Improved Regenerative Responses of Injured Spinal Cord Nerve Fibers by the Treatment of Sukjihwang(Rehmanniae radix preparat)

  • In oriental medicine, Sukjihwang (SJH, Rehmanniae radix preparat) has been used as one of the key ingredients for the prescription of several herbal decoctions and applied clinically for the treatment of several diseases including nervous system and cardiovascular disease. Here, possible growth-promoting effects of SJH on injured spinal cord axons were investigated in the rats. SJH administration increased levels of active form of ERK1/2 protein and Cdc2 proteins in the injured spinal cord tissue. Anterograde DiI-tracing of corticospinal tract axons showed that SJH-treatment enhanced axonal arborization in the injury area and extensive axonal extension into the caudal area. In SJH-treated group, glial scar formed after spinal cord injury was confined in a smaller area compared to the control group, and the trabecula structure was well observed within the injury cavity. Furthermore, increased proliferation and migration of astrocytes in the injury cavity were observed by SJH treatment. Thus, these present data provide a biological evidence on potential importance of SJH therapy for the treatment of injured spinal cord.

  • Objective: The purpose of this study was to investigate the possibility of injuries and the types of movement related to damage by body parts, and to prepare for prevention of injuries and development of a training program. Method: For this study, the experiment was conducted according to levels of 60 percentages (ST) and 85 percentages (MA) and 10 subjects from the Korean elite national weightlifting team were included. Furthermore, we analyzed joint moment and muscle activation pattern with three-dimensional video analysis. Ground reaction force and EMG analyses were performed to measure the factors related to injuries and motion. Results: Knee reinjuries such as anterior cruciate ligament damage caused by deterioration of the control ability for the forward movement function of the tibia based on the movement of the biceps femoris when the rectus femoris is activated with the powerful last-pull movement. In particular, athletes with previous or current injuries should perceive a careful contiguity of the ratio of the biceps femoris to the rectus femoris. This shows that athletes can exert five times greater force than the injury threshold in contrast to the inversion moment of the ankle, which is actively performed for a powerful last pull motion and is positively considered in terms of intentional motion. It is activated by excessive adduction and internal rotation moment to avoid excessive abduction and external rotation of the knee at lockout motion. It is an injury risk to muscles and ligaments, causing large adduction moment and internal rotation moment at the knee. Adduction moment in the elbow joint increased to higher than the injury threshold at ST (60% level) in the lockout phase. Hence, all athletes are indicated to be at a high risk of injury of the elbow adductor muscle. Lockout motion is similar to the "high five" posture, and repetitive training in this motion increases the likelihood of injuries because of occurrence of strong internal rotation and adduction of the shoulder. Training volume of lockout motion has to be considered when developing a training program. Conclusion: The important factors related to injury at snatch include B/R rate, muscles to activate the adduction moment and internal rotation moment at the elbow joint in the lockout phase, and muscles to activate the internal rotation moment at the shoulder joint in the lockout phase.

  • Tetrafluoroethylene is a colorless gas that can be used to synthesize a variety of fluoride compounds by polymerization (e.g., Teflon). Fluoride compounds have many applications in industry. There are several reports of inhalation injury from the pyrolytic product of fluoride compounds. When the polymer is heated under the conditions of inadequate ventilation, the fumes can cause polymer fume fever or pulmonary edema which manifested as symptoms such as fever, chill, profuse sweating, cough and dyspnea. However there are no reports of a direct lung injury caused by tetrafluoroethylene. We report a case of a 27-year-old male presented with acute lung injury after inhaling concentrated tetrafluoroethylene. He complained of cough and dyspnea after the accidental inhalation of tetrfluoroethylene at his workplace. The symptoms improved without any complications after conservative treatment with oxygen and steroid.

A Case of Headache and Memory Disorder after Traumatic Brain Injury Treated with Modified Seogakjihwang-tang (외상성 뇌손상 후두통과 기억장애가 발생한 환자에 서각지황탕가감방(犀角地黃湯加減方)을 투여한 1례 보고)

  • The purpose of this study is to report the clinical effects of modified Seogakjihwang-tang on a patient who had headache and memory disorder after traumatic brain injury, which were considered as the result of blood stasis. The patient had headache and memory disorder after traumatic brain injury and treated with modified Seogakjihwang-tang three times a day for 60days. As a result, the numerical rating scale and pain rating score of headache decreased and the scores of mini-mental state examination-Korea, the Korean version of Montreal cognitive assessment and the mini-memory test increased during treatment with modified Seogakjihwang-tang. Therefore, this case supports a possibility that modified Seogakjihwang-tang might be an effective treatment to patients with headache and memory disorder after traumatic brain injury.

A Case Report of a Postoperative State Patient with Cervical Spinal Cord Injury due to Ossification of Posterior Longitudinal Ligament Treated by Korean Medicine (후종인대골화증으로 발생한 경추 척수손상 환자에 대한 수술 후 한방치료 증례보고 1례)

  • Objectives: The purpose of this study is to report the effectiveness of Korean medicine treatment on postoperative state patients with cervical spinal cord injury due to ossification of the posterior longitudinal ligament. Methods: A 55-year-old Korean male patient diagnosed with cervical spinal cord injury due to ossification of the posterior longitudinal ligament was treated in an inpatient setting with Korean medicine treatment consisting of herbal medicine, acupuncture, and others from May 22 to August 21. The outcome was evaluated with the American Spinal Injury Association Impairment Scale (AIS), Spinal Cord Independence Measure-III (SCIM-III), modified Japanese Orthopaedic Association scale (mJOA), Numeric Rating Scale (NRS) before and after, or during the treatment period. Results: After 92 days of treatment, the patient's scores on all scales improved. Conclusion: The results suggest that Korean medicine treatment may be effective for postoperative patients with cervical spinal cord injury due to ossification of the posterior longitudinal ligament. Further studies should be conducted on a larger number of patients.

  • Objective: The aim of this case report is to describe three clinical cases that indicate positive effects of traditional Korean medicine for the treatment of acute headache attributed to whiplash injury. Method: Three patients were treated with a series of Korean medicine treatment, including Chuna treatment, acupuncture, pharmacoacupuncture, and herbal medicine, during the inpatient treatment period. Results: All patients showed decreases in the numerical rating scale (NRS) and headache impact test (HIT) scores. Conclusion: Traditional Korean medicine has potential benefits for the treatment of acute headache attributed to whiplash injury.

Protective Effects of Seonpyejeongcheon-tang on Elastase-Induced Lung Injury in Mice (Elastase 매개성 폐조직 손상에 대한 선폐정천탕(宣肺定喘湯)의 보호효과)

  • Objectives : This study aimed to evaluate the protective effects of Seonpyejeongcheon-tang (SJT) on elastase-induced lung injury. Materials and Methods : The extract of SJT was treated to A549 cells and an elastase-induced lung injury mouse model. Then, various parameters such as cell-based cytoprotective activity and histopathological findings were analyzed. Results : SJT showed a protective effect on elastase-induced cytotoxicity in A549 cells. This effect was correlated with analysis for caspase 3 levels, collagen and elastin contents, protein level of cyclin B 1, Cdk1, and Erk1/2, and gene expression of TNF-$\alpha$ and IL-$1{\beta}$ in A549 cells. SJT treatment also revealed a protective effect on elastase-induced lung injury in mouse model. This effect was evidenced via histopathological findings, including immunofluoresence stains against elastin, collagen, and caspase 3, and protein levels of cyclin B1, Cdc2, and Erk1/2 in lung tissue. Conclusion : These data suggest that SJT has pharmaceutical properties on lung injury. This study thus provides scientific evidence for the efficacy of SJT for clinical application to patients with chronic obstructive pulmonary disease.

A Case of Serious Caustic Injury after Ingestion of Hydrochloric Acid (염산으로 인한 중대한 부식성 손상 1례)

  • Acid ingestion can cause not only caustic injury on esophagus and stomach but also fatal outcome through systemic complications. We report the case of a patient who died early after severe caustic injury with strong acid. A 38-year-old man who ingested about 400ml of hydrochloric acid of unknown concentration was transferred to our hospital from a private clinic, in which he was managed with gastric irrigation through a nasogastric tube. He was complaining dyspnea and abdominal pain. Physical examination demonstrated tenderness and rebound tenderness on epigastric region. Severe metabolic acidosis and leukocytosis were noted. Radiological findings suggested perforation of gastrointestinal tract, although the physical signs were not typical. Endoscopy revealed caustic injuries of grade I on esophagus and of grade Ⅲ on stomach, which indicate more severe injury on the stomach than on the esophagus. Exploratory surgery was recommended but unfortunately not permitted by his family. Despite intensive measures, his vital signs deteriorated rapidly and he died 50 hours after the ingestion.

Effect of Ginseng on Renal Function in Patient with Renal Injury (인삼이 신 질환 환자에서 신 기능에 미치는 영향)

  • There are many mechanisms that mediate progression of renal injury, such as abnormalities of nitric oxide (NO) regulation, increased Platelet aggregatlon, and oxygen free radical injury. Ginseng has been known to have NO dependent vasorelaxant effect and antioxidant effect, also inhibit thrombin stimulated platelet aggregation. And these effects of ginseng may have some roles in prevention of renal injury. So we studied 24 patients with mild pathologic proteinuria and hypertension to evaluate the effect of ginseng on progression of renal injury. After 1 month treatment, creatinine clearance was significantly increased especially in the patient group with normal serum creatinine level without specific side effect. The result of this study suggest that ginseng may pre vent or retard the progression of renal injury especially in early stage.

Effects of Daesihotang-sosonggitang-gagambang on liver injury of rats induced by ccl4 and d-galactosamine (대시호탕합소승기탕가감방(大柴胡湯合小承氣湯加減方)이 $CCl_4$ 및 d-galactosamine으로 유발된 흰쥐의 간손상(肝損傷)에 미치는 영향(影響))

  • Objectives : In order to investigate the curative effect of Daesihotang-sosunggitang-gagambang on the liver injury of rats induced by $CCl_4$ and d-galactosamine. Methods : All animals were divided into 5 groups, those were normal group(untreated), control group(administrated with 0.9% Saline solution), sample I group(65mg/kg administrated), sample II group(130mg/kg administrated), positive control group(administrated with 200mg/kg silymarine). After the liver injury of rats induced by ccl4 and d-galactosamine, and cheked the serum transaminase(GOT, GPT) alkaline phosphatase(ALP), lactic dehydrogenase(LDH) for enzyme activities and triglyceride, total cholesterol amounts for serum component were measured. Result : The inhibitory effects on the serum GOT, GPT, LDH, ALP activities, serum total cholesterol content level in liver injury of rats induced by ccl4 were noted in both sample I group and sample II group. The inhibitory effects on the serum GPT, LDH activities and serum total cholesterol content level in liver injury of rats induced by d-galactosamine were noted in both sample I group and sample II group. The inhivitory effects on the serum GOT activities and triglyceride content level in liver injury of rats induced d-galactosamine were noted in sample I group, but it is not recognizable statistically. In sample II, they were noted. Conclusions : Deesihotang-sosoonggitang-gagambang has treatment effect against liver injury in rats induced by ccl4 and d-galactosamine. So it is required to study about the actions of mutual relation of medicines and path-mechanism by experiment.
