The Effects of Inhalation Method Using Essential Oils on the Preoperative Anxiety of Hystrectomy Patients (향기흡입법이 자궁적출술 환자의 수술전 불안에 미치는 효과)

  • The purpose of this study was to elucidate the effects of inhalation method using essential oils on the preoperative anxiety of hystrectomy patients, and to provide effective and holistic nursing care to them. The research design was a nonequivalent control group non synchronized design. The data were collected from February 1 to March 31, 2002 at D Medical Center in Busan. The subjects were forty one patients that were operated on under general anesthesia for hystrectomy. They were assigned to two groups, twenty one subjects in the experimental group and twenty subjects in the control group. The tool of the Visual Analogue Scale(VAS) was used to anxiety on all patients the day before surgery and the preoperative period. Then systolic and diastolic blood pressure, pulse rate levels were measured the day before surgery and the preoperative period. The experimental group received two treatments of inhalation method using essential oils of with lavender, ylang ylang, and bergamot oil. The data were analyzed by the $X^2$ test and the independent t-test. The results of this study were summarized as follows: 1. Hypothesis 1: It has been supported that the experimental group received inhalation method using essential oils might cause lower level of the preoperative VAS anxiety than that of the control group(t=-2.93, p=.006). 2. Hypothesis 2: It has been rejected that the experimental group received inhalation method using essential oils might cause lower level in the preoperative systolic blood pressure than the control group(t=-.120, p=.905). It has been rejected that the experimental group received inhalation method using essential oils might cause lower level in the preoperative diastolic blood pressure than the control group.(t=1.766, p=.085). 3. Hypothesis 3: It has been supported that the experimental group received inhalation method using essential oils might cause lower level in preoperative pulse rate than the control group(t=5.853, p=.000). According to these results, inhalation method using essential oils can be considered an effective nursing intervention that relieves the preoperative anxiety of hysterectomy patients and stabilizes vital sign partially.

Effect of Nano-sized Carbon Black Particles on Lung and Circulatory System by Inhalation Exposure in Rats

  • Objectives: We sought to establish a novel method to generate nano-sized carbon black particles (nano-CBPs) with an average size smaller than 100 nm for examining the inhalation exposure risks of experimental rats. We also tested the effect of nano-CBPs on the pulmonary and circulatory systems. Methods: We used chemical vapor deposition (CVD) without the addition of any additives to generate nano-CBPs with a particle size (electrical mobility diameter) of less than 100nm to examine the effects of inhalation exposure. Nano-CBPs were applied to a nose-only inhalation chamber system for studying the inhalation toxicity in rats. The effect on the lungs and circulatory system was determined according to the degree of inflammation as quantified by bronchoalveolar lavage fluid (BALF). The functional alteration of the hemostatic and vasomotor activities was measured by plasma coagulation, platelet activity, contraction and relaxation of blood vessels. Results: Nano-CBPs were generated in the range of 83.3-87.9 nm. Rats were exposed for 4 hour/day, 5 days/week for 4 weeks to $4.2{\times}10^6$, $6.2{\times}10^5$, and $1.3{\times}10^5$ particles/$cm^3$. Exposure of nano-CBPs by inhalation resulted in minimal pulmonary inflammation and did not appear to damage the lung tissue. In addition, there was no significant effect on blood functions, such as plasma coagulation and platelet aggregation, or on vasomotor function. Conclusion: We successfully generated nano-CBPs in the range of 83.3-87.9 nm at a maximum concentration of $4.2{\times}10^6$ particles/$cm^3$ in a nose-only inhalation chamber system. This reliable method can be useful to investigate the biological and toxicological effects of inhalation exposure to nano-CBPs on experimental rats.

Laryngomicrosurgery for the Treatment of Glottic Stenosis after Inhalation Injury in a Terrible Subway Accident in Daegu (대구 지하철 참사 흡인화상환자에서의 후두협착 치료를 위해 시행한 후두미세경 수술)

  • Background and Objectives : In order to evaluate the result of the $CO_2$ LASER laryngomicrosurgery for the treatment of glottic stenosis after inhalation injury. Patients and Methods : We retrospectively evaluated 7 patients with inhalation injury on larynx who received their surgical treatment at the Severance hospital on Jun. 2003. The average follow-up duration was 64.9 days and they were 3men and 4women. The average patient age was 30.1 years, We evaluated the preoperative state with neck CT and stroboscopy. The operation was $CO_2$ LASER laryngomicrosurgery and Mitomycin-C application. The postoperative state was evaluated with stroboscopy at POD#6 and POD#2months. Results : 6 patients had the intubation as the first care on accident. All 7 patients showed the glottic web and after operation, the vocal cord seemed to be almost normal. The glottic area was widened as 3 times as the preoperative state at POD#6. Conclusion : We could lessen the symptoms like dyspnea and hoarseness with laryngomicrosurgery and Mitomycin-C application for patients complaining some laryngeal problems because of the inhalation injury.

The Effect of Aroma Inhalation Therapy on Nausea, Vomiting, Abdominal Discomfort, Compliance and Colon Cleansing in the Preparation of Patients Undergoing Colonoscopy (아로마 흡입요법이 대장내시경 검사 대상자의 오심, 구토, 복부 불편감, 순응도 및 대장 정결도에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of aroma inhalation therapy on nausea, vomiting, abdominal discomfort, compliance, and colon cleansing for patients having a colon lavage solution before colonoscopy. Methods: This study was performed using a nonequivalent control group post-test design. The study was conducted from 20 Jun to 29 Aug, 2011. The participants were 152 patients (experimental group: 77, control group: 75) who underwent colonoscopy at A hospital, Seoul. Results: The experimental group with the aroma inhalation therapy reported significantly lower nausea and vomiting than did the control group. There were no significant differences in abdominal discomfort, compliance, and colon cleansing between experimental and control group. Conclusion: Use of an aroma inhalation therapy was efficient in nausea and vomiting among patients having a colon lavage solution before colonoscopy. Therefore, aroma inhalation therapy is recommended as an effective nursing intervention for relieving nausea and vomiting among patients having a lavage solution before colonoscopy.

  • Exposure to volatile disinfection by-products (DBPs) such as chloroform included in chlorinated tap water can occur during household activities via inhalation as well as ingestion and dermal absorption. This study was conducted to examine the significance of inhalation route of exposure since humans are unintentionally exposed to volatile DBPs while staying home. Two sets of experiments were carried out in an apartment to measure: 1) the variation of chloroform concentrations in the living room air following kitchen activities (cooking and dish-washing); and 2) the variation of chloroform concentrations in the bathroom and living room following showering. Cooking, dish-washing, and showering all contributed to the elevation of household chloroform levels. Even a few minutes of natural ventilation resulted in the reduction of the chloroform levels to the background. Estimates of daily chloroform doses and lifetime cancer risks suggested that inhalation of household air during staying home be a major route of exposure to chloroform and that ingestion be a minor one in Korean people. It is also suggested that ventilation be a simple and important measure of mitigating human exposure to volatile DBPs indoors.

  • This paper shows the study on the design of the spray-freeze dryer for the production of the pulmonary inhalation powders. Powder production and handling has been an integral part of pharmaceutical processing because of the wide use of oral dosage forms. There are a few commonly used powder preparation methods including mechanical milling, precipitaion, spray drying, freeze drying, and so on. In general, methods available for preparing inhalation powders are limited due to certain inhalation powder's sensitive nature to the processing environments. This is particularly true for preparing dry powder aerosols where the aerodynamic particle size$(<5{\mu}m)$ and the size distribution are pivotal. Supercritical fluid antisolvent and spray freeze drying have recently emerged as promising techniques for producing powders for use in microcapsulation. However, the aerosol applications of these powders are yet to be explored. The purpose of this study is to test the feasibility of using spray freeze-dried pulmonary inhalation powders for aerosolization.

  • Nowadays a huge variety of products that aim to assist to quit smoking or reduce addictive symptoms are developed and manufactured with safety evaluation, but the safety of the most recent products of interest which do not contain tobacco and nicotine, and shape cigarettes is not evaluated and guaranteed relatively. This study was carried out to evaluate the single and repeated dose inhalation toxicity and genotoxicity of H menthol (Nicotine free-tobacco free) herbal cigarettes provided by Cigastop Ltd. in ICR mice. In this study, doses which we determined to expose to mice were 40 cigarettes for 6 hours a day to mice in single dose and 20 (high dose), 10 (middle dose) and 5 cigarettes (low dose) a day for 28 days in repeated dose inhalation toxicity, in vivo chromosome aberration test and micronucleus test. The particulate substances from H menthol herbal cigarettes also were gathered and used in the Salmonella typhimurium/microsome assay (Salmonella test; Ames test). We could find neither significant changes between control and treatment groups nor dose-response effects of test material at all except serum Ca level of female middle dose treatment group in repeated dose inhalation toxicity test. In conclusion, H menthol herbal cigarettes, when applied clinically intended dose we used, might not show any toxic and/or mutagenic effect.

Toxic Effect of Inhaled Toluene on the Neural Cell (톨루엔 흡입이 신경세포에 미치는 독성)

  • Toluene inhalation increases glutamate level and its receptor in various brain regions. In this study, nitric oxide synthase (NOS) activities were investigated in various rat brain regions using NADPH diaphorase staining method which examined histochemical changes of NOS in the neural cells. Also, in vitro LDH leakage assay and MTT test were performed to investigate the toxic influences of toluene in cultured granule cell of rat cerebellum which was significantly affected with toluene in vivo. Rats were exposed to toluene of 10000 ppm for 3 days. 7 days and 14 days by 20 min $\times$ 2 times a day. NADPH diaphorase staining was processed in the different brain regions after inhalation. NADPH diaphorase staining density was not significantly changed at 3 days inhalation group, but the density decreased in proportion to the duration of toluene inhalation. Over 30% of staining density was decreased at 14 days group which was maximum duration of inhalation in this study. The tendency of staining density decrease was significant in granule cell of cerebellum. Cell death by toluene exposure was observed in cultured cerebellar granule cell. $EC_{50}$ measured with LDH leakage assay and MTT test were 43 mM and 72 mM respectively.

  • Purpose: The purpose of this study was to investigate the effects of aromatherapy on stress, fatigue, mood, and vital signs of the nurses in the operating rooms. Methods: The data were collected from September 2007 to February 2008 in the operating room at K hospital located in Seoul, Korea. A total of 48 nurses were recruited. Among them, 25 nurses were randomly assigned to an experimental group and 23 were assigned to a control group. Experimental group was provided with aroma inhalation by necklace which had essential oil mixture of 0.2 cc. For aroma inhalation, Lavender, Zeranium and Mazoram were mixed in ratio of 5:3:2. In order to test the effectiveness of aroma inhalation, the pretest and posttest results for stress, fatigue, mood, and vital signs were compared between the experimental group and the control group. The data were analyzed by unpaired t-test and ANCOVA using the SPSS/WIN 15.0 program. Results: There were statistically significant decreases in the stress and fatigue scores, and increase in the mood scores and showed significant relaxation in the vital signs after the aroma inhalation. Conclusion: The aromatherapy had the positive effects on stress, fatigue, mood, and vital signs.

A CLINICAL STUDY ABOUT COMPARISON OF INHALATION ANESTHESIA AND INTRAVENOUS ANESTHESIA WITH ORAL AND MAXILLOFACIAL PATIENTS (악안면구강외과 환자의 전신 마취에 있어서 흡입 마취와 정맥 마취의 차이에 대한 임상적 고찰)

  • Intravenous anesthesia was compared with inhalation anesthesia in 20 patients of oral and maxillofacial surgery. The patients were randomly assigned to study in two treatment groups. 20 patients were injected ketamine and propofol. 20 patients were administered Enflurane. The respond of patients consciousness and general recovery condition of the two groups were compared. Intravenous anesthesia group were awake significantly faster without complications such as nause, vomiting, and agitation after operation than inhalation anesthesia group. Full recovery time of intravenous anesthesia group was significantly 3 times less than inhalation anesthesia group. The authors conclude that intravenous anesthesia is a practical technique for oral and maxillofacial surgery patients undergoing and may be preferable to intravenous anesthesia because of the significantly short of recovery time without complications.
