임신기간과 분만 후 모체의 철 및 엽산 영양상태의 종단적 변화 (A Longitudinal Study on Maternal Iron and Folate Status During and After Pregnancy in Korean Women)

  • Anemia in women during pregnancy and after delivery has been known to affect the mother, the fetus, and the infant's growth and health status. Studies examining, changes in iron and folate status associated with anemia during pregnancy and during pregnancy, and those supplements are stopped after postpartum. However, the effects of those have not been clearly determined in pregnant and lactating Korea women. Therefore, this study was performed to determine the changes in maternal iron and folate status during pregnancy and six months after delivery longitudinally in six pregnant women who consumed supplements from 20 wk to delivery. We concluded that the iron status deteriorated during pregnancy and especially was weak in the third trimester, but had a tendency to recovery after delivery. On the other hand, the folate status deteriorated in the first and second trimester and was good in the third trimester, but had a tendency to decrease after delivery. These results suggested that the iron status was not improved despite consuming total iron supplements of 50 mg/day through diets and supplements during the second half of the pregnancy. On the other hand, the folate status improved at the end of pregnancy by consuming folate supplements of a total of 800 mg/day through diets and supplements. However, folate status was poor in the first half of the pregnancy, and the tendency of folate status to decrease during postpartum was advanced. At the point in which iron and therefore supplementation is essential. However, the effects of supplement intake time and intake dosage need to be verified and the nutritional status changes of postpartum women should be carefully monitored.

The Iron Status of Very Low Birth Weight Infants Receiving Multiple Erythrocyte Transfusions during Hospitalization in the Neonatal Intensive Care Unit

  • Purpose: We investigated the iron status of very low birth weight infants receiving multiple erythrocyte transfusions during hospitalization in the neonatal intensive care unit (NICU). Methods: We enrolled 46 very low birth weight infants who were admitted to the Kyungpook National University Hospital between January 2012 and December 2013. Serum ferritin was measured on their first day of life and weekly thereafter. We collected individual data of the frequency and volume of erythrocyte transfusion and the amount of iron intake. Results: A total of 38 (82.6%) of very low birth weight infants received a mean volume of $99.3{\pm}93.5mL$ of erythrocyte transfusions in NICU. The minimum and maximum serum ferritin levels during hospitalization were $146.2{\pm}114.9ng/mL$ and $456.7{\pm}361.9ng/mL$, respectively. The total volume of erythrocyte transfusion was not correlated to maximum serum ferritin concentrations after controlling for the amount of iron intake (r=0.012, p=0.945). Non-transfused infants took significantly higher iron intake compared to infants receiving ${\geq}100mL/kg$ erythrocyte transfusion (p<0.001). Minimum and maximum serum ferritin levels of non-transfused infants were higher than those of infants receiving <100 mL/kg erythrocyte transfusions (p=0.026 and p=0.022, respectively). Infants with morbidity including bronchopulmonary dysplasia or retinopathy of prematurity received a significantly higher volume of erythrocyte transfusions compared to infants without morbidity (p<0.001). Conclusion: Very low birth weight infants undergoing multiply erythrocyte transfusions had excessive iron stores and non-transfused infants also might had a risk of iron overload during hospitalization in the NICU.

Haematological and Immunological Response in Lambs Fed on Raw and Variously Processed Cottonseed Meal

  • An experiment was conducted with twenty crossbred male lambs to assess the effect of cotton (Gossypium) seed meal (CSM) on blood constituents and immunity. Lambs were randomly assigned to a reference diet (30% deoiled peanut meal, DPNM) and four test diets containing 40% of either raw, 45 minutes cooked, 1% $Ca(OH)_2$ and iron (1 free gossy-pol, FG : 0.3 Fe) treated CSM (replacing approximately 50%, reference concentrate mixture). These isonitrogenous and isocaloric concentrate mixtures were fed to meet 80% of protein requirements (NRC, 1985) along with ad lib maize hay for 180 days. Blood was collected at 60, 120 and 180 days post feeding. The lambs were sensitized with Brucella abortus S99 antigen after 140 days and were subjected to ELISA and delayed type hypersensitivity. Blood haemoglobin, erythrocyte count, leucocyte count, total protein, total albumin, total globulin, urea, creatinine concentration and aspartate aminotransferase activity in lambs fed on raw or processed CSM were comparable to the values of reference lambs. The higher (p<0.01) blood glucose levels observed in CSM fed lambs at 60 days of feeding was latter reduced to the levels comparable with those on reference diet at 120 and 180 days of feeding. The alanine amino transferase activity was lower in lambs fed raw and cooked CSM containing diets at 120 and 180 days of feeding. A marginal increase in serum iron and alkaline posphatase activity was observed in iron treated group and raw CSM fed lambs, respectively. The humoral immune response and DTH reactivity was lower (p<0.05) in lambs fed raw CSM (consuming 302.83 mg FG/day). Cooking, $Ca(OH)_2$ and iron treatment of raw CSM showed a positive response in alleviating the suppression of immune response owing to the reduced consumption of FG by 40.19, 17.40% and 26.73%, respectively in these diets. The present study thus indicated that consumption of 40% raw CSM (302.83 mg FG/day) though did not affect majority of the haematological and blood biochemical parameters, but markedly suppressed the immune mechanism of lambs.

  • This study was conducted to investigate to the effect of weaning food-feeding methods on Preschool-Children's food habit, food preference and iron nutritional status for the subjects of 337 children, aged 18-60 months attending day care centers in Suwon and Seoul area. It divided the subjects into two groups according to the weaning food-feeding methods during their weaning period. One group(spoon-group) was defied as one had used to spoon over two times a day for 5 months and the ohter group (bottle-group) as one had used to bottle over two times a day for 5 months in weaning periods. All of the data were analyzed based on the weaning food-feeding method, mainly using bottle or spoon during their weaning period. The study results are summarized as follows. 1) 44.2% of the mothers mostly used bottle and 46.6% of them mostly used bottle to feed weaning foods. Age difference between the two groups was not found. 2) The numbers of daily consumed foods were significantly more higher in the spoon-group than the bottle-group and the amount of daily milk consumption was more higher among bottle-group. 3) Both of food intake frequency and food preference scores of the spoon-group were significantly higher than food intake frequency and food preference scores the bottle-group. 4) Iron nutritional status assessed by Hb and Hct was low in general. Only 26-63% of the subjects at each monthly age were in normal range. The higher the children's age, the more were grouped in normal range, which was more evident among children of the bottle-group. 5) Mother's nutrition knowledge related to weaning was more higher among spoon-group than both-group. There results shows the importance of appropriate weaning food-feeding methods for Preschool-children's food habit, food preference and nutritional status especially iron nutritional status. Nutrition education for mothers with weaning aged children has to be stressed and related programs have to be intervened effectively.

The Effect of Milk Supplementation on Bone Density and Iron Status of Elderly

  • This study was performed to investigate how milk supplementation can benefit the elderly by increasing bone density but possibly harming the iron status concomitantly. Forty one elderly subjects over 65 years of age(male : 9, female : 32) participated. All subjects were apparently healthy, home staying and attending meal service for lunch at the welfare center. They were from low income area of Puchon city. One cup of milk per day was served for 10 months. The mean intake of calcium was significantly increased for females after milk supplementation. Males showed significantly increased means of triceps skinfold thickness, suprailiac skinfold thickness and waist circumference. Females showed significantly increased measurements fo three kinds of skinfold thickness, waist circumference, and hip circumference. There were no significant change in the mean bone density of lumbar spine$(L_2~L_4)$, femoral neck, ward's triangle and torchanter, but the proportion of osteopenia estimated by the T score of lumbar spine bone density was lowered from 50.0% to 34.6% for females. The mean Hb level was significantly for males. The proportion of anemia estimated by Hb(<12g/dl), Hct(<36%) and serum ferritin(<15mg/ml) were increased from 17.2% to 51.7%, from 20.7% to 44.8% and from 10.3% to 17.2%, respectively for females. It looks like milk supplementation can effect the intakes of several nutrients considered to be commonly deficient in the Korean diet fo elderly people, increase some anthropometric measurements, and decrease the proportion of osteopenia. However it can have adverse effects on iron status of females.

Incidence of Iron Deficiency Anemia in Day Scholar University Girls as Affected by Socioeconomic Status

  • Iron deficiency anemia (IDA) is the world's most common nutritional problem. It is characterized by a low hemoglobin (Hb) level and low iron status. A study was conducted to investigate the incidence of iron deficiency anemia in day scholar girls belonging to different socioeconomic strata at Punjab University, Lahore. Iron status of the subjects was estimated by measuring Hb, hematocrit (Hct), red blood cell (RBC) count, mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCR), serum iron, serum ferritin, total protein and albumin. Results indicated that females belonging to low socioeconomic strata had lower values for Hb, Hct, RBC count, total protein and albumin. Serum iron, serum ferritin, MCV and MCH values fell within the normal range for all of the socioeconomic groups. However, serum iron and ferritin varied with socioeconomic status and higher-income groups had significantly higher serum iron and ferritin. It was concluded that anemia may develop due to poor intake and absorption of iron and that those in the low-income bracket are the most affected group.

여자 청소년 및 젊은 여성의 비타민 D 결핍과 빈혈과의 연관성 분석 (Association between vitamin D deficiency and anemia among Korean adolescent girls and young women)

  • 본 연구는 2008 ~ 2014년까지 수행된 국민건강영양조사 자료를 활용하였다. 본 분석 대상자는 12 ~ 29세 여자 청소년 및 젊은 여성이며, 이들을 대상으로 비타민 D 결핍 여부에 따른 빈혈 및 철 결핍성 빈혈과의 연관성 분석을 실시하였다. 그 결과, 교란인자를 보정한 다중 로지스틱 회귀분석 모델에서 비타민 D 결핍군이 충분군보다 빈혈 및 철 결핍성 빈혈의 유병률이 유의적으로 높았다. 또한 혈청 25(OH)D 농도가 증가함에 따라 빈혈 및 철 결핍성 빈혈의 유병률이 낮아지는 선형 관계가 나타났다. 본 연구의 결과는 청소년 및 젊은 여성에서 문제가 되고 있는 비타민 D 결핍과 빈혈에 대한 예방 및 관리에 기초자료를 제공할 수 있다고 기대된다. 추후 전향적인 코호트 연구 및 임상시험 연구 설계를 이용한 후속 연구를 수행하여 비타민 D와 빈혈 사이의 명확한 인과관계를 확인할 필요가 있다고 사료된다.

  • Three types of infant food were experimentally prepared based on the average caloric requirement for, Koreans : Formula A, with 1/3 RDA for 4-6 month, formula B, with 1/2 RDA for 7-9 month, and formula C, with 2/3 RDA for 10-12 month old infants. Into each formula was added approximately 50% of rice. Analysis of the nutritive values on these formula showed no deviance Com the expected values in case of general nutrients while the percent of essential amino acids in protein was muck higher except methionine. Since iron content was found to be below the half of except values, a sufficient amount of iron as required in RDA should be additionally supplied, for example, in iron drops. The infant preference test on each formula showed much favorable acceptability : 50% of them responded as moderate, 33.3% as favorable, 12.5% as disliking, and 4.2% as disgusting. The test also showed no noticeable change in both feces and appetite.

IRON, COPPER, COBALT AND MANGANESE REQUIREMENTS IN MILK-FED CROSSBRED CALVES

  • A balance study was conducted to determine the requirements of iron, copper, cobalt and manganese in crossbred calves. Six calves were fed milk average 10 percent of their body weight and were supplemented with 15 g mineral mixture daily. A balance trial was conducted at $2\frac{1}{2}$ months of age. The primary route of excretion was through digestive tract as 99.87, 80.99, 77.27 and 99.94 percent of Fe, Cu, Co and Mn were excreted through faeces. The requirements of Fe, Cu, Co and Mn were computed using the respective mineral balance data and were found to be 169.60, 7.20, 4.48 and 8.89 mg/kg respectively.

  • The anthropometric measurements, nutrient intake, concentrations of minerals in scalp hair and urine and urinary 5-hydroxyindoleacetic acid(5-HIAA) of 30 autistic children not taking psychoactive drugs and 30 nonautistic control children were determined. The autistic children were taking significantly lower amounts of vitamin A, niacin, ascorbic acid and iron. The intake of vitamin A, niacin, and iron in autistic children were found to be 22$\%$, 75$\%$ and 58 of RDA, respectively. The decreased anthropometric measurements in height and weight of autistic children seems partly due to lower intake of these micronutrients. The food intake in vitamin and mineral group of autistic children was significantly lower. It is probably related to decreased intake of fruit in autistic children. There was no toxicity of cadmium and aluminum in both groups according to their contents in scalp hair. Autistic children showed elevated levels of hair calcium and zinc but lowered levels of copper and iron. The urinary excretion urinary excretion of 5-HIAA.
