Endotracheal Intubation of Paramedics in a Moving Ambulance (이동 중 구급차에서 1급 응급구조사의 기관내 삽관)

  • The purpose of this study is to perform the effectively advanced airway management in an emergency patient with cardiac arrest and trauma by comparing the appearance of influence in a change of visual-field height upon endotracheal intubation according to a change in position with intubation of paramedics in a moving ambulance and by analyzing the appearance of the influence in a patient's change in position with intubation upon speed in endotracheal intubation. Research subjects were randomly extracted 60(30 people for control group, 30 people for experimental group) people as the paramedics who are working at 13 fire stations in C Province. Data analysis was carried out ${\chi}^2$-test, independent t-test, paired t-test by using SPSS WIN 14.0 Version. As a result of research, to improve speed of the advanced airway management, it is considered to be likely effective in a patient's sniffing position and in the endotracheal intubation in the upper space of the main stretcher. The self-confidence in intubation after experiment increased significantly. Thus, the continuous education(training) on the intubation position and method within ambulance is considered to be likely needed for improving efficiency of the advanced airway management.

Clinical Results of Silicone Oil Injection following Vitrectomy as a Primary Procedure in Retinal Detachment (망막박리에서 일차수술로 유리체절제술과 실리콘 주입술에 대한 임상결과 분석)

  • To evaluate clinical results of silicone oil injection for retinal detachment. The 45 eyes of 44 patients underwent vitrectomy and silicone oil injection from 1999 to 2002. 1.There were no history of previous retinal surgery and ocular trauma. In 24 of the 45 eyes(53%) visual acuity increased and stabilized in 18 of the 45 eyes(40%). 2. 1 of 16 eyes(6.3%) had recurred retinal detachment after silicone oil removal. 3. Final retinal reattachment was achieved in 44 eyes(97%). Postoperative complications were cataract(16 eyes), glaucoma(10 eyes), emulsification of silicone oil(5 eyes), keratopathy(2 eyes) and recurrent retinal detachment(1 eye). These results show that silicone oil injection for primary retinal detachment yields a high rate of anatomic success and the good visual outcome

  • This study investigated hospital EMT' scope of work perceived by emergency medical service providers by surveying 122 emergency medical service providers working at university hospitals in Daejeon and Chung-nam in order to provide basic materials for improving EMT' roles and legal regulations related to their works and enhancing the quality of emergency medical services. According to the results of this study was $3.23{\pm}0.67$ for the 40 items surveyed, which was somewhat higher than average. By area, job awareness was highest for trauma care, which was followed by hospital phase and breathing assistance, and lowest for medication. In order for emergency medical service providers to recognize hospital EMT' works, first of all, EMT need to carry out their jobs and roles faithfully. Therefore, educational institutions should provide substantial education programs. What is more, legally clear definitions should be made on the job of EMT, and campaigns and monitoring should be made continuously for EMT.

Traumatic Experience and the obsessive behaviors in romantic relationship of Female Sex Workers (성매매 여성의 외상 경험과 이성 관계 집착)

  • This study investigated the characteristics of obsessive behaviors in romantic relationship of 42 female sex workers and the effect of traumatic experiences on them. Research results that there was no significant difference in loneliness, jealousy, over sensitiveness between the female sex workers group and the controls. But fear of rejection and doubt in the romantic relationship are significantly higher in the female sex workers group than in the controls. Among the subscales, childhood neglect and family secret were most frequently associated with the various obsessive behaviors. This is also the case for the control group, with family secret and physical abuse being the most. Finally, in regression analysis, in the control group, childhood physical abuse had the biggest impact on the obsessive behaviors in romantic relationship. On the other hand, the target group, childhood neglect had the biggest impact on the obsessive behaviors in romantic relationship.

A Preliminary Study on the Psychopathological protocols of the Vietnam War Agent Orange Patients and Korean Civil War Wound Soldier -Focus on Rorschach Tests- (베트남 참전 고엽제 환자와 한국전쟁 참전 상이군인의 심리적 특성에 관한 예비 연구 -로샤 반응을 중심으로-)

  • This study was carried to examine overall psychopathological patterns of the Vietnam Civil War Veterans(Agent Orange patients)and Korean Civil War veterans(wounded soldier) using Rorschach tests. Test protocols of were compared with those of twenty Vietnam Civil War Veterans group and twenty-one Korean Civil War veterans. Rorschach protocols were administrated, scored, analysed according to the Exner's comprehensive system. Specifically, each Rorschach variable was divided into 3 clusters(e.g., core section, response determinants, and several special indices. The results of t-tests between the Vietnam Civil War Veterans group and the Korean Civil War veterans showed that Vietnam Civil War Veterans group were characterized as 1)cognitive rigidity, 2) ineffective problem solving, 3)emotional represion, 4)hypervigilance, 5)anxiety and 6) depressive tendency. These patterns suggest that still Vietnam Civil War Veterans experience the past trauma and lasting psychological seguela. Other implications and limitations of this study were discussed.

Knowledge Structure of Posttraumatic Growth Research: A Network Analysis (네트워크 분석을 통한 외상 후 성장 지식구조 연구)

  • Posttraumatic growth literature has been rapidly expanding in multiple academic disciplines. Purpose of this study is to examine the knowledge structure of posttraumatic growth utilizing a network analysis. Papers published between 1996 and 2018 were searched on the Web of Science, focusing on terms related to posttraumatic growth. One thousand six-hundred and fifty-nine keywords were published 6,343 times in 1,780 papers; thus, a total of 322 keywords (5,195 appearances) were selected for the final analysis. The network analysis and network visualization tool used were NodeXL and PFnet, respectively. The keywords which appeared the most frequently were "Posttraumatic growth," followed by "Posttraumatic Stress Disease," "Cancer," and "Trauma." A total of 322 nodes have been reduced to 175 nodes and divided into a total of five groups. The five groups were "Posttraumatic Growth in Cancer, Chronic/Serious Illness, and Disability," "Posttraumatic Growth-related Psychological Variables and Psychotherapy," "Posttraumatic Growth in the Context of Death," "Cognitive Mechanisms of Posttraumatic Growth," and "Vicarious Posttraumatic Growth." This study provides a systematic overview on the knowledge structure of posttraumatic growth by quantitatively network analysis.

Maxillofacial Trauma Trends at a Tertiary Care Hospital: A Retrospective Study

  • Purpose: Maxillofacial fractures are rapidly increasing from car accidents, industrial accidents, teenaged criminal activity, and sports injuries. Accurate assessment, appropriate diagnosis, and preparing individual treatment plans are necessary to reduce surgical complications. We investigated recent trends of facial bone fracture by period, cause, and type, with the objective of reducing surgical complications. Methods: To investigate time trends of maxillofacial fractures, we reviewed medical records from 2,196 patients with maxillofacial fractures in 1981~1987 (Group A), 1995~1999 (Group B), and 2008~2012 (Group C). We analyzed each group, comparing the number of patients, sex ratio, age, fracture site, and etiology. Results: The number of patients in each period was 418, 516, and 1,262 in Groups A to C. Of note is the increase in the number of patients from Group A to C. The sex ratios were 5.6:1, 3.5:1, and 3.8:1 in Groups A, B, and C. The most affected age group for fracture is 20~29 in all three groups. Traffic accidents are the most common cause in Groups A and B, while there were somewhat different causes of fracture in Group C. Sports-induced facial trauma was twice as high in Group C compared with Group A and B. Mandible fracture accounts for a large portion of facial bone fractures overall. Conclusion: We observed an increase in facial bone fracture patients at Kyungpook National University Dental Hospital over the years. Although facial injury caused by traffic accidents was still a major cause of facial bone fracture in all periods, the percentage decreased. In recent years, isolated mandible fracture increased but mandible and mid-facial complex fracture decreased, possibly because of a reduction in traffic accidents.

Clinical Analysis of the Nasal Bone Fracture (비골골절에 대한 임상적 분석)

  • Purpose: Nasal bone fracture is the most common facial fracture. Although nasal bone fractures are considered to be minor injuries, the incidence of post-traumatic nasal deformity remains high. This study is designed to support management and patient satisfaction by classifying the simple nasal bone fracture, and survey the care method and result, which is compared with other studies. Methods: From May 2008 to April 2010, 334 patients with simple nasal bone fractures visited our hospital. The incidence, cause, types of nasal bone fracture, treatment, and complications are analyzed according to clinical examination, patient's record and radiographic images. Results: The mean age of patients was 30 years old, with 74% of the patients having been male, and 26% were female. The highest incidence of this fracture was between late teens to late twenties. The causes of nasal bone fracture were the following: having slipped or fallen down (39%), violence (16%), sports accident (14%), traffic accident (11%), industrial accident (6%), and others (16%). Patient's radiographic images were analyzed by Stranc and Robertson classification, frontal impact plane I was 38%, plane II was 16%, plane III was 1%, lateral impact plane I was 21%, plane II was 21%, and plane III was 3%. On average, surgical treatment was performed 7.2 days after trauma under general anesthesia. Closed reduction of nasal bone fracture was performed in 99% of patients. In patients with septal injuries, septal management was performed in 76.7% of cases. Aesthetic surgery was done on same time in 24% of patients. There were some complications, such as residual nasal deformity (7.2%), nasal obstruction (0.9%) and hyposmia (0.3%). Conclusion: According to this study, nasal bone fractures occurred commonly in physically active age groups (age 15~29 years), as a result of having slipped or fallen down, at afternoon and at night time. And it could be treated successfully by closed reduction and septoplasty by 7 days after trauma.

Survey and Review of Blowout Fractures (안와벽 파열골절에 대한 임상적 고찰)

  • Purpose: Blowout fractures of the orbit are common sequelae to blunt facial trauma and now increasing in number due to automobile accidents, violence and industrial disasters. There are some reports of diagnosis and treatment of this fracture, but detailed data provided in overall aspects are very few. We analysed extensive data to provide guide line of blowout fracture patients care. Methods: We retrospectively studied 387 orbital blowout fracture patients who had been followed up at least 3-6 months. Their hospital records were reviewed according to causes, fracture site, operation methods, and follow up results, etc. Results: The ratio of males to females was 7 : 3 and fractures were most often seen in the 20-29 age group. 180(47%) patients had medial orbital wall fractures, 155(40%) patients had floor fractures and 52(13%) patients had a combination of orbital floor and medial wall fractures. The highest associated bone fracture was the nasal bone(37%). The open reduction was done in the 324 patients(83.7%) and insertion materials were used in the 249 patients(77%). Total 45 patients(14%) complained of residual diplopia and 26 patients(8%) kept mild enophthalmos. 24 patients(7%) showed some restriction of extraocular muscle movements. Conclusion: We broadly surveyed the information of blow out fracture patients and believe that this study provides important prognostic information that can be of benefit to both patient and surgeon during preoperative counseling and postoperative analysis of orbital blowout fractures.

Psychosocial Outcome after Head Injury (두부외상후 심리사회적 예후)

  • Objective : This study was designed to evaluate the relationship between the initial neurosurgical or psychosocial factors and the psychosocial outcome. Patients and Methods : We analyzed 123 head-injured patients who were referred to the department of psychiatry for the evaluation of psychosocial function. We analyzed initial neurosurgical variables such as Glasgow Coma scale(GCS) score, skull fracture, CT finding, and psychosocial outcomes with regards to psychosis, personality change, depression, anxiety and IQ on Intelligence Scale. Results : Patients with mild head injury(GCS score 13-15, N=94, 76.4%) had better recovery rate on Glasgow Outcome Scale(GOS), less personality change than those with moderate or severe head injury. However, depression, anxiety and intelligence were not significantly different between two groups. The skull fracture(N=37, 30.1%) did not influence on the psychosocial outcome with reference to personality change, depression, anxiety and intelligence. The patients with abnormal CT findings(N=64, 52%) had lower recovery rate on GOS, more frequent tendency in psychosis, personality change and severe depression, less frequent in anxiety and mild depression, than patients with normal CT finding. However, levels of intelligence were not different between two groups. The patients with industrial accidents(IA) had lower educational level, milder head injury, more delay for the psychiatric evaluation (longer treatment period) than those with motor vehicular accidents(MVA). The psychosocial outcome with reference to personality change, depression, anxiety, intelligence were not different between two groups. Conclusion : These findings indicate that the more severe initial trauma, the poorer psychosocial outcome. However, it was frequently observed that patients with mild head injury suffered from mild anxiety and depression. Therefore mild head injury appeared to be more complicated by psychosocial stressors. The patients with IA, despite the fact that initial head injury was mild, required longer treatment period than MVA.
