Development and validation of LC-MS/MS for bioanalysis of hydroxychloroquine in human whole blood

  • This study aimed to analyze a high-performance liquid chromatography (HPLC) separation using a pentafluorophenyl column of parent drug hydroxychloroquine (HCQ) and its active metabolite, desethylhydroxchloroquine (DHCQ) applying to determine bioequivalence of two different formulations administered to patients. A rapid, simple, sensitive and specific liquid chromatography-tandem mass spectrometry (LC-MS/MS) method has been developed and validated for bioanalysis of HCQ and its metabolite DHCQ in human whole blood using deuterium derivative $hydroxychloroquine-D_4$ as an internal standard (IS). A triple-quadrupole mass spectrometer was operated using electrospray ionization in multiple reaction monitoring (MRM) mode. Sample preparation involves a two-step precipitation of protein techniques. The removed protein blood samples were chromatographed on a pentafluorophenyl (PFP) column ($50mm{\times}4.6mm$, $2.6{\mu}m$) with a mobile phase (ammonium formate solution containing dilute formic acid) in an isocratic mode at a flow rate of 0.45 mL/min. The standard curves were found to be linear in the range of 2 - 500 ng/mL for HCQ; 2 - 2,000 ng/mL for DHCQ in spite of lacking a highly sensitive MS spectrometry system. Results of intra- and inter-day precision and accuracy were within acceptable limits. A run time of 2.2 min for HCQ and 2.03 min for DHCQ in blood sample facilitated the analysis of more than 300 human whole blood samples per day. Taken together, we concluded that the assay developed herein represents a highly qualified technology for the quantification of HCQ in human whole blood for a parallel design bioequivalence study in a healthy male.

A Dilute-and-Shoot LC-MS/MS Method for Screening of 43 Cardiovascular Drugs in Human Urine

  • A simple, specific, and economical LC-MS/MS method was investigated for the screening of 43 prescribed antihypertensive and related drugs in human urine. The urine samples were simply prepared by diluting and mixing with internal standard before directly introduced to the LC-MS/MS system, which is fast, straightforward, and cost-effective. Fractional factorial, Box-Behnken, and I-optimal design were applied to screen and optimize the mass spectrometric and chromatographic factors. The analysis was carried out on a triple quadrupole mass spectrometer system utilizing multiple reaction monitoring with positive and negative electrospray ionization method. Chromatographic separation was performed on a Thermo Scientific Accucore RP-MS column (50 × 3.0 mm ID., 2.6 ㎛) using two separate gradient elution programs established with the same mobile phases. Chromatographic separation was performed within 12 min. The optimal method was validated based on FDA guideline. The results indicated that the assay was specific, reproducible, and sensitive with the limit of detection from 0.1 to 50.0 ㎍/L. The method was linear for all analytes with coefficient of determination ranging from 0.9870 to 0.9981. The intra-assay precision was from 1.44 to 19.87% and the inter-assay precision was between 2.69 and 18.54% with the recovery rate ranges from 84.54 to 119.78% for all drugs measured. All analytes in urine samples were stable for 24 h at 25℃, and for 2 weeks at -60℃. The developed method improves on currently existing methods by including larger number of cardiovascular medications and better sensitivity of 12 analytes.

Prevalence study of respiratory pathogens in Korean cats using real-time polymerase chain reaction

  • Pathogens such as feline herpesvirus, feline calicivirus, Bordetella bronchiseptica, Chlamydia felis, Mycoplasma felis and Pasteurella multocida usually cause feline upper respiratory tract disease (URTD). Real-time PCR was used to analyze the detection and prevalence of the most common respiratory pathogens in cats with (n=69) and without respiratory signs (n=31). Pathogens were detected in 53 cats, divided into 37 (69.8%) with a single pathogen, 15 (28.3%) with two pathogens, and 1 (1.9%) with three pathogens. M. felis had the highest detection rate in 29 (42.0%) cats, P. multocida was detected in 18 (26.1%), FHV in 10 (14.5%), FCV in 7 (10.1%), B. bronchiseptica in 3 (4.3%), and C. felis in 2 (2.9%). M. felis was the most frequently detected pathogen in cats living outdoors without vaccination. Of the 37 cats infected with single pathogen, nasal discharge was observed in 13 (35.1%), ocular signs in 6 (16.2%), drooling in 5 (13.5%), dyspnea in 3 (8.1%), and asymptomatic in 10 (27.0%). In 51 outdoor and 49 indoor cats, pathogens were detected in 35 (68.6%) and 18 (36.7%) cats, respectively. Of the 29 cats infected with M. felis, 22 (75.9%) showed respiratory signs, and 7 (24.1%) were healthy. In the age of the 53 positive cats, 10 (18.9%) were under the age of 1 year, 26 (49.1%) were aged 1~3 years, and 17 (32.1%) were aged 3 years or older. Although the number of cats in the study was small, the results can provide valuable data on the prevalence of URTD in Korean cats.

Cu-SCR 촉매의 De-NOx 성능 향상을 위한 연구 (Research for Performance Improvement of De-NOx of Cu-SCR Catalysts)

  • 화석연료를 기반으로 하는 내연기관의 엄격한 배기가스규제를 충족시키기 위해 자동차와 선박용 후처리장치의 비중이 점차로 증가하고 있다. 디젤엔진은 $CO_2$ 배출량이 적고 강력한 파워와 연료의 경제성을 가지고 있으며, 상용차뿐만 아니라 승용차에서도 시장의 수요가 증가하고 있다. 디젤 연료 특성으로 인하여 질소산화물은 국부적인 고온연소 영역에서 생성되며, 입자상물질은 확산연소 영역에서 생성이 된다. LNT와 urea-SCR 촉매는 디젤엔진에서 NOx를 저감시키기 위한 후처리장치로 개발되어져왔다. 이 연구는 가혹해지고 있는 배기가스 규제 대응을 위해 소형과 중 대형 디젤기관에 많이 사용되고 있는 Cu SCR 촉매의 NOx 저감 성능 향상을 목적으로 한다. $5Cu-2ZrO_2$/Zeolyst(Si/Al=13.7)SCR 촉매는 $5Cu-2ZrO_2$/93Zeolite(Si/Al=2.9) 촉매에 비해 촉매온도 $300^{\circ}C$ 이상에서 약 5-50% 수준으로 de-NOx 성능이 높았다. Zeolite는 zeolyst에 비해 금속의 분산도가 낮고 평균 입경이 커짐에 따라 촉매의 반응속도가 저하되었다. 10wt% Cu가 담지된 $10Cu-2ZrO_2$/88Zeolyst 촉매는 $200^{\circ}C$에서 40%, $350^{\circ}C$에서 약 65%로 NOx 정화성능이 가장 높았고, Cu의 이온이 제올라이트의 결정화합물인 Al과의 이온교환율이 증가함에 따라 다른 촉매에 비해 20-40% de-NOx 성능이 향상되었다.

생강 삼투압 건조 시 물질이동 특성과 품질에 미치는 효과 (Mass transfer Characteristic during Osmotic Dehydration of Ginger and Its Effect on Quality)

  • 생강의 삼투압 건조 중 설탕용액의 농도와 온도 및 침지시간에 따른 내부 물질이동을 생강 내부의 수분손실, 설탕흡수, 몰랄농도 및 속도매개 변수로써 조사하였고 갈색화 반응정도를 평가하였으며 데치기와 삼투압건조를 각각 거친 후 열풍건조 시킨 것에 대하여 복원 시 텍스쳐를 비교하였다. 용액의 농도와 온도가 증가함에 따라서 수분손실, 설탕흡수, 몰랄농도 및 속도매개 변수가 증가하였으며, 용액농도와 온도에 관계없이 수분손실, 설탕흡수, 몰랄농도는 초기 3분간 침지과정에서 빠르게 이루어진 후 증가현상이 둔화되었다. $80^{\circ}C$의 60 Brix 설탕용액에서 18분간 침지하였을 때 40.05 g 수분손실/100 g생강으로 초기 수분함량 83.02%(wet basis) 기준으로 약 52%의 수분을 제거하는 효과를 갖는다. 속도매개 변수는 설탕용액의 농도 보다는 온도에 의한 변화가 컸다. 갈색화 정도는 설탕용액의 농도와 온도에 관계없이 15분간 침지시켰을 때 가장 낮은 O.D.값을 나타내었으며, 대조구(O.D.=0.132)와 비교하였을 때 $80^{\circ}C$에서 40 또는 50 Brix의 설탕용액에 15분간 침지시킨 것이 가장 낮은 값(O.D.=0.027)을 보였다. 데치기 공정을 시킨 것과 대조구를 열풍건조 시킨 다음 끓는 물에서 3분간 복원시킨 후의 관통힘은 차이가 나타나지 않았으나 삼투압 건조시킨 생강은 대조구 보다 $22{\sim}34%$ 적게 나타났다.

항결핵제 투여후 중합효소연쇄반응으로 추적한 폐결핵 환자들의 치료반응 관찰 (Monitoring of Pulmonary Tuberculosis by Polymerase Chain Reaction After Antituberculous Treatment)

  • 배 경: 중합효소연쇄반응은 폐결핵의 진단에 유용한 검사법으로 도입되어 각 병원에서 널리 이용되고 있으나, 사균과 생균이 모두 양성으로 증폭되기 때문에 결핵환자들의 경과관찰에는 적당하지 못한 것으로 인식되고 있다. 그러나 항결핵제 투여후 중합효소연쇄반응 검사에서 음성으로 전환되는 예를 발견하여, 항결핵제 투여후 중합효소연쇄반응의 음전여부와 음전시기를 알아 보고자하였다. 방 법: 1996년 12월 1일부터 1997년 10월 31일까지 대구효성가톨릭 의료원을 방문하여, 중합효소연쇄반응에서 양성이 나온 16례의 폐결핵환자들을 대상으로, 항결핵제 투여하고 2주-1개월 간격으로 객담을 채취하여 총 113 검체를 대상으로 균배양, 항산성염색 및 중합효소연쇄반응법 등의 추적검사를 시행하였다. 폐결핵 환자들은 흉부 방사선 소견에 따라 분류하였고, 임상경과는 항결핵제 투여 후 1년간 관찰하였다. 결 과: 항결핵제를 투여한 16명의 환자들은 중합효소연쇄반응법에서 모두 30주 내에 음전되었고 평균 음전기간은 $16{\pm}8$주였다. 대상군은 흉부상사선 소견상 경증 8례, 중등도 4례 및 증증 4례로 분류되었고, 각각의 평균 음전기간은 경증 $9{\pm}5$주, 중등 $20{\pm}8$주 및 중증 $23{\pm}2$ 주였다. 항결핵제에 감수성을 보인 예에서는 약제를 복용함에 따라 중합효소연쇄반응 산물이 점차 감소하다가 증폭되지 않는 양상을 나타내었다. 결 론: 중합효소연쇄반응이 양성이었던 폐결핵환자들은 항결핵제 투여 후 30주 이내 모두 음전되었고, 폐침윤 정도가 넓을수록 평균 음전기간이 길었으며, 항결핵제로 치료함에 따라 중합효소연쇄반응 산물이 감소되었다. 따라서 중합효소연쇄반응은 폐결핵 환자들의 치료반응 판정과 효율적인 경과 관찰에 유용하게 사용될 수 있을 것이다.

동위원소희석 액체크로마토그래피 질량분석법에 의한 분유 내 콜레스테롤의 정량 (Quantitative analysis of cholesterol in infant formula by isotope dilution liquid chromatography-tandem mass spectrometry)

  • 분유 내 콜레스테롤을 정량분석하기 위한 일차분석법으로 동위원소희석 질량분석법을 개발하였다. 내부표준물질로 콜레스테롤-d4를 사용하여 분유에 첨가하였다. 분유에 지방산과 에스테르형태로 결합되어 있는 콜레스테롤을 자유 콜레스테롤로 바꾸어주기 위해 비누화과정을 수행하였다. 비누화과정은 반응온도, 반응시간, 사용한 KOH의 농도에 따라서 최적화하였으며, 그 결과로 분유시료 0.1 g에 대해 70 ℃에서 180 분 동안 8 M의 KOH 0.8 mL를 첨가하여 반응을 진행시키는 최적화조건을 확립하였다. 이와 같은 조건으로 실험을 진행하여 재현성은 0.23%, 확장불확도는 95%의 신뢰범위에서 1.9%로 추정되었다. 확립된 동위원소희석 질량분석법의 유효화를 위해 분유내에서 콜레스테롤의 인증값을 가지는 NIST SRM을 측정하였고 이 결과가 인증값과 불확도 범위내에서 일치하는 것을 확인하였다.

Epidermal Growth Factor Receptor Mutations in Non-Small Cell Lung Cancers in a Multiethnic Malaysian Patient Population

  • Background: Mutations in the tyrosine kinase domain of the epidermal growth factor receptor (EGFR) in non-small cell lung cancer (NSCLC) are predictive of response to EGFR-targeted therapy in advanced stages of disease. This study aimed to determine the frequency of EGFR mutations in NSCLCs and to correlate their presence with clinical characteristics in multiethnic Malaysian patients. Materials and Methods: In this prospective study, EGFR mutations in exons 18, 19, 20 and 21 in formalin-fixed paraffin-embedded biopsy specimens of consecutive NSCLC patients were asessed by real-time polymerase chain reaction. Results: EGFR mutations were detected in NSCLCs from 55 (36.4%) of a total of 151 patients, being significantly more common in females (62.5%) than in males (17.2%) [odds ratio (OR), 8.00; 95% confidence interval (CI), 3.77-16.98; p<0.001] and in never smokers (62.5%) than in ever smokers (12.7%) (OR, 11.50; 95%CI, 5.08-26.03; p<0.001). Mutations were more common in adenocarcinoma (39.4%) compared to non-adenocarcinoma NSCLCs (15.8%) (p=0.072). The mutation rates in patients of different ethnicities were not significantly different (p=0.08). Never smoking status was the only clinical feature that independently predicted the presence of EGFR mutations (adjusted OR, 5.94; 95%CI, 1.94-18.17; p=0.002). Conclusions: In Malaysian patients with NSCLC, the EGFR mutation rate was similar to that in other Asian populations. EGFR mutations were significantly more common in female patients and in never smokers. Never smoking status was the only independent predictor for the presence of EGFR mutations.

Ultrafine Particle Events in the Ambient Atmosphere in Korea

  • In this study, real time measurements of particle number size distribution in urban Gwangju, coastal Taean, and industrial Yeosu in Korea were conducted in 2008 to understand the occurrence of ultrafine particle (UFP) (<100 nm) events, the variation of its concentration among different sampling sites, and UFP formation pathways. Also, to investigate seasonal and long-term variation of the UFP number concentration, data were collected for the period of 5 years (2007, 2008, 2010, 2011, and 2012) in urban Gwangju. Photochemical and combustion events were found to be responsible for the formation of UFP in the urban Gwangju site, whereas only photochemical event led to the formation of UFP in the coastal Taean site. The highest UFP concentration was found in industrial Yeosu (the average UFP number fractions were 79, 59 and 58% in Yeosu, Gwangju, and Taean, respectively), suggesting that high amount of gas pollutants (e.g., $NO_2$, $SO_2$, and volatile organic carbon (VOC)) emitted from industries and their photochemical reaction contributed for the elevated UFP concentration in the industrial Yeosu site. The UFP fraction also showed a seasonal variation with the peak value in spring (61.5, 54.5, 50.5, and 40.7% in spring, fall, summer, and winter, respectively) at urban Gwangju. Annual average UFP number concentrations in urban Gwangju were $5.53{\times}10^3\;cm^{-3}$, $4.68{\times}10^3\;cm^{-3}$, $5.32{\times}10^3\;cm^{-3}$, $3.99{\times}10^3\;cm^{-3}$, and $2.16{\times}10^3\;cm^{-3}$ in the year 2007, 2008, 2010, 2011, and 2012, respectively. Comparison of the annual average UFP number concentration with urban sites in other countries showed that the UFP concentrations of the Korean sites were lower than those in other urban cities, probably due to lower source strength in the current site. TEM/EDS analysis for the size-selected UFPs showed that the UFPs were classified into various types having different chemical species. Carbonaceous particles were observed in both combustion (soot and organics) and photochemical events (sulfate and organics). In the photochemical event, an internal mixture of organic species and ammonium sulfate/bisulfate was identified. Also, internal mixtures of aged Na-rich and organic species, aged Ca-rich particles, and doughnut shaped K-containing particles with elemental composition of a strong C with minor O, S, and K-likely to be originated from biomass burning nearby agricultural area, were observed. In addition, fly ash particles were also observed in the combustion event, not in the photochemical event.
