Arthroscopic Rotator Cuff Repair: Serial comparison of outcomes between full-thickness rotator cuff tear and partial-thickness rotator cuff tear

  • Purpose: To compare the results of arthroscopic rotator cuff repair and subacromial decompression in partial thickness rotator cuff tear (PTRCT) with those in full thickness rotator cuff tear (FTRCT). Subjects and method: Of the 46 patients who were rested of the rotator cuff tear based on the operational findings, 42 patients who were able to receive a serial follow-up for 2 years were selected as the study subjects. The average age of the patients at the time of the operation was 55 years, and the mean duration of the follow-up was 34 months. The subjects included 22 cases of PTRCT and 20 cases of FTRCT. In terms of rotator cuff repair, the average number of tendon to tendon repair (TTR) was 1 in both PTRCT and FTRCT, and that of tendon to bone repair (TBR) was 1 and 3 in PTRCT and FTRCT, respectively. The average number of use of suture anchor was 1 and 2 in PTRCT and FTRCT, respectively. The level of shoulder pain and function of the subjects were measured using shoulder functional evaluation score of American shoulder and elbow society (ASES score) at before and 2 years following the operation. Results: At the final follow-up following the operation, PTRCT group showed changes in scores from 7.2 to 0.9 on average pain score and 34 to 91 on ASES score, whereas FTRCT group showed changes in scores from 7.6 to 1.2 on pain score and 29 to 88 on ASES score. There were no significant differences between the two groups (P > 0.05). The average range of motion of shoulder significantly increased in both groups at the final follow-up in comparison with the pre-operative time point. The evaluation at the final follow-up showed that 93% of the total subjects showed good or excellent results, and 95% showed satisfactory results from the procedure with regard to pain reduction and functional outcomes. Two cases of the 3 fair results were caused by acromioclavicular arthritis. Conclusion: It may be anticipated that arthroscopic rotator cuff repair and subacromial decompression may bring satisfactory post-operative outcomes in both PTRCT and FTRCT on pain relief and functional recovery. However, careful preoperative examination of the acromioclavicular joint is critical to avoid failures of these procedures.

소형견의 내측 슬개골 탈구 정복술 후 근육 내 전기자극의 재활효과 (Rehabilitative Effect of Intramuscular Electrostimulation after Reconstruction of Medial Patellar Luxation in Small Sized Dog)

  • 개의 내측 슬개골 탈구는 가장 많이 발생하는 관절 질환 중 하나로 정복 수술 후 넙다리빗근에 전기 자극을 실시하여 재활효과를 관찰하기 위하여 본 연구를 수행하였다. 본 연구는 내측 슬개골 탈구가 있는 소형견 12두를 대상으로 슬개골 탈구 정복술 후에 입원 2주 동안 수술한 후지의 앞쪽 넙다리빗근에 근육 내 전기자극 치료를 적용한 군(n = 8)과 대조군(n = 4)으로 나누어 관절가동범위, 근육 둘레, 파행 및 체중지지 정도를 측정하였다. 근육 내 전기자극을 실시한 군은 대조군에 비하여 관절가동범위, 근육 둘레, 파행점수 및 체중지지 정도 등 네 가지 사항 모두 수술 후 5일에는 유의성을 나타냈고(p < 0.01), 수술 후 10일에는 파행점수를 제외한 관절가동범위, 근육둘레 및 체중지지 정도의 항목에서 유의성을 보였다(p < 0.05). 이상의 결과로 볼 때, 소형견의 내측 슬개골 탈구 정복술을 실시한 후, 넙다리빗근에 전기자극을 실시하면 초기 재활치료에 도움이 많을 것으로 사료된다.

8개월된 유아 골육종 환자에서의 사지 보존술 (Osteosarcoma in an 8 Month-Old Infant treated with Limb Sparing Operation)

  • 원발성 악성 골종양중에서 가장 흔한 골육종은 10세에서 14세사이에 호발하며, 6세이하에서는 발병이 드문 것으로 알려져 있다. 문헌고찰에 의하면 지금까지 보고된 최소 연령의 골육종환자는 13개월된 여아로써 제2 중수골에 발생한 골육종이었다. 저자들은 생후 8개월된 남아의 우측 근위 경골에 발생한 골육종에 대한 치험례를 보고하고자 한다. 환아는 광범위 절제술 및 동종골 이식술을 시행하였으며 술 후 20개월에 동종골 이식편 근위 접합부에 내반 변형이 나타나서 변형교정 및 근위 비골 전이술을 시행하였다. 술 후 51개월의 추시관찰에서 잔류 병변은 보이지 않았으며 슬관절 운동범위는 15도에서 75도로 측정되었다. 영유아의 골육종은 매우 드물지만 감별진단으로 고려해야 할 것이며 수술적 치료에서 절단술 대신 사지 보존술도 가능하며 향후 발생할 수 있는 하지 부동에 대한 해결방안을 고려하여야 할 것으로 사료되었다.

중노동자에서 발생된 주관절 후방부의 충돌 병변 (The Impingement of The Posterior Elbow in The Heavy Workers)

  • 목적: 굴곡 구축과 신전제한이 발생한 중노동 작업자에서 발견된 주관절 주두 골극과 상완골 주두와 골곡 사이의 충돌을 발견하고 이 병변이 주관절 신전장애에 미치는 영향과 제거 효과를 파악하고자 하였다. 대상 및 방법: 무거운 물건을 들어올리는 중노동자에서 발생한 통증을 수반하는 신전제한을 갖은 주관절에 대하여 관절경을 시술하였다. 6명 7예를 대상으로 하였으며 환자의 평균 연령은 43 세였다. 내원 당시 동통을 동반한 관절운동 제한을 호소하였으며 평균 관절운동 범위는 굴곡구축 $17^{\circ}$, 후속 굴곡 $87^{\circ}$였다. 결과: 전 예에서 술 후 2개월 추시상 평균 굴곡구축 $2^{\circ}$($15^{\circ}$ 호전), 후속굴곡 $122^{\circ}$($35^{\circ}$ 호전)를 보였으며 술 후 1년 추시상 평균 굴곡구축 $3^{\circ}$($14^{\circ}$ 호전), 후속굴곡 $113^{\circ}$($26^{\circ}$ 호전)의 호전을 얻을 수 있었다. 얻어진 관절 운동 범위 내에서는 동통이 없는 상태를 보였으며, 합병증은 발견되지 않았다. 결론: 만성 반복성 외상에 의해 발생된 주관절 후방부의 퇴행성 강직에 대하여 충돌부위만을 선택적으로 제거하는 방식은 관절운동회복과 동통의 경감에 있어서 효과적이고 조기 재활이 가능한 술식으로 사료된다.

합병증이 병발된 슬관절 치환술에 시행한 관절경술의 효용성 (The Effectiveness of Arthroscopy in Complicated Knee Arthroplasty)

  • 목적: 슬관절 치환술 후에 발생한 합병증의 치료를 위해 시행한 관절경술의 결과를 알아보고 그 효용성을 확인하고자 하였다. 대상 및 방법: 1992년 5월부터 2008년 6월까지 본원에서 슬관절 치환술을 시행받은 후 합병증으로 인하여 관절경적 치료를 받았던 25예를 연구 대상으로 하였다. 25예 중 슬관절 전치환술을 시행받았던 경우가 19예, 부분치환술을 시행받았던 예가 6예였다. 모든 환자에서 관절경을 시행하기 전에 합병증의 원인을 밝히기 위해서 이학적 검사와 방사선 검사를 시행하였으며, 감염이 의심되는 경우 관절액 천자 및 혈액학적 검사를 추가로 시행하였다. 결과: 관절경술 시행 시 진단으로는 전치환술을 시행한 예에서는 감염 11예, 관절의 유착 및 섬유화로 인한 운동 제한 6예, 연부 조직의 충돌 2예 등이 있었으며, 부분 치환술을 시행한 예에서는 반월상 연골 파열, 유동성 치환물의 아탈구, 혈관절증, 관절 내 시멘트 유리체, 연부 조직 충돌 및 슬관절 강직으로 인한 운동 제한이 각각 1예 씩 총 6예가 있었다. 감염이 있던 11예 중 9예에서 관절경술만으로 치료가 되었고, 관절 강직이 있던 7예는 평균 슬관절 운동 범위가 술 전 $65^{\circ}$에서 최종 추시 시 $105^{\circ}$로 향상 되었으며, 나머지 예에서도 관절경술로 성공적인 치료 결과를 보였다. 결론: 슬관절 치환술 후 발생한 합병증의 치료로 시행한 관절경술은 적절한 환자의 선택 시 안전하고 효과적인 방법임을 확인할 수 있었다.

  • 최근 ITU-T/VCEG과 ISO/IEC MPEG은 공동으로 차세대 비디오 압축 표준 기술인 High efficient video coding (HEVC)를 제정하고 있다. HEVC는 H.264/AVC 표준 기술과 비교해 보면 매우 향상된 압축 효율을 보여 주고 있으며, 특히 Full HD 이상의 매우 큰 사이즈의 동영상 및 그에 따른 고화질 비디오 서비스를 주요 목표로 하고 있다. 그러나 품질과 압축 효율 향상을 위해 새로운 구조와 압축 도구들이 추가됨으로써 인코더의 연산 복잡도가 매우 증가되어 있으며, 이에 화질의 열화를 최소한으로 유지하면서 계산량을 감소시키는 연구가 필요해 지고 있는 상황이다. 본 연구에서는 화면 간 예측 시에 부호화 단위의 분할을 효율적으로 조기 종료하는 방법을 제안한다. 제안된 방법은 현재 코딩 단위 (Coding unit)의 움직임 벡터 정보, 율-왜곡 비용, 그리고 상위 코딩 단위에서의 개별 사이즈에 대한 평균 율-왜곡 비용값을 활용하여 분할을 조기에 중단한다. 실험 결과를 통하여 제안된 방법이 기존의 ECU 방법보다 약 10% 정도로 속도 향상이 있으며, HM 참조 소프트웨어와 비교 시 BD-rate 증가는 1.975% 정도로 매우 적게 나타남을 보인다.

슬관절 통증 환자의 한의학적 치료 호전도와 BMI 와의 상관성 (The Correlation between Korean Medical Treatment on Knee Joint Pain and BMI)

  • Objectives : This study was designed to assess the general distribution, clinical effectiveness of Korean medical treatment on knee pain and the correlation between Korean medical therapy on knee pain and BMI. Methods : This is an observational study. 65 patients admitted to Daejeon Jaseng Hospital of Korean Medicine with musculoskeletal disorders were observed from July, 2014 to July, 2015. They were analyzed according to sex, age, pain lesion, body mass index(BMI) and treatment efficacy. All patients received a combination of treatments during hospitalization, including acupuncture, pharmacopuncture, herbal medicines and physical therapy. A zero to ten numerating rating scale (NRS) assessing pain, Western Ontario and McMaster Universities Arthritis Index(WOMAC) index and Range of Motion(ROM) was used before and after treatments. Statistical correlations among assessment measurements were evaluated by examining the paired t-test and the Pearson's correlation coefficients. Results : The average BMI of all patients was $24.03{\pm}2.95kg/m^2$. According to clinical definition of obesity by Korean Society for the Study of Obesity, 33.8% of all patients was within normal weight, 1.5% was underweight, 33.8% was overweight, 24.6% was obesity and 6.2% was morbid obesity. For knee pain patients, NRS (Numeric Rating Scale) decreased from $6.28{\pm}1.63$ to $3.94{\pm}2.03$(p<0.001). WOMAC index decreased from $48.14{\pm}17.63$ to $40.37{\pm}18.28$(p<0.001). But, there were no significant correlations in statistics among BMI, knee pain index and knee pain index improvement. Conclusions : Korean medical combination treatment might be effective in reducing pain and improving functional disorders for patients with knee pain. This study further confirmed the efficacy of Korean medical treatment on knee pain. But, more studies on correlation between knee pain and obesity and development of assessment measurement are needed.

정상성인에 있어 배부근 스트레칭 운동이 척주 유연성에 미치는 영향 (Effects of Back Muscle Stretching on the Flexibility of Spinal Column of Normal Adults)

  • Purpose : The purpose of this study was to evaluate the effects of back muscle stretching on the flexibility of spinal column. Methods : The subjects were consisted of healthy adults (18 of females, 22 males ; mean aged 21.83) from 18 to 29. All subjects randomly assigned to the control group, back muscle stretching group. back muscle stretching group received back muscles stretching for 20 minutes per day and 3 times a week during 3 week period. Spine motion analyzer (Spinal Mouse) was used to measure the flexibility of spinal column. All measurement of each subjects were measured at pre-experiment, after 10 days, and after 21 days. Results : The results of this study were summarized below 1. The sacral tilt angle of the hip joint of control group, back muscle stretching group was no significantly differences at pre-experiment and after 10 days(p>0.5), but differency of each group occurred at after 21 days(p<0.5). the sacral tilt angle significantly increased at the back muscle stretching group rather than the control group. 2. The thoracic vertebral tilt angle of the control group, back muscle stretching group was no significantly differences at pre-experiment, after 10 days, after 21 days(p>0.5). 3. The lumbar vertebral tilt angle of the control group, back muscle stretching group was no significantly differences at pre-experiment, after 10 days, after 21 days(p>0.5). 4. The spinal tilt angle of control group, back muscle stretching group was no significantly differences at pre-experiment and after 10 days(p>0.5), but differency of each group occurred at after 21 days(p<0.5). the spinal tilt angle significantly increased at the back muscle stretching group rather than the control group(p>0.5). 5. The length of the spinal column of control group, back muscle stretching group was no significantly differences at pre-experiment and after 10 days (p>0.5), but differency of each group occurred at after 21 days(p<0.5). the length of the spinal column significantly increased at the back muscle stretching group rather than the control group(p<0.5). Conclusion : These data suggests that 3-week back muscle stretching improved the flexibility of sacrum, spinal column, and also improved spinal column lengthening. Additional randomized controlled trials to more fully investigate treatment effects and factors that may mediate these effects are needed.

상완신경총 손상 환자에서 시행한 견관절고정술 (Shoulder Arthrodesis in Brachial Plexus Injury Patient)

  • Purpose: To analyze the clinical and radiologic results of the shoulder arthrodesis in brachial plexus injury patients with flail upper extremity. Material and Method: From Aug 1978 to April 2008, 29 shoulders in 29 patients with brachial plexus injury with shoulder fusion, we evaluated 20 shoulders in 20 patients, more than 1 year follow up. The average follow-up period was 6.45years (range: 1year~24years). There were 13 men and 7 women, and the mean age at the time of trauma was 32.0years(2~65 years). The type of injury was a motorcycle accident in 11 patients, in car accident in 5, pedestrian accident in 3, and fall from a height in 1. The lesion of injury was root and trunk in 1 patient, trunk and cord in 1, trunk in 18. Surgery was performed on the whole arm type paralysis in 12 patients, lower arm type paralysis in 8 patients. The preoperative visual analog scale score was 8.7(7~10). When the trapezius and serratus anterior muscle were in function, operation was performed. 18 patients were processed to the additional operation. Gracilis free flap in 6 patients, neurotization in 3, Steindler flexor plasty in 6, and tendon transfer in 3 were performed. Fixation was conducted with cancellous screws in 13 patients, Knoles pins in 5, and cancellous screws and Knoles pins in 2. The position of the arthrodesis at operation was $28.5^{\circ}$($20~45^{\circ}$) in abduction, $30.3^{\circ}$($20~45^{\circ}$) in flexion, and $30.8^{\circ}$($20~40^{\circ}$) in internal rotation. Result: The follow up visual analog scale score was 3.4(0~7). Postoperatively, shoulder spica cast was applied for 15.3weeks(8-20weeks). The median time to bony union was 17.7weeks(9~28weeks). Average range of motion was $32.0^{\circ}$($15~40^{\circ}$) of abduction, $24.0^{\circ}$($10~40^{\circ}$) of flexion, and $18.5^{\circ}$($10~30^{\circ}$)of internal rotation. Conclusion: The shoulder fusion in brachial plexus injury patients is one of the good methods to relieve pain, improve the function and stabilize the flail shoulder joint.
