A Case of a Patient of Pontine Hemorrhage with Clinical Features of Cerebellar Dysfunction (소뇌기능이상의 임상양상을 특징으로 보인 뇌교부출혈 환자 1례)

  • I experienced a case of a patient with clinical features of cerebellar dysfunction in the intracranial hemorrhage which encroached the basis of lower pontine and all parts of pontine tegmentum. So I report this case with bibliographical inquiry. In addition, I applied the treatment of Oriental medicine to sequelae of intracranial hemorrhage like disorders of eye movement, central dizziness, cerebellar tremor and ataxias but the effect did not meet my expectation. I anticipate more clinical studies and reports on this hereafter.

  • Micro electrochemical machining was investigated to machine micro internal features. This method uses a micro disk tool electrode and can easily machine micro features inside of a micro hole, which are very difficult to make by the conventional processes. In order to limit the machining area and localize the electrochemical dissolution, ultra short pulses were used as power source and a micro disk electrode with insulating layer on its surface was used as a tool electrode. By electrochemical process, internal features, such as groove array, were fabricated on the stainless steel plate.

  • Synovial sarcoma is a malignant soft tissue neoplasm that occurs frequently in the extremities of young adults, near large joints. The lung is a common site of metastasis but an extremely unusual primary site for synovial sarcoma. We report an unusual case of primary synovial sarcoma that arose in the lung of a 59-year-old woman. The tumor had histologic and immunophenotypic features consistent with biphasic synovial sarcoma These features included of an intimate admixture of cytokeratin and epithelial membrane antigen(EMA)-positive neoplastic epithelial cells and vimentin-positive fibroblast-like spindle cells. The patient had a closed thoracomy drainage and doxycycline pleurodesis for malignant loculated effusion and showed tumor extension in the left whole lung 4 months after pleurodesis. This case is an usual addition to the small number of published reports on primary pulmonary synovial sarcoma The distintive features of this neoplasm allow it to be distinguished from a variety of primary and metastatic malignancies in the lung.

Retrospective Study for Safety of Xie-xia Fa (瀉下法) by Euphorbia kansui Radix: Analysis of Clinical Features, Liver and Kidney Functions (감수(甘遂)를 이용한 사하법(瀉下法)의 안전성에 대한 후향적 연구: 임상반응, 간기능, 신기능을 중심으로 한 평가)

  • Objectives: The purpose of this study was to report the safety of diarrhea-inducing treatment (Xie-xia Fa, 瀉下法) by Euphorbia kansui Radix, especially in the respect of clinical features, liver and kidney functions. Methods: Patients who were treated with Euphorbia kansui Radix powder between September 2013 and September 2014 were assessed retrospectively. Results: Less than 6 grams dosage of Euphorbia kansui Radix can induce several mild intestinal reactions but have no harmful effects on liver and kidney functions. Conclusions: Through this study, diarrhea-inducing treatment by less than 6 grams of Euphorbia kansui Radix can considered safe enough to be used in local clinics, but a cautious approach is needed.

  • Background and Aim: Oxaliplatin hypersensitivity is a well-known adverse reaction but the prevalence varies and data for frequency and clinical features have not been reported for Korea. Here we evaluates the prevalence and risk factors for hypersensitivity reactions to oxaliplatin after chemotherapy. Methods: Clinical information on all patients treated with oxaliplatin was retrospectively reviewed in electronic medical records between August 2009 and July 2010 in Seoul National University Bundang Hospital. Patients who experienced hypersensitivity reactions to oxaliplatin were compared with those who did not. Results: A total of 393 patients received oxaliplatin, with 42 (10.7%) experiencing hypersensitivity reactions including three cases of anaphylaxis. Median cycle of the first hypersensitivity reaction was 8. Reactions correlated with lower dexamethasone doses. Other variables were not significant. Conclusions: The prevalence of hypersensitivity reactions was 10.7%, symptoms being mostly mild and cutaneous. Lower dexamethasone doses could be a predictor for hypersensitivity reactions to oxaliplatin.

  • PCP remains the leading cause of deaths in patients with AIDS. As familiarity with PCP increases, atypical manifestations of the diseases are being recognized with greater frequency. There are following "atypical" manifestations of PCP ; 1) interstitial lung response that include diffuse alveolar damage, bronchiolitis obliterance, interstitial fibrosis, and lymphoplasmocytic infiltrate 2) striking localized process frequently exhibiting granulomatous features 3) extensive necrosis & cavitation 4) extrapulmonary dissemination of the disease. A wide variety of pathologic manifestations may occur in PCP in human immunodeficiency virus-infected patienst and that atypical features should be sought in lung biopsies from patients at risk for PCP. We had experienced a case of PCP, which presented with severe hypoxia, progressive dyspnea and fine crackles. It was diagnosed as PCP in AIDS with manifestation of BOOP by open lung biopsy and showed good response to Bactrim & corticosteroid therapy.

Clinical and Endoscopic Features of Colonic Anisakiasis in Korea

  • To analyze the clinical and endoscopic features of colonic anisakiasis. A retrospective chart review of 20 patients with colonic anisakiasis, who were diagnosed by colonoscopy at 8 hospitals between January 2002 and December 2011, was performed. Patients' mean age was $53.6{\pm}10.74years$. Seventy percent patients were men. Acute abdominal pain was a common symptom that mostly developed within 48 hr after the ingestion of raw fish, and which lasted for 1-28 days. Sixty percent patients had ingested raw fish before the diagnosis of colonic anisakiasis and 40% patients were incidentally found to have colonic anisakiasis during the screening colonoscopies. Leukocytosis and eosinophilia were each found in 20% of the patients. In all patients who underwent colonoscopy, the worms were removed with biopsy forceps, except in 1 case, and a definite diagnosis of anisakiasis was made. In some cases of colonic anisakiasis, colonoscopy may be helpful in the diagnosis and treatment to avoid surgical intervention.

  • The purpose of this study is to suggest the direction of the future fashion by grasping the cyber fashion, which is discussed outstandingly in the recent fashion world, into internal expression and external features. The result of this study is as follows: Cyber fashion means the application of the electronic image, dynamic phenonmenon of machine, and the effects of light to fashion, and it has some external features of geometrical pattern, dynamic structure, and high-tech material and color: Aside from this noticeable characteristics, cyber fashion has some internal features of the direction toward future, the anti-culture, and the surreal. In this cyber fashion, first, computer will be introduced and used as the means to realize a dream of human being. Second, it will be designed with the ideal feature of future society. Third, it will be possible to develope material and design to solve ecological issue of human beings. Fourth, the fashion to give the peace and stability to human being will be popular.

Imatinib-induced hepatitis treated by corticosteroids in a patient with metastatic gastrointestinal stromal tumor

  • Imatinib mesylate is currently used as the first-line treatment for metastatic gastrointestinal stromal tumors (GISTs). Imatinib-induced hepatotoxicity in patients with GIST is very rare. Its features vary from subclinical elevation of serum aminotransferase to clinically apparent acute hepatitis, which is associated with immunologic reactions. Imatinib-induced hepatotoxicity with autoimmune-like features can be treated by the discontinuation of imatinib mesylate and the administration of oral steroids. Here, we report a case of late-onset imatinib-induced hepatitis with autoimmune-like features in a patient with metastatic GIST, which was improved by oral corticosteroids.
