로 유도된 골관절염 Rat에 미치는 영향)

  • Objectives This study was carried out to know the effects of Keonbodan (hereinafter referred to KBD) in osteoarthritis induced by Monosodium iodoacetate(hereinafter referred to MIA) on Wistar rat. Methods Osteoarthritis was induced by injection of MIA into left knee joint cavities of rat. Osteoarthritis rats were divided into 4 groups (normal (n=6), control (n=6), indomethacin (n=6), KBD (n=6) group). The control group was administered normal saline and indomethacin group was administered indomethacin (2 mg/kg). And the KBD group was administered KBD (142 mg/kg). Each groups were administered by orally for 4 weeks. This experiment were carried out in vivo. In vivo, at the end of the experiment (5 weeks after MIA injection), effects on hepatotoxicity and nephrotoxicity, cytokines in serum, arachidonic acid, osteocalcin, MMP-9, TIMP-1 and cartilage volume were evaluated. And histopathological examinations on the articular structures of knee joints were performed. Results 1. In weight-bearing measurement, level of weight was increased. 2. In order to hepatotoxicity and nephrotoxicity, ALT, AST, BUN and creatinine were tested. And there were no significant changes. 3. In serum, levels of TNF-$\alpha$, IL-$1{\beta}$ were significantly decreased. IL-6 was insignificantly decreased. 4. In serum, level of MMP-9 and TIMP-1 was decreased. 5. In serum, level of $LTB_4$, $PGE_2$ and osteocalcin was decreased. 6. In ${\mu}$CT-arthrography, the cartilage volume was greater than that of the control group. 7. The joint damage induced by osteoarthritis was lesser than the control group in histopathologic observation (H&E, Safranin-O staining). Conclusions These results demonstrated that KBD suppressed the osteoarthritis- inducing effects of MIA in rat. And further studies are required to find out more effective substance and anti-osteoarthritic mechanism in the future.

Effects of Saengkanggamchotang (SKT) on MIA-Induced Osteoarthritis in Rats (생강감초탕(生薑甘草湯)이 골관절염 유발 Rat에 미치는 영향)

  • Objectives This study was performed to investigate the effects of Saengkanggamchotang (SKT) on the monosodium iodoacetate (MIA) induced osteoarthritis in rats. Methods Osteoarthritis was induced by injection of MIA (50 ul, 60 mg/ml) into knee joints of rats. Rats are divided into a total of 4 groups (normal, control, positive comparison group, SKT treated group, each n=6). Normal group are not treated at all without inducing osteoarthritis whereas control group were induced for osteoarthritis by MIA and oral medicated with 20 ml of distilled water per day. Positive comparison group was injected with MIA and after 7 days, that was taken indomethacin (30 mg/kg/mouse). SKT treated group was injected with MIA and after 7 days that was taken SKT (30 mg/kg/mouse). Positive comparison group and SKT treated group were oral medicated for each substance a total of 4 weeks with one time per day. After experiments (from 1 week after injection of papain to 4 weeks elapsed), the functions of liver and kidney, Prostaglandin E2, inflammatory cytokine (IL-$1{\beta}$, IL-6, TNF-${\alpha}$), osteocalcin, TIMP-1, MMP-9 within serum. Knee joint structures were observed by H&E, safranin-O staining method, and amount of cartilage were measured by ${\mu}CT$-arthrography. Results 1) Hind paw weight bearing ability was significantly improved. 2) Functions of liver and kidney were not affected. 3) Prostaglandin E2, osteocalcin, TIMP-1, MMP-9 in serum were significantly decreased. 4) Inflammatory cytokine IL-$1{\beta}$ was significantly decreased, and IL-6, TNF-${\alpha}$ were decreased but had not significant. 5) In terms of histopathology, significantly reduced subsidence of cartilage and bone in H&E staining. And in Safranin O staining, proteoglycan content in synovial membrane was significantly increased compared with control group. 6) Destruction of cartilage on ${\mu}CT$-arthrography was significantly reduced. Conclusions Based on all results mentioned above, Saengkanggamchotang (SKT) is believed to be meaningful for suppressing the progress of osteoarthritis and its treatments.

Pre-post Comparisons on Physical Symptoms, Balance, Muscle Strength, Physical Functioning, and Depression in Women with Osteoarthritis after 12-week Tai Chi Exercise (타이치 운동교실 참여 후 골관절염 여성환자의 통증, 체력, 신체기능 및 우울의 변화)

  • Objective. The 12 forms of Sun-style Tai Chi exercise has been developed specifically for arthritis patients in order to reduce their symptoms and to improve physical functioning. This quasi-experimental study examined the changes in pain, balance, muscle strength and physical functioning in women with osteoarthritis at the completion of the 12 week Tai Chi exercise program. Methods. The patients with osteoarthritis who signed the consent form were screened by their primary physician according to the inclusion criteria and invited to the study. Total of 66 osteoarthritis women with an average age of 63 years were participated in the Tai Chi exercise. At the completion of 12 weeks, 34 patients completed both pretest and posttest measures with 48% of overall dropout rate. Outcome measures were physical symptoms, balance, muscle strength, physical functioning, and depression. Paired t-test was utilized to examine differences between pre and post-measures. Results. After participating in the Tai Chi exercise program, the women with osteoarthritis showed significant improvements in their physical fitness measures, and consequently in their physical functioning. In physical fitness test, there were significant improvements in balance, flexibility, muscle strengths of knee, grip, and back muscles after the Tai Chi exercise. However, No significant differences were found in pain and stiffness of their knee joints and depression measure. Conclusion. The 12 forms of Tai Chi exercise has been found safely applicable to the older women with osteoarthritis for 12 weeks, and effective in improving balance, flexibility, and muscle strengths, and consequently lessening difficulties of performing their activities of daily life.

Effects of a Heel Wedge on the Knee Varus Torque During Walking (보행 시 무릎관절 내번토크에 미치는 후족왯지의 영향)

  • In the present study, knee varus torque and mediolateral accelerations were measured using the three-dimensional motion analysis system and a linear accelerometry in odor to determine the effect of heel wedges during walking. Wedges were inclined with 10$^{\circ}$ and 15$^{\circ}$ in medial and lateral directions respectively Both knee varus torques and mediolateral accelerations showed two distinct positive peaks in loading response and preswing. Medial wedges resulted in significantly increased both knee varus torque and lateral acceleration in loading response, compared with the barefoot walking(p<0.05). On the other hand, lateral wedges decreased them in loading response(p<0.05). This became more significant for more inclined wedges. However, no significant correlations were found between knee varus torque and lateral acceleration according to the angle of heel wedges in preswing. From this study, it was found that a lateral wedge would be helpful to treat osteoarthritis, decreasing knee varus torque in loading response. In addition, lateral acceleration of the knee joint might be an alternative to determine the effect of wedges and the alignment of the knee joint during walking, instead of measuring knee torque by the three-dimensional motion analysis.

Effects of Complex Exercise Program on Pain and Muscle Activation in elderly Patients with Knee Osteoarthritis (복합운동이 퇴행성 슬관절염 노인환자의 통증과 근활성도에 미치는 영향)

  • This research is performed for the elderly patients who have knee osteoarthritis joint and who are applied to complex exercise program to know that how the complex exercise affects on pain and muscle activation. This research is tested 23 elderly patients who have knee osteoarthritis in hospital B. It divided into exercise group(n=2) and control group(n=1), by doing general physical therapy for each case three times a week, 40 minutes, among 10 weeks and measured before and after VAS and muscle activation. For the case exercise group, supplementally, applied to complex exercise program three times a week, 40 minutes, among 10 weeks and also measured before and after VAS and muscle activation. After the test is performed in 10 weeks, the results of this test are as follows. First, both exercise group and control group, they have more in pain after experiment rather than before, respectively(p<0.05). but There was no significant statistical difference between two groups(p>0.05). Second, exercise group have in muscle activation after experiment rather than before(p<0.05). and There was significant statistical difference between two groups(p<0.05). The test is implied that if the people who has knee osteoarthritis wants to get more good effects with complex exercise, they should get more opportunities to complex exercise. as well as fitness, resistance exercise and grafted to the easily create an exciting program to improve the quality of life of seniors in the program will be studied in future study.

  • Objectives : To determine whether individualized acupuncture (IA) provides greater pain relief and improved function compared with standardized acupuncture (SA) in patients with osteoarthritis of the knee. Methods: Design is a randomized single blind controlled trial with two intervention arms (IA vs SA) of 6 weeks' duration and 3 months follow up. An orthopedist diagnosed 50 patients' symptoms as knee osteoarthritis. Interventions were applied by 2 residents studying the acupuncture and moxibustion specially in Dongguk University International Hospital. Primary outcome measure is pain as measured by visual analogue scale (VAS). Secondary measures of pain and disability include WOMAC, SF-36, LFI and KHAQ. Results : The 50 participants were well matched for age, sex, bad side, VAS, WOMAC, SF-36, LFI and KHAQ scores at the baseline. 3 participants dropped out. There have not been any adverse events in both groups. Participants in both groups experienced great improvement in all measures except SF-36at 3 and 6 weeks. At 18 weeks, the IA group experienced significantly greater improvement than the SA group in VAS but not in WOMAC, SF-36, LFI and KHAQ. Conclusions : Manipulation of IA seems to provide pain relief longer for osteoarthritis of the knee when compared with SA.

Arthroscopic Treatment of Osteoarthritis of the Knee(Comparison of the groups with and without mechanical symptom) (슬관절 퇴행성 관절염의 관절경적 치료 - 기졔적 증상이 있는 군과 없는 군과의 비교 -)

  • Purpose: The purpose of this study is to evaluate the clinical results for the treatment of osteoarthritis of the knee by arthroscopic procedure according to presence or absence of mechanical symptoms. Materials and Methods: We studied sixty patients (60 knees) who underwent arthroscopic procedure for the treatment of osteoarthritis of the knee from October,2000 to January,2002. The average follow up period was 15 months(12-30 months). We evaluated the mechanical symptoms by simple radiographs, history taking and physical examination. We analysed the clinical results by Baumgaertner's scale. Results: The Baumgaetner's score improved from 3.1 preoperatively to 8.1 postoperatively in the group with mechanical symptoms, whereas the scores in the group without mechanical symptom improved from 3.5 preoperatively to 5.4 postoperatively. The patients with mechanical symptoms were satisfied more than the patients without mechanical symptoms. Conclusion: The arthroscopic surgery could be one of good alternative treatment methods for the osteoarthritis of the knee with mechanical symptoms . But, more than these patients will be needed for further evaluation.

  • Objectives : The purpose of this study is to evaluate the feasibility of massive clinical research and to make a basic analysis on the effectiveness and safety of moxibustion treatment on knee osteoarthritis compared to usual care. Methods and Results : This study is a protocol for a pilot randomized controlled trial. Forty participants are assigned to the moxibustion group (n=20) and usual care group (n=20). Participants assigned to the moxibustion group receive moxibustion treatment on the affected knee(s) at six standard acupuncture points (ST36, ST35, ST34, SP9, Ex-LE04 and SP10) three times per week for four weeks (total of 12 sessions). Participants in the usual care group don't receive moxibustion treatment during the study period and follow-up are made on the 5th, 9th and 13th weeks after random allocation. Both groups are allowed to use any kind of treatment, including surgery, conventional medication, physical treatment, acupuncture, herbal medicine, over-the-counter drugs and other active treatments. Education material that explains knee osteoarthritis and current management options and self-exercise is provided for each group. The pain scale of the Korean Western Ontario and McMaster Universities Questionnaire (K-WOMAC) is the primary outcome measurement used in this study. Other subscales of the K-WOMAC, the Short-Form 36 Health Survey (SF-36), Beck Depression Inventory (BDI), Physical Function test, Patient Global Assessment, and Pain Numeric Rating Scale (NRS) are used as outcome variables to evaluate the effectiveness of acupuncture. Safety is assessed at every visit. Conclusions : The result of this trial will provide a basis for the effectiveness and safety of acupuncture treatment for knee osteoarthritis.

An Effect of Muscle Strengthening Exercise Program on Muscle Strength, Pain, Depression, Self- efficacy, and Quality of Life of Patients with Knee Osteoarthritis (근력강화 운동프로그램이 퇴행성 슬관절염대상자의 근력, 통증, 우울, 자기효능감 및 삶의 질에 미치는 영향)

  • In an attempt to investigate the effect of a muscle strengthening exercise program on muscle strength, pain, depression, self-efficacy and quality of life of patients with knee osteoarthritis, a pre-experiment, one group pre-test & post-test design, was planned. Muscle strengthening exercise was carried out from May 22 through August 14, 1995 at isokinetic exercise room in rehabilitation department of University Hospital in Taejon. The subjects were seven female clients conveniently sampled from University Hospital located in Taejon, between 39 and 61 years of age, who had a osteoarthritis in knee. Muscle strengthening exercise program was composed of three sessions per week, one isokinetic exercise at angular velocity of 60° and 180° with Cybex isokinetic dynamometer and two resistance home exercise sessions with elastic band. Data were analyzed with frequency, percentage of change, Friedman test, Duncan test using SAS program. Results were obtained as follows : 1) Flexion and extension muscle strength at angular velocity of 60° and 180° were increased after 12weeks' exercise than those of before experiment. But exept flexion muscle strength at angular velocity of 180°(F=3.34, P=0.0261), there was no statistically significant difference among muscle strengths, which is measured every 3 weeks. 2) Pain was decreased after 6weeks' exercise than that of before experiment, and after 12weeks' exercise than that of 6weeks' exercise. There was statistically significant difference (F=4.28, P=0.0396). 3) Depression was increased after 6weeks' exercise than that of before experiment, and after 12weeks' execise than that of 6weeks' exercise. There was no statistically significant difference between before experiment and after 6weeks' exercise. But, there was statistically significant difference between after 6weeks' exercise and 12weeks' exercise(F=9.38, P=0.0035). 4) Self-efficacy was decreased after 6weeks' exercise than that of before exercise. But, it was increased after 12weeks' exercise than that of be-fore exercise and after 6weeks' exercise. But there was no statistically significant difference (F=1.46, P=0.2706). 5) Quality of life was increased after 6weeks' exercise than that of before exercise, and after 6weeks' exercise than that of 12weeks' exercise. But there was no statistically significant differ once (F=1.06, P=0.3816). Thus, the significant of muscle strengthening exercise for the improvement of muscle strength, pain, depression, is verified. But, this study was a pre-experiment with small size subjects. So, con-trolled experimental study is necessary to determine the effect of this muscle strengthening exercise program on muscle strength, pain, depression, self-efficacy, and quality of life of patients with knee osteoarthritis.

Effect of a modified maneuver for quadriceps muscle setting with co-contraction of the hamstrings on patients with knee joint osteoarthritis (퇴행성 슬관절염 환자에 대한 수정된 대퇴사두근과 슬괵근 운동의 동시수축 효과)

  • Purpose : The purpose of this study was to investigate a modified maneuver for quadriceps setting exercise for patients with knee osteoarthritis. Methods : The patients were randomly divided into a modified maneuver for quadriceps setting exercise group(MQG) and conventional quadriceps setting exercise group(CQG). Total of 28 patients received a modality with training for 1 hour, three times a week for 12 weeks. Results : Each group showed significant reductions in the WOMAC(Western Ontario McMaster Universities Osteoarthritis) Index, mobility, and muscle strength after 12 weeks. In pain, physical function, and hamstring muscle strength, there was a statistically significant difference between groups. Conclusion : According to the results, MQG experienced less pain, physical function, and increased more hamstring muscle strength than NQG.
