후종인대골화증으로 발생한 경추 척수손상 환자에 대한 수술 후 한방치료 증례보고 1례 (A Case Report of a Postoperative State Patient with Cervical Spinal Cord Injury due to Ossification of Posterior Longitudinal Ligament Treated by Korean Medicine)

  • Objectives: The purpose of this study is to report the effectiveness of Korean medicine treatment on postoperative state patients with cervical spinal cord injury due to ossification of the posterior longitudinal ligament. Methods: A 55-year-old Korean male patient diagnosed with cervical spinal cord injury due to ossification of the posterior longitudinal ligament was treated in an inpatient setting with Korean medicine treatment consisting of herbal medicine, acupuncture, and others from May 22 to August 21. The outcome was evaluated with the American Spinal Injury Association Impairment Scale (AIS), Spinal Cord Independence Measure-III (SCIM-III), modified Japanese Orthopaedic Association scale (mJOA), Numeric Rating Scale (NRS) before and after, or during the treatment period. Results: After 92 days of treatment, the patient's scores on all scales improved. Conclusion: The results suggest that Korean medicine treatment may be effective for postoperative patients with cervical spinal cord injury due to ossification of the posterior longitudinal ligament. Further studies should be conducted on a larger number of patients.

  • The quinolones are broad-spectrum antibiotics and enhanced antimicrobial activity has extended the use of the quinolones beyond the traditional indications for quinolone antibiotics in the treatment of urinary tract infections. The quinolones are effective in a wider variety of infectious diseases, including skin and respiratory infections. Because of their excellent safety and tolerability, they have become popular alternatives to penicillin and cephalosporin derivatives in the treatment of various infections. A retrospective study was performed to evaluate efficacy and safety of IV quinolones for inpatient use. Total 117 patients who administerd quinolones for longer than 3 continuous days at community hospital from October 1st, 2008 to December 31st, 2008 were reviewed. The criterias for drug evaluation were included the validation of indication, outcome, dosage and side effects. In the results, ciprofloxacin 13 (total 93), levofloxacin 3 (total 59) and moxifloxacin 2 (total 19) cases were not met the criterias based on the culture results. Major indications were pneumonia (ciprofloxacin 16.3%, levofloxacin 67.8%, moxifloxacin 84.2%), urinary tract infection (ciprofloxacin 44.1%), skin infection (ciprofloxacin 7.5%, levofloxacin 20.3%, moxifloxacin 10.5%), intra-abdominal infection (ciprofloxacin 10.8%, moxifloxacin 5.3%), etc.. In the results of quinolone monotherapy, the frequencies were each ciprofloxacin 74.2%, levofloxacin 50.8% and moxifloxacin 47.4%. In the results of dosage validation, the validities were each ciprofloxacin 54.8%, levofloxacin 94.9% and moxifloxacin 100.0%. In the results of duration validation, the validities were each ciprofloxacin 59.1%, levofloxacin 78.0% and moxifloxacin 89.5%. Adverse drug reactions were reported for total 49 cases and those were gastrointestinal tract effects including nausea, vomiting, diarrhea and central nervous system effects including headache, dizziness. In summary, the quinolones appropriately used for hospitalized patients based on this study. A focused approach emphasizing "correct use of quinolones" may reduce development of antimicrobial resistance and maximize class efficacy. Consequently, correct use of antibiotics will contribute to decrease medical expenses for person and community.

대학부속한방병원 및 국립의료원 한방진료부 뇌졸증환자에 대한 한방치료 현황 분석 (Clinical study on C.V.A patients in Hospital attached to college of oriental medicine and N.M.C. department oriental medicine)

  • The clinical study was carried out on 1737 cases of inpatient and outpatient which have been diagnosed as C.V.A at hospital attached to 8 colleges of oriental medicine in Korea or National Medical Center from January 1st 1994 to December 31st 1994. The results were as follows; 1. In this study, the case of Occlusive C.V.D was 70.1%, Cerebral hemorrhage was 20.5%, Transient ischemic attack (T.I.A) was 5.3%, Subdural hemorrhage was 3.0%, Subarachnoid hemorrhage was 0.8% 2. It was confirmed by C.T (20%), E.K.G. (19%), X-ray (19%), Urinalysis (19%), Hematoscopy (20%). 3. The mean days of hospital treatment was 27.88 days, mean days of ambulation was 70.34 days. The mean days of hospital treatment of Occlusive C.V.D, Cerebral hemorrhage, T.I.A., Subdural hemorrhage, Subarachnoid hemorrhage were 25.79, 39.32, 12.49, 16.23, 23.40 days, respectively. The mean days of ambulation of Occlusive C.V.D, Cerbral hemorrhage, T.I.A., Subdural hemorrhage, Subarachnoid hemorrhage were 74.40, 93.68, 69.10, 29.75, 32.57 days, respectavely. 4. Oriental medical treatment of C.V.A was mainly Acupuncture (25%), Paper of Chinese herbs (22%), Chinese physiotherapy (14%), Extract of Chinese herbs (11%). 5. Oriental medical physiotherapy for C.V.A was mostly E.S.T., Kinesiatrics, electric negative therapy, others Aerohydrotherapy, interferential current therapy (I.C.T.), Carbon, Samhogi, T.E.N.S., Ultra-sound, Infra-red, Microwave, T.D.P., Ultraviolet, S.S.P., Chinese herbs beth, Prarffin bath, Magnetic treatment and tractions.

병원관리에서 의료보험 입원진료비의 병원자체심사 결과의 분석 연구 - 일 대학병원의 퇴원전 심사를 중심으로 (Analytic Study of the Hospital Self Inspection Results with the Medical Insurance Inpatient Fee on the View-point of the Hospital Management. -based on the University hospital Pre-discharge inspection-)

  • The purposes of this study were to evaluate the results of the hospital self inspection with the medical insurance and to offer basic materials to the medical insurance inspection and the education of medical insurance. The study was undertaken with 4,730 cases among the total 13,810 medical insurance in patients from Jan. 1990 to Dec. 1990 at one university hospital in Pusan. The major contents of the inspection were the omission of diagnosis and medical fee, curtailment, application mistake, the rates of inclusion, subtraction and total accumulation. The data were collected using patients charts and bills. The results of the paper analysis were as follows. 1. From the pre-discharge hospital self inspection, major omission were treatment and material fee but medication fee were moderately high and high curtailment was operation fee. 2. Decreasing order of operation fee adjustment were digestive(22.4%) muscular(22%) and neuro system operation(21.4%). Majority of the medication fee adjustments were injection form of medication(95.7%). 50% of the treatment fee adjustments were composed of injection fee(27.9%) and dressing or post-operative dressing fee(22.3%). 74.7% of material costs were composed of oxygen(30.6%), blood and the blood composed materials(44.1%). 3. Pre-discharge inspection showed 6% adjustment rate, 4.3% addition and 2.1% curtailment rate. Most of the adjustment were omission(66.1%). 4. Omission were divided by event omission(92.6%)and application mistake(7.4%). The decreasing order of omission fee were operation(21.84%), treatment(18.71 %) diagnosis(18.68%), medication (14.53%) and material costs(10.84%). So operation and treatment part were the major part of the total omission fee(40.55%). 5. The average omission of diagnosis were 1,800 per month.

  • Objectives : The purpose of this study is to investigate the effects of warm needling therapy in a patient with cervical myelopathy caused by ossification of posterior longitudinal ligament. Methods : A 53-year-old Korean male patient diagnosed with cervical myelopathy due to ossification of posterior longitudinal ligament was treated with warm needling therapy on posterior neck aside from the routine inpatient treatment from June 26th to July 16th. Clinical improvement was evaluated with visual analogue scale(VAS), Japanese orthopedic score(JOA), grip and release test, finger escape sign and patient satisfaction was measured using a five-point Likert scale. Results : After 21 days of treatment, the patient showed improvement in VAS, JOA, grip and release test and finger escape sign. The patient also expressed satisfaction with the treatment. Conclusions : The results suggest that warm needling therapy may be an effective additional treatment for cervical myelopathy and other symptoms related to ossification of posterior longitudinal ligament(OPLL). Further studies should be carried out regarding possible long-lasting effects.

전립선암의 척추전이로 인해 압박골절이 발생한 환자에 대한 한방처치 1례 (A Case Report of Integrative Korean Medicine Treatment of a Patient with a Compression Fracture Arising from Metastasis of Prostate Cancer to the Spine)

  • Objective: The aim of this study was to report the effects of traditional Korean medicine as a treatment for the pain of a compression fracture in a patient with metastatic prostate cancer. Methods: A 60-year-old male patient with a compression fracture was treated with herbal medicines, acupuncture, and pharmacopuncture for 25 days. His chief complaints were severe low back pain, pain and numbness in both legs, and right groin pain. The effect of treatment was evaluated by the numerical rating scale (NRS) score for pain, the Oswestry Disability Index (ODI) score, and blood test results (CBC-diff count, CRP, ESR, etc.). Results: After 25 days of inpatient treatment, the patient's pain was controlled. The NRS and ODI scores were lower and most pathological symptoms had decreased. Conclusions: Traditional Korean medicine can be a solution for patients with compression fractures arising from metastasis of prostate cancer to the spine.

Anemia Screening, Prevalence, and Treatment in Pediatric Inflammatory Bowel Disease in the United States, 2010-2014

  • Purpose: We examined the prevalence of anemia, annual screening for anemia, and treatment of anemia with iron among children with inflammatory bowel disease (IBD). Methods: A retrospective study of U.S. pediatric patients with IBD was performed in the MarketScan commercial claims database from 2010-2014. Children (ages 1-21) with at least two inpatient or outpatient encounters for IBD who had available lab and pharmacy data were included in the cohort. Anemia was defined using World Health Organization criteria. We used logistic regression to determine differences in screening, incident anemia, and treatment based on age at first IBD encounter and sex. Results: The cohort (n=2,446) included 1,560 Crohn's disease (CD) and 886 ulcerative colitis (UC). Approximately, 85% of CD and 81% of UC were screened for anemia. Among those screened, 51% with CD and 43% with UC had anemia. Only 24% of anemia patients with CD and 20% with UC were tested for iron deficiency; 85% were iron deficient. Intravenous (IV) iron was used to treat 4% of CD and 4% UC patients overall and 8% of those with anemia. Conclusion: At least 80% of children with IBD were screened for anemia, although most did not receive follow-up tests for iron deficiency. The 43%-50% prevalence of anemia was consistent with prior studies. Under-treatment with IV iron points to a potential target for quality improvement.

A traditional East Asian herbal complex, Majainhwan for constipation in Parkinson's disease: A retrospective study

  • Objectives: Parkinson's disease (PD) patients experience various non-motor symptoms, among which constipation is the second most common after anosmia. However, there are no clear guidelines or effective treatment for constipation in PD. Methods: To investigate the efficacy of Majainhwan (MH) on constipation in patients with PD, we conducted a retrospective chart review study of PD or Parkinsonism patients with constipation who received outpatient or inpatient treatment and whose previous laxatives were replaced by MH from August 2016 to July 2019. Results: In this study, a total of 68 patients' medical records were reviewed. Among the 44 outpatients with MH treatment, "effective" results were observed in 86.4% patients. Similarly, among the 24 inpatients, "effective" results were noted in 95.8% patients. The adverse effect was "diarrhea" reported in five cases. Conclusion: Based on these findings, we could suggest that MH is relatively safe and may be effective in the treatment of constipation in patients with PD.

요통을 동반한 요추 추간판 탈출증 환자에 대한 Sandbag Motion Style Acupuncture Treatment 호전도 비교 연구: 무작위배정 대조군연구 (The Comparative Study on the Effect of Motion Style Acupuncture Treatment Using Sandbag in Lumbar Disc Herniation with Low Back Pain: A Randomized Controlled Trial)

  • Objectives The purpose of this study was to compare the effects of MSAT (Motion style acupuncture treatment) using Sandbag regarding low back pain with HIVD of L-SPINE. Methods Forty patients received inpatient treatment from March 16 to May 16, 2015 in the Daejon-Jaseng oriental medicine hospital were divided into 2 groups by blocked randomization: Group A (n=20) is routine treatment on HIVD of L-SPINE with MSAT using Sandbag and Group B (n=20) is routine treatment on HIVD of L-SPINE (acupuncture, Shinbaro pharmacopuncture, herb medication, chuna treatment and physiotherapy). The MSAT (Motion style acupuncture treatment) using Sandbag was administered each day and both groups were received routine treatment each day. Evaluations were made before treatment, after 3 days and 7 days, and before discharge using the NRS (numeric rating scale) and ODI (oswestry disability index). The statistically significance was evaluated by SPSS 22.0 for windows. Results The NRS (numeric rating scale) and ODI (oswestry disability index) of both A and B groups after 3, 7 days and before discharge of treatment significantly decreased (p<0.05). Regarding group comparison, the NRS (numeric rating scale) and ODI (oswestry disability index) of A group was significantly lower than the B group at 3, 7 days, before discharge (p<0.05). Regarding improvement of the NRS (numeric rating scale) and ODI (oswestry disability index), A group was significantly lower than the B group at 3 day (NRS), 7 day (ODI) (p<0.05). There was significant difference between two groups at 7 days (NRS) and before discharge (p<0.05). Conclusions Compared to routine treatment, the MSAT (Mostion style acuputure treatment) using Sandbag significantly improved the NRS (numeric rating scale) and ODI (oswestry disability index) after 3, 7 days, and before discharge. Because early intervention determines the grade of Low back pain with HIVD of L-SPINE, the MSAT (Motion style acupuncture treatment) using Sandbag will be clinically helpful to patients at the early stage.

하지방사통을 동반한 요추 추간판 탈출증 환자에 대한 이상근 MSAT 치료군과 비치료군의 후향적 호전도 비교 연구 (Comparison of Effects between MSAT on Piriformis Muscle and Non-MSAT on Piriformis Muscle in Patients with HLD with Leg Radiating Pain: A Retrospective Analysis)

  • Objectives The purpose of this study was to compare the effects of MSAT (Mostion style acuputure treatment) on piriformis muscle regarding leg radiating pain with HIVD of L-SPINE. Methods Forty patients who received inpatient treatment from Febuarary 1 to March 31, 2014 in the Daejon-Jaseng oriental medicine hospital were divided into 2 groups by blocked randomization : Group A (n=20)=routine treatment on HIVD of L-SPINE with MSAT on piriformis muscle and Group B (n=20)=routine treatment on HIVD of L-SPINE (acupuncture, Shinbaro pharmacopuncture, herb medication, chuna treatment and physiotherapy). The MSAT (Mostion style acuputure treatment) on piriformis muscle was administered each day and both groups were received routine treatment each day. Evaluations were made before treatment, after 3 days and 7 days, and before discharge using the NRS (numeric rating scale) and ODI (oswestry disability index). The statistically significance was evaluated by SPSS 18.0 for windows. Results Compared to before treatment, the NRS (numeric rating scale) and ODI (oswestry disability index) of both A and B groups after 3, 7 days and before discharge of treatment significantly decreased (p<0.05). Regarding group comparison, the NRS (numeric rating scale) and ODI (oswestry disability index) of A group was significantly lower than the B group at 3, 7 days, before dischanre (p<0.05). Because early intervention determines the grade of Leg radiating pain with HIVD of L-SPINE, the MSAT (Mostion style acuputure treatment) on piriformisat the early stage will be clinically helpful to patients.
