Purpose: The purpose of this study was to evaluate the efficacy and safety of Montelukast sodium in the prevention of bronchopulmonarydysplasia (BPD). Methods: The Interventional study was designed as a multicenter, prospective, and randomized trial, with open labeled and parallel-experimental groups, 66 infants were enrolled and allocated to either the case group (n=30) or the control group (n=36) based on gestational age (GA). Infants in the case group were given Montelukast sodium (Singulair) based on their body weight (BW). Zero week was defined as the start time of the study. Results: The incidence of moderate to severe BPD was not different between the groups (case group: 13 of 30 [43.3%] vs. control group: 19 of 36 [52.8%], P=0.912). Additionally, secondary outcomes such as ventilation index, mean airway pressure and resort to systemic steroids were not significantly different. There were no serious adverse drug reactions in either group, and furthermore the rate of occurrence of mild drug related-events were not significantly different (case group: 10 of 42 [23.8%] vs. control group: 6 of 48 (15.8%), P=0.414). Conclusion: Montelukast was not effective in reducing moderate or severe BPD. There were no significant adverse drug events associated with Montelukast treatment.
Total implanted central venous port (TIAP, Chemoport) is widely used in oncology patients because it does not require external dressing and restricts patient activity. Chemoport only requires monthly flushes of heparinized saline to keep the patency of the catheter and probably less prone to infectious complications than tunneled catheter. Despite the extensive use of permanent central venous access in oncology patients, there are only few reports about clinical experience of the Groshong catheter. The purpose of this study is to compare the complication rate between the traditional open-ended (non-valved) chemoports and valved chemoports (Groshong catheter connected to TIAP). During 5 years (Jan 2006 to May 2010), 438 patients received chemoport insertion procedure in our interventional radiology department. Among them 30 patients was referred to our department for problematic chemoports. We compared the cause of problematic chemoports between two types of chemoports (valved, vs. non-valved). Valved chemoports had higher referral rates than non-valved chemoports. When there is a need to insert valved port, different method of insertion and maintenance procedure seems to be necessary. More than 20 ml of flusing with heparinized saline after blood sampling could be a good suggestion. Adequate care of chemoport is essential for long patency. Also following the guideline from the manufacturing company is necessary.
The purpose of this study was to develop a theory appropriate to the context by identifying the career motivation formation process from the perspective of academic high school students using a grounded theory approach. In this study, participants were selected among non probability sampling was used for sampling by artificially reputational case selection methods. Forty two highschool students(21 male, 21 female) were participated in this study. Research data were collected mainly collected using in-depth interview data were analyzed by applying the grounded theory method of Strauss and Corbin (1998). According to the results, 319 concepts and 56 sub-categories, and 19 categories were derived n the open coding process. Academic high school students' formation of STEM career motivation were influenced by contextual conditions of "STEM-related career think that the instruments' causal conditions and 'STEM education experience in inside and outside of school' named 'STEM career understanding and self-understanding' using the strategy of being influenced in interventional conditions of social support and obstacle 'for the central phenomenon of "STEM efforts to achieve career goals, it appeared as a result of" satisfaction for STEM careers. And it had a 5-step process over time that the formation process of STEM career motivation. This is expected to provide homes, schools, communities, and contribute to have a new insight on the education of the country, given the direction of career education and counseling intervention and the basic data used to develop and apply STEM career education.
Background: In mediastinal lymph node sampling in non-small cell lung cancer (NSCLC) it is important to determine the appropriate treatment as well as to predict an outcome. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a recently developed, accurate, safe technique in patients with NSCLC for sampling mediastinal lymph nodes. We sought to determine the usefulness of EBUS-TBNA in mediastinal staging with NSCLC considered to be operable. Methods: We retrospectively reviewed the records of 142 patients who underwent EBUS-TBNA for mediastinal staging in the Asan Medical Center, Korea from July 2008 to July 2010. If patients were in an operable state, they underwent subsequent surgical staging. Diagnoses based on biopsy results were compared with those based on surgical results. Results: We performed EBUS-TBNA in 184 mediastinal lymph nodes in 142 NSCLC patients. Almost all of the EBUS-TBNA samples were from the lower paratracheal (112, 60.9%) and subcarinal (57, 31.0%) lymph nodes. In 142 patients, 51 patients (35.9%) were confirmed with malignant invasion of the mediastinal lymph node by EBUS-TBNA and 91 (64.1%) patients were not confirmed. Among the 91 patients, 64 patients (70.3%) underwent surgical staging. 3 patients (4.7%) who were misdiagnosed by the EBUS-TBNA were confirmed by surgery. After Diagnostic sensitivity of EBUS-TBNA, the prediction of mediastinal metastatsis was 94.4% and specificity was 100%. The procedures were performed safely and no serious complications were observed. Conclusion: We demonstrated the high diagnostic value of EBUS-TBNA for mediastinal staging.

Purpose: The number of patients who take warfarin is growing and so is the number of complications. Hemorrhage is the major complication, but the clinical characteristics and outcomes have not been determined for Korean patients. Therefore, we tried to evaluate the characteristics of the patients with hemorrhagic complications after taking warfarin as anticoagulation therapy. Methods: We retrospectively reviewed the medical records of the patients who visited the emergency room with bleeding complications after taking warfarin anticoagulation at the out-patient clinic for 1 year from 1 st January 2008. We compared between two groups (the major hemorrhage group vs. the minor hemorrhage group) according to the clinical criteria, the unstable vital signs that required blood transfusion, transfusion more than 2 units of blood, the need for further laboratory follow-up, the need for interventional treatment and the development of critical complications or death due to bleeding. Results: There were 150 patients who met the criteria and had acute hemorrhagic complications (the major group: 90 patients and the minor group: 60 patients). In the major hemorrhage group, the frequent sites of bleeding were the gastro-intestinal system (40 patients), lung (14 patients) and intracranium (7 patients). At the emergency room, the major group showed a higher initial INR of the activated prothrombin time than did the minor group (p=0.02). The bleeding sites of the fatal cases were the gastro-intestinal system (3 patients), lung (3 patients) and intracranium (3 patients), but the percentage of fatality was the highest for intracranium bleeding. Conclusion: In the major hemorrhage group, gastrointestinal bleeding was the most frequent complication and fatality was the highest for intracranium bleeding. An initially higher INR showed a greater risk of major bleeding, but not more fatalities.
Background: Although many clinicians know about the reducing effects of the pulsed and low-dose modes for fluoroscopic radiation when performing interventional procedures, few studies have quantified the reduction of radiation-absorbed doses (RADs). The aim of this study is to compare how much the RADs from a fluoroscopy are reduced according to the C-arm fluoroscopic modes used. Methods: We measured the RADs in the C-arm fluoroscopic modes including 'conventional mode', 'pulsed mode', 'low-dose mode', and 'pulsed + low-dose mode'. Clinical imaging conditions were simulated using a lead apron instead of a patient. According to each mode, one experimenter radiographed the lead apron, which was on the table, consecutively 5 times on the AP views. We regarded this as one set and a total of 10 sets were done according to each mode. Cumulative exposure time, RADs, peak X-ray energy, and current, which were viewed on the monitor, were recorded. Results: Pulsed, low-dose, and pulsed + low-dose modes showed significantly decreased RADs by 32%, 57%, and 83% compared to the conventional mode. The mean cumulative exposure time was significantly lower in the pulsed and pulsed + low-dose modes than in the conventional mode. All modes had pretty much the same peak X-ray energy. The mean current was significantly lower in the low-dose and pulsed + low-dose modes than in the conventional mode. Conclusions: The use of the pulsed and low-dose modes together significantly reduced the RADs compared to the conventional mode. Therefore, the proper use of the fluoroscopy and its C-arm modes will reduce the radiation exposure of patients and clinicians.
The purpose of this study is to examine those factors affecting Male Abusive Behavior on Intimate Female Partner Violence. The primary aim of this study is to determine the association between Intimate Partner Violence and coping strategies of male abusive behavior. The sample included 121 male abusive behavior who are referred by Counsel for Family Violence. For statistical analysis, descriptive statistical methods and hierarchical multiple regression were employed. Results indicated that male abusive behavior expressed more aggressive cognitive biases and irrational beliefs than nonviolent men. Both of problem-solving and avoidance coping to deal with relationship conflicts were related to abusive behavior of male. Specially, men who used higher levels of avoidance coping strategies was more likely related to physical abuse, less use of problem-solving coping was related to psychological abuse. Hostility and low marital satisfaction have also been associated with Intimate Partner Violence. Drinking is a risk factor for psychological abuse. Results are discussed implication for developing theoretical and interventional meanings of social welfare practice.
The aim of this study was to investigate occupational and individual risk factors and working conditions in relation to musculoskeletal symptoms in street cleaners. Investigation was conducted through a survey of 395 male street cleaners employed by the government office in Seoul, Gyeonggi and Chung-Nam from July to August of 2009. The control group was comprised of 143 male drivers and security guards. Risk factors for musculoskeletal symptoms in street cleaners were investigated by multiple logistic regression analysis and also evaluated ergonomic risk factors by assessing working conditions of 4 street cleaners. As a result of symptom questionnaires, all of the prevalent rates of musculoskeletal symptoms in street cleaners had significantly higher results than those of the control group(p<0.05). On binary logistic regression analysis of musculoskeletal symptoms, street cleaners showed significant higher odds ratio as 18.84(95%CI: 6.56-54.12) in the arm/elbow, 10.49(95%CI: 4.29-25.65) in the hand/wrist compared to the control group. Both absence of rest breaks and exposure to ergonomic risk factors showed to be important internal risk factors of musculoskeletal symptoms among street cleaners. The exposure levels of QEC(Quick exposures checklist) in street cleaners were revealed to be higher on the shoulder/arm, wrist/hand, and neck than back, or from stress. The findings appear to show that street cleaners were high-risk group of work-related musculoskeletal disorders. Therefore street cleaners require a holistic interventional strategy, including adequate arrangement of rest breaks, improvement of working tools and control of individual risk factors such as obesity and smoking.
Background: Workers laboring in steel industries in tropical settings with high ambient temperatures are subjected to thermally stressful environments that can create well-known risks of heat-related illnesses and limit workers' productivity. Methods: A cross-sectional study undertaken in a steel industry in a city nicknamed "Steel City" in Southern India assessed thermal stress by wet bulb globe temperature (WBGT) and level of dehydration from urine color and urine specific gravity. A structured questionnaire captured self-reported heat-related health symptoms of workers. Results: Some 90% WBGT measurements were higher than recommended threshold limit values ($27.2-41.7^{\circ}C$) for heavy and moderate workloads and radiational heat from processes were very high in blooming-mill/coke-oven ($67.6^{\circ}C$ globe temperature). Widespread heat-related health concerns were prevalent among workers, including excessive sweating, fatigue, and tiredness reported by 50% workers. Productivity loss was significantly reported high in workers with direct heat exposures compared to those with indirect heat exposures ($x^2=26.1258$, degrees of freedom = 1, p < 0.001). Change in urine color was 7.4 times higher among workers exposed to WBGTs above threshold limit values (TLVs). Conclusion: Preliminary evidence shows that high heat exposures and heavy workload adversely affect the workers' health and reduce their work capacities. Health and productivity risks in developing tropical country work settings can be further aggravated by the predicted temperature rise due to climate change, without appropriate interventions. Apart from industries enhancing welfare facilities and designing control interventions, further physiological studies with a seasonal approach and interventional studies are needed to strengthen evidence for developing comprehensive policies to protect workers employed in high heat industries.

Unlike conventional radiographic examinations, angiointerventional procedures have a high risk of radiation exposure to patients or operators due to prolonged radiation exposure time. This study was undertaken to examine effects of reducing the radiation risk by applying dose reduction fiber (DRF) shielding cloth during angiography. To investigate the properties of DRF shielding cloth, we measured the scattered radiation below and above a human phantom using a glass dosimeter, at site distances 10 cm away from the irradiated field. The results obtained reveal a 15 ~ 31% reduction of scattered radiation in the irradiation field, and 53 ~ 70% reduced radiation measured after phantom transmission. Taken together, our data indicate that application of DRF shielding cloth for radiation reduction at non-procedural sites during interventional procedure results in reduction of scattered doses to patients and operators, without affecting the medical examinations. We propose the use of DRF shielding during angiointerventional procedures, in order to reduce the risk of radiation exposure of patients and operators.
