Reliability of rehabilitative ultrasound imaging for measuring the gluteus maximus muscle at rest and during contraction

  • Objective: The aim of this study was to investigate the inter-rater and intra-rater reliability of rehabilitative ultrasound imaging (RUSI) for measurement of muscle thickness with changes in angles of the gluteus maximus (GM) at rest and during contraction. Design: Cross-sectional study. Methods: Twenty-two healthy men volunteered for this study. GM muscle images were obtained in the resting position and during prone hip extension with knee flexion at hip abduction angles of $0^{\circ}$ and $30^{\circ}$, respectively. Two examiners randomly measured the thickness of the GM twice in three different positions. The first position was a comfortable prone position. The second position was prone hip extension with knee flexion (PHEKF) to $90^{\circ}$. The third position was achieved by hanging a 1-kg weight on the ankle of the lifted leg during PHEKF with the angle of the lifted leg the same as the second position. Intra-class correlation coefficients (ICCs), standard error measurements, and minimal detectable changes were used to estimate reliability. Results: The intra-rater reliability ICCs (95% confidence interval) of the GM were >0.870, indicating good reliability. Inter-rater reliability ICCs ranged from 0.668 to 0.913. The reliability of measurements of muscle thickness at each position was similar to the reliability of the angle change. Differences in muscle thickness and ratios for each position with $0^{\circ}$ and $30^{\circ}$ of hip abduction were not statistically significant. Conclusions: In the present study, the intra-rater reliability of muscle thickness measurements of the GM was good, and the inter-rater reliability was moderate to good. Reliable RUSI measurements of wide and large muscles, such as the GM muscle at rest and during contraction, are feasible. Further investigation is required to establish the reproducibility of the protocols presented in this study.

Reliability and validity of rehabilitative ultrasound images obtained using a hands-free fixed probe in measuring the muscle structures of the tibialis anterior and the gastrocnemius

  • Objective: This study aimed to investigate the reliability and validity of muscle thickness (MT) and pennation angle (PA) measurements of the ankle muscle, including the tibialis anterior (TA) and the medial gastrocnemius (MGCM), using a hands-free fixed probe and to compare it with the conventional linear probe. Design: Observational inter-rater reliability study. Methods: Thirty-three healthy subjects (20 male, 13 female) were included. In all subjects, ultrasound images were acquired from the TA and MGCM using a hands-free fixed probe and a conventional linear probe in random sequence by two examiners at two time-points within a 7-day interval. MT and PA were calculated on the taken images. Intra-class correlation coefficients (ICC), 95% confidence intervals, standard error of measurement and the Pearson's correlation coefficient were used to estimate reliability and validity. And also, Bland-Altman plots were generated for a visual representation of MT and PA at the TA and MGCM. Results: The ICC for all intra-rater reliability was 0.943 to 0.995 and that for all inter-rater reliability was 0.928 to 0.993, indicating excellent reliability. A significantly high correlation was observed between MT and PA at the TA and MGCM with use of the hands-free fixed probe and the conventional linear probe (r>0.938; p<0.001). Conclusions: The hands-free fixed probe provided excellent images for measurement of the MT and PA of the TA and MGCM and is a useful device for making clinical measurements of muscle structure without grasping of the probe.

Association between Taql polymorphism of vitamin D receptor gene and vertical growth of the mandible: A cross-sectional study

  • Objective: To determine whether the gonial angle on digital panoramic radiographs is associated with vitamin D receptor (VDR) Taql polymorphism. Methods: Genomic DNA samples were collected from the buccal mucosa of patients aged 26-43 years. TaqMan assay for single nucleotide polymorphism genotyping was used to detect the genotype of Taql polymorphism. The gonial angle was measured bilaterally on panoramic radiography. The normal gonial angle was fixed as 121.8°, and it represented the cutoff value for the high gonial angle (HGA) and low gonial angle (LGA) groups. Various genetic models were analyzed, namely dominant (homozygous [AA] vs. heterozygous [AG] + polymorphic [GG]), recessive (AA + AG vs. GG), and additive (AA + GG vs. AG), using the chi-squared test. Results: The reliability of the gonial angle measurement was analyzed using a random sample (26%) of the tests, with the intra-examiner correlation showing an intra-class correlation coefficient of 0.99. The frequencies of the AA, AG, and GG genotypes of rs731236 polymorphism were 40.5%, 41.9%, and 17.6% in the HGA group and 21.8%, 51.0%, and 27.2% in the LGA group, respectively (P = 0.042). A statistically significant difference was observed in the allele frequencies between the two groups (P = 0.011). Moreover, a significant correlation was observed in the dominant genetic model. Conclusions: Taql polymorphism in the VDR gene plays a critical role in the vertical growth of the mandible and decreased gonial angle.

Assessment of The Accuracy of The MR Abdominal Adipose Tissue Volumetry using 3D Gradient Dual Echo 2-Point DIXON Technique using CT as Reference

  • In this study, in order to determine the validity and accuracy of MR imaging of 3D gradient dual echo 2-point DIXON technique for measuring abdominal adipose tissue volume and distribution, the measurements obtained by CT were set as a reference for comparison and their correlations were evaluated. CT and MRI scans were performed on each subject (17 healthy male volunteers who were fully informed about this study) to measure abdominal adipose tissue volume. Two skilled investigators individually observed the images acquired by CT and MRI in an independent environment, and directly separated the total volume using region-based thresholding segmentation method, and based on this, the total adipose tissue volume, subcutaneous adipose tissue volume and visceral adipose tissue volume were respectively measured. The correlation of the adipose tissue volume measurements with respect to the observer was examined using the Spearman test and the inter-observer agreement was evaluated using the intra-class correlation test. The correlation of the adipose tissue volume measurements by CT and MRI imaging methods was examined by simple regression analysis. In addition, using the Bland-Altman plot, the degree of agreement between the two imaging methods was evaluated. All of the statistical analysis results showed highly statistically significant correlation (p<0.05) respectively from the results of each adipose tissue volume measurements. In conclusion, MR abdominal adipose volumetry using the technique of 3D gradient dual echo 2-point DIXON showed a very high level of concordance even when compared with the adipose tissue measuring method using CT as reference.

만성 뇌졸중 환자의 앞 톱니근과 하부 등세모근의 비대칭성 연구 (The Study of Asymmetrical of the Serratus Anterior and Lower Trapezius Muscles in Chronic Stroke Patients)

  • PURPOSE: The purpose of this study was to compare the muscle architecture of serratus anterior and lower trapezius using rehabilitative ultrasound imaging (RUSI) in affected and unaffected side of chronic stroke patients. METHODS: The participants were thirty five patients with stroke hemiplegia in this study. RUSI was used to measure the muscle thickness of the serratus anterior and lower trapezius muscles. We compared the muscle thickness according to affected side and sex, determined the reliability of the measurement image. Independent t-test, intra-class correlation coefficient (ICC) and standard error of measurement (SEM) were used for statistical analysis. RESULTS: Significant difference in muscle thickness of serratus anterior was observed between affected and unaffected side (p<.001). Muscle thickness according to gender showed a significant difference in unaffected side of serratus anterior (p<.05). Compare asymmetry ratio of serratus anterior and lower trapezius muscle thickness showed a significant difference (p<.001). ICC for intra-reliability was .944~.962 in serratus anterior and .873~.925 in lower trapezius muscle thickness, respectively. SEM was .001~.004 in serratus anterior and .002~.008 in lower trapezius muscle. CONCLUSION: This study, using RUSI, showed significant difference in muscle thickness of serratus anterior in affected and unaffected side of stroke patients. RUSI is a practical tool for measuring soft-tissue thickness in the scapular region muscle of stroke.

A Systematic Review on Trunk Impairment Scale for Stroke Patients

  • Objective: The purpose of this study was to systematically review the trunk impairment scale that are used to assess the trunk control of stroke patients. Design: A systematic review Methods: Stroke subjects were categorized as acute, subacute, chronic. In this systematic review, the studies published between 2000 and 2020 were selected. A literature search using the keywords 'QUADAS', 'stroke', 'trunk impairment scale'. Data sources included RISS, GOOGLE Scholar and DBpia. We assessed the quality of assessment tools using Quality Assessment of Diagnostic Accuracy Studies tool. Results: We reviewed 18 studies. 7 of the 18 studies reported reliability results, 10 reported validity results. The QUADAS tool quality evaluation of 17 literatures extracted except for one randomized control test among 18 literatures showed a range of 3 to 13 points. 5 of the 18 studies are presented with the Cronbach alpha coefficient indicating reliability using internal consistency, all of which are more than 0.8. All studies that presented test-retest reliability, intra-rater reliability, and inter-rater reliability showed high agreement with an intra-class correlation coefficient of 0.75 or more. Conclusions: A systematic review of the study of the application of the trunk impairment scale for stroke patients will help provide criteria for future studies and application of the trunk impairment scale in clinical practice.

정상 성인에 대한 정량적 무릎힘줄반사 검사의 측정자간 신뢰도 (Inter-Rater Reliability of Quantitative Knee Tendon Reflex Test for Healthy Subjects)

  • The purpose of this study was to evaluate inter-rater reliability of the amplitude and first knee extension angles in deep tendon reflex test by using an electrical hammer. Twenty-five healthy adults participated in the study. Compound muscle action potential is elicited by tapping the knee tendon with an electrical hammer in deep tendon reflex tests. The amplitude and knee extension angle were simultaneously measured. The mean value of the amplitude and the knee extension angles through three time trials for each tester, are used for determining the inter-rater and Intra-class Correlation Coefficients (ICCs) reliabilities. According to the results, the ICCs of the amplitude is .280 and that of the knee extension angle is .789. Pearson correlation coefficients of the amplitude of the action potential and the knee extension angles are .685, showing significant statistically moderate correlation. Inter-rater reliability in the amplitude was not significant. More objective and quantitative deep tendon reflex tests should be done to obtain higher reliability in further studies.

Reliability study of the Pectoralis Minor Muscle Thickness Measurement using Rehabilitative Ultrasound Imaging

  • PURPOSE: This study examined the imaging procedure of pectoralis minor muscle thickness and assessed the intra- and inter-rater reliability of the muscle thickness measured by two raters using rehabilitative ultrasound imaging (RUSI) in healthy individuals. METHODS: Fifteen participants (aged 21 - 28, seven females, and eight males) were involved in the study. The primary rater palpated the coracoid process and the fourth rib, defined as the width of the index finger lateral to the sternum to avoid breast tissues, and lined the two landmarks. The second examiner checked 1 / 3 (1st point) and 1 / 2 (2nd point) of the line length as measurement points. The two raters obtained right side muscle images of the participants at a standardized sitting position using RUSI with a 7.5 MHz linear transducer at 40mm depth. For intra-rater reliability, the principal rater took three images per point and tried to take one more with an interval. For the inter-rater reliability, the other rater performed the same tasks as the principal rater on the same day. The reliability was analyzed using the intra-class correlation coefficient (ICC), the standard error of the measurement (SEM), and Bland and Altman plots. RESULTS: The reliability at all points was excellent for the same rater (ICC3,1 = .973 - .978, SEM = .042 - .046), and between raters (ICC2,1 = .939 - .959, SEM = .059 - .097). CONCLUSION: These findings show that the RUSI could be reliable for examining the pectoralis minor muscle thickness in healthy individuals at all measurement sites.

Bio-Inspired Object Recognition Using Parameterized Metric Learning

  • Computing global features based on local features using a bio-inspired framework has shown promising performance. However, for some tough applications with large intra-class variances, a single local feature is inadequate to represent all the attributes of the images. To integrate the complementary abilities of multiple local features, in this paper we have extended the efficacy of the bio-inspired framework, HMAX, to adapt heterogeneous features for global feature extraction. Given multiple global features, we propose an approach, designated as parameterized metric learning, for high dimensional feature fusion. The fusion parameters are solved by maximizing the canonical correlation with respect to the parameters. Experimental results show that our method achieves significant improvements over the benchmark bio-inspired framework, HMAX, and other related methods on the Caltech dataset, under varying numbers of training samples and feature elements.

  • We have designed and implemented an optoelectronic hardware of binary phase extraction joint transform correlator (BPEJTC) which provides higher peak-to-sidelobe ratio than many other versions of JTC that has been published so far and does not produce correlation peaks due to intra-class association, to construct a multi-target tracking system. The digital processing unit controlling the entire system plays the part of modifying and binarizing the joint transform power spectrum (JTPS) and the optical processing unit is mainly used to take fourier transform operations. Some experimental results conducted by designed system along with its architecture showed the processing rate of 6 frames per second, thereby the potential applicability of the proposed system to real-time multitarget tracking system is given.
