The Effect of Knee Muscle Imbalance on Motion of Back Squat (무릎 근력의 불균형이 백 스쿼트 동작에 미치는 영향)

  • The purpose of this study was to investigate the effect of muscle imbalance on motion of back squat. The isokinetic muscle strength of the 8 subjects was recorded for the knee flexion/extension by the cybex 770 dynamometer. Each subject performed 3 back squats with the long barbell with an intensity of 25% body weight(BW), 50%BW, 100%BW, 125%BW. During the back squat through the recorded kinematic data the subjects' maximum flexion and extension knee angle, center of mass displacement and V-COP were calculated for evaluation of the stability of the movement. For the statistical analysis independent t-test was used. Knee flexion angle and COM displacement are dominated by the reciprocal muscle ratio. V-COP factor was dominated by bilateral extension deficit. Based on the results we can know that as the intensity of the squat increased to a level control was difficult because the muscles' imbalance influenced the movement.

  • 본 연구의 목적은 소방용 coverall의 디자인이 소방수들의 동작기능성에 어떤 영 향을 주는지에 관하여 고찰하여, 보다 기능적 인 coverall을 개발, 궁극적으로 작업자의 노동력을 향상시키고, 보다 안락한 환경 속에서 작업할 수 있도록 도와주는데에 있다. 미국 캔사스 지역의 전문 소방수를 대상으로, interview 및 video analysis를 통하여, 그들이 현재 사용하고 있는 current coverall의 동작기능면에서의 문제점을 조사, 분석한 후, 이를 바탕으로 prototype coverall을 개발하였다 prototype coverall의 동작기능성을 객관적으로 측정하기 위하여 10가지 실험동작(shoulder adduction/ abduction, shoulder flexion/extension, trunk flexion/extension(standing), hip flexion/extension(knee), hip flexion/extension(trunk), upper leg flexion, trunk lateral flexion, hip adduction, trunk flexion/ extension(sitting), upper leg flexion)을 선택하여 이의 관절각도(range-of-motion)를 Leighton flexometer, goniometer, computer-aided method를 이용하여 측정하였으며, 착용자의 주관적 평가를 위하여서는 23 항목의 wearer acceptability scale를 사용하였다. ANOVA(Analysis of Variance)와 LSD(Least Significant Difference)를 이용한 통계처리 결과, prototype coverall이 current coverall에 비해 동작기능성 (특히 crotch, torso, hip movement)에 있어서 우수함을 보이고 있다.

  • Good stability and complete range of motion should be the ultimate goal of a rehabilitation program after ACL reconstruction. In previous years. the rehabilitation of the ACL reconstructed knee focused on protecting the new ligament by blocking terminal knee extension, hut, despite good stability, this approach led to numerous postoperative complications. Nowadays, most of surgeons agree the accelerated rehabilitation program based on the concept of ligamentization and clinical experience. Accelerated rehabilitation program consists of maintain of full extension of the knee, early weight bearing and prompt recovery of ROM, and closed kinetic chain exercise. Meeting this goal requires effective communication between-members of the health care team-the physician, physical therapist, atheletic trainer, and the patient. We have to know the importance of rehabilitation, knowlege about the physical therapy, and to introduce for special physical therapist and equipment.

As, in relation to all movements of a human being, the movements such as mutually walking, running, rotating, and jumping are attained endlessly through the ground amid the interaction with the ground, in terms of the harmonious movement of the upper limbs and the lower limbs, related to the basic movement in ballet, the type of a movement depends on the size and direction of the force that presses down the ground (Fz, Fx, Fy) amid the interaction with the ground. Therefore, aiming to correctly and efficiently perform a rotational movement in Arabesque, this study analyzed factors of the force manifestation through GRF (Ground Reaction Force), by dividing into preparing, stepping, standing, rotating, and finishing stages (events (1) ${\sim}$ (5)), targeting the subjects of 4 elite female students who majored in ballet. 1. At the No.5 position of the preparing stage, It is necessary that support the ground with left and right foot balance, 2. As the stepping stage is the phase ranging from the event (2), in which a plie movement of bending a knee is started, to the event (3) of stretching a knee, Rebunding motion is not good, and One have a position with ankle and knee flextion condition in order to stretch strengthly in event (3) position 3. At the event (1) position, It is necessary that exert the Fz reaction force at the event (3) position. Because large stretch force help to have a toe on position easily and show a active motion 4. Correlation Between Knee Muscle Strength and Maximal Cycling Speed Measured Using 3D Depth Camera in Virtual Reality Environment

  • Background: Virtual reality (VR) programs based on motion capture camera are the most convenient and cost-effective approaches for remote rehabilitation. Assessment of physical function is critical for providing optimal VR rehabilitation training; however, direct muscle strength measurement using camera-based kinematic data is impracticable. Therefore, it is necessary to develop a method to indirectly estimate the muscle strength of users from the value obtained using a motion capture camera. Objects: The purpose of this study was to determine whether the pedaling speed converted using the VR engine from the captured foot position data in the VR environment can be used as an indirect way to evaluate knee muscle strength, and to investigate the validity and reliability of a camera-based VR program. Methods: Thirty healthy adults were included in this study. Each subject performed a 15-second maximum pedaling test in the VR and built-in speedometer modes. In the VR speedometer mode, a motion capture camera was used to detect the position of the ankle joints and automatically calculate the pedaling speed. An isokinetic dynamometer was used to assess the isometric and isokinetic peak torques of knee flexion and extension. Results: The pedaling speeds in VR and built-in speedometer modes revealed a significantly high positive correlation (r = 0.922). In addition, the intra-rater reliability of the pedaling speed in the VR speedometer mode was good (ICC [intraclass correlation coefficient] = 0.685). The results of the Pearson correlation analysis revealed a significant moderate positive correlation between the pedaling speed of the VR speedometer and the peak torque of knee isokinetic flexion (r = 0.639) and extension (r = 0.598). Conclusion: This study suggests the potential benefits of measuring the maximum pedaling speed using 3D depth camera in a VR environment as an indirect assessment of muscle strength. However, technological improvements must be followed to obtain more accurate estimation of muscle strength from the VR cycling test.

  • Prosthetic replacement is one of the most common methods of reconstruction after resection of malignant tumor around the knee. Gait analysis provides a relative objective data about the gait function of patients with prosthesis. The purpose of this study was to compare the gait pattern of the patients who underwent limb salvage surgery with prosthesis for distal femur and that of patients with prosthesis for proximal tibia. This study included ten patients (4 males, 6 females, mean age 22.7 years, range 14-36) who underwent a wide resection and Kotz hinged modular reconstruction prosthesis replacement and six normal adult(Control). The site of bone tumor was the distal femur (Group 1) in six patients and proximal tibia (Group 2) in 4 patients. The follow-up period ranged from 15 to 82 months (mean : 33 months). The evaluation consisted of clinical assessment, radiographic assessment, gait analysis using VICON 370 Motion Analysis System. The gait analysis included the linear parameters such as, walking velocity, cadence, step length, stride length, stance time, swing time, single support and double support time and the three-dimensional kinematics (joint rotation angle, velocity of joint rotation) of ankle, knee, hip and pelvis in sagittal, coronal and transverse plane. For the kinetic evaluation, the moment of force (unit: Nm/kg) and power (unit: Watt/kg) of ankle, knee and hip joint in sagittal, coronal and transverse plane. In the linear parameters, cadence, velocity, step time and single support were decreased in both group 1 and group 2 compared with control. Double support decreased in group 2 compared with control significantly(p<.05). In contrast to our hypothesis, there was no significant difference between group 1 and group 2. Effect of Saddle to Pedal Length in Kayak Ergometer on Rowing Motion and EMG Activation in Elite Kayak Players (엘리트 카약 선수들의 에르고미터를 이용한 로잉 동작 시 안장과 페달의 거리가 로잉 패턴 및 근 활성도에 미치는 영향)

  • The purpose of this study was to identify the saddle to pedal length contributing to successful performance in kayak using a kayak ergometer. Ten male elite kayak players participated in this study. players were tested on the kayak ergometer which was varied saddle to pedal length by the knee flexion angle(90deg; 120deg; 150deg) to measure stroke frequency, paddling amplitude, joint angle, RoM and angular velocity, foot pressure and force, iEMG using the 3D motion system, foot pressure system and EMG wireless system. At a results, rowing at 120deg on knee flexion angle showed higher stroke frequency and paddling amplitude than other knee flexion angles. RoM at upper extremity showed not significant difference between knee flexion angles. But there were significant differences in thorax and pelvis rotation RoM, knee flexion-extension RoM in each condition. In addition, foot pressure, force and iEMG were significantly different in knee flexion angles. Study showed that changed of saddle to pedal length affected rowing performance kinds of stroke frequency, paddling amplitude. The most important thing, increased range of motion in pelvic and thorax has occurred by force that generated foot-bar to seat. Not only that, but it seems to be attributed to a technical adaptation developed to maximum rowing performance.

Results of Conventional and Accelerated Rehabilitation Following ACL Reconstruction (전십자인대 재건술 후 고식적 재활과 적극적 재활의 결과)

  • The purpose of this study is to compare the results between conventional and accelerated rehabilitation program following ACL reconstruction using bone-patellar tendon-bone. Conventional rehabilitation focused on protecting the new ligament by blocking terminal extension and avoiding active quadriceps function in the terminal degrees of extension. But there is current trend toward early postoperative mobilization and intensive. so called 'accelerated', rehabilitation stressing hyperextension of the knee. The results of intraarticular ACL reconstruction with conventional and accelerated rehabilitation were prospectively compared for one year postoperatively in a series of 27 patients. Range of motion and thigh circumference were checked preoperatively, and weekly up to 8 weeks, 3 months. 6 months, and 1 year postoperatively. Stress radiologic test, KT-1000 arthrometer. Cybex II dynamometer were checked in preoperatively, and 3 months, 6 months, and 1 year postoperatively. There were no differences of objective stability and restoration of muscle power. But the accelerated group had a low incidence of extension loss. excellent range of motion, and less difference of thigh circumference. We concluded that accelerated rehabilitation program is recommendable due to superiority in terms of range of motion, especially less extension loss without increasing laxity of knee joint.

Effects of Unilateral and Bilateral Movement on Muscle Strength and Activity During Maximum Contraction (최대 근수축시 외측운동과 양측운동이 근력과 근활동에 미치는 영향)

  • In the present study, we aimed to elucidate how muscle strength and activity are affected by movement pattern(bilateral [BLM] & unilateral movement [ULM]) and movement velocity($0^{\circ}$/s, $60^{\circ}$/s, $120^{\circ}$/s) at maximum effort, and to elucidate the relationship between a left/right asymmetry and bilateral deficit. A total of 18 healthy males participated in the study. Each participant performed maximum knee extension bilaterally and unilaterally while the EMG and moment were recorded, and then the relationships between the asymmetry and bilateral deficit were analyzed. The peak moments for the isokinetic motion at $60^{\circ}$/s and $120^{\circ}$/s and overall muscle activities of lower extremity were significantly reduced for the BLM in comparison to the ULM. And though the asymmetry in ULM were maintained during BLM at all velocities, the bilateral deficits at the velocity of $0^{\circ}\acute{y}$/s and $120^{\circ}\acute{y}$/s were significantly correlated with increased asymmetries of muscle strength in ULM. In conclusion, the reduction in the muscle strength exhibited in bilateral knee extension was shown to arise partially from a reduction in muscle activity, and left/right asymmetry was found to be associated with mechanical reduction in bilateral movement. These findings suggest that training aimed at increasing muscle strength must involve methods and strategies intended to reduce left/right asymmetry.

The Effects of Visual Biofeedback Information on Hyperextended Knee Control

  • Purpose: A hyperextended knee is described as knee pain associated with an impaired knee extensor mechanism. Additionally, a hyperextended knee may involve reduced position sense of the knee joint that decreases the individual's ability to control end-range knee extension movement. The purpose of this study was to investigate the effects of visual biofeedback information for plantar pressure distribution on knee joint angle and lower extremity muscle activities in participants with hyperextended knees. Methods: Twenty-three participants with hyperextended knees were recruited for the study. Surface electromyography signals were recorded for the biceps femoris, rectus femoris, gastrocnemius, and tibialis anterior muscle activities. The plantar pressure distribution was displayed and measured using a pressure distribution measuring plate. Knee joint angle kinematic parameters were recorded using a motion analysis system. The visual biofeedback condition was the point at which the difference between the forefoot and backfoot plantar foot pressure on the monitor was minimized. The Wilcoxon signed-rank test was used to determine the significance between the visual biofeedback condition and the preferred condition. Results: The knee joint angle was significantly decreased in the visual biofeedback condition compared to that in the preferred condition (p<0.05). The rectus femoris and gastrocnemius muscle activities were significantly different between the visual biofeedback and preferred conditions (p<0.05). Conclusion: The results of this study showed that visual biofeedback of information about plantar pressure distribution is effective for correcting hyperextended knees.
