제2형 당뇨병 환자의 칼슘 섭취가 혈중지질, 혈압, 염증 및 혈당 관련 지표에 미치는 영향 (Dietary Calcium Intake is associated with Blood Lipid Profile, Blood Pressure, Inflammatory State and Insulin Resistance in Type 2 Diabetes Patients)

  • This study was conducted to determine the association between dietary calcium intake and biomarkers related to lipid and glucose metabolism and inflammation in Korean patients with type 2 diabetes. Seventy-five subjects (41 males, 34 females) were recruited from a group of patients who had visited the department of endocrine medicine. Data on anthropometric characteristics, clinical indices such as hemoglobin A1c and C-reactive protein (CRP), and dietary nutrient intakes were collected. Subjects were divided into three groups on the basis of their calcium intake [< EAR (below estimated average requirement), EAR-RNI (between EAR and recommended nutrient intake), > RNI (above RNI)]. Average calcium intake of < EAR, EAR-RNI, > RNI groups were $462.7{\pm}18.7$, $649.7{\pm}12.8$, and $895.7{\pm}21.7mg$, respectively. Energy intake was not different among groups but intakes of protein, total and saturated fatty acids were significantly higher in > RNI group than < EAR group. Analysis of covariance revealed that HDL cholesterol level was significantly higher in EAR-RNI group, as compared to < EAR group after adjustment with confounders such as age, sex, BMI and energy intake (p < 0.05). Levels of CRP and homeostasis model assessment 2-insulin resistance (HOMA2-IR) were significantly lower in EAR-RNI group. Total cholesterol level was higher in EAR-RNI and > RNI groups, although within the normal range. Our results suggest that dietary calcium intake may influence the levels of HDL-cholesterol, CRP and HOMA2-IR and subsequently, help management/treatment of type 2 diabetes patients.

Uncertainty reduction of seismic fragility of intake tower using Bayesian Inference and Markov Chain Monte Carlo simulation

  • The fundamental goal of this study is to minimize the uncertainty of the median fragility curve and to assess the structural vulnerability under earthquake excitation. Bayesian Inference with Markov Chain Monte Carlo (MCMC) simulation has been presented for efficient collapse response assessment of the independent intake water tower. The intake tower is significantly used as a diversion type of the hydropower station for maintaining power plant, reservoir and spillway tunnel. Therefore, the seismic fragility assessment of the intake tower is a pivotal component for estimating total system risk of the reservoir. In this investigation, an asymmetrical independent slender reinforced concrete structure is considered. The Bayesian Inference method provides the flexibility to integrate the prior information of collapse response data with the numerical analysis results. The preliminary information of risk data can be obtained from various sources like experiments, existing studies, and simplified linear dynamic analysis or nonlinear static analysis. The conventional lognormal model is used for plotting the fragility curve using the data from time history simulation and nonlinear static pushover analysis respectively. The Bayesian Inference approach is applied for integrating the data from both analyses with the help of MCMC simulation. The method achieves meaningful improvement of uncertainty associated with the fragility curve, and provides significant statistical and computational efficiency.

우리나라 농촌지역 노인들의 영양점검표(DETERMINE)를 통한 계절별 영양상태 판정의 신뢰도 분석 연구 (Reliability of Nutritional Screening Using DETERMINE Checklist for Elderly in Korean Rural Areas by Season)

  • It is very important to screen the elderly for nutritional risk, because nutritional status is a critical factor to maintain their health. Some nutrition checklists used in Korea for the elderly are from other countries. Reliability of those checklist in Korea is not studied enough. This survey was done for the elderly over 65-years-old who live in Hong-cheon, An-dong, Dam-yang and Yeon-gi in Korea (subject; summer: 146, winter: 145) to study the reliability of DETERMINE checklist which is adopted widely in Korea. Using the score of DETERMINE checklist, the elderly were divided as high, middle and low risk groups. For nutritional assessment for those elderly, dietary assessment using 24 recall, anthropometry, biochemical assessment and health condition were used. Results for the checklist showed that percentage below EAR for energy intake and protein intake in winter were higher in the high risk group than other groups. The intakes of phosphorus and iron and most vitamins were below the DRI. The percentage of subjects with intake below DRI was highest in the high risk group. Sensitivity, specificity, and positive predictive values using the DETERMINE were calculated using 6 point as a cut-off point. Subjects were divided into two groups by MAR (MAR < 0.75:undernutrition, MAR < 0.75 : normal). Sensitivity recorded 49.4% and 34.3%, specificity did 61.9% and 65.4 and Positive predictive value did 62.1% and 46.0% each for summer and winter. Results of screening using DETERMINE Checklist were not matched with dietary assessment but not with anthropometric and biochemical measurement. In conclusion DETERMINE 'Checklist' is shown be a good screening tool for finding out risk groups for dietary intake in the elderly, It needs to verify reliability and validity through large-scale survey.

Dietary intake of n-3 and n-6 polyunsaturated fatty acids in Korean toddlers 12-24 months of age with comparison to the dietary recommendations

  • BACKGROUND/OBJECTIVES: Adequate dietary fatty acid intake is important for toddlers between 12-24 months of age, as this is a period of dietary transition in conjunction with rapid growth and development; however, actual fatty acid intake during this period seldom has been explored. This study was conducted to assess the intake status of n-3 and n-6 polyunsaturated fatty acids by toddlers during the 12-24-month period using 2010-2015 Korea National Health and Nutrition Examination Survey data. SUBJECTS/METHODS: Twenty-four-hour dietary recall data of 12-24-month-old toddlers (n = 544) was used to estimate the intakes of ${\alpha}$-linolenic acid (ALA; 18:3n-3), eicosapentaenoic acid (EPA; 20:5n-3), docosahexaenoic acid (DHA; 22:6n-3), linoleic acid (LA; 18:2n-6), and arachidonic acid (AA; 20:4n-6), as well as the major dietary sources of each. The results were compared with the expected intake for exclusively breastfed infants in the first 6 months of life and available dietary recommendations. RESULTS: Mean daily intakes of ALA, EPA, DHA, LA, and AA were 529.9, 22.4, 37.0, 3907.6, and 20.0 mg/day, respectively. Dietary intakes of these fatty acids fell below the expected intake for 0-5-month-old exclusively breastfed infants. In particular, DHA and AA intakes were 4 to 5 times lower. The dietary assessment indicated that the mean intake of essential fatty acids ALA and LA was below the European and the FAO/WHO dietary recommendations, particularly for DHA, which was approximately 30% and 14-16% lower, respectively. The key sources of the essential fatty acids, DHA, and AA were soy (28.2%), fish (97.3%), and animals (53.7%), respectively. CONCLUSIONS: Considering the prevailing view of DHA and AA requirements on early brain development, there remains considerable room for improvement in their intakes in the diets of Korean toddlers. Further studies are warranted to explore how increasing dietary intakes of DHA and AA could benefit brain development during infancy and early childhood.

Contribution of foods to absolute nutrient intake and between-person variations of nutrient intake in Korean preschoolers

  • BACKGROUND/OBJECTIVES: The aim of this study was to analyze specific foods influencing absolute nutrient intake and between-person variations of nutrient intake among Korean preschoolers. SUBJECTS/METHODS: This study included 2,766 participants aged 1-5 years in the 2009-2013 Korea National Health and Nutrition Examination Surveys. Dietary data were obtained from a 24-h dietary recall method. Major food sources of absolute nutrient intake were evaluated based on percent contribution of each food. To assess the contribution of specific foods to between-person variations in nutrient intake, stepwise multiple regressions were performed and cumulative $R^2$ was used. RESULTS: White rice and milk were main food sources of energy, protein, carbohydrate, phosphorus, iron, potassium, thiamin, riboflavin, and niacin. The percentage of fat contributed by milk was 21.3% which was the highest, followed by pork, soybean oil, and egg. White rice accounted for 25% and 40% of total variability in total energy and carbohydrate intakes, respectively. About 39% of variation in calcium intake was explained by milk while 40% of variation in phosphorous intake was explained by cheese. The top 10 foods contributing to between-person variations in nutrient intakes were similar with food items that mainly contributed to absolute nutrient intakes. The number of foods explaining 90% of absolute amounts of nutrient intakes varied from 28 for vitamin A to 80 for iron. CONCLUSIONS: This study identified specific foods that contributed to absolute nutrient intakes and between-person variations in nutrient intakes among Korean preschoolers. Our findings can be used to develop dietary assessment tools and establish food-based dietary guidelines for young children.

요양병원 입원 노인의 자아존중감에 따른 건강상태와 간이영양상태 비교 (Comparison of Health Status and Mini Nutritional Assessment according to Self-esteem of Elderly in Care Hospital)

  • The purpose of the study was to examine health status and mini nutritional assessment (MNA) according to self-esteem status of elderly in care hospital. The survey was carried out from $13^{th}$ to $31^{st}$ of September, 2014 in five care hospitals. Analysis was performed for 226 subjects. Evaluation criteria included demographics, clinical status, MNA, health-related quality of life (EQ-5D) and self-esteem. The SE I, SE II and SE III groups were divided by self-esteem scores. Data were analyzed one-way ANOVA, Duncan' multiple range test and Pearson's correlation using SPSS 15.0. Education level, religion, dental condition and medical endurance type were significantly different in all groups. Many elderly people were normal BMI ($20.94{\sim}21.67kg/m^2$). Self-esteem significantly increased in order of SE I, SE II and SE III. Mobility, usual activity and anxiety or depression were significantly different in all groups, and EQ-5D of SE II and SE III groups were significantly higher than that of SE I group. Comparison of MNA screening score in elderly patients was as follows : Score for food intake, psychological stress or acute disease, neuropsychological problems, pressure scores or skin ulcer, protein intake, mode of feeding, nutritional status and health status in SE I group were significantly lower than those of SE II and SE III groups. Mobility and drug intake of SE I group were significantly lower than those of SE III groups. Fruit and vegetable intake SE I group were significantly higher than those of SE II and SE III groups. MIS (Malnutrition Indicator Score) was significantly different among the SE I, SE II and SE III groups. We conclude that self-esteem score is positively correlated with protein intake, nutrition status, health status and MIS in elderly care hospital. To improve nutritional status of elderly in care hospitals, systematic nutrition management and self-esteem education program should be implemented.

선진국의 토양위해성평가 모델 비교분석 연구 (Comparative Study of Soil Risk Assessment Models used in Developed Countries)

  • 우리나라는 최근에 토양위해성평가 지침을 마련하였으며, 앞으로 토양위해성모델 개발에 대한 필요성이 대두되고 있다. 선진국에서는 자국 내 부지특성과 노출경로를 고려한 토양위해성평가 모델을 토양정책 전반에 활용하고 있다. 본 연구에서는 선진국에서 위해성평가에 이용되는 대표적인 오염토양 위해성평가 모델을 비교분석하였다. 연구대상이 된 모델은 미국, 영국, 네덜란드에서 사용하고 있는 CalTOX, CLEA, CSOIL로서, 노출경로, 토지이용도 그리고 노출량 산정식을 중심으로 비교분석하였다. 모델 검토시 우선적으로 비교 분석된 항목은 노출시나리오, 노출경로, 입력변수의 공통사항이며, 이러한 분석결과를 토대로 선진국의 토양위해성 모델들이 공통적으로 포함하고 있는 노출경로를 추출하여, 국내에 적용 가능한 토양 위해성평가 모델개발의 기초자료로 제안하였다. 인체노출량 산정식에서는 일반적으로 미국식 방법이 국내 상황에서 사용이 용이한 것으로 판단되며, 비산먼지나 휘발물질 흡입의 경우는 네덜란드식이 기본값이 제공되어 있으므로 사용하기가 편리할 것으로 판단된다.

Microplate Reader를 이용하여 측정한 혈액의 엽산 농도와 실측량 기록법에 의한 엽산 섭취량 (Blood Folate Level Determined by a Microplate Reader and Folate Intake Measured by a Weighted Food Record)

  • Microbiological method using a 96-well microplate reader for folate assay was established, and folate intake and blood folate concentrations of 23 female college students were assessed. To evaluate folate intake, dietary data were collected by a 3-day weight food record, and serum and RBC folate concentrations were measured by the new method. The coefficient of variation for the new method was less than 10%. Mean daily folate intake of the subjects was 126.7${\mu}g$ which is only 50.7% of the RDA. The mean concentrations of serum and RBC folate were 7.46ng/ml and 294.4ng/ml, respectively, which were within the normal range. These results indicate that folate intake seems to be underestimated due to incomplete food composition database. Therefore, folate database should be appropriately in order to asses folate intake accurately.

국민건강영양조사를 활용한 담수어패류 섭취량 산정: 위해성 평가를 위한 파라메타 도출 (Estimating Freshwater Fish Intake for Human Health Risk Assessment Using Korea National Health and Nutrition Examination Survey)

  • Freshwater fish intake is a critical parameter for deriving water quality criteria and water quality standards for protection of human health based on human health risk assessment. Although the freshwater fish intake parameter should be accurate and representative of Korean fish consumption for the water quality criteria to be reliable, data are limited in Korea and have low reliability. In this study, Korean National Health and Nutrition Examination Survey data from 2008-2010 were analyzed to reevaluate freshwater fish consumption. Based on these results, an average consumption rate of 3.0 g/day per person, a $90^{th}$ percentile consumption rate of 0.0 g/day per person, an average consumption rate of 65.7 g/day per fish consumer, and a $90^{th}$ percentile consumption rate of 153.4 g/day per fish consumer were proposed for derivation of water quality criteria using a conservative approach and various exposure scenarios.

  • In 1990, Tolerable Daily Intake (TDI) of 10 pg TCDD/kg/day for dioxins based on carcinogenicity and reproductive toxicity was determined by WHO/EURO, that resulted in the establishment of TDIs in other countries. In Japan, Ministry of Health and Welfare and Environment Agency, respectively established the TDI of 10 pg TCDD/kg/day and Health Risk Assessment Index of 5 pg TCDD/kg/day in 1996. Accumulation of new scientific data, especially by molecular toxicology since 1990, resulted in the reevaluation of TDI by WHO-ECEH and IPCS in May, 1998. At this meeting, it was stressed that \circled1 toxic effects of dioxin is mediated through Ah-receptor in both animals and humans, \circled2 use of ebody burdeni concept is better than the use of traditional NOAEL/UF approach, \circled3 inclusion of coplanar PCBs in the TDI by the use of new WHO-TEF. LOAELs (0.16~200 ng TCDD/kg/day) obtained from reproductive toxicity and immunotoxicity in rats, and neurobehavioral toxicity and induction of endometriosis in rhesus monkeys are calculated to be the body burden of 10~50 ng TCDD/kg that is 14~37 pg TEQ/kg/day as human daily intake. Finally TDI of 1~4 pg TEQ/kg/day was established by applying the UF of 10. In Japan, reproductive toxicity and immunotoxicity in rats were used to obtain LOAELs (100~200 ng TCDD/kg/day). Finally TDI of 4 pg TEQ/kg/day was established in June 1999 by applying the UF of 10 to human daily intake of 43.6 pg TEQ/kg/day which corresponds to the body burden of 86 ng TCDD/kg.
