Salvage Endoscopic Resection for Residual Lesion after Definitive Chemoradiotherapy in Esophageal Cancer

  • Definitive chemoradiotherapy (CRT) with its significant efficacy and safety in esophageal cancer is reserved for patients with unresectable tumor or those who decline surgery. However, the incidence of locoregional failure or recurrence after definitive CRT remains high. Although esophagectomy is the standard treatment for locoregional failure or recurrence, this approach is associated with high mortality and morbidity. A 56-year-old man diagnosed with esophageal squamous cell carcinoma who refused to undergo surgery received definitive CRT. An endoscopy for response assessment performed after 2 months revealed a residual lesion, which was completely resected by salvage endoscopic submucosal dissection. To the best of our knowledge, endoscopic resection in locoregional failure or recurrence after definitive CRT is very rarely reported, and there are no guidelines or consensus to date. Here, we report a case of successful salvage endoscopic resection of residual lesion after definitive CRT.

Literature Review on Traditional Chinese Medicine Treatment of Gastric Cancer (위암의 중의약 치료에 대한 문헌적 고찰)

  • Objectives: This study was investigated in order to analyse the characteristics of recent Chinese medicine treatment of gastric cancer, and provide literature basis for the development of effective therapy of gastric cancer by reviewing Chinese journals. Methods: The literature on Chinese medicine treatment of gastric cancer were collected, analyzed and summarized from the China National Knowledge Infrastructure (CNKI) from 2000 to 2013. Results: Physicians have not shown consistent opinion with the pattern identification of gastric cancer. Recently, traditional Chinese Medicine patent prescriptions have been much used in the treatment of gastric cancer, which is classified into the reinforcement of healthy qi and elimination of the pathogenic factors. Chinese medicine combined with western medicine can improve the immune system and quality of life, while reducing toxic side effects. Conclusions: Further studies on traditional Chinese Medicine are needed to increase the survival rate of gastric cancer and effectiveness of combination therapy with western medicine.

  • The purpose of this study is to develop BSC model of social enterprise. Performance analysis tool of BSC have been brought over from the business world, designed and created from the perspectives of profit-based businesses. The BSC is a strategic performance measurement and management tool designed for the private sector acting as a communication/information and learning system, to measure 'where we are now' and 'where to aim for next'. It prescribes a plan for translating 'vision' and 'strategy' into concrete action across four perspectives at different stages, depending on the business. These perspectives are 'financial', 'customer', 'internal processes' and 'learning and growth', each of which is connected by cause-and-effect relationships that reflect the firm's strategy. Social aims of social enterprise are to accomplish desired outcomes which are to employ vulnerable people and to provide social services. The measurement factors of financial perspective are stable funding, efficiency of budgeting, stakeholders' financial supports, and trade profit. The measurement factors of customer perspective are government, social service users, employees, local communities, sup plier, social activity company, and partnership with external organizations. The measurement factors of internal process perspective are organizational culture, organizational structure/management, internal/external communication, quality of products and services, information sharing. The measurement factors of learning and growth perspective are training and development, management participation, knowledge sharing, leadership of CEO and manager, and learning culture.

A Human Case of Hymenolepis diminuta in a Child from Eastern Sicily

  • We report a case of Hymenolepis diminuta infection in a 2-year-old child living in a suburban area of Catania, Italy. This case was initially referred to us as Dipylidium caninum infection, which was not cured after being treated twice with mebendazole. However, by analyzing the clinical presentation and stool samples we arrived to the diagnosis of H. diminuta infection. The case presented with atypical allergic manifestations which had never been reported as clinical features of symptomatic H. diminuta infection; remittent fever with abdominal pain, diffuse cutaneous itching, transient thoracic rash, and arthromyalgias. The patient was treated with a 7-day cycle of oral niclosamide, which proved to be safe and effective. This case report emphasizes that a correct parasitological diagnosis requires adequate district laboratories and trained personnel. In addition, we recommend the importance of reporting all H. diminuta infection cases, in order to improve knowledge on epidemiology, clinical presentation, and treatment protocols.

Review of the Domestic Trend of Pulse Diagnosis Studies in Korea (맥진 연구의 국내 동향에 대한 고찰)

  • Objectives: The aim of this study was to present several proposals for future pulse diagnosis practice and research by investigating the trend of pulse diagnosis studies in Korea. Methods: We searched online medical databases, including National Discovery for Science Leaders (NDSL), the Oriental Medicine Advanced Searching Integrated System (OASIS), the Research Information Sharing Service (RISS), and the Korean Traditional Knowledge Portal (KTKP), for pulse diagnosis articles in Korea. We selected articles on pulse diagnosis but excluded duplicate articles, articles irrelevant to pulse diagnosis, and articles published in foreign countries. Results: In the first screening, 801 articles were selected. We found 251 articles and classified them according to category. The medical engineering field had 148 articles. A total of 24 articles were related to algorithms for pulse wave detection, 34 to sensors, five to pressurization technology, 16 to systems, 11 to remote medical service, five to mobiles, nine to trends, and 44 to basic research. The Korean medicine field had 103 articles. A total of 41 articles were devoted to literature reviews, 20 to case reports, 11 to constitutional medicine, six to experimental studies, and 25 to relevant research. Conclusion: More efforts to practice pulse diagnosis for various diseases should be made and the results actively published.

The effect of suction pipe leaning angle on the internal flow of pump sump

  • A better flow condition for the intake of pump is provided by the sump pump that connects the forebay to the intake of the pump station. If the suction sump is improperly shaped or sized, air-entraining vortices or submerged vortices may develop. These phenomena may greatly affect pump operation if vortices become sufficiently large. Moreover, any remaining vortices in the pump flow passage may result in an increase in the noise and vibration of the pump. Therefore, the vortices in the pump flow passage must be reduced to achieve good pump sump station performance. In this study, the effect of suction pipe leaning angle on the pump sump's internal flow is investigated. Additionally, a pipe type with an elbow shape is investigated. The results show that the air entraining vortices occur under the condition of a water level ratio H/D = 1.31 for each suction pipe type.

A Pulmonary Paragonimiasis Case Mimicking Metastatic Pulmonary Tumor

  • Pulmonary paragonimiasis is a relatively rare cause of lung disease revealing a wide variety of radiologic findings, such as air-space consolidation, nodules, and cysts. We describe here a case of pulmonary paragonimiasis in a 27-year-old woman who presented with a 2-month history of cough and sputum. Based on chest computed tomography (CT) scans and fluorodeoxyglucose positron emission tomography (FDG-PET) findings, the patient was suspected to have a metastatic lung tumor. However, she was diagnosed as having Paragonimus westermani infection by an immunoserological examination using ELISA. Follow-up chest X-ray and CT scans after chemotherapy with praziquantel showed an obvious improvement. There have been several reported cases of pulmonary paragonimiasis mimicking lung tumors on FDG-PET. However, all of them were suspected as primary lung tumors. To our knowledge, this patient represents the first case of paragonimiasis mimicking metastatic lung disease on FDG-PET CT imaging.

  • A 3 year-old pregnant female Yorkshire terrier dog was presented with major complaints of polyuria and polydipsia. Laboratory tests found persistent hyperglycemia and glucosuria. Abdominal ultrasound study found 2 dead fetuses. Ovarian hysterectomy was performed to remove the dead fetuses. After ovarian hysterectomy, the level of blood glucose was returned to normal range. In addition, the clinical signs and urine glucose were all disappeared. Based on findings on laboratory tests and follow-up study, the case was diagnosed as gestational diabetes mellitus. To our best knowledge, this is the first case report describing a gestational diabetes mellitus of dog in Korea.

Seroprevalence of Encephalitozoon cuniculi in Pet Rabbits in Korea

  • Encephalitozoon cuniculi is a microsporidian parasite commonly found in rabbits that can infect humans, causing encephalitozoonosis. The prevalence of encephalitozoonosis is not well documented, even when many clinics suspect pet rabbits as being highly infected. This study investigated the seropositivity of E. cuniculi using ELISA. The examination of 186 rabbits using ELISA showed that 22.6% (42/186) were seropositive against E. cuniculi. In analysis with healthy status, all 42 seropositive sera were collected from clinically normal rabbits. Moreover, the gender and age of pet rabbits did not have anysignificant effect on E. cuniculi infection. To the best of our knowledge, this is the first report to describe the seroprevalence of E. cuniculi in pet rabbits and suggests that pet rabbits could act as an important reservoir of encephalitozoonosis for both pet animals and humans in Korea.

A Clinical Study on the Shape of Normal Liver by Scintiphoto (형광(螢光) "카메라"상(像)에 의(依)한 정상간(正常肝)의 전면형태(前面形態)에 관(關)한 연구(硏究)(예보)(豫報))

  • A knowledge of the wide ranges in shape of liver is important in the interpretation of hepatic scintigraphy as some variants may be easily confused with pathological abnormalities. In this study, the variations in shape in 250 cases of normal liver obtained by scintiphoto were categorized into 9 types by means of the arbitrary standard for classification. Following were the results; 1) Among 250 normal cases, type 5 was 118(47.2%) with the highest prevalence and type 3 was 2 (0.8%) with the lowest. 2) A high incidence of obesity was noted in type 1, type 4 and type 8. That of ideal weight was noted in type 2, type 5 type 6 and type 9. 3) A high ratio of cardiac impression in scintiphoto was noted in type 4, type 7 and type 8. That of square left lobe was noted in type 2, type 6 and type 9. Finally that of tailed left lobe was in type 4, type 7 and type 8.
