노로바이러스 및 로타바이러스 감염의 역학 및 기후요인과의 관계: 천안시, 2010-2019 (Molecular epidemiologic trends of norovirus and rotavirus infection and relation with climate factors: Cheonan, Korea, 2010-2019)

  • 바이러스감염으로 인한 설사는 전 세계적으로 공중 보건의 주요 문제이며 사망률의 큰 부분을 차지하고 있지만 기후데이터를 이용하여 분석한 연구는 많지 않다. 따라서 본 연구는 설사를 유발하는 바이러스인 Rotavirus Gr.A, Norovirus G-I & GII의 감염과 기후와의 인과관계를 분석하여 조기진단과 치료를 용이하게 하고 계절성 질환을 더욱 관리하고 예방하는데 기여하고자 하였다. 2010년 6월부터 2019년 12월까지 단국대학교 병원에서 시행한 대변 시료 4,009개의 설사바이러스 6종의 멀티플렉스 역전사 PCR(mRT-PCR)검사결과와 다양한 기후 요인 중 체감온도, 상대습도, 일조율과의 상관관계를 후향적으로 분석하였다. 4,009 개의 대변 샘플 중 985 개는 Rotavirus Gr.A, Norovirus G-I & GII 감염에 대해 양성이었다. 이 985 건 중 95.3 % (n = 939)는 10 세 미만으로 Rotavirus Gr.A, Norovirus G-I & GII는 10 세 미만 환자에서 높은 감염률을 보였다. 우리는 기상 빅데이터와 연령, 시기별 감염분석 등에 기초하여 Rotavirus Gr.A의 감염이 상대 습도에 따라 유의한 상관 관계가 있음을 확인하였고 Norovirus G-II의 감염은 체감 온도와 유의한 상관 관계가 있음을 확인하였다. 본 연구는 기후요인에 따른 Rotavirus Gr.A, Norovirus G-I & GII 감염의 분포에 대한 이해도를 향상시킴으로써 바이러스성 설사질환과 관련된 보건정책의 설정이나 계절적 영향에 대한 새로운 통찰력을 제공하는데 유용한 자료가 될 것으로 기대한다.

Magnetic Resonance Imaging Patterns of Post-Operative Spinal Infection : Relationship between the Clinical Onset of Infection and the Infection Site

  • Objective : To investigate the magnetic resonance imaging (MRI) findings and the patterns of postoperative spinal infection according to the passage of time. Methods : Institutional review board approval was obtained, and informed consent was not obtained for the retrospective review of patients' medical records. A total of 43 patients (27 men and 16 women; mean age, 64) diagnosed with postoperative spinal infection were included in this study. We retrospectively reviewed the MRI findings and the medical records and categorized the infection sites based on MRI, i.e., anterior, posterior, and both parts. The duration of the clinical onset from surgery was divided, i.e., acute (${\leq}2weeks$), subacute (2-4 weeks), and late (>4 weeks). Results : Postoperative spinal infection was involved in the posterior part in 31 (72%), anterior part in two (4.7%), and both parts in 10 patients (23.3%). Abscess or phlegmon in the back muscles and laminectomy site were the most common MRI findings. The number of patients with acute, subacute, and late clinical onset were 35, two, and six, respectively (mean, 33.4 days; range, 1-730 days). The mean duration of the clinical onset was 12 days in the posterior part, 15.2 days in both parts, and 456.5 days in the anterior part. Conclusion : Postoperative spinal infection usually occurred within four weeks in the posterior part and over time the infection was considered to spread into the anterior part. For the evaluation of postoperative spinal infection, the posterior surgical field was more important than the vertebral body or the disc space on MRI.

Climate Factors and Their Effects on the Prevalence of Rhinovirus Infection in Cheonan, Korea

  • The use of big data may facilitate the recognition and interpretation of causal relationships between disease occurrence and climatic variables. Considering the immense contribution of rhinoviruses in causing respiratory infections, in this study, we examined the effects of various climatic variables on the seasonal epidemiology of rhinovirus infections in the temperate climate of Cheonan, Korea. Trends in rhinovirus detection were analyzed based on 9,010 tests performed between January 1, 2012, and December 31, 2018, at Dankook University Hospital, Cheonan, Korea. Seasonal patterns of rhinovirus detection frequency were compared with the local climatic variables for the same period. Rhinovirus infection was the highest in children under 10 years of age, and climatic variables influenced the infection rate. Temperature, wind chill temperature, humidity, and particulate matter significantly affected rhinovirus detection. Temperature and wind chill temperature were higher on days on which rhinovirus infection was detected than on which it was not. Conversely, particulate matter was lower on days on which rhinovirus was detected. Atmospheric pressure and particulate matter showed a negative relationship with rhinovirus detection, whereas temperature, wind chill temperature, and humidity showed a positive relationship. Rhinovirus infection was significantly related to climatic factors such as temperature, wind chill temperature, atmospheric pressure, humidity, and particulate matter. To the best of our knowledge, this is the first study to find a relationship between daily temperatures/wind chill temperatures and rhinovirus infection over an extended period.

The Epidemiological Survey of Nasal Colonization of Methicillin Resistant Staphylococcus aureus in Patients and Doctors

  • Methicillin resistant Staphylococcus aureus (MRSA) is one of the most common nosocomial pathogens. Many hospitals are facing the problems which they have to use expensive antibiotics and suffer from long term hospital study of patients due to MRSA. This study is to survey MRSA nasal colonization of patients and doctors, and to investigate the mode of transmission of MRSA by pulsed field gel electrophoresis (PFGE) and then use these data to prevent further spread of cross infection and reduce nosocomial infection. Subjects of this study were 201 patients with MRSA infection at an university hospital in Busan from Sept. 1997 to Aug. 1998. Bacterial genotypes of MRSA strains isolated from nares and wound of patients (14 cases) and nares of doctors (8 cases) were analyzed by PFGE. Nasal cultures of 20 I patients for detecting nasal colonization of MRSA were performed and incidence rate of nasal colonization was 40% (80/201). Among 201 patients MRSA were acquired from hospital in 140 (70%) patients and were acquired from community 61 (30%) patients. Among 14 pairs of MRSA from colonized or infected sites and anterior nares, DNA patterns of 10 pairs (71.4%) were equal. 86% (12/14) MRSA strains isolated from patients and 12.5% (1/8) MRSA strains isolated from doctors show same pattern. DNA patterns were changed in some doctors after nasal oint. Treatment. It could be inferred that the most sources of MRSA in hospital are the endemically existing MRSA. Therefore, we believe that it would be necessary to control MRSA nasal colonization of the patients and the related medical teams to reduce the medical cost and to improve the efficacy of medical cares.

경상남도 북부지역 돼지 사육농가에 대한 돼지호흡기복합감염증 양상 조사 (Characterization of the infection pattern of porcine respiratory disease complex (PRDC) in the northern area of Gyeongsangnam-do, Korea)

  • The prevention of porcine respiratory disease complex (PRDC) is very important because of its high infection-rates in the swine farms and the economic impact in swne industry in Korea. To control the prevalence of PRDC, it is important to know about infection patterns of it. Therefore, this study aimed to investigate the infection patterns of PRDC in the northern area of Gyeongsangnam-do. To this end, the infection of porcine reproductive and respiratory syndrome virus (PRRSV), porcine circovirus type 2 (PCV2), Actinobacillus pleuropneumoniae (APP), Mycoplasma hyopneumoniae (MH), and Swine influenza virus (SIV) were examined using 120 pig lung tissues by PCR analysis. As a result, single pathogen positive specimens were 25.0% and the others (75.0%) were turned out to be PRDC with at least two pathogens. Among PRDCs, 50 specimens (41.7%) was infected with PRRSV, PCV2, MH and SIV. Ten specimens (8.3%) showed triple infections of PRRSV, PCV2 and MH. Double infected specimens for PRRSV and PCV2 were 10 (8.3%), and for PCV2 and APP were 20 (16.7%).

PCR-Based RELP Analysis of ureC Gene for Typing of Indian Helicobacter pylori Strains from Gastric Biopsy Specimens and Culture

  • Since culture of Helicobacter pylori is relatively insensitive and cumbersome, molecular detection and typing of H. pylori isolates are gaining importance for strain differentiation. In the present study genomic DNA of 42 gastric biopsies and H. pylori isolates from corresponding patients were analyzed and compared by PCR-based RFLP assay. The 1,132-bp product representing an internal portion of ureC gene of H. pylori was amplified by PCR and digested with restriction enzymes HindⅢ, AiuⅠ and PvuⅠ. The HindⅢ, AluⅠ and PvuⅠ digestion produced 4, 7, and 2 distinguishable RFLP patterns respectively from 42-H. pylori isolates. By combining all three restriction enzyme digestions, 15 RFLP patterns were observed. However, when PCR products from 42 gastric biopsy specimens were digested by restriction enzymes HindⅢ, AluⅠ and PvuⅠ, we observed 5, 8 and 2 RFLP patterns, respectively. Patterns from 34 of 42 gastric biopsy specimens matched those of corresponding H. pylori isolates from respective patients. Patterns from the remaining eight biopsy specimens differed and appeared to represent infection with two H. pylori strains. The patterns of one strain from each of these biopsies was identical to that of the isolate from corresponding patients and the second pattern presumably represented the co-infecting strain. From the study, it appears that PCR-based RFLP analysis is a useful primary tool to detect and is distinguish H. pylori strains from gastric biopsy specimens and is superior to culture techniques in the diagnosis of infection with multiple strains of H. pylori.

혈청학적 분석을 통한 돼지 생식기호흡기증후군의 농장단위 감염유형 (Infection patterns of porcine reproductive and respiratory syndrome virus by serological analysis on a farm level)

  • Porcine reproductive and respiratory syndrome (PRRS) is the most economically important viral infectious disease in pig populations worldwide. This study was conducted to better understand the epidemic and dynamics of PRRS virus (PRRSV) on each farm and to evaluate the risk of PRRSV infection in Korea. Interviews with pig farmers were carried out to obtain PRRS vaccination programmes in 60 pig farms throughout Korea. Blood samples were also collected from the 59 pig farms to investigate outbreak patterns of each farm. Vaccination against PRRS was performed in 16.7% farms for breeding pigs and 8.3% of farms for nursery pigs. According to the seroepidemiological analysis, 56 (94.9%) out of 59 farms were considered to be affected by PRRSV infection. The results revealed that 68.9% of sows tested were seroconverted and interestingly, gilt herds had the highest seropositive rate (73%), suggesting that gilts may play a key role in PRRSV transmission in sow herds. Among the PRRS-affected piglet herds, 33 (55.9%), 14 (23.7%) and 6 (10.2%) farms were initially infected with PRRSV during the weaning, suckling and nursery period, respectively. It seems likely, therefore, that PRRSV infection predominantly occurs around the weaning period in piglet herds. Based on antibody seroprevalence levels in both sow and piglet groups, we were able to classify patterns of PRRSV infection per farm unit into 4 categories; category 1 (stable sow groups and non-infected piglet groups), category 2 (unstable sow groups and non-infected piglet groups), category 3 (stable sow groups and infected piglet groups), and category 4 (unstable sow groups and infected piglet groups). Our data suggested that 43 (72.9%) farms were analysed to belong to category 4, which is considered to be at high-risk for PRRS outbreak. Taken together, our information from this study will provide insight into the establishment of an effective control strategy for PRRS on the field.

Diagnostic Value of Early Inflammatory Reaction in Postoperative Infection of the Lumbar Spine

  • Objective : Our aim is to evaluate the early changes of biologic markers such as white blood cell[WBC] count, erythrocyte sedimentation rate[ESR] and C-reactive protein[CRP] in early diagnosis of postoperative infection and to differentiate infection from inflammatory reaction in lumbar spine surgery. Methods : We reviewed 330patients who had undergone spinal operations between May 1999 and October 2001. For this study, the patients were classified into two groups, which include a group that underwent spinal decompressive surgery without instrumentation[SD], and the other group that underwent fusion surgery with spinal instrumentation[SI]. And each group was also subdivided into two groups respectively, one with infection and the other without infection. We retrospectively analyzed the WBC count, ESR and CRP preoperatively and postoperatively, according to their operation type and postoperative infection history. Results : Inflammatory indices were physiologically affected by instrumentation itself. But ESR and CRP elevations were more prolonged and sustained under infection. In SD patients without infection, ESR and CRP were stabilized 5 days after surgery. In SI patients without infection, CRP was stabilized about 7days after surgery, but ESR showed sustained and variously elevated. In both SD and SI groups, the stabilization of CRP was the most reliable behavior of surgery without infection. Conclusion : C-reactive protein is most sensitive parameter for postoperative spine infection. The knowledge of the inflammatory indices and their relatively uniform patterns with or without infection offers surgeons the ability to infer the state of surgical wound.

Multiclass Botnet Detection and Countermeasures Selection

  • The increasing number of botnet attacks incorporating new evasion techniques making it infeasible to completely secure complex computer network system. The botnet infections are likely to be happen, the timely detection and response to these infections helps to stop attackers before any damage is done. The current practice in traditional IP networks require manual intervention to response to any detected malicious infection. This manual response process is more probable to delay and increase the risk of damage. To automate this manual process, this paper proposes to automatically select relevant countermeasures for detected botnet infection. The propose approach uses the concept of flow trace to detect botnet behavior patterns from current and historical network activity. The approach uses the multiclass machine learning based approach to detect and classify the botnet activity into IRC, HTTP, and P2P botnet. This classification helps to calculate the risk score of the detected botnet infection. The relevant countermeasures selected from available pool based on risk score of detected infection.

Gene Expression Profiles in Genetically Different Mice Infected with $Toxoplasma$ $gondii$: ALDH1A2, BEX2, EGR2, CCL3 and PLAU

  • $Toxoplasma$ $gondii$ can modulate host cell gene expression; however, determining gene expression levels in intermediate hosts after $T.$ $gondii$ infection is not known much. We selected 5 genes ($ALDH1A2$, $BEX2$, $CCL3$, $EGR2$ and $PLAU$) and compared the mRNA expression levels in the spleen, liver, lung and small intestine of genetically different mice infected with $T.$ $gondii$. ALDH1A2 mRNA expressions of both mouse strains were markedly increased at day 1-4 postinfection (PI) and then decreased, and its expressions in the spleen and lung were significantly higher in C57BL/6 mice than those of BALB/c mice. BEX2 and CCR3 mRNA expressions of both mouse strains were significantly increased from day 7 PI and peaked at day 15-30 PI ($P$<0.05), especially high in the spleen liver or small intestine of C57BL/6 mice. EGR2 and PLAU mRNA expressions of both mouse strains were significantly increased after infection, especially high in the spleen and liver. However, their expression patterns were varied depending on the tissue and mouse strain. Taken together, $T.$ $gondii$-susceptible C57BL/6 mice expressed higher levels of these 5 genes than did $T.$ $gondii$-resistant BALB/c mice, particularly in the spleen and liver. And ALDH1A2 and PLAU expressions were increased acutely, whereas BEX2, CCL3 and EGR2 expressions were increased lately. Thus, these demonstrate that host genetic factors exert a strong impact on the expression of these 5 genes and their expression patterns were varied depending on the gene or tissue.
