아동의 개구운동과 두개안면골격형태의 상관성 -측모두부방사선 계측법적 연구- (A cephalometric study on the relationship between mandibular opening movement and morphology of craniofacial skeleton)

  • 최대 개구운동시 하악골의 개구능력과 과두의 활주 운동 양상을 관찰하고 두개안면골격 형태요소와의 상관성을 알아보기 위해 경희의료원 교정과에 내원한 10세 전후의 아동 68명의 최대감합위 및 최대개구위 측모두부방사선사진을 분석하고 통계 처리하여 다음과 같은 결론을 얻었다. 1. 최대개구량의 평균치는 47.1mm, 최대개구운동시 과두의 이동 직선거리는 18.1mm, 과두의 수평 이동거리는 17.5mm, 수직이동거리는 3.8mm였으며 이동경사도는 $13.1^{\circ}$이었다. 2. 전방두개저의 길이, 하악골 및 상악복합체의 전후방적인 길이가 길수록, 하악상행지가 전방으로 경사할수록, 후안면 고경이 크고 하악의 경사도가 작을수록 개구량이 컸다. 3. 하악상행지가 직립된 경사도를 가질수록, 하악골의 만곡도가 클수록 과두는 수직적인 활주운동을 하였다. 4. 상악복합체가 전후방적으로 길수록 과두는 전방으로 길게 활주운동을 하였다. 이상의 내용으로 볼 때 최대개구위 측모두부방사선사진은 부정교합 환자의 개구운동시 하악골의 운동양상을 평가할 수 있는 진단 자료로서 유용하며 하악골의 개구운동 양상은 두개안면골격의 특정한 형태적 요소와 연관성을 가지며 악관절의 기능적 해부 형태에 관한 정보를 제공하는 것으로 사료된다.

Original Article 2 - 전방개교합이 동반된 하악과두흡수 환자의 임상적 특징 및 악안면 골격형태에 관한 연구 (Clinical and cephalometric characteristics in patients with anterior open bite and mandibular condylar resorption)

  • The aim of this study was to investigate clinical and cephalometric characteristics of patients with anterior open bite and mandibular condylar resorption. Total of 45 subjects were recruited from the patients who visited the Department of Oral Medicine, Kyungpook National University Hospital in 2006 for treatment of temporomandibular dysfunction. After taking patient's history concerning temporomandibular joint dysfunction, clinical examination was performed and panoramic, transcranial, and lateral cephalometric radiographs were obtained. The cephalometric data were compared to those of normal Korean population. The results were as follow: 1. Clinical characteristics 1) Female was 4.6 time more prevalent than male, and 82.2% of the subjects were in second and third decade. 2) Angle's Class I occlusal type was 51%, Class II was 29%, and Class III was 20%. 3) The mean value of the overjet and overbite were 3.2mm and -1.1mm, respectively. 4) Most of the patients had parafunctional oral habits. 2. Cephalometric characteristics 1) SNA showed no significant difference between condyle resorption and normal group. However, in female resorption group, SNB was lesser and ANB was greater than those in normal group. 2) SN-GoMe and FMA increased in resorption group. 3) Palatal plane angle did not show significant difference between resorption and normal group. 4) Total posterior facial height was significantly smaller and total anterior facial height showed no significant increase as compared with those of the normal group. 5) Mandibular body length did not show any significant difference between resorption and normal group.

악안면 기형 환자들의 발현 양상, 원인 요소 및 외과적 교정 방법에 관한 역학적연구 (A RETROSPECTIVE STUDY ON THE CLINICAL MANEFESTATIONS, ETIOLOGIC FACTORS OR SURGICAL CORRECTION METHODS OF THE MAXILLOFACIAL DEFORMITY PATIENTS)

  • Recently the goal of orthognathic surgery has been focused on esthetic improvement of the patients. Also early corrective surgery was favorable selected by most of the oral and maxillofacial surgeons. We should consider the etiologic factor of the patient's dentofacial deformities when treatment is planned, because this is the major factor in estimating the predictability or stability of result. The more researches were carried on the etiologic factors of the dentofacical deformities, The more possibility of the early surgical correction will be increased. The authors analyzed about etiologic factors and epidemiologic studies of the forth patients who had received the orthognathic surgery. The analyzed results were as follows: 1. The predilection ration between male and female was 17:23, and 32 patients (80%) of 40 patients were aged twenties. 2. 26patients(65%) complained estetic problems as well as functional problems. 10 patients(25%) complained only esthetic problems, and 4 of 40 patients complained only functional problems. 3. Mandibular prognathism was found to be done most frequently(25, 39%). Facial asymmetry (13, 20%) and angle hypertrophy were found to be next in sequence. 4. Sagittal split ramus ostetomy was done most frequently(27, 35%). Lefort I osteotomy(13, 17%), angle reduction (12,16%), and genioplasty(11, 15%) were done also. 5. The number of the cases due to nonspecific etiologic factor was 22(55%), that of cases due to inhertied tendency was 12(30%), that of cases due to congenital anomaly was 3(7.5%), and that of cases due to trauma was 3(7.5%). 6. The number of patients who got only maxilliary surgery was 2(5%), that of patients who got only mandibular surgery was 23(57.5%), and that of patients who got simultaneous two jaw surgery was 15(37.5%).

Radiomorphometric analysis of edentulous posterior mandibular ridges in the first molar region: a cone-beam computed tomography study

  • Purpose: The aim of our study was to determine the prevalence and degree of lingual concavities in the first molar region of the mandible to reduce the risk of perforating the lingual cortical bone during dental implant insertion. Methods: A total of 163 suitable cross-sectional cone-beam computed tomography images of edentulous mandibular first molar regions were evaluated. The mandibular morphology was classified as a U-configuration (undercut), a P-configuration (parallel), or a C-configuration (convex), depending on the shape of the alveolar ridge. The characteristics of lingual concavities, including their depth, angle, vertical location, and additional parameters, were measured. Results: Lingual undercuts had a prevalence of 32.5% in the first molar region. The mean concavity angle was 63.34°±8.26°, and the mean linear concavity depth (LCD) was 3.03±0.99 mm. The mean vertical distances of point P from the alveolar crest (Vc) and from the inferior mandibular border were 9.39±3.39 and 16.25±2.44, respectively. Men displayed a larger vertical height from the alveolar crest to 2 mm coronal to the inferior alveolar nerve (Vcb) and a wider LCD than women (P<0.05). Negative correlations were found between age and buccolingual width at 2 mm apical to the alveolar crest, between age and Vcb, between age and Vc, and between age and LCD (P<0.05). Conclusions: The prevalence of lingual concavities was 32.5% in this study. Age and gender had statistically significant effects on the lingual morphology. The risk of lingual perforation was higher in young men than in the other groups analyzed.

Sequential treatment from mandibulectomy to reconstruction on mandibular oral cancer - Case review I: mandibular ramus and angle lesion of primary intraosseous squamous cell carcinoma

  • Primary intraosseous squamous cell carcinoma (PIOSCC) is very rare type of squamous cell carcinoma (SCC) that occurs within the jaw and arises from remnants of odontogenic epithelium with no connection to the oral mucosa. This study reports two cases of PIOSCC of the mandible. Reported in this article are two cases of PIOSCC of the mandible that were treated with resection and reconstruction using a fibular free flap. The first case was a 36-year-old male patient who complained of right mandibular pain. Computed tomography (CT) and panoramic radiograph revealed a large radiolucency in the mandibular ramus area. At first, an odontogenic keratocyst was tentatively diagnosed, and an excision procedure was carried out at another clinic. A final biopsy after cyst enucleation revealed well-differentiated SCC, so we proceeded with segmental mandibulectomy and reconstruction using a fibular free flap. The second case was a 48-year-old male patient with left mandibular pain. CT and panoramic radiograph revealed irregular radiolucency in the mandibular angle area near tooth #38. At first, osteomyelitis was tentatively diagnosed, and a curettage was carried out. A later biopsy revealed well-differentiated SCC, so segmental mandibulectomy and reconstruction with a fibular free flap were secondarily performed. Our two cases have had no recurrence. The facial appearance of both patients is satisfactory, and the neo-mandibular body created using a fibular bone transfer displays adequate bony volume.

Comparison of the bite force and occlusal contact area of the deviated and non-deviated sides after intraoral vertical ramus osteotomy in skeletal Class III patients with mandibular asymmetry: Two-year follow-up

  • Objective: The objectives of this study were to compare the time-dependent changes in occlusal contact area (OCA) and bite force (BF) of the deviated and non-deviated sides in mandibular prognathic patients with mandibular asymmetry before and after orthognathic surgery and investigate the factors associated with the changes in OCA and BF on each side. Methods: The sample consisted of 67 patients (33 men and 34 women; age range 15-36 years) with facial asymmetry who underwent 2-jaw orthognathic surgery. OCA and BF were taken before presurgical orthodontic treatment, within 1 month before surgery, and 1 month, 3 months, 6 months, 1 year, and 2 years after surgery. OCA and BF were measured using the Dental Prescale System. Results: The OCA and BF decreased gradually before surgery and increased after surgery on both sides. The OCA and BF were significantly greater on the deviated side than on the non-deviated side before surgery, and there was no difference after surgery. According to the linear mixed-effect model, only the changes in the mandibular plane angle had a significant effect on BF (p < 0.05). Conclusions: There was a difference in the amount of the OCA and BF between the deviated and non-deviated sides before surgery. The change in mandibular plane angle affects the change, especially on the non-deviated side, during the observation period.

한국인과 스칸디나비아계 백인의 두부자세와 두개안면구조의 형태에 관한 비교연구 (A comparative study on head posture and craniofacial morphology between koreans and scandinavian caucasians)

  • 본 연구는 두부자세를 형성하는 두개저와 경추가 이루는 각의 차이가 두개안면구조의 형태에 어떠한 차이로 나타나는지를 파악해 보기 위하여, 51명의 한국인 성인 남자와 120명의 스칸디나비아계 백인, 성인 남자를 대상으로 자연상태의 두부위치로 촬영하여 얻은 두부방사선계측사진에서 두부자세변수, 두개저, 경추, 상악골, 하악골, 상하악 악골관계 그리고 치아, 치조에 관련된 변수를 설정, 이를 서로 비교함으로써 다음과 같은 결과를 얻었다. 1. NSL/OPT로 표현되는 두개와 경추가 이루는 각은 한국인에서 평균 9.28도 더 크게 나타났다. 2. n-s로 표현되는 전두개저의 길이는 한국인에서 평균 4.66 mm가 더 짧게 나타났다. 3. sp-pm 및 ss-pm으로 표현되는 상악골 기저의 길이는 한국인에서 각각 평균 2.75 mm, 4.65 mm더 짧은 양상을 보였으며, n-sp로 표현되는 상악골의 전방부 고경, 그리고 s-pm으로 표현되는 상악골의 후방부 고경은 각각 2.60 mm, 2.06 mm더 긴 양상을 보이는 한편, NSL/NL로 표현되는 상악골의 두개저에 대한 기울기는 차이를 보이지 않았다. 4. pg-tgo로 표현되는 하악골체의 길이와 ar-tgo로 표현되는 하악골지의 고경은 두 표본에서 동일한 양상을 보였으나, ML/RL로 표현되는 하악우각부는 한국인에서 3.22도 작은 양상을 보였고, NSL/ML로 표현되는 두개저에 대한 하악골의 경사도는 한국인에서 2.44도 더 큰 양상을 보였다. 5. s-n-sp 및 s-n-ss로 표현되는 상악골의 두개저에 대한 돌출도, 그리고 s-n-sm으로 표현되는 하악골의 두개저에 대한 돌출도는 차이를 보이지 않았다. 6. ss-n-pg로 표현되는 상,하악골의 전후방적 관계는 한국인에서 1.44도 더 큰 양상을 보였으나. NL/ML로 표현되는 수직적관계는 차이를 보이지 않았다. 7. n-go으로 표현되는 전안면고경은 한국인에서 5.57 mm 더 큰 양상을 보였다. 8. CL/ML로 표현되는 하악치조의 돌출도는 한국인에서 5.71도 더 큰 양상을 보였으며, ILs/ILi로 표현되는 상,하악 전치가 이루는 각은 한국인에서 3.08도 더 작은 양상을 보였다. 이상의 결과는 두개저와 경추가 이루는 두부자세에 따라 두개안면구조의 형태가 영향을 받을 수 있음을 보여주었다.

하악지치 발치 후 우측 교근에 발생한 방선균증의 치험례 (ACTINOMYCOSIS OF THE RIGHT MASSETER FOLLOWING THE EXTRACTION OF LOWER THIRD MOLAR)

  • Actinomyces is a part of the normal oral flora, but under certain circumstances it may become pathogenic. Actinomycosis is a chronic granulomatous infective disease caused by microaerophilic Gram-positive bacteria of the genus actinomyces. It can involve almost any system, but principally affects the head and neck. Because the lesions in the submandibular region and the angle of the jaw give the face a swollen, indurated appearance, actinomycosis of mandible can be easily misdiagnosed in its acute or early state of infection. In these cases the disease usually presented as a swelling suggestive of an abscess or mimicking a neoplasm. The yield from standard cultures was poor and repeated sampling and anaerobic culture may be needed to obtain a positive culture. So actinomycosis should always be considered in a differential diagnosis of all infections of the cervicofacial area. Diagnosis of actinomycosis is made based on the histopathology, the clinical presentation and past dental history. We experienced a case of actinomycosis in the masseter muscle and present the case with review of literature.

설계 풍속 상향 조정에 따른 Quay crane용 제용량 쐐기형 레일 클램프 설계 (Design of the Various Capacity Wedge-type Rail Clamp for a Quay crane According to the Design Wind Speed Criteria Change)

  • Recently many countries have concentrated their effort on the port automation, in order to be the Hub-port, that the Ultra Large Container Ship could come alongside the Quay, in their region. As the magnitude of the container ship increase, that of the Quay crane increases from 50ton-class to 61ton-class more and more. The wind speed criteria to design the structures used in the port was upgraded from 20m/s to 40m/s due to change of the weather condition. Our laboratory could have the ability to design the wedge type rail clamp for 50ton-class Quay crane in 30m/s wind speed. Accordingly we analyzed the load condition of the Quay crane about 40m/s wind speed upgraded from 20m/s and designed the wedge type rail clamp for 50ton and 61ton-class Quay crane.

Age, extraction rate and jaw surgery rate in Korean orthodontic clinics and small dental hospitals

  • Objective: This study aimed to investigate the current data regarding age, sex, and Angle Classification of Korean orthodontic patients and influence of these factors on the tendency to undergo extraction and orthognathic surgery. Methods: The recent trends of Korean orthodontic patients were assessed using questionnaire survey. The questionnaires were e-mailed to orthodontists who met the study criteria; 58% of the orthodontists opened the e-mails, and 27.7% replied to the e-mails. In all, the medical records of 11,340 patients who underwent orthodontic treatment at private clinics and small dental hospitals in Korea were analyzed. Results: The percentage of female patients in the study sample was 69.6%, and the average age of the patients was 19.87 years. The percentage of patients who were older than 19 years was 50.2%. Class II and Class III malocclusions were noted in 33.6% and 23.6% of patients, respectively. Extraction and orthognathic surgery were performed in 60.4% and 6.9% of patients, respectively. Conclusions: The results showed that there were a high percentage of adult, Class II malocclusion and extraction patients in private practices and small dental hospitals during the study period. Further, a relatively high proportion of adult patients opted to undergo orthognathic surgery.
