Objective: Hwa-byung is one of the cultural concept of distress in Korea resulted from chronic accumulated anger. It is characterized by various symptoms like stuffy in the chest, hot or heat sensation, something pushing up in the chest, feeling of mortification, and a flush of anger. This protocol aims to explore the efficacy and safety of Huanglian-jie-du decoction on various somatic symptoms and insomnia in patients with Hwa-byung. Methods: This is study protocol for a randomized, double-blind, placebo-controlled trial. A total of 44 patients will be randomly assigned to the experimental group or the placebo group in a 1:1 ratio. All medications will be taken orally 3 times per day for 7 consecutive days. The primary outcomes are the mean changes in Patient Health Questionnaire of physical symptoms (PHQ-15) and Insomnia Severity Index (ISI) after the 7 days of administration. The secondary outcomes include the scales to assess stress response, symptoms of Hwa-byung, and state anger. Conclusion: The results of this study will provide high quality and explorative evidence to investigate the effect of Huanglian-jie-du decoction on Hwa-byung.
Objectives : This study aimed to suggest distinct signs of Greater yin disease (tàiyīnbìng, 太陰病) by researching the etymology of three terms: zi-tong (自痛), jie-ying (結硬), and yin-er (因爾), which are exclusive found only in Greater yin disease (太陰病) in Gangpyeong-shanghanlun, but not in other categories. Methods : We studied the etymologies of the three terms that are unique in Greater yin disease, then found some pathological signs that are related with them through the four mental illness cases, which include somatization, obsessive-compulsive behavior, delusion, and panic disorder. Results : Based on the definitions of each term, we diagnosed the four patients who had mental illnesses as having Greater yin disease, and we observed meaningful improvements after administering herbal medication. After Gyejigajakyak-tang administration, the Insomnia Severity Index (ISI) score decreased from 18 to 7 and the Beck's Anxiety Index (BAI) score decreased from 36 to 18 in the first case, the ISI score decreased from 27 to 16 and the BAI score decreased from 50 to 33 in the second case, and the ISI score decreased from 23 to 4 and the BAI score decreased from 34 to 5 in the third case. In the last case, the ISI and BAI scores were 16 and 22, respectively, at the first visit, and it was found that the scores had changed to 6 and 22, respectively, at the last visit. Conclusions : We found that the unique terms in Greater yin disease, including zi-tong (自痛), jie-ying (結硬), and yin-er (因爾), can be interpreted as groundless arbitrary assumption, resting strong tension, and psychological projection based on the etymological hypothesis. Therefore, we suggest these as specific signs of Greater yin disease.
본 연구는 라텍스 매트리스가 수면의 질과 편안함에 미치는 영향을 평가하기 위하여 수행되었다. 대상자들은 임상의의 진찰을 통해 중증의 불면증을 비롯한 수면에 영향을 줄 수 있는 질병이 없는 건강한 성인 11명(남 5명, 여 6명, 나이 37.7세, 키 167.8cm, 체중 67.0kg)으로 구성되었다. 대상자들은 개인 생활공간에서 7일 동안 사용하던 스프링 매트리스에서 수면을 취하였고, 이를 웨어러블 디바이스(Fitbit)와 수면일지, PSQI, ISI, 만족도 설문을 사용하여 기록하였다. 이어서 대상자들의 매트리스를 라텍스 매트리스로 교체하여 14일 동안 사용하게 하였고, 이를 기록하였다. 그 결과 Fitbit 상에서 라텍스 매트리스를 사용 후 주중 수면 시간이 62.9분, 주말 수면 시간은 53.2분 증가하는 것으로 나타났으며, ISI 설문에서는 3.8점이 유의하게 감소하였다. PSQI 설문에서는 poor sleeper가 10명에서 7명으로 줄었으나 통계적으로 유의하지는 않았고, 만족도 설문에서는 허리, 목, 어깨의 편안함이 유의하게 증가하였다. 이러한 결과로 미루어 볼 때, 라텍스 매트리스는 객관적 및 주관적 수면의 질에 긍정적인 영향을 미칠 수 있을 것으로 보여진다.
목적: 섬망은 말기암환자에서 생길 수 있는 중요한 합병증으로 임상적으로나 삶의 질 적인 측면에서 매우 중요한 문제이다. 이에 호스피스 병동에 입원한 말기암 환자에서 섬망 발생과 관계된 위험 요인들을 알고자 연구를 시행하였다. 방법: 2011년 5월부터 2012년 9월까지 일개 지역 두개의 종합병원 내 호스피스 병동에 입원한 환자 중 의식이 명료하고 다른 정신과적 질환이나 약물 중독이 없는 환자를 대상으로 전향적으로 추적하여 섬망 발생 여부를 확인하였다. DSM-IV 진단기준에 맞게 2명의 의사에 의해 진단된 경우 섬망군으로 분류하였다. 입원 시 인지기능(mini-mental status examination, MMSE), 우울 점수(Beck depression inventory, BDI), 불안 점수, 불면 척도(Insomnia Severity Index, ISI), 혈액검사 결과와 섬망 발생 여부를 로지스틱 회귀분석으로 비교하고, 유의한 항목에 대해 Cox의 비례위험모형으로 교차비를 구하였다. 결과: 선정기준에 부합하는 연구 대상자 96명 중 41명(42.7%)에서 섬망이 발생하였다. 로지스틱 회귀분석 결과 일차 암 위치, 인지기능 장애(MMSE < 24), 우울($BDI{\geq}16$), 불면($ISI{\geq}15$)의 Odds ratio (OR)이 통계적으로 유의한 결과를 보였다. 이들 4가지 인자에 대해 입력 방법을 이용해 시행한 Cox 회귀분석 결과, 우울의 OR 5.130(95% CI 2.009~13.097), 인지기능저하의 OR 5.130 (95% CI 2.009~13.097)로 나타났다. 결론: 호스피스 병동에 입원한 말기암환자에서 우울하거나 인지기능 장애가 있을 경우 섬망 발생의 위험이 유의하게 높았다. 호스피스 환자에게 우울이나 인지기능에 대한 주의 깊은 관찰이 필요할 것이다.
Objectives: This case report aims to report the clinical effectiveness of the combination treatment of Korean medicine with Guibiondamtang-gamibang on Hwa-byung patients. Methods: Case 1 experienced stress at work and complained of chest discomfort, nausea, dizziness, insomnia. Case 2 experienced huge stress about money, and complained of palpitation, chest discomfort, dizziness and insomnia. They received combination treatment of Korean medicine during hospitalization. The treatment included Guibiondamtang-gamibang, acupuncture, moxibustion, pharmacoacupuncture, and fumigation. The effects were evaluated through Hwa-byung scale, instrument of oriental medical evaluation for Hwa-byung, insomnia severity index (ISI) and heart rate variability (HRV) test. Results: After the treatment, the clinical symptoms such as chest discomfort, palpitation, dizziness, nausea and insomnia were improved. Conclusions: This case report shows that the combination treatment of Korean medicine with Guibiondamtang-gamibang may be effective for treating Hwa-byung patients.
Objective : The present study had examined the psychiatric symptoms and physical health consequences for the bereaved parents of the high school students who died in the 2014 Sewol ferry accident. Methods : Forty bereaved parents participated in the study. The authors administered self-report questionnaires about the parents' health behaviors and psychiatric symptoms. The authors also conducted laboratory tests to assess the parents' physical health at 18 and 30 months after the accident. Univariate descriptive statistics were performed to report the prevalence and severity of psychiatric symptoms and health-related behaviors. Paired t-test and Mcnemar test were performed to compare the 18-and 30-month findings. Correlation analysis between psychiatric symptoms and laboratory findings were performed to find a relationship between the two variables. Results : At 30 months after the accident, most of the bereaved parents still appeared to suffer from complicated grief (97.5%), post traumatic stress disorder (80%), insomnia (77.5%) and severe depression (62.5%) based on the scores on the Inventory of Complicated Grief (ICG), the PTSD Check List-5 (PCL-5), the Insomnia Severity Index (ISI) and the Patient Health Questionnaire-9 (PHQ-9). One quarter of the bereaved parents reported high-risk drinking, and 47.5% reported increased drinking amount and frequency after the accident. In objective laboratory results, 55% of the bereaved parents were obese as defined by body mass index ${\geq}25$. The parents' mean low-density lipoprotein shows a significant increase over time (118.5 mg/dL at 18 months. vs. 132.5 mg/dL at 30 months. paired t-test t=-4.061, $$p{\leq_-}0.001$$). Total cholesterol and low-density lipoprotein at 30 months after the accident were in clinically borderline high range. In correlation analysis, triglyceride was positively correlated with ISI. Conclusion : The loss of children in the Sewol ferry accident, a disaster caused by human error, continued to have considerable impact on the victims' parents' mental and physical health 18 and 30 months after the accident. A longitudinal study following the parents' physical health would be necessary to investigate the long-term effects of this traumatic experience on physical health.
Purpose: The purpose of this study was to identify associations among the sleep disturbance, fatigue, job stress, and blood and body fluid (BBF) exposure of shift-work nurses. Methods: A total of 299 shift-work nurses from two tertiary hospitals were enrolled in this study. We used the Insomnia Severity Index (ISI), and the Fatigue Severity Scale (FSS), and the Korean Occupational Stress Scale-Short Form (KOSS-SF) to evaluate sleep disturbance, fatigue, and job stress, respectively. The data were analyzed using t-test or chi-squared test and Logistic regression analysis using the SPSS 23.0 program. Results: We found that 43.8% of participants reported BBF exposure over the past year. Splash or exposure to broken skin of BBF were most frequent (56.9%), and followed by needlestick injuries (30.4%) and sharp injuries (12.8%). Age, hospital, working period, level of stress, sleep disturbance ($ISI{\geq}15$), fatigue (FSS $score{\geq}4$), job demand and organizational climate subset in KOSS-SF were significantly associated with BBF exposure in shift-work nurses. In multivariate analysis after adjusting age and hospital, the risk factors of BBF exposure in shift-work nurses were the level of stress and fatigue (FSS $score{\geq}4$). Conclusion: Fatigue and job stress were related to BBF exposure in shift-work nurses. Our results suggest that management of sleep disturbance, fatigue, and high job stress in shift-work nurses is needed to reduce risk of BBF exposure.
Objectives: The purpose of this study was to evaluate the reliability and validity of the instrument on pattern identification for insomnia (PIT-Insomnia) and verify the correlation between PIT-Insomnia and psychological tests. Methods: Two evaluators examined the pattern identification of the participants who met insomnia disorder diagnostic criteria of the Diagnostic and Statistical Manual of Mental Disorder, Fifth Edition (DSM-5) and took the Insomnia Severity Index (ISI) score over 15 once manually and twice using the PIT-Insomnia to measure the inter-rater and test-retest reliability. We also conducted the following surveys: the Pittsburgh Sleep Quality Index (PSQI), the Korean version of Beck's depression inventory (K-BDI), the Korean version of the State-Trait Anxiety Inventory (STAI-K), the Korean Symptom checklist-95 (KSCL-95), and the EuroQol-5 dimension (EQ-5D), to measure concurrent validity and correlation between the PTI-Insomnia and psychological tests. Results: 1. The test-retest reliability analysis of the pattern identification results showed moderate agreement, and test-retest reliability analysis of each pattern identification score showed agreements from poor to moderate. 2. The inter-rater reliability analysis of the pattern identification results via manual showed slight agreement, when analysis was performed with calibration, the inter-rater reliability analysis of the pattern identification results via manual showed fair agreement. 3. The concordance analysis between results via manual and the PIT-Insomnia showed poor agreement, when the analysis was performed with calibration, concordance analysis showed fair agreement. 4. The concordance analysis between the PIT-Insomnia and the PSQI showed positive linear correlation. 5. The concordance analysis between the PIT-Insomnia and the PSQI, K-BDI, STAI-K, KSCL-95, and EQ-5D showed that non-interaction between the heart and kidney have positive linear correlation with the K-BDI, anxiety item of KSCL-95, dual deficiency of the heart-spleen have positive linear correlation with somatization item of KSCL-95, paranoia item of KSCL-95, heart deficiency with timidity have positive linear correlation with stress vulnerability item of KSCL-95, parania item of KSCL-95, phlegm-fire harassing the heart have positive linear correlation with K-BDI, paranoia item of KSCL-95, depressed liver qi transforming into fire have positive linear correlation with the anxiety item of KSCL-95, parania item of KSCL-95, all pattern identification have negative linear correlation with EQ-5D. Conclusions: The PIT-Insomnia has moderate agreement of reliability and reflects the severity of insomnia since it has some concurrent validity with the PSQI. There are some correlations between the PTI-Insomnia with specific psychological tests, so we could suggest it can be used appropriately in the clinical situation.
Purpose: Temporomadibular disorders (TMDs) can result in chronic pain, which is often associated with psychological and sleep disturbance. Increased levels of psychological and sleep impairments are often related with poor treatment outcomes. The purpose of the present study was to evaluate clinical features, psychological profiles and the prevalence of clinical insomnia in TMD patients with chronic pain and to suggest an effective treatment approach. Methods: A total of 200 TMD patients who had visited the Pusan National University Hospital dental clinic for treatment of their pain were recruited from June 2018 through December 2019. TMD patients were classified into an acute (n=100) and chronic (n=100) group and compared the clinical symptoms. The primary diagnosis of TMD were categorized as TMD with joint pain group (TMD_J), TMD with muscle pain group (TMD_M) and TMD with joint-muscle mixed pain group (TMD_JM). Self- report measures of sleep quality and psychological profiles were evaluated via the Insomnia Severity Index (ISI) and the Korean Stress Response Inventory (SRI). Independent t-tests, Mann-Whitney U-tests, and chisquared test were used for the statistical analysis. Results: Chronic TMD patients showed higher pain intensity, as well as higher prevalence of related symptoms (headache, musculoskeletal pain) and myogenous pain. They also had significantly higher scores in all SRI parameters and a higher percentage of clinical insomnia than acute TMD patients. Conclusions: Based on the above results, psychological profiles and sleep quality assessments are necessary to provide essential data that will allow for improved treatment of chronic TMD patients.
목적: 치아를 상실하여 치료를 받기 위해 내원하는 보철과 환자를 대상으로 처음 내원할 당시 스트레스에 대한 반응과 수면의 상태가 어떤지 살펴보고자 하였다. 대상 및 방법: 2018년부터 2년간 부산대학교병원 보철과에 신환으로 접수한 236명의 성인 환자(남자 94명, 여자 142명, 24 - 86세)를 대상으로 검사 및 상담, 설문조사를 시행하였다. 잔존치 수, 무치악 여부, 저작 가능 여부, 향후 보철치료 방향을 기록하였으며, 시각화 아날로그형 척도(Visual analogue scale, VAS), 스트레스 반응 척도(Stress reponse inventory, SRI) 및 불면증 심각도 지수(Insomnia severity index, ISI)를 도구로 사용하여 환자의 주관적 구내불편감, 스트레스 반응, 수면 상태를 측정하였다. 결과: 전체 보철과 신환에서 SRI 의 7개 항목 중 '피로'가 유의하게 높은 평균값을 나타냈고 '공격성'이 유의하게 낮은 값을 보였다 (P < .001). 무치악자와 완전유치악자는 VAS로 측정한 구내 불편감 값에 유의한 차이가 있었다 (P = .004). 20개 미만의 치아가 잔존한 경우, 스스로 저작이 불가능한 경우, 가철성 보철물을 치료계획을 잡게 되는 환자의 경우는 그 반대의 경우에 비하여 VAS 값, SRI 중 '우울'과 '피로' 값이 유의하게 높게 드러났다 (P < .05). VAS 수치가 심각에 해당하는 환자의 경우 심각에 해당하지 않는 환자에 비하여 SRI 항목 중 '긴장', '신체화', '우울', '피로', '좌절'의 값이 유의하게 높았다 (P < .05). 임상적 불면증은 전체 환자 중 11.4%에서 나타났고, 여자가 남자보다 수면의 질이 유의하게 낮았다 (P = .044). 불면증을 가진 사람은 정상수면자에 비해 VAS값과 7가지 모든 SRI 항목에 대해 유의하게 높은 점수가 나타났다 (P < .05). 결론: 보철 신환에서 스트레스에 대한 반응 검사(SRI)를 시행하였을 때 7가지 스트레스 반응 항목 중 '피로' 항목이 높게 나타났으며, 잔존치 수와 저작 기능으로 나타나는 구강 상태, 불면증 유무는 주관적 구내불편감, 스트레스 반응 항목 및 정도에 영향을 주었다.

제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
