Effect of curcumin in the prevention of experimentally induced nephrolithiasis in rats by ethylene glycol and Vitamin D3

  • Curcumin (CMN) is known to have beneficial role in anorexia, coryza, cough, diabetic wounds, and hepatic disorders apart from its inherent antioxidant effects. Therefore, the present study was aimed to evaluate antioxidant effect of CMN in prevention of nephrolithiasis in rats-induced by ethylene glycol (EG) and Vitamin D3 (Vit. D3). Male Wistar rats (175 - 200 g) were randomized in groups like control, EG + Vit. D3 induced nephrolithiatiatic rats, CMN treated rats, CMN + EG + Vit. D3 treated rats, Vit. E + EG + Vit. D3 treated rats. Urine was collected weekly throughout the experimental protocol and estimated for calcium oxalate (CaO) count. After completion of experimental protocol serum was estimated for blood urea nitrogen and creatinine. Both the kidneys were excised and used to evaluate levels of biomarkers of oxidative stress and calcium oxalate crystal deposition by histopathological studies. Administration of EG and Vit. D3 to rats resulted in increased oxidative stress, hyperoxaluria and renal deposition of CaO crystals. Supplementation with CMN improves kidney function, reduces elevated oxidative stress, urinary oxalate level and renal deposition of CaO which shows its protective action in nephrolithiasis. The increased deposition of stone in the kidney and stone forming constituents of nephrolithiatic rats were effectively lowered by treatment of CMN.

  • Ultrasonogram for kidney, spleens liver, intestine and heart was evaluated 11 Chriu horses and 14 Thoroughbred horse. The kidney was determined at both flan and both 17th intercostal spaces. The spleen was determined at the left 13-17 intercom spacer and the liver was determined at the right 7-13 intercostal space. The heart was determined at the right 4-6 intercostal space and left 3-6 intercostal space. The length, of rig kidney at the Thoroughbred horse, Thoroughbred foul, Cheju horse and Cheju foul were 16.2, 12.6, 13.1 and 11.2 mm, respectively. The width of right kidney at the Thoroughbed horse, Thorughbred foul, Cheju horse and Cheju foul were 5.4, 4.3, 4.6 and 4.2 mm, respectively. The depth of right kidney at the Thoroughbred horsed Thoroughbred foul, Cheju horse and Chrju foul were 5.2, 4.4, 4.5 and 4.3 mm, respectively. Similar ultrasonographic measurements were obtained for the left kidney, The left ventricular end-diastolic diameter at the Thoroughbred horse, Thoroughbred fouls Cheju horst and Cheju foul were 107, 83, 85 and 73 mm, respectively. The left ventricular end-systolic diameter were at the Thoroughbred horse, Thoroughbred foul, Cheiu horse and Cheiu foul were 63, 52, 53 and 45 mm, respectively. Also, the interventricular septum in end-diastole, interventricular septum in end-systoles left ventricular wall end-diastoles left ventricular wall end-systoles right ventricular end-diastole diameter, aorta and left atrium at t Thoroughbred horse, Thoroughbred foul, Chriu horse and Chriu foul were measured. Experimental renal stone and enterolith of colon were observed by ultrasonography.

Analysis of Twinkling Artifacts Caused by Kidney Stones on Abdominal Ultrasound (복부 초음파 검사에서 신장결석으로 인해 발생되는 Twinkling 인공물에 관한 분석)

  • Kidney stones are largely classified into kidney stones, ureter stones, and urolithiasis depending on the location of their occurrence. Therefore, in this study, from January 2019 to June 2021, kidney stones found in 112 patients with flank pain or who visited for abdominal ultrasonography at a general hospital located in Daegu were diagnosed with urolithiasis. We wanted to investigate the effect on twinkling artifacts. As a result of the study, the incidence of twinkling artifacts due to kidney stones was relatively high in the longitudinal scan among the scan methods. As the number of kidney stones increased, the incidence of twinkling artifacts increased by 1.296 times (p<0.05). As the kidney stone size increased, the incidence of twinkling artifacts increased by 0.086-fold (p<0.05). It was found that the number and size of kidney stones are factors affecting twinkling artifacts. Since the effect of kidney stones on twinkling artifacts is related to the number and size of kidney stones, continuous attention should be paid to helping the detection of kidney stones by using variables affecting twinkling artifacts.

Emphysematous pyelonephritis associated with calcium oxalate uroliths detected on computed tomography in an awake dog

  • An 11-year-old female mixed-breed dog admitted due to anorexia and vomiting. Radiography showed 7.6-mm uroliths in the right proximal ureter and an enlarged kidney. Type 1 emphysematous pyelonephritis (EPN) was diagnosed using computed tomography (CT) in the awake dog. Right ureteronephrectomy was performed, and Escherichia coli was isolated. The stone was consisted of 80% calcium oxalate. At the 6-month follow-up, no complications were observed. This is the first report of EPN caused by calcium oxalate in a dog. In addition, we suggest performing CT in weak animals to diagnose EPN without anesthesia.

Changes in Oxalate and Phytate Concentrations During Soymilk Processing from the Seeds of Korean Soybean Cultivars

  • A high dietary oxalate intake may lead to calcium oxalate (CaOx) kidney stones in the gastrointestinal tract. Most soy foods contain high concentrations of oxalate and/or phytate. This study analyzed the changes in oxalate (Ox), phytate ($InsP_6$), and calcium (Ca) during soymilk processing from the seeds of Korean recommended soybean cultivars (cvs). The contents of Ox, $InsP_6$, and Ca in 21 cvs ranged from 14, 108, and 148 to 231, 279, and 246 mg/100 g of dry seed, respectively. Seven cultivars were selected from the 21 cvs by the distributions of Ox, $InsP_6$, and Ca. Then, each contents of soymilk prepared from the 7 cvs were determined. All contents were lower in soymilk than in seeds, but the Ox to $InsP_6$ ratios changed from varying ratios (0.1-0.8) to normal ratios (0.8-1.0) in all cvs except 'Paldalkong'. Consequently, during soymilk processing, the Ox content was decreased and the $InsP_6$ content was remained higher than the Ox content although the Ox was likely to be less reductive than the $InsP_6$. These results may provide better information for minimizing the risk of formation of Ox kidney stones due to consumption of soy products.

A Study on the Diagnosis of Urinary Stone Location by Abdominal Positioning Variations (요로결석 위치 진단에 대한 복부자세 변화에 따른 연구)

  • Patients who visit the emergency room with urinary stones have difficulty lying down in a supine position due to severe pain when performing the KUB test. The purpose of this study was to find methods to reduce the patients' pain and image distortion, and obtain medical images with high diagnostic values. After checking the standard classification of disease and cause of death, the target group consisted of 121 patients who had clearly distinguished stones from computed tomography. Patients with stones in the ureteralvesical junction were excluded. Qualitative image evaluation was performed by confirming the location of the stone in the computed tomography images. and evaluated the rate of visual discrimination of stones possible through KUB and abdominal plain X-ray. Quantitative image evaluation was performed on the KUB, abdominal plain X-ray images. The transverse process of the first lumbar vertebrae served as the standard point, and the length from this point to the lower part of the stone was measured. Results from looking at the rate of visual discrimination of stones possible through KUB and abdominal plain X-ray showed: 94 patients (77.6%) for KUB images and 91 patients (75.2%) for computed tomography images. The standard deviation for KUB and abdominal X-ray was 3 (2.4%). Comparing and analyzing the location from KUB images and abdominal plain X-ray images, the stone position was 10.1 mm in the kidney, 10.5 mm in the ureteropelvic junction, and 9.7 mm in the ureters. It was shown that the stone moved 10 mm on average with significant statistical difference (P<0.05). In cases where the pain is so severe that it is impossible to perform the test in the supine position, an alternative may be to check the stone position by performing a modified KUB test by having the patient stand in a vertical position. In the future, this will provide convenience to both the examiner and the patient when performing the examination, and it will contribute with its reproducibility.

Removal of Renal Calculi by Nephrotomy in Two Dogs (개에서 신장결석의 제거를 위한 신절개술)

  • A 10 years old female Poodle weighing 4.38 kg and a 10 years old female Maltese weighing 4.33kg were referred to veterinary leaching hospital of college of veterinary medicine, Konkuk uni-versity because of hematuria and severa anorexia. Preoperative hematological and srum chemical values revealed that counts of WBC were $78.4{\times}10^3$,/TEX>/ul in the Poodle and $21.6{\times}10^3$in the Maltese, concentrations of BUN 105 and 13 mg/dl, concentrations of creatinine 4.0 and 1.4 mg/dl, concentration of ALKP 542 U/L in the Poodle and concentration of ALF 37 U/L in the Maltese, respectively. Radiographic findings were observed radiopaque bilateral nephroliths in the Poodle and right renal cal- culus in the Maltese. Ultrasonographic findings were showed hyperechoic mass with severe acoustic shadowing, which were ]ocated in the center of both kidneys parenchyma in the Poodle and in right kidney in the Maltese. Bisectional nephrotomy in right kidney for removal of calculus, which had ,sin- gle stone and more large than splitted stones in lets kidney in the Poodle, was performed under isnf- lurane anesthesia in 100% $O_2$. The time from temporary occlusion of blood flow to suture of incisioned kidney after removal of renal calculi was elapsed 12 minutes. During operation and until 24hrs after operation was infusioned mannitol and tranexanic acid solution. There were normal urination with yellow color in the Poodle at 3 days after operation and in the Maltese at 1 day and good appetite after 7 days and 1 day after operation. At 7 days and 9 days after operation, hematolgoical and seam chemical values repealed that counts of WBC were $29.8{\times}10^3/u1 in the Poodle and 13.1 {\itmes}10^3$/ul in the Mal- tese, concentrations of BUN 13 and 10 mg/dl. concentrations of creatinine 1.3 and 1.1 mg/dl, con-centrations of ALT 34 and 97U/L, concentraion of AST 10 U/L in the Poodle, respectively, By ultra-sonographic view there was not observed hyperechoic mass and radippaque calculus in operated kid-ney in radiographic view.

A Case of Urinary Tract Infection in Calf with Hypospadias (요도하열 송아지에서 요로감염증례)

  • Hypospadias is a rare congenital malformation of the urethra reported in cattle. The urethral lumen of male indigenous Korean calf is open along the ventral aspect of the penis in the perineal region. Renal abscess and renal stone formation causing urinary tact infection has not been reported in hypospadia calves. The objective of this study was investigation for renal abscess and renal stone formation through autopsy. Histopathological examination and laboratory tests were performed. At autopsy, the pustules were formed on the right renal cortex, and the renal medulla abscess were formed on right and left part of the renal pelvis. Histopathological finding, this case was diagnosed as severe acute suppurative and necrotizing pyelonephritis, and severe chronic interstitial nephritis with fibrosis and moderate multifocal acute cystitis with edema. Milky exudate of the kidney has been identified as Actinomyces meyeri using the VITEK-2 system for identification of bacteria, and the stone has been identified as carbonate apatite using FT-IR system for quantification analysis. This case report describe the hypospadias complicated with urinary tract infection due to carbonate apatite stones and Actinomyces meyeri.

Antioxidant Activities of Ribes diacanthum Pall Extracts in the Northern Region of Mongolia

  • Ribes diacanthum Pall (RDP) is a member of the Saxifragaceae family. The plant is traditionally used in Mongolia for the treatment of various ailments associated with kidney and bladder's diseases, cystitis, kidney stone, and edema. This study was aimed to investigate antioxidant activities of different solvent extracts of whole Pall plants, based on ferric-reducing antioxidant potential (FRAP), 2,2'-azinobis(3-ethybenzothiazoline-6-sulfonic acid) ($ABTS{\cdot}+$) radical scavenging activity, 1,1-diphenyl-2-picrydrazyl ($DPPH{\cdot}$), and hydroxyl (${\cdot}OH$) radical scavenging activities. Additionally, total flavonoids and phenolic contents (TPC) were also determined. The ethyl acetate extract of RDP (EARDP) had a remarkable radical scavenging capacity with an $IC_{50}$ value of 0.1482 mg/mL. In addition, EARDP was shown to be higher in total phenolic and flavonoid contents than the methanol extract of RDP (MRDP). Moreover, the EARDP had the predominant antioxidant capacity, DPPH, hydroxyl, and ABTS radical scavenging activities and ferric reducing power. These results suggest a potential for R. diacanthum Pall extract as a functional medicinal material against free-radical-associated oxidative damage.

A Case of Idiopathic Renal Hypouricemia with SLC22A12 Gene Mutation Showing General Weakness and Incidental Renal Stone

  • Idiopathic renal hypouricemia (iRHUC) is a rare hereditary disease caused by a defect in urate handling of renal tubules. Type 1 renal hypouricemia (RHUC1) is diagnosed with confirmation of a mutation in SLC22A12 gene which encodes a renal urate-anion exchanger (URAT1). The majority of iRHUC patients are asymptomatic, especially during childhood, and thus many cases go undiagnosed or they are diagnosed late in older age with complications of hematuria, renal stones, or acute kidney injury (AKI). We report a case of a 7-year-old boy with subtle symptoms such as general weakness and dizziness and revealed hypouricemia and incidental nephrolithiasis. Homozygous mutations were detected in the SLC22A12 (c.774G>A) by molecular analysis. The present case suggests that fractional excretion of uric acid (FEUA) screening could be better followed by the coincidental discovery of hypouricemia, to prevent conflicting complications of iRHUC, even with normal urine uric acid to creatinine ratio (UUA/UCr), and sequential genetic analysis if needed.
