OPEN REDUCTION OF MANDIBULAR CONDYLE FRACTURES WITH AND WITHOUT DISCAL INJURY : A CASE REPORT (관절원판의 손상과 관련된 하악과두 골절의 관혈적 정복술의 치험례)

  • This is case report of open reduction of condylar fractures with or without discal injury. Many articles described open reduction and internal fixation of condylar fractures emphasize the method of bony reduction and fixation without mention of the position of the disc. So we like to present our cases of open reduction of condylar fractures in conjunction with reconstruction of the disc and associated structures. The pupose of this presentation is to emphasize several well - established principles of trauma management and management of temporomandibular joint injuries, such as 1) in situations of traumatic injury, anatomic restoration is the goal of treatment and, 2) anatomic alignment of the TMJ disc over the condyle is preferable to disc subluxation because the latter may lead to chronic pain, limitation of opening and degenerative arthritis. Although our case is small with short term follow up, we believe that open reduction and internal fixation of condylar fractures in conjunction with disc repair is a biologically sound approach to those fractures indicated for open surgery. Long term follow up will allow better judge the validity of this treatment approach to us.

RADIOGRAPHIC STUDY ON THE INTERRELATION BETWEEN BONE DEF ORMANS AND CONDYLAR HEAD POSITION IN THE TMJ ARTHROSIS (악관절증에서의 골형태이상과 과두위변화와의 상호관계에 관한 방사선학적 연구)

  • The author analysed the interrelation between the morphologic changes of bone Structures and the position of condylar head from the routine radiographs of 134 cases of the temporomandibular joint arthrosis. The frequencies of coincidence between the site of bone defrmity and condylar head positional change were examined. Also, the positional changes of condylar head and the direction of condylar movement in relation to the kind of bone deformities were observed. The results obtained were as follows; 1. In 52.65 per cent of total cases, the site, of positional change of condylar head was coincided with the site of bone deformans. The frequencies of the coincidence between these in the five items among seven items examined were above 53 per cent. From the results, it seems that the positional changes of condylar head were related with the morphological change of bone structure. 2. Eburnation and erosion. revealed frequently positional changes in the opening and closing position of the mouth, although in the early stages of the TMJ arthrosis. 3. In the bone deformans, during opene position of the mough 44.81 per cent of total cases revealed backword movement and 37.74 per cent showed forward movement. In closed position of the mouth, downward movement was revealed in 35.23 per cent of total cases and upward movement 28.41 per ,cent of total cases. 4. In the cases showing eburnation, the frequencies of coincidence between the site of positional change and bone deformans were 58.57 per cent of the total cases. that means it was high in the early stages of the TMJ arthrosis.

  • The purpose of this study was to evaluate the effect of vegetation on the cutting slope applied by vegetation system in the whole land. The engineering characteristics of slopes were investigated using the face mapping and physical tests. From the statistical methods, a relationship between a geologic stratum and degree of covering was independent. Therefore, the afforestation of slope was affected more by condition of vegetation as direction and opening of joints, and a topsoil state than by conventional classification of layer. It was concluded that the adjustable vegetation on the slope considering condition of geometric shape of slope, joint data and spontaneous type could make useful early afforestation.

Histological Observation of Osteochondrosis Dissecans Occurred in Mandibular Condyle (하악과두에 생긴 박리성 뼈연골증의 조직학적 관찰)

  • A 57 years old female complained of severe pain on the right temporomandibular joint (TMJ) area. Her right condyle had been partly resected under surgical operation 13 years ago due to condyle hypertrophy, thereafter she felt dull pain on TMJ area and recently the lesion became severely swelled and painful leading to cancer phobia. The present radiological views showed slightly enlarged and sclerosed condyle with increased radiopacity, but its articular sliding function was almost disable during mouth opening. The patient's TMJ lesion was carefully managed with conservative physiotherapy and pain treatment. The microsection of condyle head obtained from the previous operation was re-evaluated histologically, and it was finally diagnosed as osteochondrosis dissecans (OCD), exhibiting hyperplastic proliferation of cartilage in condyle head and marked vascular dilatation in epiphyseal zone. This abnormal cartilage tissue was distinguishable from normal cartilage tissue found in the peripheral cartilaginous cap of the same microsection. The involved cartilage cap showed thick hypertrophic chondrocyte zone with horizontal and vertical clefts accompanying diffuse hyaline degeneration. The superficial fibrous zone of cartilage cap was thickened and frequently peeled off, while lower hypertrophic zone of cartilage cap was highly cellular and proliferative. Consequently, the endochondral ossification became aberrant and resulted pre-mature apoptosis of many hypertrophic chondrocytes, followed by diffuse and mild inflammatory reaction in the underlying marrow tissue. Therefore, it was suggested that this hypertrophic condyle lesion, OCD, be differentiated depending on radiological and histological features from ordinary condyle hyperplasia, osteochondroma, and osteoarthritis, and that the pathological confirmation of OCD may provide a reliable modality for dental and medical treatment of chronic and painful TMJ lesion.

Development and Evaluation of the Korean Army's Ergonomic Flight Jacket (인간공학적 육군 비행재킷의 개발 및 평가)

  • This study used a preliminary survey to help develop an ergonomic flight jacket that is suitable for the working environment and mission performance. The results are as follows. The ergonomic sleeve pattern was designed with a forward 165° incline that considers a shoulder joint direction suitable for the motion; in addition, a closely design opening provided warmth and safety from fire. As a result of the dimensional suitability, pilots evaluated that sleeve length and total length of the developed flight jacket were a little long (p<.01), while flight engineers and crew evaluated that those of the developed flight jacket were appropriate (p<.01). Pilots evaluated that chest circumference and waist circumference were large (p<.05), while flight engineers and crews evaluated that those of the developed flight jacket were appropriate. The evaluation of the motion suitability indicated that pilots, flight engineers and crew found the developed flight jacket more comfortable than the current flight jacket (p<.05, p<.01, p<.001). The evaluation of the usability of pockets and penholders indicated that pilots, flight engineers and crew found the developed jacket easier to use (p<.01). The flight engineers and crew evaluated that the appearance of the developed flight jacket was better than the current flight jacket (p<.05). The results of this study show that the difference of environment and mission performance has a significant influence on evaluation; therefore, it is necessary to develop separate military uniforms that included a winter flight jacket to reflect the needs of each group.

A 10-year overview of chronic orofacial pain in patients at an oral medicine center in Iran

  • Background: Orofacial pain is defined as pain felt in the soft or hard tissues of the head, face, mouth, and neck. Chronic orofacial pain is often challenging to diagnose and difficult to treat. Due to the lack of available information about the prevalence and clinical form of orofacial pain, this study aimed to evaluate the characteristics of chronic orofacial pain in patients presenting at the Department of Oral Medicine of Shahid Beheshti Dental School between 2012 and 2022. Methods: In this retrospective study, we evaluated the files of 121 patients at the Department of Oral and Maxillofacial Diseases of Shahid Beheshti Dental School, which were completed during 2012-2022. We extracted the required information from these files. Results: In total, 121 files were included in the study (30 male, 91 female). The mean age of the patients was 43.68 ± 16.79 years. The most common diagnosis in patients with chronic orofacial pain was temporomandibular disorders (TMD) (55.3%). Among pain-related factors, psychological factors showed the highest frequency (30.5%). Opening and closing (43.8%) had the highest frequency among factors that increased pain, and the rest (6.6%) had the highest frequency among the factors that reduced pain. Most patients experienced unilateral pain over the masseter area. Most patients reported their pain intensity to be greater than 7 in the verbal analog scale (VAS). The most common symptom associated with pain was joint noise (37.1%). Conclusion: A ten-year retrospective evaluation of patient files showed that more than half of the patients with chronic orofacial pain had TMD.

Strain energy release rates in the curved spar wingskin joints with pre-embedded delaminations

  • Any pre-existed delamination defect present during manufacturing or induce during service loading conditions in the wingskin adherend invariably shows a greater loss of structural integrity of the spar wingskin joint (SWJ). In the present study, inter-laminar delamination propagation at the critical location of the SWJ has been carried out using contact and multi-point constraint finite elements available with commercial FE software (ANSYS APDL). Strain energy release rates (SERR) based on virtual crack closure technique have been computed for evaluation of the opening (Mode-I), sliding (Mode-II) and cross sliding (Mode-III) modes of delamination by sequential release of multi point constraint elements. The variations of different modes of SERR are observed to be significant by considering varied delamination lengths, material properties of adherends and radius of curvature of the SWJ panel. The SERR rates are seen to be much different at the two pre-embedded delamination ends. This shows dissimilar delamination propagation rates. The maximum is seen to occur in the delamination front in the unstiffened region of the wingskin. The curvature geometry and material anisotropy of SWJ adherends significantly influences the SERR values. Increase in the SERR values are observed with decrease in the radius of curvature of wingskin panel, keeping its width unchanged. SWJs made with flat FRP composite adherends have superior resistance to delamination damage propagation than curved composite laminated SWJ panels. SWJ made with Boron/Epoxy (B/E) material shows greater resistance to the delamination propagation.

Experimental Study on Static Behavior of Laterally Strengthened Spliced Prestressed Concrete Girder using Bending Moment Connector (휨연결재에 의해 횡방향으로 보강된 분절 프리스트레스트 거더의 정적거동에 관한 실험적 연구)

  • The main purpose of this study is to investigate the static behavior of spliced prestressed concrete girder with bending moment connector and lateral prestressing. Four (4) spliced girders and one (1) monolithic girder had been fabricated and tested to compare their static behaviors. Same geometry and materials are used to fabricate these spliced and monolithic girders. A monolithic girder and one (1) spliced girder without lateral bending connector are used as control specimens to estimate the performance of three (3) spliced girders with lateral bending connector. Deflections at the middle of girders have been measured for evaluation. Also, strains of the concrete at the middle of span and connection points have been measured. It was found from the result that laterally strengthened spliced girders showed improved ultimate strength but less stiffness compared to the monolithic girder at the ultimate state. Laterally strengthened spliced girder also showed improved strength as well as improved stiffness compared to the spliced girder without lateral strengthening.

Hindfoot Alignment Change after High Tibial Valgization Osteotomy in a Patient with an Ipsilateral Fused Ankle: A Case Report (족관절이 유합되어 있는 상태에서 동측 슬관절 고위경골외반절골술을 시행한 이후 후족부 정렬의 변화: 증례 보고)

  • Ankle arthrodesis was performed on a 55-year-old male patient with an active lifestyle who developed severe arthritis in the left ankle. Over the follow-up period, high tibial valgization osteotomy was conducted for painful medial knee joint arthritis with genu varum deformity to correct overall lower limb alignment from varus to valgus with respect to the fused ankle. This study was conducted to investigate how hindfoot alignment would change when the overall alignment of the lower limb shifted from varus to valgus with the ipsilateral ankle in a fused state. Conclusively, while no intrinsic changes in the hindfoot alignment were observed following the alteration of lower limb alignment, the hindfoot naturally adjusted to valgus deviation in response to limb valgus realignment. Moreover, symptoms changed in line with this adjustment. Given the absence of similar case studies or reports, a review of relevant literature is included to contribute to knowledge of this subject.

Clinical Effect of Avocado/Soybean Unsaponifiables on Osteoarthritis of the Temporomandibular Joint (측두하악관절의 골관절염 치료에서 Avocado/Soybean Unsaponifiables의 효용성)

  • The aim of this study was to investigate the beneficial effect of a daily dose of 300 mg of ASU taken for more than 3 months on the subjects diagnosed as osteoarthritis of temporomandibular joint by RDC/TMD. Total 68 outpatients(15-54y) of female except menopause in Orofacial Pain Clinics of the Pusan National University Hospital were randomly assigned to either an ASU group(n=36) or a placebo group(n=32). The pain, noise and limited mouth opening(LOM) were evaluated by numerical analogue scale(NAS, range 0-10) and maximum comfortable opening(MCO) were measured by milimeter scale. The difference of simple uptake rate(SUR) on bone scan, hot spot(HS) on coronal SPECT, condylar bony changes on CT between the ASU and placebo groups were compared to investigate the objective effect. The obtained results were as follows. 1. Comparison of the NAS of pain, noise, LOM and MCO before treatment and 3, 6 and 9 months after treatment showed no significant difference between the ASU and placebo groups. 2. Comparison of the NAS of pain, noise, LOM and MCO before treatment and 3, 6 and 9 months after treatment showed no significant difference between the ASU and placebo groups without splint treatment, but showed more increased MCO in the ASU group than the placebo group with splint treatment at 6, 9 months after treatment. 3. Comparison of the NAS of pain before treatment and 3, 6 and 9 months after treatment that the NAS of pain at first visit divided into two groups(above or below 6) showed more decreased the NAS of pain in the ASU group than the placebo group that the NAS of pain at first visit was above 6. 4. Comparison of the NAS of pain, noise, LOM and MCO during 6 months period showed improvement of clinical symptoms within group, but no significant difference between subjects. 5. The simple uptake ratio(SUR) on bone scan and hot spot(HS) on coronal SPECT showed more increased SUR and HS in affected side than non-affected side of the ASU and placebo groups. 6. Comparing of condylar bony changes, osseous remodeling were observed highest, osteophyte lowest in the affected and non-affected side of the two groups. After treatment, comparison of condylar bony changes were observed more decreased erosive features in the ASU group than the placebo group.
