Effect of Hemodialysis on Dogs with Acute Renal Failure Induced by Ethylene Glycol (Ethylene Glycol로 유발된 급성신부전증 이환견에 대한 혈액투석효과)

  • The aim of this study is to clarify the therapeutic effect of hemodialysis (HD) on acute renal failure in dogs experimentally induced by ethylene glycol (EG) ingestion in experiment 1(HD on 12 and 36 hours after EG injestion) and experiment 2(HD on 24 and 48 hours after EG injestion). Clinical signs such as vomiting, depression, ataxia and knuckling were improved on experiment 1 and 2. White blood cell (WBC) and platelets (PLT) were decreased, and packed cell volume (PCV) was increased after hemodialysis, however, no significant differences were observed in experiment 1 and 2. BUN, creatinine and phosphorus was significantly decreased in experiment 1 and 2, respectively. These results suggested that hemodialysis was effective for acute renal failure in dogs induced by EG ingestion.

Effect of Drinking Fermented Milk on the Improvement of Defecation in Constipated Female Students (농후 발효유에 의한 여대생의 변비개선 효과)

  • The aim of this study was to investigate the effect d concentrated fermented milk on the improvement of defecation in female students with constipation. This yogurt was fermented with Bifidobacterium, L. acidophilus, and S. thermophilus and supplemented with oligosaccharides, dietary fiber, and yeast and mushroom extracts. 44 female students with constipation were sampled as subjects and fed 150ml fermented milk per day for 2 weeks period. After each defecation, the subjects were examined for the status of stool evacuation by questionnaires. The frequency of stool evacuation, fecal amount, time spending for evacuation, fecal color and shape, and refreshing feeling after evacuation were remarkably improved after ingestion of fermented milk for 2 weeks. These effects were greater in the heavy constipation group than in the mild group. The fecal odor did rot decrease significantly after 2 weeks of intake of fermented milk, but the odor thended to decrease after the intake of fermented milk These results indicated that the ingestion of concentrated fermented milk is effective to improve the constipation.

Effect of Korean red ginseng extract on liver damage induced by shortterm and long-term ethanol treatment in rats

  • Korean red ginseng (KRG) is prepared by the process of steaming the roots of Panax ginseng. In this study, the feeding effects of KRG-water extract (KRGE) on ethanol-induced liver damage were elucidated by measuring serum biomarkers in rats. Serum ${\gamma}$-glutamyltranspeptidase (g-GT) activity and the concentration of malondialdehyde (MDA) were significantly increased by short-term and long-term ethanol treatment in rats, whereas the activities of serum glutamate pyruvate transaminase (GPT) and glutamate oxaloacetate transaminase (GOT) did not respond. Pretreatment with KRGE maintained the activity of serum GPT, and the MDA concentration induced by short-term ethanol ingestion remained within the normal range. However, co-feeding of KRGE to rats decreased the concentration of MDA but failed to modulate the serum ${\gamma}$-GT activity induced by long-term ethanol treatment. Our studies suggest that in rats, it appears that KRGE does not sufficiently reverse the physiological response evoked by long-term ethanol ingestion to maintain normal conditions, in view of the serum biomarker ${\gamma}$-GT, regardless of KRGE's favorable antioxidant activity.

A Case Report of Glacial Acetic Acid Ingestion Complicated with Hepatic Necrosis (빙초산 음독과 합병된 간괴사 1례)

  • Caustic ingestion can produce a progressive and fatal injuries to esophagus, stomach and other organs. Reported exposure to acetic acid results injuries to gastrointestinal tract, hemolysis and disseminated intravascular coagulation is general, but causing hepatic necrosis by direct injuries are rare. A 47-year-old man visited our emergency medical center complaining odynophagia and abdominal pain after ingesting glacial acetic acid ($99\%$) with suicidal ideation. At the time of arrival, the patient complained mild abdominal pain but a few hours later the patient complained severe abdominal pain with markedly elevated liver enzymes. The Abdominal Computerized Tomography showed diffuse gastric wall edema and density of wedge shaped hypodense area in right hepatic dome showing focal hepatic necrosis without significant inflammation. This seems likely to be a direct effect of the noxious agent on hepatocyte involving the portal circulation.

  • Hydrofluoric acid (HF), one of the strongest inorganic acids, is used mainly for industrial purpose. Hydrofluoric acid injuries has a potential for both systemic as well as severe local tissue destruction. One of the most serious consequences of severe exposure to HF by any route is marked lowering of serum calcium (hypocalcemia) and other metabolic changes, such as hypomagnesemia and which may result in a fatal outcome if not recognized and treated. promptly cardiotoxicity is not well known except arrhythmias, which are a primary cause of death. We report a case of myopericarditis by ingestion of hydrofluoric acid.

Early Gastrin Response to Single Glucose and Casein Feeding (포도당 및 Casein 액 단일투여에 의한 조기 Gastrin 반응)

  • The early plasma gastrin responses to single oral glucose or casein solution were studied in the same normal subjects on different days. After an overnight fast, glucose or casein solution was ingested within few minutes at the breakfast time. The plasma gastrin responses to these solutions were compared and contrasted with the concentration when the subjects received glucose solution intravenously. Results were as follows: 1) Rapid intravenous glucose infusion did not produce any changes in the plasma gastrin concentration. 2) Plasma gastrin concentration rose and peaked within 10 minutes after an oral liquid ingestion and then decreased substantially by 15 minutes, but remained slightly above fasting levels at 60 minutes. 3) There was no significant difference between the mean plasma gastrin concentrations after glucose of casein ingestion, but each fluid produced a significant increase in serum gastrin above fasting levels. 4) The subjects who produced high plasma gastrin response to glucose solution did so to casein solution. Conversely a low response to one solution reflected a low response to the other solution. 5) From the above results, authors discussed that individual responsibility rather than the sorts of meals is the factor in the determination of the magnitude of the early gastrin response.

Ingestion Exposure to Nitrosamines in Chlorinated Drinking Water

  • Objectives: N-Nitrosodimethylamine (NDMA) is classified as a probable human carcinogen by the United States Environmental Protection Agency (US EPA) and is formed during the chlorination of municipal drinking water. In this study, selected nitrosamines were measured in chlorinated drinking water collected from Chuncheon, Kangwon-do, Republic of Korea, and a risk assessment for NDMA was conducted. Methods: Twelve water samples were collected from 2 treatment plants and 10 household taps. Samples were analyzed for 6 nitrosamines via solid-phase extraction cleanup followed by conversion to dansyl derivatives and high-performance liquid chromatography-fluorescence detection (HPLC-FLD). Considering the dietary patterns of Korean people and the concentration change of NDMA by boiling, a carcinogenic risk assessment from ingestion exposure was conducted following the US EPA guidelines. Results: NDMA concentrations ranged between 26.1 and 112.0 ng/L. NDMA in water was found to be thermally stable, and thus its concentration at the end of boiling was greater than before thermal treatment owing to the decrease in water volume. The estimated excess lifetime carcinogenic risk exceeded the regulatory baseline risk of $10^{-5}$. Conclusions: This result suggests that more extensive studies need to be conducted on nitrosamine concentration distributions over the country and the source of relatively high nitrosamine concentrations.

  • The physicochemical equivalencies of drugs are not usually correlate to the generic equivalencies of drugs and the generic equivalencies of drugs produced by different manufacturers or different formulations are being called in question frequently. The bioabailability of two formulations of oxytetracycline and erythromycin stearate were performed in healthy human volunteers. At the same time, the disintegration testes were performed with randomly sampled materials in question. For the biological evaluation of new oxytetracycline formulation; tablet(250mg), two-way cross over study in 10 healthy young volunteers was performed using oxytetracycline capsule (250mg) as reference, Erythromycin stearate (250mg) tablets and capsules produced by different manufacturers were compared in a two-way cross over study in 12 subjects with same manner of oxytetracyclines. oxytetracycline tablets showed somewhat slow disintegration rate, but appeared not statistical differences in serum concentrations from the reference, up to six hours after ingestion. Erythromycin stearate capsules disintegrated more rapidly than enteric coated tablets. Serum concentrations of capsules were more variable and markedly lower (P<.005 after 2hrs) than the enteric coated tablets. Rapid disintegration of capsules may result in destruction of active chemicals owing to the interaction with gastric acid and the above factor may contribute mainly to the low serum level after ingestion of capsules.

Should Workers Avoid Consumption of Chilled Fluids in a Hot and Humid Climate?

  • Despite provision of drinking water as the most common method of occupational heat stress prevention, there remains confusion in hydration messaging to workers. During work site interactions in a hot and humid climate, workers commonly report being informed to consume tepid fluids to accelerate rehydration. When questioned on the evidence supporting such advice, workers typically cite that fluid absorption is delayed by ingestion of chilled beverages. Presumably, delayed absorption would be a product of fluid delivery from the gut to the intestines, otherwise known as gastric emptying. Regulation of gastric emptying is multifactorial, with gastric volume and beverage energy density the primary factors. If gastric emptying is temperature dependent, the impact of cooling is modest in both magnitude and duration (${\leq}5$ minutes) due to the warming of fluids upon ingestion, particularly where workers have elevated core temperature. Given that chilled beverages are most preferred by workers, and result in greater consumption than warm fluids during and following physical activity, the resultant increased consumption of chilled fluids would promote gastric emptying through superior gastric volume. Hence, advising workers to avoid cool/cold fluids during rehydration appears to be a misinterpretation of the research. More appropriate messaging to workers would include the thermal benefits of cool/cold fluid consumption in hot and humid conditions, thereby promoting autonomy to trial chilled beverages and determine personal preference. In doing so, temperature-based palatability would be maximized and increase the likelihood of workers maintaining or restoring hydration status during and after their work shift.

Scombroid Fish Poisoning and Histamine Food Poisoning (스콤브로이드 생선 중독과 히스타민 식중독)

  • Scombroid fish poisoning (SFP) is a form of histamine food poisoning caused by the ingestion of improperly stored fish. The term "scombroid" derives from the family name of the fish family first implicated, such as tuna and mackerel. On the other hand, non-scombroid fish species, such as sardine and herring, can also cause histamine poisoning. The histamine is converted from histidine by a bacterial enzyme in the causative fish. Because the symptoms of SFP can easily be confused with food allergies, it is believed to have been significantly under-reported. In 2016, an outbreak of SFP occurred among primary school students who had eaten yellowtail steak in Korea. The most common findings consisted of a rapid onset of flushing of the face and trunk, erythematous and urticarial rash, diarrhea, and headache occurring soon after consuming the spoiled fish. Usually, the course is self-limiting and antihistamines can be used successfully to relieve symptoms, but several life-threatening SFP cases have been reported. Clinical toxicologists should be familiar with SFP and have competency to make a differential diagnosis between fish allergy and histamine poisoning. SFP is a histamine-induced reaction caused by the ingestion of histamine-contaminated fish, whereas a fish allergy is an IgE-mediated reaction. This review discusses the epidemiology, pathophysiology, diagnosis, treatment, and preventive measures of SFP.
