Purpose: This study was to find a correlation between the bilateral reciprocal leg press test and a the balance in chronic stroke patients. Methods: Eighteen patients performed an isokinetic leg press test consisting of a bilateral reciprocal and unilateral mode. Following the isokinetic leg press testing, subjects performed the balance test: Berg Balance Scale (BBS), Timed Up & Go (TUG) test, and stability limit. Pearson product moment correlation coefficients were used to determine the correlation between the mean score of the isokinetic leg press test, balance test in both affected and non-affected side. Results: This study indicated a significant correlation between the bilateral reciprocal leg press test and stability limit. There were significant correlation between non-affected side bilateral leg press(NBL) and BBS (r=0.501), affected side bilateral leg press (ABL) and non-affected side stability limit(NS) (r=0.614), ABL and total stability limit (TS) (r=0.493), NBL and affected side stability limit(AS) (r=0.480), NBL and NS (r=0.560), NBL and TS (r=0.563), among the patients. Conclusion: Measurement of the lower extremity strength using the bilateral reciprocal leg press test can be used as an evaluating tool of the balance test.
The role of eccentric muscle activities in functional everyday activities and sport is important and equally significant to concentric conditions. Eccentric and concentric exercise and evaluation are, therefore, very important. The purposes of this study were to measure eccentric md concentric peak torgue, percentage of peak torque, average power and percentage of average power of trunk flexors and extensors by using the Cybex NORM isokinetic dynamometer, and to standardize the value obtained. Thirty four young volunteers were tested, 17 females and 17 males, who had no history of back pain or abnormality. Each subjects were tested on three repetitions for isokinetic concentric and eccentric contraction at 5 velocities. The results were as follows; 1) Peak torque percent body weight of eccentric contraction were significantly greater than concentric contraction at each angular velocity and in trunk flexors and extensors(p<0.01). 2) Peak torque percent body weight of concentric contraction were significantly decreased as the angular velocity increased both male and female(p<0.01). 3) Peak torque percent body weight of eccentric contraction were not significantly changed as the angular velocity increased both male and female(p<0.05), 4) Peak torque percent body weight of male were significantly greater than female at each angular velocity and in concentric and eccentric contraction of trunk flexors and extensors(p<0.01)
Purpose: The aim of this study was to examine the activation of the contralateral upper and lower extremities and trunk muscle during ipsilateral upper extremity diagonal isokinetic exercise. Methods: Twenty-one healthy male subjects with no history of shoulder injury undertook ipsilateral diagonal isokinetic exercise at 60, 120, and $180^{\circ}/sec$, utilizing a standard Biodex protocol. Muscle activation amplitudes were measured in the upper trapezius, pectoralis major, biceps brachii, rectus abdominis, external oblique, rectus femoris, adductor longus, and biceps femoris muscles using electromyography. A one-way analysis of variance and paired t-tests were conducted, and the data were analyzed using SPSS, version 21.0. Results: The results revealed no statistically significant interaction between motion and angular velocity and no statistically significant contralateral muscle activation according to angular velocity (p>0.05). However, they revealed statistically significant contralateral muscle activation according to motion (p<0.05). Conclusion: These results suggest that the movements involved in contralateral upper extremity diagonal isokinetic exercise can enhance muscle strength in patients affected by stroke, fracture, burns, or arthritis.

The purpose of this study was to examine the effects of 8 weeks of resistance exercise on gait-related abilities and the differences in age groups to provide reference materials for application of training by age group. Subjects in their 30s, 40s, and 50s were recruited and subjected to a lower extremity exercise, in which the intensity gradually increased three times a week for eight weeks. The gait-related variables, torque at 0.18 sec, acceleration time, and torque at 30°, were assessed through isokinetic tests before and after the 8-week training. The torque at 0.18 sec showed a significant increase in the 50s group. There was no significant difference in acceleration time according to time and group. The torque at 30° showed a significant increase in the 30s group. In conclusion, 8 weeks resistance training can increase neuromuscular activity in adults in their 50s group, and changes in training variables are needed to increase torque at 30° that affects knee stabilization in adults over 40s group.
[Purpose] The combined effect of different types of post-exercise treatment has not been fully explored. We investigated the effect of combined cold water immersion (CWI) and compression garment (CG) use after maximal eccentric exercise on maximal muscle strength, indirect muscle damage markers in the blood, muscle thickness, and muscle soreness score 24 h after exercise. [Methods] Ten men performed two trials (CWI + CG and CON) in random order. In the CWI + CG trial, the subjects performed 15 min of CWI (15℃), followed by wearing of a lower-body CG for 24 h after exercise. In the CON trial, there was no post-exercise treatment. The exercise consisted of 6 × 10 maximal isokinetic (60°·s-1) eccentric knee extensions using one lower limb. The maximal voluntary contraction (MVC) and maximal isokinetic (60°·s-1) strength during knee extension, as well as the indirect muscle damage markers, were evaluated before exercise and 24 h after exercise. [Results] The maximal muscle strength decreased in both trials (p < 0.001), with no difference between them. The exercise-induced elevation in the myoglobin concentration tended to be lower in the CWI + CG trial than in the CON trial (p = 0.060). The difference in the MVC, maximal isokinetic strength, muscle thickness, and muscle soreness score between the trials was not significant. [Conclusion] CWI followed by wearing of a CG after maximal eccentric exercise tended to attenuate the exercise-induced elevation of indirect muscle damage markers in the blood.
Purpose: This study compared the effects of the elastic resistance and general training for college Taekwondo athletes, and investigated the biomechanical ability and lower extremity function according to the movement speed of the knee joint. Methods: Twenty university student Taekwondo athletes participated voluntarily in this study. The subjects performed general resistance training for four weeks, followed by elastic resistance training for another four weeks. The biomechanical parameters during the fast and slow isokinetic conditions, Y-balance ability, and vertical jump ability were measured three times: before training, after general resistance training, and after elastic resistance training. Statistical analysis was performed under isokinetic conditions and the intervention effect. Results: The biomechanical ability differed significantly between fast and slow isokinetic conditions (p<0.05). An analysis of the training method revealed a significant difference in the maximum knee extension speed and the posterior-lateral direction of the Y-balance test (p<0.05). Conclusion: Elastic resistance training had a more positive effect on the functional task performance than biomechanical factors related to muscular strength compared to general resistance training. In addition, speed-related knee biomechanical information of subjects with strong physical abilities could be utilized in sports physiotherapy.
Isokinetic evaluation of trunk flexors and trunk extensors was performed at $60^{\circ}/sec\;and\;120^{\circ}/sec$ of angular velocity by using cybex 6000TEF Unit on 31 healthy male white workers and 15 post-operative HILD patients with no significant difference in mean age and mean body weight between two groups, and compared each other. The purpose of this study is to obtain the isokinetic normative strength values and endurance latins for Dunk extensors and trunk flexors, and is to provide a guideline for rehabilitation program of post-operative HILD patients. The collected data were analyzed by ANOVA, Duncan's Nyktuoke Range Test, and Pearson correlation coefficiency in PC-SAS program, The results obtained were as follow ; 1. Post-operative subjects has lower isokinetic values than normal subjects in peak torque, peak torque $\%$ by body weight, total work, total work $\%$ by body weight, average power, average power $\%$ by body weight, TAAE of trunk flexors and trunk extensors, and there are significant differences with statistic value in trunk extensors at $60^{\circ}/sec$ and in trunk flexors and trunk extensors at $120^{\circ}/sec$ between two groups(p<0.05). 2. Pest-ooperative subjects has lower values for angle of peak torque than normal subjects in trunk extensors, and there are significant differences with statistic value at $60^{\circ}/sec$ and $120^{\circ}/sec$ between two groups. 3. Post-operative subjects has higher values for endurance ratios than normal subjects in trunk extensors and flexors, but there are no significant differences with statistic value between two groups. 4. Post-operative subjects has higher values than normal subjects in peak torque ratios, total work ratios, average power ratios of trunk flexors to trunk extensors, and there are significant differences with statistic value between two groups(p<0.01). 5. There is significant positive-correlation with statistic valve between peak torque and height and body weight in normal subjects(p<0.05), but Thjere is significant negative-correlation between peak torque of trunk extensor at $120^{\circ}/sec$ and age (p<0.05). 6. There is significant positive-correlation with statistic value between peak torque of trunk flexors and body weight in post-operative subjects (p<0.05), but There is significant negative-correlation between peak torque of trunk extensor add age (p<0.05). In conclusion, post-operative subjects have greater weakness in trunk musculature than normal subjects, especially there is more significant weakness in trunk extensors than in trunk flexors
Purpose: The goal of this study was to evaluate differences in strength deficits between (i) patients with stage 1 or 2 impingement syndrome and (ii) patients with rotator cuff tears. Materials and Methods: We enrolled 43 patients with stage 1 or 2 impingement syndrome (group 1) and 21 patients with rotator cuff tears (group 2). The isokinetic strength of both groups was evaluated at $60^{\circ}/sec$ for external rotation, internal rotation, adduction and abduction. We measured the peak torque, total work, average power of bilateral sides, peak torque relationship to body weight, and the ratio between unilateral agonist and antagonist. Results: The isokinetic strength deficits assessed in ratio of peak torque to body weight for group 1 and group 2 respectively were; $28.48{\pm}23.76%$ and $29.12{\pm}32.81%$ for abduction (p=0.929), $7.20{\pm}13.98%$ and $18.94{\pm}19.52%$; for adduction (p=0.021), $16.88{\pm}13.76%$ and $25.80{\pm}24.07%$; for external rotation (p=0.221), and $14.1{\pm}25.67%$ and $29.02{\pm}35.06%$ for internal rotation (p=0.059). For average power and total work, group 2 showed a significantly greater deficit for adduction and internal rotation than group 1. Conclusion: Those with rotator cuff tears have more isokinetic muscle strength deficits than those with stage 1 or 2 impingement syndrome. Progression from stage 1 and 2 impingement syndrome to stage 3 may result in greatest changes in strength deficits for internal rotation and adduction.
After warming-up exercise for 20 minutes, Isokinetic measurement of trunk strength for flexor and extensor was done by using Cybex 6000 TEF Unit on 91 healthy male white workers from 22years old to 49 years old, and compared each other. 20 repetitions of trunk extension-flexion were done at $120^{\circ}$/sec angular velocity. After resting for 1 minutes, Four repetitions at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec) were done with 30 seconds of resting interval between each angular velocity. The purpose of this study is to obtain the isokinetic normative strength values for trunk extensors and flexors, and is to know the correlation between age, height, weight of subjects and data from isokinetic trunk strength measurement, and is to provide a guideline for exercise program of male white collar workers The collected data were analyzed by ANOVA, Duncan's Multiple Range Test, and Pearson correlation coefficiency in PC-SAS program. The results obtained were as follow; 1. There is significant positive-correlation with the statistic value between weight and peak torque of trunk muscles at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec)(p<01), between height and peak torque of trunk muscles at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec) except peak torque of trunk flexor at $60^{\circ}$/sec(p<01). 2. There is nagitive-correlation between age and peak torque of trunk muscles at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec), there is significant differences with statistic value between age and peak torque of trunk extensor at $120^{\circ}$/sec(p<.01). 3. Mean peak torque and mean peak torque % by body weight of trunk extensor is 1.1 times higher values than trunk flexor at $60^{\circ}$/sec. 4. There is the increase in peak torque angle of trunk flexor with increasing of age, and the decrease in peak torque angle of trunk flexor with increasing of age at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec). there is significant differences with statistic value in peak torque angle of trunk flexor at $120^{\circ}$/sec(p<.01). 5. There is significant decrease in endurance ratio of trunk extensor with increasing of age at $120^{\circ}$/sec(p<.01). In conclusion, peak torque of trunk extensor is 1.1 times higher values than trunk flexor in healthy male white collar workers.
The tendinous ends of neglected achilles tendon rupture tend to retract and separate with atrophy due to gastrosoleus muscle contracture, leaving a wide gap occupied with fibroadipose scar tissue. It is almost impossible to perform simple end-to-end anastomosis after the intervening scar tissue being excised. Therefore many surgical procedures have been proposed to reconstruct the large gap. We treated three such cases by V-Y advancement flap and double Krackow suture technique, and their postoperative strength of triceps surae were evaluated with Cybex isokinetic strength testing. All patients returned to preinjury activities with satisfaction, but the ankle plantar flexor power showed about 20-30% deficit.
