A Case Report of a Chronic Schizophrenia Treated with Combined Treatment of Korean and Western Medicine

  • Objectives: Schizophrenia is difficult to treat effectively and the antipsychotics used have many side effects. However, few studies have focused on the combined treatment of Korean and Western medicine as an alternative. In this study, we reported an inpatient with chronic schizophrenia who was treated by a combination of Korean and Western medicine. Methods: We experienced a case of a diagnosed schizophrenia patient as whose chief complaint was avolition, diminished emotional expression and hallucination. The patient was treated with Western medicine and Korean traditional treatment including acupuncture, moxa and herbal medicine. The Brief Psychiatric Rating Scale (BPRS) and the Positive and Negative Syndrome Scale (PANSS) were used for assessment. Results: After treatment, symptoms involving avolition, diminished emotional expression and hallucination were improved; furthermore, the scores of the BPRS and the PANSS were decreased by approximately 50%, respectively. In addition, there were no notable side effects. Conclusions: The combined treatment of Korean and Western medicine can be an effective and well-tolerated treatment modality for patients with chronic schizophrenia.

A Case Report of Traditional Korean Medicine Treatment for Broca's Aphasia Associated with Cerebral Hemorrhage after Middle Cerebral Artery Infarction (중대뇌동맥 뇌경색 이후 발생된 뇌출혈에 동반된 브로카실어증(Broca aphasia) 환자에 대한 한의치료 증례보고 1례)

  • Objective: The aim of this case report was to present the effects of traditional Korean medicine on a patient with Broca's aphasia associated with cerebral hemorrhage after middle cerebral artery infarction. Methods: The Korean version of the Western Aphasia Battery (K-WAB) and the Evaluation of Articulator Performance were used to evaluate the language functions of the patient. Herbal medication and acupuncture were administered to improve the patient's symptoms. Results: The inpatient and outpatient treatments improved the patient's symptoms. The aphasia quotient (AQ) score of the K-WAB test before treatment was 6.4, but it increased to 21.4 after treatment. The Evaluation of Articulator Performance score improved by 0.0 points before treatment, by 1.0 points after 9 days of treatment, and by 1.5 points after 42 days of treatment. Conclusions: This case report suggest that Korean medical therapy can be effective in improving the language functions of patients with Broca's aphasia.

A Review of the Domestic Study Trends on Korean Medicine Treatment after Hysterectomy (자궁적출술 후 한의약 치료에 대한 국내 임상 연구 고찰)

  • Objectives: The purpose of this review is to analyze the domestic study trends on Korean medicine treatment for patients after hysterectomy. Methods: We investigated the studies on Korean medicine treatment for patients who undertaken hysterectomy via searching 4 online databases up to October 2020. After searching studies, we analyzed selected studies. Results: 12 clinical studies were selected and all studies were case reports. The most common cause of hysterectomy was myoma of uterine, followd by adenomyosis. The major complaints of post-hysterectomy were abdominal pain, low back pain and fatigue, general weakness. Herbal medicine was used in all studies. In addition, acupuncture, moxibustion and cupping were performed. The duration of inpatient treatment ranged from 5 to 39 days. All cases showed that chief complaints and accompanying symptoms were improved. Conclusions: Korean medicine treatment is an effective treatment option for patients after hysterectomy. Further large, well-designed studies are needed to establish the foundation of Korean medicine treatment after hysterectomy.

Five Clinical Cases of Facial Chuna Manual Therapy with Korean Medicine Treatment for Acute Bell's Palsy

  • This study individually analyzed the effects of Korean Facial Chuna Manual Treatment (K-FCMT) combined with Korean medicine (KM) treatment (acupuncture, electroacupuncture, pharmacopuncture, and herbal medicine) on five patients with acute Bell's palsy who visited Dongsuwon Korean Medicine Hospital between August 1 and 31, 2022. During inpatient treatment, two of the five patients received K-FCMT 5-6 times a week, and the other three received the same frequency during outpatient treatment for approximately 2 weeks. Patients with a House-Brackmann grading scale (HBGS) score of ≤4 and Yanagihara unweighted grading system (Y-score) ≥7-8 began to recover after the 2nd-3rd sessions of K-FCMT, which entered the recovery phase quickly. As patients entered the recovery phase (7-9th sessions of K-FCMT), symptoms improved to HBGS scores of 1-2 and Y-scores of 35-40 points. This study suggests the possibility of applying K-FCMT combined with KM treatment to patients in the acute stage of Bell's palsy.

An Analysis of Tasks of Nurses Caring for Patients with COVID-19 in a Nationally-Designated Inpatient Treatment Unit (국가지정 입원치료병상에 입실한 COVID-19 환자를 돌보는 간호사의 업무분석)

  • Purpose: The purpose of this study was to provide foundational knowledge on nursing tasks performed on patients with COVID-19 in a nationally-designated inpatient treatment unit. Methods: This study employs both quantitative and qualitative approaches. The quantitative method investigated the content and frequency of nursing tasks for 460 patients (age ≥ 18 y, 57.4% men) from January 20, 2020, to September 30, 2021, by analyzing hospital information system records. Qualitative data were collected via focus group interviews. The study involved interviews with three focus groups comprising 18 nurses overall to assess their experiences and perspectives on nursing care during the pandemic from February 3, 2022, to February 15, 2022. The data were examined with thematic analysis. Results: Overall, 49 different areas of nursing tasks (n = 130,687) were identified based on the Korean Patient Classification System for nurses during the study period. Among the performed tasks, monitoring of oxygen saturation and measuring of vital signs were considered high-priority. From the focus group interview, three main themes and eleven sub-themes were generated. The three main themes are "Experiencing eventfulness in isolated settings," "All-around player," and "Reflections for solutions." Conclusion: During the COVID-19 pandemic, it is imperative to ensure adequate staffing levels, compensation, and educational support for nurses. The study further propose improving guidelines for emerging infectious diseases and patient classification systems to improve the overall quality of patient care.

The Lived Experience of Relapse and Treatment Among the Alcoholics (알코올 중독 환자의 재발과 치료 경험)

  • Purpose: The purpose of this study is to explore the subjective experiences of relapse and treatment among the alcoholics. Methods: Phenomenological methodology was used for the study. Participants were seven people with inpatient alcoholics at the alcohol treatment center at K hospital in K province. Data were collected via in-depth interviews from March to September, 2012 and analyzed using Colazzi's framework. Results: Six themes and seventeen meanings were formulated for the relapse and treatment experiences of participants with alcoholism. Six themes were 'Lack of true insight', 'Lack of strength to live a life', 'Unable to overcome an urge to drink', 'Being drawn away from others', 'Falling into the powerlessness and despair', and 'Being foothold of self-growth'. Conclusion: The results from this study revealed the relapse experiences among alcoholic patients. Therefore, the findings can provide the framework and direction for developing the relapse prevention program for alcoholic patients. In addition, practitioners in the treatment center or hospital need to consider their relapse and treatment experience.

A Case Report of a Central Pontine Myelinolysis Patient Treated with Korean and Western Medicine (중심성 뇌교 수초용해증 환자에 관한 한양방 복합치료 1례)

  • Objectives : The purpose of this study is to report the clinical effect of Korean and Western medical treatment on a hemiplegic inpatient with central pontine myelinolysis(CPM). Methods : A patient who was diagnosed with central pontine myelinolysis(CPM) was treated with acupuncture, herbal medicine, Korean physical therapy, along with receiving continuous Western medication and rehabilitation treatment. The patient was evaluated through Manual Muscle Testing(MMT), measurement of grip strength, dysarthria, and dysuresia. The Korean and Western medical treatment was conducted for 4 weeks. Results : After the 4 weeks of Korean and Western medical treatment, MMT grade improved from 2 to 4 and grip strength improved from 6 to 13. Dysarthria and dysuresia also improved. Conclusion : According to these results, this report suggests that Korean and Western medical treatment could be effective in the treatment of central pontine myelinolysis(CPM) patients.

A Case of Dural Ectasia with Low Back Pain and Sciatica Treated with Integrative Korean Medical Treatment

  • Dural ectasia is defined as ballooning or expansion of the dural sac surrounding the spinal cord. This report describes a rare case of low back pain and sciatica, suspected as being dural ectasia. The patient was hospitalized for 45 days, and underwent integrative Korean medical treatment, including pharmacopuncture, acupuncture, herbal medicine, Chuna therapy, cupping therapy, and physiotherapy. The effect of the treatment was evaluated using the numerical rating scale, Oswestry disability index, European quality of life 5 dimensions, and subjective symptoms. After inpatient treatment, the pain the patient experienced was significantly reduced and the evaluation indices improved. This case report suggested that integrative Korean medical treatment could be an effective therapeutic choice for low back pain and sciatica, with dural ectasia. Further clinical studies are needed to support this observation.

A Case Report of Reflux Esophagitis After Gastrectomy due to Gastric Ulcer (위궤양으로 인한 위절제술 후 발생한 역류성 식도염 치험 1례)

  • Objectives: The purpose of this study was to report the effectiveness of Korean medicine in the treatment of reflux esophagitis. Methods: A 62-year-old male patient had reflux esophagitis that occurred one year after a proximal gastrectomy for gastric ulcer in 2020. The patient underwent drug treatment in 2021 and inpatient treatment in 2022 at Mokpo Hankook Hospital, but he did not improve. We treated him with Korean medical treatments, including herbal medicines (Pyungjingunbi-tang-gami), acupuncture, and moxibustion. His symptom severity was assessed with a daily visual analog scale (VAS) for heartburn, upper abdomen pain, and acid reflux. Results: After treatment, the patient's symptoms were improved. The severity of heartburn and acid reflux was reduced from VAS 7 to VAS 1, and his upper abdomen pain was improved from VAS 6 to VAS 1. The symptom frequency was also reduced. Conclusion: Korean medicine treatment could be an effective and quick treatment for reflux esophagitis.

Two Patients Who Were Hospitalized for Clavicle Fracture Caused by a Traffic Accident and Improved with Korean Medicine Complex Treatment (교통사고로 발생한 쇄골 골절로 입원하여 한방복합치료로 호전된 환자 2예)

  • To report improvement of 2 hospitalized patients who had clavicle fractures due to traffic accident with Korean medicine complex treatment. We collected a data from February 2020 to April 2022. A total of 2 patients were studied and we compared numeric rating scale (NRS), neck disability index (NDI), range of motion (ROM) at admission and discharge date of hospitalization. After receiving an average of 28.5 days of Korean medicine inpatient treatment, both patients had reduced NRS, NDI scores and increased ROM. After Korean medicine complex treatment, 2 patients with clavicle fracture showed to have been effective. But further studies need to be done.
