Development of $5^{th}$ percentile female finite Element Model for Crashworthiness Simulation - Part I Articulated Rigid Body Model (충돌 안전도 해석을 위한 $5^{th}$ percentile 성인 여성 유한요소 모델 개발 - Part I 다물체 동력학 모델 개발)

  • In order to investigate the small female occupant behavior and accompanying injury mechanisms in vehicular trash event, a finite element model of $5^{th}$ percentile female has been developed. The model consists of articulated rigid body, which represents the morphology of small female body, and internal components with anatomical details. Articulated rigid body model serves as a basic platform for joining the detail internal skeletons and organs, while itself can be used for representing the overall kinematics of small female occupant. The modeling details such as anthropometry and finite element structure as well as validation results for the articulated rigid body model are introduced in this paper. The second part of the modeling, i.e. the internal components with anatomical details of small female are presented in subsequent part II of the paper.

Construction of Magnetic Resonance Imaging Inside-out probes for Internal Imaging (핵자기공명 영상법을 위한 내부 영상용 뒤집음-탐침의 제작)

  • In imaging the samples or human internal organs in a tube shape, general RF-probe types (that encircles a sample or places on top of the sample) are usually unsuitable for the internal imaging due to the degradation of signal-to-noise ratios(SNR's). In the present study the inside-out probes for Magnetic Resonance Imaging (MRI) have been constructed in the three different shapes such as an anti-solenoidal, a saddle and a dual surface types which are positioned as close to the area as possible by putting the probe inside the tubelike sample to improve filling factor. RF-field distributions have also been calculated depending upon the geometrical changes of anti-solenoid probes. Moreover, the performance of the inside-out probes has been checked by measuring SNR's of the images acquired. The inside-out probes constructed in this study produced better SNR's and rf-field uniformity in the area close to the probes in comparing with any other commercial probes. There is a high possibility that the constructed probes in the present study are applicable to the diagnosis of human bodies.

  • In mass casualty situation due to radiological accidents, it is important to start aggressive management with rapid triage decisions. External contamination needs immediate decontamination and internal contamination should be treated with special expertise and equipment to prevent the rapid uptake of radionuclides by target organs. Acute radiation syndrome shows a sequence of events that varies with the severity of the exposure. More severe exposures generally lead to more rapid onset of symptoms and severe clinical findings. After the massive exposure, various systems of the body reflect their severe damages that can lead to death within hours or up to several months. The disease progression has classically been divided into four stages: prodromal, latent, manifest illness, and recovery or death. Three characteristic clusters of symptoms including the hematopoietic syndrome, the gastrointestinal syndrome and the cerebrovascular syndrome are all associated with the acute radiation syndrome. The standard medical management of the patients with a potentially survivable radiation exposure includes good medical, surgical and supportive measures. Specific treatment with cytokines and bone marrow transplantation should be considered. The management of internal contamination is much the same as the treatment of poisoning. The standard decontamination should be applied to reduce uptake, and the chelating agents can be administered to enhance the clearance of radioisotopes. Radioactive iodine ($^{131}I$) as one of the nuclear fission products can increase the incidence of thyroid cancer in children. Potential benefit of potassium iodide prophylaxis is greater especially in neonates, infants and small children.

Internal parasites of cats in Iri and its vicinity (이리지방 고양이의 장내기생충 감염상황)

  • As a series of basic study for the prevention of zoonotic parasites, fecal samples and internal organs of cats in Iri and its vicinity were examined for helminth and their ova or protozoan oocysts from November 1994 to March 1995. The results obtained were summarized as follows; Out of the 133 samples, 101(75.9%) were positive and 135(101.5%) were cumulative positive for parasites. The following were identified in the samples and positive numbers of each species detected were ; Toxocura cari, 51;Isospora spp., 27;Spirometra erinacei, 26;Tasnka taentaeformis, 24;Diphyllobothrium latum, 4;and Capillaria spp., 3, respectively. In analyzing the state of infection, 64.9%(61 heads) of all the cats inspected had a single infection, while the rest(excluding the 32 uninfected cats) had mixed infections : 27.6%(26 heads), 6.4%(6 heads) and 1.1%(1 heads) had double, triple, quadruple Infections, respectively. From these results, it can be concluded that since the helminth, zoonotic parasites, in Korean autochthonal cats cause not only various disorders, but also can be very harmful to human beings, they are considered to be of great significance to public health.

Prevalence of internal parasites in the laboratory rats (실험용 흰쥐의 내부기생충 감염실태조사)

  • A survey on the prevalence of internal parasites in the laboratory rats in the Chonnam and Kyunggi areas was carried out. A total of 208 rats was selected for necropsy and the parasites were collected from the organs and the fecal samples. The infection rate of the internal parasites was high as 63.9% and eight species of parasites were identified. Among the species identified A tetraptera(21.6%) and H diminuta(20.7%) appeared with relatively higher infection rates than any other species identified. A significant difference in numbers of parasites was recognized between the regions as 8 species in Chonnam and 4 species in Kyunggi. The laboratory rats reared in stainless steel cage showed the higher infection rate than those in polycarbonate cage, with some exception in H spumosa and H nana. In this survey T crassicauda was firstly in the laboratory rats in Korea.

  • Atypical antipsychotics are more effective than typical antipsychotics and have fewer side effects such as tardive dyskinesia and extrapyramidal symptoms; therefore, prescriptions of atypical antipsychotics are increasing. However, recently, it has been reported that atypical antipsychotics have a higher incidence of diabetes, hyperglycemia, and obesity than typical antipsychotics. Atypical antipsychotics induce obesity-inhibiting appetite-related receptors such as serotonin and dopamine. Decreased exercise due to improving psychotic symptoms, and genetic characterictics can also cause weight gain. Hyperglycemia and hypoglycemia were another metabolic problem related to treatment with atypical antipsychotics. The mechanisms of hyperglycemia were mainly related obesity, decreased anorexigenic hormones, and increased insulin resistance in multiple organs. There are also reports that genes related to diabetes have an effect on the incidence of diabetes mellitus treated with atypical antipsychotics. On the other hand, although it is not clear why hypoglycemia occurs, it documented in case reports all over the world. There are more reports of atypical antipsychotics than typical antipsychotics and these are frequently reported in Asians. Further research on the mechanism of hypoglycemia related to atypical antipsychotics is strongly recommended.

A Rare Case of Undifferentiated Carcinoma of the Colon Directly Invading the Duodenum

  • Colorectal cancers often invade adjacent organs; however, direct duodenal invasion is rare. Adenocarcinoma is the most common type of colorectal cancer, but an undifferentiated carcinoma type is unusual. Herein, we present a case of undifferentiated carcinoma of the colon that directly invaded the duodenum and metastasized to distant lymph nodes. An 85-year-old female patient was admitted with a 7-cm-sized colonic mass invading the duodenum, detected by computed tomography. Positron emission tomography revealed fluorodeoxyglucose uptake in the colon, duodenum, and aortocaval lymph nodes. A large encircling mass in the ascending colon and an ulcerated mass in the duodenum were revealed by colonoscopy and esophagogastroduodenoscopy, respectively. Pathologic examinations of the colon and the duodenum revealed nonglandular, diffusely infiltrating atypical round cells, confirming undifferentiated carcinoma of the colon. The histologic type of this tumor was distinguished using immunohistochemical (IHC) markers. Finally, microscopic characteristics and IHC markers aided in identifying the histologic type of colorectal cancer.

A Case of Jejunal Diverticulitis with Perforation Combined with Intussusception Caused by Inflammatory Fibroid Polyp (천공을 동반한 공장게실염과 염증성 섬유양 용종으로 인한 장중첩증이 동반된 1예)

  • Diverticulosis of the small intestine is a rare entity, compared with that of duodenum or colon, and is found in only 1% of autopsied patients. The main complications are diverticulitis with or without a perforation, obstruction and hemorrhage, which are associated with a high mortality. Intussusception is primarily a disease of childhood; with only 5 to 10% of cases occurring in adults. In contrast to childhood intussusception, 90% of adult intussusception cases are had an associated pathologic processes. An inflammatory fibroid polyp is an uncommonly localized non-neoplastic lesion of the gastrointestinal tract. It occurs most often in the stomach and secondly in the ileum. It rarely occurs in other organs such as the colon, jejunum, duodenum and esophagus. We report a case of jejunal diverticulitis with a perforation combined with intussusception caused by an inflammatory fibroid polyp. A 78-year-old female presented with abdominal pain, fever and chill. Contrast CT scan showed intussusception of the ileum. The patient was treated with a small bowel segmental resection. After surgery, the specimen showed jejunal diverticulitis with perforation.

A Case Report of a Non-small Cell Lung Cancer Patient Metastasis to Vertebra with Cancer Pain Improved by Traditional Korean medicine including Aconitum Ciliare Decaisne Pharmacopuncture (비소세포성 폐암의 척추 전이로 발생한 암성통증에 대한 초오 약침을 포함한 한방 치료 1례)

  • Objectives : The purpose of this case report is to examine clinical application of Traditional Korean medicine including aconitum ciliare decaisne pharmacopuncture for cancer pain caused by bone metastasis of non small cell lung cancer. Methods : The patient diagnosed as non small cell lung cancer was treated with pharmacopuncture, acupuncture, electroacupuncture and herbal medicine. We used NRS(Numeric rating scale) and ECOG PS(Eastern Cooperative Oncology Group Performance Status) to observe the effect of the treatment. Results : After the treatment, NRS of cancer pain and ECOG PS score decreased. Also, the frequency of using rapid-onset opioids decreased. Conclusions : This study suggests Traditional Korean medicine treatment including aconitum ciliare decaisne pharmacopuncture is effective in cancer pain control caused by metastasis to bone with multiple organs with non small cell lung cancer.

  • Sarcoidosis is a multi-systemic granulomatous disorder of unknown etiology. The characteristic pathological finding is the presence of non-caseating granulomas. The lungs are primarily affected, however other organs may be involved causing various symptoms and ambiguous laboratory findings can be present. There are a few reported cases of sarcoidosis with elevated tumor markers. We describe a 68-year-old woman presenting with sarcoidosis showing elevated serum carcinoembryonic antigen (CEA). The possibility of cancer arising from serum CEA such as gastrointestinal cancer, breast cancer and lung cancer was excluded. A transbronchial lung biopsy demonstrated a non-caseating granuloma without necrosis. As a result prescribed 30 mg prednisolone daily to the patient and serum CEA was decreased after 1 month of treatment. We report a case of pulmonary sarcoidosis with elevated serum CEA.
