Kangaroo mother care practices for low birthweight newborns in a district hospital in Indonesia

  • Purpose: Kangaroo mother care (KMC) was introduced in Indonesia 30 years ago, but the extent of its use has not been fully documented. Therefore, this study aimed to examine the use of KMC and evaluate the characteristics of infants who received KMC at Koja District Hospital in North Jakarta, Indonesia. This retrospective cohort study recorded the characteristics of infants with birthweights less than or equal to 2,200 g at the above-mentioned hospital. Methods: Data collected from infant registers included gestational age, birthweight, Apgar score, number of complications, history of neonatal intensive care unit treatment, and KMC status. Cox regression analysis was conducted. Results: This study found that 57.7% of infants received KMC. Infants with birthweights over 1,500 g were 2.16 times (95% CI: 1.20-3.89) more likely to receive KMC. Conclusion: Efforts to promote KMC are recommended, specifically for infants with birthweights greater than 1,500 g. KMC for infants with other conditions can also be considered based on the infants' stability.

  • 출산율, 결흔율 감소 및 이혼을 증가 등의 사회적 현상으로 인해 태어나면서부터 건강에 대한 관심이 증대되고 있는 실정이다. 특히 의사 표현 능력이 부족한 소아의 질환 진단은 직접 내원해서 진단을 받아야 하는 불편함을 가지고 있다. 이를 위해 본 연구에서는 재택 기반으로 소아 울음소리를 통해 음성 분석학적 요소를 추출하여 정상 소아와 질환을 앓고 있는 소아와의 비교, 분석을 통해 소아 질환을 진단할 수 있는 시스템을 개발하고자 한다. 특히, 본 논문은 소아에게 가장 쉽게 걸릴 수 있는 소아호흡기 질환 중 소아감기, 소아폐렴 및 소아천식을 대상으로 실험을 수행하였으며 울음소리의 특징 요소를 추출하여 진단기기로 개발하고자 한다. 이를 위해 소아호흡기 질환이 인체의 음성 기관을 자극하는 질환임을 가정하고 음성학적 분석 요소 중 조음기관과 관련된 주파수대역폭분석을 통한 방법을 실험하였으며 이를 정상 소아와 소아호흡기 질환을 앓고 있는 환자를 비교, 분석하였다. 이와같은 방법을 통해 정상 소아에 비해 호흡기 질환을 앓고 있는 소아가 주파수 대역폭이 짧게 형성되는 결과를 추출하였다.

초산모 배우자의 아동기 경험, 부성애착, 부성역할에 관한 연구 (Childhood Experiences, Paternal Attachment and Paternal Role of Primiparous Spouses)

  • Purpose: This study aims to provide foundational data for preparing for educational programs on the role of a father that are designed to help primiparous spouses carry out the paternal role successfully. Methods: This study was conducted in two OB/GYN clinics and a study sample of 277 primiparous spouses who satisfied the selection criteria was selected. Results: Of the correlation among the childhood experiences, paternal attachment and role of a father of the subjects, the childhood experiences had a positive correlation with the paternal-infant attachment and the role of a father. The paternal-infant attachment and the role of a father also had a positive correlation. Conclusion: An infant is influenced primarily by his or her parents, who will have immense influences on the child's growth and development. Since the paternal-infant attachment is closely related to the role of a father, the development of positive paternal-infant attachment must be preceded in order for a primiparous spouse to carry out the role of a father successfully. Therefore, the development of nursing intervention programs is needed to foster the proper understanding of the role of a father, accommodate the practical needs of primiparous spouses and nurture the paternal-infant attachment for the promotion of realistic, successful role of a father.

통합감각자극이 저체중아의 성장 및 모아 상호작용에 미치는 효과 (The Effects of Multimodal Sensory Stimulation Combined with Chiropractic Therapy on Growth and Mother-Infant Interaction in Infants with Low Birth Weight)

  • Purpose: This study was conducted to investigate the effects of multimodal sensory stimulation on growth and mother-infant interaction in infants with low birth weight (LBW). Method: A non-equivalent control group time-series study design was used. The participants were 38 LBW infants and their mothers (19 in the intervention group and 19 in the control group). The data were collected from September 1, 2003 to March 31, 2004. For the mothers in the intervention group, this researcher instructed mothers in the multimodal sensory stimulation therapy, in turn the mothers used these techniques on their infants once a day during the 4-week research period. The researcher measured weight, length, and head circumference of the LBW infants once a week for 4 weeks and made a film of the mother playing with the infant for 5 minutes in the last week of the research period. Results: Compared to the control group, LBW infants in the intervention group showed significant increases in weekly weight gain (F=3.82, p=.012) and had significantly higher scores for mother-infant interaction (t=3.93, p>.000). Conclusion: The results suggest that multimodal sensory stimulation therapy can be used to increase the growth of LBW infants and improve mother-infant interaction.

영아 어머니의 과도한 수유 관련 요인 (Factors Associated with Pressure to Eat as a Feeding Practice among Mothers with Infants)

  • Purpose: This study aimed to identify factors-both infant-related and maternal-associated with pressure to eat as a feeding practice among mothers with infants. Methods: This study used a cross-sectional design and included 163 mothers of infants aged 2~12 months. Of the 180 self-reported questionnaires that were distributed, 163 (91%) were included in the data analysis. Multiple regression analysis was used to identify the factors associated with pressure to eat as a feeding practice among the mothers. Results: Infant's temperament (β=-.17, p=.035), mother's body mass index (β=-.16, p=.048), and concern about the infant being underweight (β=.30, p=.001) were associated with pressure to eat as a feeding practice among mothers. The explanatory power of these variables in the predictive model was 19.2%. Conclusion: Educational programs should be developed for improving mother's awareness of cues from infants with difficult temperament. In addition, educational interventions regarding the correct evaluation of infant's weight are needed to relieve mother's concern about their infant's being underweight. These interventions might be helpful to reduce the prevalence of pressure to eat as a feeding practice among mothers with infants.

Knowledge on Sudden Unexplained Infant Death-related Safe Sleep Practices and Infant Cardiopulmonary Resuscitation in Pediatric Nurses

  • Purpose: Sudden unexplained infant death (SUID) is a major contributor to infant mortality, and pediatric nurses have the responsibility to educate parents on SUID-reducing strategies. This study was conducted to measure pediatric nurses' knowledge of SUID-related safe sleep practices (K-SSSP) and infant cardiopulmonary resuscitation (K-ICPR). Methods: In total, 136 pediatric nurses were administered a survey including K-SSSP (13 items), K-ICPR (5 items), confidence in K-SSSP education (1 item; 5 points), and other factors relating to SUID experiences or education. Results: The correct answer rates of the K-SSSP and K-ICPR were 62.6% and 62.5%, retrospectively. The mean score for confidence in K-SSSP education was 2.6±0.9. Only 18 nurses (13.2%) responded that they educated parents on the content of the K-SSSP, while 76 nurses had received education on SUID. Positive relationships were observed between K-SSSP scores and higher education, between K-ICPR scores and having own child(ren) and clinical experience, and between confidence in K-SSSP education and higher education or having one's own child(ren). Nurses caring for newborns performed more SUID education than nurses working in other units. Conclusion: There is a profound need to implement a systemic educational program on SUID and strategies to reduce SUID for pediatric nurses.

초산모의 양육스트레스, 영아신호에 대한 민감성, 양육환경 (Parenting Stress, Maternal Sensitivity to Infant Cues and Child Rearing Environment of First-time Mothers)

  • Purpose: The purpose of this study was to identify relations among the parenting stress, maternal sensitivity to infant cues, parenting environment of first-time mothers. Method: The participants were 194 first-time mothers of babies aged 1-6 months who visited well-baby clinics in 5 hospitals. The data were collected from April 15 to June 15, 2003. Results: The parenting stress level was moderate with a mean score of 2.4(range 1-5). The parent domain and parent-child relationship domain of the parenting stress scale were significantly correlated with maternal sensitivity to infant feeding cues (r=-.178, p<.05; r=-.197, p<.01). Parenting stress was significantly correlated with childrearing environment(r=-2.19, p<.01). Parenting stress and childrearing environment were significantly different according to the educational level of the mothers and their prenatal care. Conclusions: Nursing interventions to reduce parenting stress in first-time mothers are needed to improve maternal sensitivity to infant cues and childrearing environment which foster infant development.

영아기질 측정도구(WBL)의 신뢰도 및 타당도 평가 (Confirmatory Analysis of Infant Temperament Inventory (WBL; What my baby is like))

  • This study was conducted to test the reliability and validity of "What my baby is like(WBL)" that was developed by Pridham et al. for measuring infant temperament. 37 subjects were participated four times-1, 3, 6, and 12months, and 146 cases were analyzed. The construct validity for the scale was tested by factor analysis with four factor loading solution based on the previous study. One item was excluded because total-item correlation was too low, so that the number of total items were eighteen. The analysis yielded four well defined factors: amenability and persistence, activity, reactivity, and adaptability. These four factors explained 57.62% of the variance in the infant temperament. The criterion related Validity was confirmed by the relationship between the infant temperament and HOME, and between the infant temperament maternal confidence in childrearing. The reliability of the scale was determined with Cronbach's alpha coefficient and Guttman split-half coefficient. Cronbach's alpha was .86, for 18 items, and Guttman split-half cofficient was .81. In subscale analysis, Cronbach's alpha was .56~.84. In conclusion, WBL showed a high degree of validity and reliability, suitable in Korean infants for measuring temperament. In the further study, it would be necessary to refine the concept of temperament and to analyze the correlation between WBL and other temperament inventory.

모아의 조기 접촉이 모성의 애착행위에 미치는 영향 (Effects of Early Contact on Maternal Infant Attachment)

  • Purpose: The purpose of this study was to evaluate the effects of early contact between mothers and infants on attachment behaviors. Methods: The participants were purposely allocated to an experimental group (n=20) and a control group (n=20). For the experimental group, early contact between mothers and newborns took place for 10 minutes. Mother-infant attachment was assessed in the nursery. Data analysis was done using frequency, percentage, $x^2$ test, mean, standard deviation, and t-test with the SPSS/WIN program. Results: The experimental group practicing early mother-infant contact, showed a higher degree of identifying behaviors (p<.05), modalities of interaction (p<.05), and caretaking behaviors (p<.05) than the control group. Conclusion: The early mother-infant contact at 2 hours post-delivery had positive effects on mother-infant attachment. Therefore an early contact can be recommended for all mothers-to-be as a way of improving mother-infant relationships.

The Effects of the 5-step Method for Infant Cardiopulmonary Resuscitation Training on Nursing Students' Knowledge, Attitude, and Performance Ability

  • Purpose: The purpose of this study was to examine the effects of an infant cardiopulmonary resuscitation (CPR) training program that applied the 5-step method on the knowledge, attitudes, and performance ability of nursing students in terms of enhancement and sustainability. Methods: Sixty-one nursing students (28 in the experimental group and 33 in the control group) from D city participated in this study. Data were collected from April 25 to December 15, 2016. The experimental group and control group received infant CPR education using the 5-step method and the traditional method, respectively. The outcome variables were measured 3 times (pretest and posttest at 1 week and 6 months after training) Results: There were significant differences in attitude (t=2.68, p=.009) and performance ability (t=4.56, p<.001) between the groups at 1 week after training, as well as in sustained performance ability at 6 months after training (F=6.76, p=.012). Conclusion: The 5-step method of infant CPR training was effective for improving performance ability in a sustained manner and promoting a positive attitude. Therefore, it is recommended that nursing students, as infant CPR novices, receive training using this effective method.
