초음파를 통해 경과관찰한 비복근 부분파열 환자의 한의학적 치료: 증례보고 (Korean Medical Treatment for Partial Rupture of Gastrocnemius Muscle Observed by Ultrasonography: A Case report)

  • Gastrocnemius muscle partial rupture is a common muscle injury. This case is report on a patient with a gastrocnemius muscle partial rupture who was continuously observed with ultrasonography while receiving Korean medicine treatment. Acupuncture, pharmacoacupuncture, herbal medicine, physical therapy and rehabilitation treatment were performed on a patient diagnosed with gastrocnemius partial rupture. The improvement of symptoms was evaluated using Numeric Rating Scale (NRS), Range of Motion (ROM) of ankle, circumference of calf and size of hematoma by using ultrasonography. NRS decreased more than 90%, ROM of ankle joint gradually improved, calf circumference improved, and hematoma reduced by more than 90% when measured by ultrasonography. In this study, gastrocnemius muscle partial rupture was observed with ultrasonography, and it is considered that the Korean medical treatment is useful for gastrocnemius muscle partial rupture.

외상으로 인한 슬개골 골절을 가진 환자의 국소진동을 동반한 물리치료 중재가 신체기능에 미치는 영향: 사례군 연구 (The Effects of Physical Therapy Intervention with Local Vibration on Physical Function in Patients with Traumatic Patella Fracture: Case Series)

  • Background: The purpose of this study was to investigate the effects of physical therapy intervention with local vibration on the physical function of patients with traumatic patella fractures. Methods: This study recruited 6 subjects who had suffered traumatic patella fractures. The study was conducted for an average of 12.8 weeks. Before the treatment (2 weeks post-surgery), they were evaluated using the numeric pain rating scale (NPRS), the Korean-version of the impact of event scale-revised (IES-R-K), pressure pain threshold (PPT), range of motion (ROM) of the knee joint, and the Korean knee injury and osteoarthritis outcome score (K-KOOS) and were reevaluated after 7 and 12 weeks, post-surgery. This study was conducted according to ORIF Patella Fracture Post-Operative Rehabilitation Protocol after applying local vibration. The protocol consists of Phases 1~5 and this study has been applied from phase 2. Results: A comparison of the performance of the participants before and after the intervention showed a decrease in NPRS (9.83±.41→4.83±.98), IES-R-K (68.67±2.73→23.83±2.40), and K-KOOS (Function, Daily living: 70.5±5.96→34.0±3.35, Function, Sports and Recreational activities: 22.83±2.32→10.77±1.37, Quality of Life: 19.33±7.33→7.33±.52) scores. And the ROM (Knee flexion: 30.0±4.47°→128.73±3.6°, Knee extension: -6.83±2.48°→-1.33±1.03°) and PPT (9.67±.52kg/cm2→22.44±2.33kg/cm2) scores increased. Conclusion: These results show that physical intervention with local vibration using a Blackroll® booster and head can help to improve pain, physical function, and psychological status. Also, it was possible to select interventions depending on the patient's condition and the desired goal, using physical intervention with the Blackroll® booster technique.

관현악단 전문 연주자와 대학생 연주자의 연주 관련 근골격계 증상의 발생 특성에 관한 비교 (A Comparative Study on the Characteristics of Performance-related Musculoskeletal Symptoms between Orchestra Performers and University Student Performers)

  • Background: Instrument players are exposed to occupational musculoskeletal symptoms due to constant movement and strain to meet the needs of their audience. This can cause dissonance between the player and the instrument and can cause medical problems in the musculoskeletal system such as pain, tendinitis, muscle spasms, and joint hyperextension from overuse of body parts. Objective: The purpose of this study is to comparatively analyze the characteristics and occurrence of musculoskeletal symptoms in professional and student players, and to find out the awareness of the need for health care professionals and special exercise programs for orchestra players. Methods: The subjects were professional orchestral musicians 191 from the three symphony orchestras and music students 209 from the four universities volunteered to participate in this study; 393 subjects (98.25%) completed the questionnaire. symptom prevalence and related factors of playing-related musculoskeletal disorders (PRMDs) were surveyed with a self-administered questionnaire. Standardized Nordic Questionnaire was complemented by adding a visual-among scale and used to diagnose PRMD. Results: High prevalence of musculoskeletal complaints in both professional musicians (85.30%) and student musicians (82.30%). Professional musicians and student musicians also suffered the highest problem in shoulder and neck complaints. Significant differences were found between groups that recognized of necessity for specialized health professionals, professional organizations, and special programs for musicians. Conclusions: Professional musicians and student musicians, like orchestra players, are already exposed to overuse syndrome. However, student musicians lack awareness of the risk of injury compared to orchestra players. We suggest that an appropriate treatment management program for the prevention and early treatment of musculoskeletal system damage should be provided to performers at an early stage.

Comparison of Tibialis Anterior Muscle Thickness with 4 Different Toe and Ankle Postures: Ultrasonographic Study

  • Purpose: Ankle dorsiflexion is an essential element of normal functions, including walking, activities of daily living and sport activities. The tibialis anterior (TA) muscle functioned as a dorsiflexor and as a dynamic stabilizer of the ankle joint during walking and jumping. This study aimed to compare TA muscle thickness using ultrasonography according to the four different toe and ankle postures for the selective TA strengthening exercise. Methods: This study were recruited 26 (males: 15, females: 11) aged 20-30 years, with no injury ankle and calf in the medical history, had normal dorsiflexion and inversion range of motion (ROM). The thickness of the TA muscle was measured by ultrasonography in the four different toe and ankle postures: 1. Ankle dorsiflexion with all toe extension and ankle inversion (ITEDF); 2. Ankle dorsiflexion with all toe flexion and ankle inversion (ITFDF); 3. Ankle dorsiflexion with all toe extension and neutral position (NTEDF); 4. Ankle dorsiflexion with all toe flexion and neutral position (NTFDF). One-way repeated analysis of variance (ANOVA) and Bonferroni correction were used to confirm the significant difference among conditions. The level of statistical significance was set at α=0.01. Results: TA muscle thickness with ITFDF was significantly greater than in any other ankle positions, including ITEDF, NTFDF, and NTEDF (p<0.01). Conclusion: Among the four toe and ankle postures, isometric contraction in ITFDF postures showed the greatest increase in thickness of TA rather than ITEDF, NTEDF, and NTFDF postures. Based on these results, ITFDF can be recommended in an efficient way to selectively strengthen TA muscle.

하지 화상 환자의 재활치료를 위한 보행보조 로봇훈련의 선행연구 (The Pilot Study of Robot-Assisted Training for the Lower Extremity Rehabilitation of Burn Patients)

  • Purpose: Scar contracture influence the outcome of burn patients significantly. This study aims to investigate the feasibility of robot-assisted training for the lower extremity rehabilitation of burn patients. Methods: This pilot study was conducted on 7 burn patients for 8 weeks between January 2019 and November 2019. Two of 7 patients withdrew from this study because one had skin abrasion on the legs which thigh fastening devices were applied on and the other was not participate in the assessment at 4 weeks after training. Final 5 patients received gait training with SUBAR® and numeric rating scale (NRS), 6-minutes walking test, and range of motion in flexion and extension of knee and ankle joint were evaluated before training, 4 weeks and 12 weeks after training. Results: The subjects had a mean age of 51.8±98 years, mean total burn surface area of 30.8±13.7%, mean duration from injury to 1st assessment of 102.8±39.3 days. Anyone of 5 patients did not have musculoskeletal or cardiovascular side effects such as increased or decreased blood pressure or dizziness. The significant improvement in NRS, gait speed, and range of motion in knee extension and ankle plantarflexion after robotic training (all P<0.05). Conclusion: Robot-assisted training could be feasible for the rehabilitation of burn patients and it could improve muscle strength and range of motion in lower extremities, and gait function.

전위성 치골 골절의 경피적 고정술: 술기보고 (Percutaneous Screw Fixation in a Displaced Pubic Fracture: Technical Note)

  • 골반골 골절은 고에너지 손상이며 주변 조직 및 장기의 손상이 동반되는 경우가 흔하다. 골반 외상 환자에서 적절한 치료가 제공되지 않으면 사망률이 높아지므로 수상 초기에 적극적인 치료를 요하게 된다. 하지만 광범위한 수술을 시행하기에는 환자의 상태가 허락되지 않는 경우가 많아서 최소 침습적인 수술을 고려하게 된다. 치골의 경피적 고정은 피부 절개를 크게 하지 않고도 골반의 전방부 안정성을 획득할 수 있는 방법으로 비교적 쉽게 적용할 수 있어 여러 문헌에서 소개되고 있다. 그러나 대부분의 문헌에서는 비전위성 골절에 대한 나사 고정을 기술하고 있으며 전위성 골절을 경피적 나사 고정으로 치료할 때 비구의 관절면을 침범하지 않는 위치로 나사를 삽입하는 것이 어려운 점은 간과되고 있다. 저자는 치골의 전위성 골절의 경피적 나사 고정 시 구부러진 유도핀을 이용하여 관절면의 침범을 피하면서 유관나사를 쉽게 삽입할 수 있었기에 본 술식을 소개하고자 한다.

Arthroscopic release in adhesive capsulitis of the shoulder: a retrospective study with 2 to 6 years of follow-up

  • Background: This study aimed to evaluate the response rate to arthroscopic release treatment in adhesive capsulitis of the shoulder (ACS) for patients with refractory to conservative treatment. Methods: In this retrospective study, 51 patients (age mean, 49.1±5.6 years) with unilateral adherent capsule underwent arthroscopic releasing surgery for the shoulder capsule. Etiologies of the ACS in 30 patients were idiopathic: 10 patients were affected after surgery and 11 patients following trauma. The patients were evaluated in terms of shoulder function, satisfaction rate, pain intensity, and joint range of motion (ROM) based on a Constant score, a Simple Shoulder Test, the visual analog scale, and four movements, respectively. Results: The mean Constant score before surgery was 48.2±3.5 and reached 74.4±6 and 77.0±6.3 at 6 months and the final follow-up, respectively (p<0.001). The mean scores of pain intensity, a Simple Shoulder Test, and ROM showed significant improvement at all follow-ups (p<0.001). Sex, age, and diabetes did not have any significant effect on patient recovery. However, patients who experienced ACS after surgery had poorer results than others at all follow-up points. Conclusions: Arthroscopic releasing surgery of the shoulder in patients with ACS refractory to conservative treatment produces rare complications and an effective injury response. It seems that patients suffering ACS following surgery have a weaker response to the treatment.

족삼리 양릉천 전침 자극 후 흰쥐 통증 모델에서 microRNA의 차등 발현 (Differential Expression of microRNAs Following Electroacupuncture Applied to ST36 and GB34 in Rat Models of Chronic Pain)

  • Objectives : Some acupoints are commonly utilized to treat a variety of diseases. The acupoints appear to have a wide range of effects caused by several mechanisms. The purpose of this study is to investigate into the potential role of microRNAs (miRNAs) in the multipotent effects of individual acupoint stimulation. Methods : We examined the miRNA expressions in the dorsal root ganglia (DRG) of neuropathic or inflammatory pain rats following ST36 and GB34 electroacupuncture (EA) stimulation. Neuropathic pain was induced by L5 spinal nerve ligation. Inflammatory pain was induced by knee joint injection of Complete Freund's adjuvant (CFA). EA was given under gaseous anesthesia with the same parameters (1mA, 2Hz, 30 min) in 5 consecutive days. Pain behaviors and miRNA expressions were analyzed. Results : In rats with neuropathic and inflammatory pain, EA treatments significantly enhanced the paw withdrawal threshold and weight-bearing force. After nerve injury, 36 miRNAs were upregulated in the DRG of neuropathic rats, while EA downregulated 10 of them. Furthermore, 14 miRNAs were downregulated following nerve damage, while one was increased by EA. 15 miRNAs were increased in the DRG of inflammatory rats following CFA injection, while 5 were downregulated by EA. Furthermore, 17 miRNAs were downregulated following CFA injection, while 7 were increased by EA. The miRNAs rno-miR-335, rno-miR-381-5p, rno-miR-1306-3p, and rno-miR-1839-3p were regulated by EA in both models. Conclusions : In two pain models, EA applied to ST36 and GB34 regulated miRNA expression differently. There appeared to be both acupoint-specific and non-specific miRNAs, and miRNA regulation of differential protein expression may modulate a variety of EA mechanisms.

Comparative Effects of Novel Modified Sleeper and Cross-body Stretching on Scapular Anterior Tilting and Shoulder Internal Rotation in Subjects With Anterior Tilted Scapular and Shoulder Internal Rotation Deficits

  • Background: Posterior capsule tightness (PCT), commonly seen in overhead athletes, is a soft tissue adaptation that is also noted in non-throwers. PCT is associated with scapular and humeral kinematic alterations, significant restriction of shoulder internal rotation (IR) range of motion (ROM), and significant scapular anterior tilting. Sleeper and cross-body stretches (CBS) are suggested for PCT and IR deficits, and have been modified since introduction. A novel modified sleeper stretch (NMSS) was designed in this study to prevent the risk of anterior translation of the humeral head. Though the effects of posterior shoulder stretching exercise have been widely studies, to the best of our knowledge, no previous studies have investigated the effectiveness of posterior shoulder exercises in decreasing scapular anterior tilting. Objects: To compare the immediate effects of two posterior shoulder stretching exercises (NMSS and CBS) on scapular anterior tilting and shoulder IR ROM. Methods: Thirty-two subjects with anteriorly tilted scapula and IR deficits [mean age: 24.3 ± 2.5 years; 15 males and 17 females] participated in this study. Subjects were randomly assigned to either the NMSS or CBS groups. Scapular anterior tilting (at rest and at shoulder 60° active IR) and shoulder IR ROM were measured before and immediately after intervention. Results: Scapular anterior tilting significantly decreased, while the shoulder IR ROM significantly increased in both groups. However, there was no significant group-by-time interaction effect or significant difference between the groups. Conclusion: Both stretching exercises were effective in restoring shoulder IR ROM and decreasing scapular anterior tilting.

Effects of Flossing Band Technique, Static and Dynamic Stretching on Hamstring on Knee Range of Motion, Muscle Activity, and Proprioception

  • Purpose: This study aimed to compare the lasting effects of the flossing band technique, dynamic and static stretching on hamstring on range of motion (ROM), muscle activity, and proprioception to identify the most effective pre-exercise method for preventing injuries. Methods: Thirty participants were randomly assigned to the flossing band (FB), dynamic stretching (DS), and static stretching (SS) groups, with 10 subjects in each. Measurements included muscle activity of the biceps femoris vis surface electromyography, knee ROM and proprioception during active knee extension and flexion using a smart joint goniometer. Assessments were conducted before, immediately after, 15, and 30 minutes after each intervention. Results: Proprioception showed no significant differences among groups at any time point. Significant differences in knee ROM were observed in the FB group (except between 15 and 30 minutes after), DS group (except between immediately after and 15 minutes after, and between 15 and 30 minutes after), and SS group (except between before and 15 minutes after, and between before and 30 minutes after). Muscle activity in the FB (except between before and 30 minutes after, and between 15 and 30 minutes after) and SS (between before and immediately after, between immediately after and 30 minutes after, and between 15 and 30 minutes after) groups showed significant differences, while the DS group exhibited no significant changes. Conclusion: Although direct comparisons did not establish superiority, within-group analyses indicated that the flossing band technique exhibited longer-lasting effects than dynamic and static stretching, providing valuable insights for injury prevention program design.
