Medial and Lateral Canthal Reconstruction with an Orbicularis Oculi Myocutaneous Island Flap

  • Background The eyelid and canthal areas are common locations for cutaneous tumors. The medial canthus includes, among many other apparatuses, the canthal tendon and lacrimal canaliculi, and its characteristic thin and supple skin is hard to mimic and restore using tissue from other regions. Accordingly, reconstruction of the canthal area can prove challenging for surgeons. Although various methods, such as skin grafts and local flaps from adjacent regions, have been utilized for reconstructive purposes, they present known disadvantages. However, we were able to successfully reconstruct both lateral and medial canthal area defects by using orbicularis oculi myocutaneous island flaps. Methods Our study included seven patients who underwent medial or lateral canthal region reconstruction, using orbicularis oculi myocutaneous island flaps, between 2011 and 2014, following either cutaneous tumor excision or traumatic avulsion injury. Results Five patients had basal cell carcinoma, one had squamous cell carcinoma of the eyelid, and one had sustained a traumatic avulsion injury of the eyelid and canthal area. Entire flap loss was not observed in any patient, but one-a heavy smoker-showed partial flap loss, which healed with secondary intention and yielded acceptable results. Donor site morbidity was not observed, and all patients were satisfied with their surgical outcomes. Conclusions The canthal regions can be successfully reconstructed with orbicularis oculi myocutaneous island flaps. These flaps offer several key advantages, including similarity in texture, color, and thickness to the recipient site and a negligible incidence of donor site morbidity.

Reconstruction of Disarticulated Knee Stump by Using Distally Based Anterolateral Thigh Island Flap (역혈류성 전외측대퇴섬피판을 이용한 무릎 잘린끝의 재건)

  • Purpose: The basic vascular anatomy and versatility of the anterolateral thigh flap was reported firstly by Song in 1984 and then by Zhang who introduced the reverse flow pattern of this flap. In this case, the authors reviewed various articles and their experiences with the distally based anterolateral thigh flap and applied it for coverage of bone-exposed wound occurred at the distal of the disarticulated knee stump. We consequently reported the reliability and resourcefulness of this flap in the difficult and limited situation. Methods: A 67-year-old-man who had suffered from arteriosclerotic obliterans inevitably underwent the disarticulation at knee joint due to clinical deterioration. He presented to our clinic with soft tissue necrosis and bone exposure at the stump. We debrided the wound and conducted the distally based anterolateral thigh island flap by transecting proximal portion of descending branch of the lateral circumflex femoral artery and the $14{\times}10cm$ sized flap was transferred to cover the defect. The pedicle measured 14 cm in length with pivot point 7 cm above the patella. Results: The postoperative course was mainly uneventful except early venous congestion for 4 days and subsequent partial skin loss. The wound was healed by secondary intension and no other sequelae had been observed during follow-up period of 12 months. Conclusion: Despite the presence of various reconstructive choices, the distally based anterolateral thigh island flap can be designed to repair soft tissue defects around the knee region, providing its reliable blood supply and long pedicle length, especially in the challenging cases.

Taxonomic Characteristics of Peyssonnelia capensis Montagne(Peyssonneliaceae, Rhodophyta) from Jeju Island in Korea (제주도산 홍조 아프리카바다표고(Peyssonnelia capensis Montagne)의 분류학적 특성)

  • The characteristics of Peyssonnelia capensis Montagne (Peyssonneliaceae, Rhodophyta) from Jeju Island were investigated in order to clarify the species entity and re-evaluate taxonomic position. The morphological variations of the Peyssonnelia capensis from Jeju Island were divided into two types, the thin thallus type and the thick thallus type. The thin thallus type is characterized by thin, soft, orbicular, slightly lobed thallus with entire to undulate margin, and compact rhizoids. The thick thallus type has thick and stiff thallus with sinuate margin and scattered rhizoids, and has more branches than the thin thallus type. The basal layer and the epithallium of thin thallus type are similar to those of thick thallus types in their structure. However, the hypothallial cells of the thick thallus type are longer than those of the thin thallus type. The erect filament of thick thallus type has more cells than those of the thin thallus type. Tetrasporangial nemathecia are distinctly protruded. Mature tetrasporangia of this species are divided cruciately into four spores as those of other Peyssonneliaceae. In addition to their morphological and anatomical distinctness between two types, the nucleotide sequence analyses of SSU and ITS2 region make no differences among populations from Jeju and other localities in Korea. Therefore these two morphological variation may not base on genetic variation.

First Report of Root Rot of Dendropanax trifidus Caused by Fusarium oxysporum in Korea (Fusarium oxysporum에 의한 황칠나무 묘목 뿌리썩음병 발생 보고)

  • Dendropanax trifidus belonging to the family Araliaceae, is a warm-temperate evergreen tree distributed in Jeju Island, Bogil Island, Geomun Island, Geoje Island, Wando, and Haenam in Korea. In June 2021, a root rot disease in which branches of Dendropanax trifidus seedlings turned brown and shrunk was discovered at the seedling cultivation facility in Naju-si, Republic of Korea. To identify the root rot fungus, three strains were isolated from the diseased tissues of seedlings and their mycological characteristics were investigated on potato dextrose agar. In addition, a molecular phylogenetic analysis was performed using sequences of the internal transcribed spacer (ITS) region and translation elongation factor 1-α (EF1-α) gene. The fungus was identified as Fusarium oxysporum. For pathogenicity test, the roots of seedlings were immersed in the conidia suspension of the strains and planted. After 20 days inoculation, root rot and browning symptoms were confirmed in the inoculated plants. This is the first report of F. oxysporum on D. trifidus in Korea.

Catch and reproductive biology of common octopus Octopus vulgaris in the coastal waters of Jeju Island, Korea (제주해역 참문어(Octopus vulgaris)의 어획실태와 생식생태)

  • The purpose of this study is to provide information about annual variations in catch size and changes in reproductive biology in the common octopus, Octopus vulgaris, a commercially important species in Jeju Island. Samples were collected from coastal waters of Jeju Island, Korea by coastal trap fishery from January to November 2021. Octopus vulgaris have been decreasing in this region since 1984. The mantle length (ML) ranged from 4.5 to 17.8 cm and body weight (BW) ranged form 88.5 to 2,657.5 g. A χ2-test revealed that the number of males was significantly greater than females (χ2 = 32.712, df = 10, P<0.05). Mature females were found from January to July and male occurred January to August. The gonadosomatic index value was higher from July to August than other months. The relationship between mantle length and body weight was BW = 2.4527ML2.3139 (female) and BW = 2.6785ML2.3159 (male). At 50% group maturity, female and male average weight was estimated to be 554.7 g and 330.6 g, respectively.

The Reconstruction of Foot using Medial Plantar Flap (내측 족저 피판을 이용한 족부의 재건)

  • Purpose : Plantar surfaces, calcaneal area, and region of Achilles insertion, which are extremely related with weight-bearing area and shoes application, must be reconstructed with glabrous and strong fibrous skin. Numerous methods of reconstructing defects of these regions have been advocated, but the transfer of similar local tissue as a cutaneous flap with preservation of sensory potential would best serve the functional needs of the weight-bearing and non-weight-bearing surfaces of this region. Therefore it is recommended to use the limited skin of medial surface of foot that is similar to plantar region and non-weight-bearing area. In this paper we performed the medial plantar flap transfered as a fasciocutaneous island as one alterative for moderate-sized defects of the plantar forefoot, plantar heel, and area around the ankle in 25 cases and report the result, availability and problem of medial plantar flap. Materials and methods : We performed proximally based medial plantar flap in 22 cases and reverse flow island flap in 3 cases. Average age was $36.5(4{\sim}70)$ years and female was 3 cases. The causes of soft tissue defect were crushing injury on foot 4 cases, small bony exposure at lower leg 1 case, posterior heel defect with exposure of calcaneus 8 cases, severe sore at heel 2 cases, skin necrosis after trauma on posterior foot 4 cases, and defect on insertion area of Achilles tendon 6cases. Average follow up duration was 1.8(7 months-9.5 years) years. Results: Medial plantar flaps was successful in 22 patients. 18 patients preserved cutaneous branches of medial plantar nerve had sensation on transfered flap but diminished sensation or dysesthesia. At the follow up, we found there were no skin ulceration, recurrence of defect or skin breakdown in all 18 patients. But there was one case which occurred skin ulceration postoperatively among another 4 cases not contained medial plantar nerve. At the last follow up, all patients complained diminished sensation and paresthesia at medial plantar area distally to donor site, expecially with 4 patients having severe pain and discomfort during long-time walking. Conclusion : Medial plantar island flap based on medial plantar neurovascualr pedicle have low failure rate with strong fibrous skin and preserve sensibility of flap, so that it is useful method to reconstruct the skin and soft tissue defect of foot. But it should be emphasized that there are some complications such like pain and paresthesia by neuropraxia or injury of medial plantar nerve at more distal area than donor site. We may consider that medial plantar flap have limited flap size and small arc of rotation, and require skin graft closure of the donor defect and must chose this flap deliberately.

Macrobenthic community on the soft-bottom around the Youngjong Island, Korea (영종도 주변해역의 저서동물 군집)

  • Macrobenthic fauna were collected seasonally using the van Veen Grab $(0,1/m^2)$ to investigate the benthic faunal assemblages on the soft-bottoms around the Youngjong Island during October 1991 to July 1992. A total of 266 species was identified. Of these polychaetes comprised 111 species $(41.7\%)$; crustaceans $75(28.2\%)$ molluscs, $59 (22.2\%)$ and others including echinoderms, $27(7.9\%)$. Mean density and biomass were estimated to be 498 $ind./m^2$ and 54.8$g./m^2$, respectively. Polychaetes were the most dominant faunal group in terms of abundance $(332\;ind./m^2)$ and number of species as well, whereas echinoderms were predominant in biomass $(332\;g./m^2)$. The dominant species were Mediomastus sp., Heteromastus sp., Nipponomysella oblongata, and Nephts polyranchia; the abundance of these species showed seasonal variations. The study area was divided into three regions by cluster analysis based on the similarity of species composition. The first region consisted of intertidal flat (G-I); the second, shallow subtital region of muddy sand (G-II); the third, channel region of mud sediments (G-III). The intertidal flat showed the highest density, and the channel was the lowest density, but the Highest in species diversity. Distribution of macrobenthic faunal assemblages of the study area seemed to be controlled by sedimentary facies and duration of tidal exposure.

Restoration Model of Evergreen Broad-leaved Forests in Warm Temperate Region(IV) - Vegetation Structure of the Case Study Areas - (난대 기후대의 상록활엽수림 복원 모형(IV) - 사례지의 식생구조 -)

  • To study restoration model of evergreen broad-leaved forests in warm temperate region, vegetation structure was studied at Wando(Island) as a case study. Quercus acuta was a dominant species at evergreen broad-leaved forests in Wando(Island). Majority of evergreen broad-leaved forests was a thirty years old coppice forest. Reforested vegetation and deciduous broad-leaved forests was developed at a mid-slope districts and a piedmont. Deciduous broad-leaved forestsconsisted of Quercus serrata, Carpinus tschonoskii, Carpinus coreana, etc., was developed at a ridge and higher districts. Evergreen broad-leaved woody plants were growing at a forest floor of deciduous broad-leaved forests. The species over sixty percent of constanty ratio in forty seven plots were Ligustrum japonicum, Trachelospermum asiaticum var. intermedium, Quercus acuta and Eury japonica. The vascular plants in the Wando(Island) was summarized as 488 taxa which composed as 101 families, 321 genus, 426 species, 56 varieties, 5 forms and 1 hyvrid. Evergreen broad-leaved woody stecies was 32 taxa which composed as 23 genus, 30 species and 2 varieties. The species such as Liliope platyphylla of Liliaceae and Pueraria thunbergii of Leguminosae, etc. was recorded as the highest values for their widely distribution in the areas. On the contrary, and forty taxa of plants such as Viburnum erosum of Caprifoliaceae, Traceholospermum asiaticum var. intermedium was recorded as over 50% of constancy ratio. Two hundred and nine taxa of plants such as Juglans manshurica of Juglandaceae, Cornus walteri of Cornaceae and Rodotypos scandens of Rosaceae, etc. was showed the specific trends due to long-term artificial disturbance. The forest of Pinus thunbergii showde the highest species diversities(155 species per 600m$^{2}$), while the Cinnamomum japonicum-Tracheolospermum asiaticum var. intermedium community showed the lowest species diversities(23 species per 600m$^{2}$).

A Comparison of Jeju Island, Ulleung Island, and Chungbuk region Students for Igneous rocks Observation and Knowledge (화성암 관찰과 지식에 대한 제주도, 울릉도, 충북 지역 학생들의 비교)

  • The purpose of this study was to compare characteristics of students lived in volcanic region or not while observing the igneous rocks basalt and granite. For the study we recruited 68 fifth-grade students in Jeju Island, 29 fifthgrade students in Ulleung Island and 61 fifth-grade students in inland area area. This study was conducted as follows: After observing of granite and basalt, the students wrote observations. At the end of observation they also wrote all knowledges about granite and basalt except for the observations. The results are as follows: First, igneous rocks observations did not show a statistically significant difference between regional characteristics variables. Second, the amount of knowledge that students knew about granite and basalt except for the observation facts also did not show significant differences. Third, in the results of qualitative analysis for knowledges about granite and basalt except for the observation facts, many students knew about the igneous origin and about the substance but there were few statements that reflected the characteristics of the location. This results will provide a meaningful implication that the science class must consist of the use a various examples associated with a real-life.

Development of sweet potato double cropping system in the southern island area of Korea

  • In Korea, the average air temperature has been elevated twice faster than the average global warming. And the climate warming is characterized by the smaller rise of air temperature in summer and the greater rise of air temperature in spring and winter. Therefore, the number of frost-free days to determine the cultivation ability of crops has increased by more than 15 days in 10 years according to climate warming. This climate warming trend has extended and is projected to extend not only the sweet potato growing season but also the sweet potato early cultivating area to higher altitude and latitude region. This study was carried out to evaluate the possibility of sweet potato double cropping in the southern island area of Korea by assessing the growth and yield performance of sweet potato cultivated at extremely-early and -late time. We had performed at Yokji Island Yokji Island($E128^{\circ}$ 18' $N34^{\circ}$ 36'), a representative specified complex area of sweet potato cultivation in southern Korea. As the test varieties, the major cultivars of the this region, Shinyulmi and early hypertrophic cultivars, Dahomi were used. The prior cropping were planted with PE film mulching on March 30 and April 10, and harvested after 110 days. So the succeeding cropping were planted without PE film mulching on July 25 and August 5 according to the harvesting time of the prior sweet potato and harvested after 120 days. As a control, it was harvested on September 15, 120 days after planted on May 15. Each experimental plot had an area of 12 square meters consisting of 4 beds, and was planted one at a time at intervals of 25cm. We had investigated growth characteristics - main vine length, node number, branch number, total vine yield, and tuberous root characteristics - tuberous root number, average weight, starch value, and etc. After harvesting, we analyzed the economic effects by examining the postharvest quantity, the input labor, the management cost, and the income. The total yield of marketable products in prior and succeeding cropping was 46~70% higher than that of control. The average unit price of sweet potato was 36% higher than the conventional culture, and the gross income increased by 98%, but the operating cost increased by 83%, and the farm income increased by 103%. There are considerations such as the difficulty of enlargement of cultivation area due to lack of labor in limited space and the need for watering measures due to spring drought. However, if the area of application for sweet potatoes double system is increased by 10%, it can be used as a new cropping system.
