경도의 슬관절 십자 인대 손상환자에 대하여 시행한 열 위축술 (Radiofrequency Shrinkage Method for Minor Degree of Cruciate Ligament Injury of knee joint)

  • 목적: 재건술의적응이되지않는전,후방십자인대손상환자에서radiofrequency 방식을이용한열위축술을시행하여술기및단기추시결과를보고하고자한다. 대상및방법 : 재건술의적응이되지않는전방십자인대9예, 후방십자인대5예를대상으로하였으며평균추시기간은6개월이었다. 결과 : 일상생활에서느꼈던불안정성은감소됐으며파행이나통증등이현저하게호전되었다. 다만후방십자인대에서시행한경우시간이지남에따라다시슬관절이완이증가하는소견을관찰할수있었다. 결론 : Radiofrequency를이용한열위축술은많은활동을요하는젊은사람의슬관절십자인대손상에서, 특히경도의부분파열및수술의적응이되지않는이완환자에서증상을보일경우시도해볼수있는술식중의하나로추천할만하다. 그러나본결과는단기추시이므로장기추시와대조군연구가요할것으로사료된다.

족부에서 발생한 장딴지신경의 신경외막 결절종 (Epineural Ganglion Cyst of the Sural Nerve at the Foot: A Case Report)

  • Purpose: Ganglion cysts of peripheral nerve are uncommon. Ganglion cysts located within the nerve and extraneural ganglia that cause symptomatic nerve compression have been reported. We report an unusual case of epineural ganglion cyst confined to the epineurium of the sural nerve at the foot. Methods: A 45-year-old woman was referred because of a mass on the foot. She had six months' history of pain and numbness in the right small toe. During the examination of the lesion, multinodular cystic mass was identified arising from the epineurium of the sural nerve. The nerve fascicles were compressed by the cyst, but the cyst wall clearly did not invade the fascicle. With the aid of surgical microscope, the epineural cyst was completely excised along with epineural tissue to which it was attached, and the sural nerve was decompressed. There was no relationship between the cyst and either the joint capsule or tendon sheath. Since the cyst was on the periphery of the nerve it was possible to remove the cyst intact without damaging the underlying fascicles. Results: The postoperative course was uneventful. Pathologic examination showed a ganglion cyst with a degenerated collagen fibers and contained a yellowish, jelly-like mucinous substance. No neural elements were identified within the cystic wall. Her sensory impairment improved progressively. At the 15 months follow-up, she was asymptomatic with no neurological deficits. Conclusion: Rarely, ganglion cysts can involve peripheral nerves, leading to varing degrees of neurological deficits. Intraneural intrafascicular ganglion may be difficult to separate from the neural elements without nerve injury. Epineural ganglion, subcategorized as intraneural extrafascicular ganglion, can be removed without damage to the underlying nerve.

토끼 모델에서 굴근 인대 봉합 후 가딕스®의 유착 방지 효과 (Adhesion Prevention with Guardix® (Sodium Hyaluronate) After Flexor Tendon Repair in Rabbits)

  • Purpose: Adhesion is the most common and troublesome complication after repair of flexor tendon injury. Recently, use of sodium hyaluronate derivatives for adhesion prevention is increasing. A commercial product, Guardix$^{(R)}$, sodium hyaluronate(NaHe) combined with carboxymethylcellulose(CMC) has been newly developed as a preventive material for adhesion. We have investigated its effect in rabbits. Methods: Twenty seven male New Zealand white rabbits were operated under ketamine anesthesia. After tendon repair in zone II of the hind paw, Guardix$^{(R)}$(experimental group) or normal saline(control group) was administered. Biomechanical tests were performed to estimate adhesion formation at 2, 4, 8, and 12 weeks after the operation. Maximum tensile load to flex the distal interphalangeal joint 50 degree from its resting state(MTL50) was measured, depicting the amount of adhesion formed. Subsequently, breaking strength was assessed. Results: There were no postoperative complications such as infection, wound dehiscence, or hematoma. MTL50 was significantly lower in the experimental group than in the control group at 4, 8, 12 weeks (p<0.05). Mean value of MTL50 was 6.64N in the experimental group and 28.53N in the control group at 12 weeks after surgery. There were no significant differences in breaking strength. Conclusion: Our results indicate that Guardix$^{(R)}$ is helpful in reducing adhesion formation and does not interfere with normal healing processes of the tendon.

서울시 초.중.고등학교에서 5년간(2000~2004년) 발생한 학생의 비의도적 손상 및 보상급여비 현황 (Incidence of Nonfatal Unintentional Injuries among Students, and Compensation Payment for Five Years (2000~2004) in School, Seoul)

  • Purpose: The purpose of this study was to identify incidence of nonfatal unintentional injuries (accidents) among students, and to investigate compensation payment for five years (2000~2004) in schools located in Seoul. Methods: Subjects were 14,783 students injured among elementary, middle and high school students. The accumulated data for 5 years (2000~2004) was from the Seoul School Safety and Insurance Association. Chi-square, ttest, logistic regression, and MANCOVA were conducted. Results: The most frequent accidents occurred among males (73.2%), and in special school students (3.86 per 1,000) followed by middle school students (3.05 per 1,000), in May (0.25 per 1,000) followed by June and October, in physical education classes (0.73 per 1,000) followed by special activities (0.40 per 1,000), recess (0.31 per 1,000) and lunch (0.29 per 1,000). Fractures (0.86 per 1,000) were followed by mild injuries (0.39 per 1,000), joint and ligament injuries (0.31 per 1,000), and teeth injuries (0.26 per 1,000). After adjusting for potential confounding variables, the highest means of final compensation payment was for burns (810 thousand KRW) followed by the teeth injuries (506 thousand KRW), cleaning injuries (550 thousand KRW) followed by injuries incurred during special activities (510 thousand KRW) and injuries incurred at special schools (556 thousand KRW). Conclusion: In school, the highest incidences of nonfatal unintentional injuries were in special schools, among males, fractures and physical education. However, the highest compensation payments were for burns, and injuries taking place at special schools and during cleaning hour. Proper health education including teaching healthy habits for safety/injury prevention is needed to prevent injuries and decrease compensation.

홈 트레이닝을 위한 운동 동작 분류 및 교정 시스템 (Pose Classification and Correction System for At-home Workouts)

  • 홈 트레이닝을 하는 사람들은 전문적인 대면 지도가 없기 때문에 잘못된 자세로 동작을 하여 신체에 무리가 올 수 있다. 본 연구에서는 자세 예측 모델과 다층 퍼셉트론을 이용하여 사용자의 자세를 교정 해주는 "영상 데이터 기반 동작 분류 및 자세 교정 시스템"을 제안한다. 자세 예측 모델로 뼈대 정보를 예측한 후 심층 신경망을 이용하여 어떤 운동 동작인지를 분류한 뒤, 올바른 관절의 각도를 알려주며 교정이 이루어진다. 이 과정에서 동작 분류 모델의 성능을 향상시키기 위해 연속적인 프레임들의 결과를 고려하는 투표 알고리즘을 적용하였다. 다층 퍼셉트론 기반 모델을 자세 분류 모델로 사용했을 때 0.9의 정확도를 가진다. 그리고 투표 알고리즘을 통해 분류 모델의 정확도는 0.93으로 향상된다.

광범위한 안면외상 환자에서의 미세술기를 이용한 재건술 (Microsurgical reconstruction of posttraumatic large soft tissue defects on face)

  • Our faces can express a remarkable range of subtle emotions and silent messages. Because the face is so essential for complex social interactions that are part of our everyday lives, aesthetic repair and restoration of function are an important tasks that we must not take lightly. Soft-tissue defects occur in trauma patients and require thorough evaluation, planning, and surgical treatment to achieve optimal functional and aesthetic outcomes, while minimizing the risk of complications. Recognizing the full nature of the injury and developing a logical treatment plan help determine whether there will be future aesthetic or functional deformities. Proper classification of the wound enables appropriate treatment, and helps predict the postoperative appearance and function. Comprehensive care of trauma patients requires a diverse breadth of skills, beginning with an initial evaluation, followed by resuscitation. Traditionally, facial defects have been managed with closure or grafting, and prosthetic obturators. Sometimes, however, large defects cannot be closed using simple methods. Such cases, which involve exposure of critical structures, bone, joint spaces, and neurovascular structures, requires more complex treatment. We reviewed and classified causes of significant trauma resulting in facial injuries that were reconstructed by microsurgical techniques without simple sutures or coverage with partial flaps. A local flap is a good choice for reconstruction, but large defects are hard to cover with a local flap alone. Early microsurgical reconstruction of a large facial defect is an excellent choice for aesthetic and functional outcomes.

Korean Medical Treatment Including Miniscalpel Acupuncture for Patients After Rotator Cuff Tear Surgery: A Report of Two Cases

  • This study aimed to report the effect of Korean medical treatment including miniscalpel acupuncture on 2 patients who underwen tsurgery for rotator cuff tear. They were treated for almost 4 weeks at the Department of Acupuncture and Moxibustion, Dong-Eui University Korean Medicine Hospital. Visual analog scale (VAS), and range of movement (ROM) were used to evaluate treatment effects. In both patients, shoulder pain and restriction of shoulder joint movement improved after miniscalpel acupuncture treatment. In Case 1, shoulder pain decreased from a VAS score 8 to a VAS score 3, and ROM of the shoulder improved from flexion $100^{\circ}$ to $160^{\circ}$, extension $10^{\circ}$ to $30^{\circ}$, abduction $90^{\circ}$ to 1$30^{\circ}$, adduction $10^{\circ}$ to $40^{\circ}$, internal rotation $10^{\circ}$ to $50^{\circ}$, and external rotation $10^{\circ}$ to $50^{\circ}$. In Case 2, shoulder pain, which was rated a VAS score 8 at first-visit, disappeared, and ROM of the shoulder recovered to normal range. These results suggest miniscalpel acupuncture may contribute to the recovery process after rotator cuff tear surgery.

Evaluation of the Relationships Between Kellgren-Lawrence Radiographic Score and Knee Osteoarthritis-related Pain, Function, and Muscle Strength

  • Background: Knee osteoarthritis (OA) diagnosis using Kellgren-Lawrence scores is commonly used to help decision-making during assessment of the severity of OA with assessment of pain, function and muscle strength. The association between Kellgren-Lawrence scores and functional/clinical outcomes remains controversial in patients with knee OA. Objects: The purpose of this study was to examine the relationships between Kellgren-Lawrence scores and knee pain associated with OA, function during daily living and sports activities, quality of life, and knee muscle strength in patients with knee OA. Methods: We recruited 66 patients with tibiofemoral knee OA and determined knee joint Kellgren-Lawrence scores using standing anteroposterior radiographs. Self-reported knee pain, daily living function, sports/recreation function, and quality of life were measured using the knee injury and OA outcome score (KOOS). Knee extensors and flexors were assessed using a handheld dynamometer. We performed Spearman's rank correlation analyses to evaluate the relationships between Kellgren-Lawrence and KOOS scores or muscle strength. Results: Kellgren-Lawrence scores were significantly negatively correlated with KOOS scores for knee pain, daily living function, sports/recreation function, and quality of life. Statistically significant negative correlations were found between Kellgren-Lawrence scores and knee extensor strength but not flexor strength. Conclusion: Higher Kellgren-Lawrence scores were associated with more severe knee pain and lower levels of function in daily living and sports/recreation, quality of life, and knee extensor strength in patients with knee OA. Therefore, we conclude that knee OA assessment via self-reported KOOS and knee extensor strength may be a cost-effective alternative to radiological exams.

발의 형태학적 특성에 따른 시공간 보행 변인과 하지의 기능 및 통증 차이 (Differences in Spatiotemporal Gait Parameters and Lower Extremity Function and Pain in Accordance with Foot Morphological Characteristics)

  • Objective: The aim of this study was to investigate the differences in spatiotemporal gait performance, function, and pain of lower-extremity according to foot morphological characteristics. Method: This case-control study recruited 42 adults and they were classified into 3 groups according to foot morphology using navicular-drop test: pronated (≥ 10 mm), normal (5~9 mm), and supinated (≤ 4 mm) feet. Spatiotemporal gait analysis and questionnaires including Foot and Ankle Ability Measure activities of daily living / Sports, Western Ontario and McMasters Universities Osteoarthritis Index, Lower Extremity Functional Scale, International Physical Activity Questionnaire, and Tegner activity score were conducted. One-way analysis of variance was used for statistical analysis. Results: The pronated feet group showed longer loading response and double limb support in both feet and increased pre-swing phase in non-dominant feet. The supinated feet group demonstrated a longer swing phase in non-dominant feet and single limb support in dominant feet. However, there was no significant group difference in function and pain of knee joint and lower-extremity between groups. Conclusion: Our results indicated that abnormal spatiotemporal gait performance according to foot morphology. Although there was no difference in lower extremity dysfunction and pain according to the difference in foot morphology, they have the possibility of symptom occurs as a result of continuous participation in activities of daily living and sports. Therefore, individuals with pronated or supinated foot should be supplemented by utilizing an orthosis or training to restore normal gait performance.

Effects of Flossing Band Exercise on Range of Motion, Vertical Jump in Taekwondo Demonstration Athletes with Functional Ankle Instability

  • 이 연구는 플로싱밴드 운동이 기능적 발목 불안정성 태권도 시범단 선수들의 관절가동범위와 수직점프에 어떠한 영향을 미치는지 분석하여 플로싱 밴드의 효과를 알아보는데 목적이 있다. D 대학교에 재학 중인 남성 태권도 시범단 선수 21명을 대상으로 기능적 발목 불안정성 그룹(FAIG, n=7명), 경미한 발목 불안정성 그룹(MAIG, n=7명), 발목 안정성 그룹(ASG, n=7명)으로 구분하였다. 모든 그룹은 일회성 플로싱밴드 운동 프로그램을 실시하였다. 그 결과 FAIG의 발등굽힘(Dorsi flexion, p<.01), 안쪽번짐(Inversion, p<.01), 가쪽번짐(Eversion, p<.05)에서 유의하게 관절가동범위가 증가하였으며, 수직점프(p<.01)에서도 유의한 증가를 보였다. FAI를 가진 태권도 시범단 선수들의 플로싱밴드 운동은 발목의 관절가동범위와 수직점프를 증가시킴으로 경기력 향상과 발목손상의 위험을 줄일 수 있는 하나의 중재방법이 될 수 있는 가능성을 확인하였다.
