환자의 주관적 증상 텍스트에 대한 진료과목 분류 모델 구축 (Classification Modeling for Predicting Medical Subjects using Patients' Subjective Symptom Text)

  • 의료 인공지능 분야에서 의사의 판단에 도움을 줄 수 있는 질환 예측 및 분류 알고리즘에 대해선 많은 연구가 이뤄져왔지만, 의료 소비자의 정보 획득과 판단에 도움을 줄 수 있는 인공지능에 대해선 상대적으로 관심이 적다. 네이버 지식인에 지난 1년 간 자신의 증상엔 어떤 병원을 가야할 지 질문하는 질문 건수만 해도 15만 건이 넘는다는 사실은 의료소비자들에게 적합한 의료정보의 제공이 필요하다는 반증이기도 하다. 따라서 본 연구에선 의료소비자들이 자신의 증상에 대한 진료과목을 선택하는데 도움을 줄 수 있도록 네이버 지식인에서 환자들이 직접 서술한 증상 텍스트를 수집하여 8개 진료과목을 분류하는 분류모델을 구축했다. 우선 환자의 주관이 개입된 데이터의 타당성과 객관성을 확보하기 위해 객관적 증상 텍스트(서울응급의료 정보센터에서 정리한 진료과목 별 주요 질환 증상)와 주관적 증상 텍스트(지식인 데이터) 간 유사도 측정을 수행하였다. 유사도 측정 결과, 두 텍스트가 동일한 진료과목의 증상일 경우 상이한 진료과목의 증상 텍스트에 비해 상대적으로 높은 유사성을 가진다는 것을 입증했다. 상기 절차를 따라 타당성을 확보한 주관적 증상 텍스트를 대상으로 릿지회귀모델을 사용하여 분류모델을 구축한 결과 0.73의 정확도를 확보할 수 있었다.

  • In 1189's ZhongXiuZhengHuoJingShiZhengLeiBeiYongBenCao(重修政和經史證類備用本草), we took a look at the medicinal herbs that mentioned JoSeon(朝 鮮), GoRyeo(高麗), BaekJe(百濟), SinRa(新羅), DongIn(東人), DongHae(東海), YoDong(遼東), and BalHae(渤海). In the face of Korea's Oriental medicine's reality of having medical texts only after the 2nd half of the GoRyeo(高麗) Dynasty, this study will provide the basis of finding the origin of Indigenous Herbal Medical Science that was founded in the late-GoRyeo(高麗), early-JoSeon(朝鮮) period.

  • With the recent increase of social demand of knowledge on traditional medicine, the construction of database is becoming a pressing matter. The reality is that while the social desire to adapt the high quality knowledge is growing fast, the field of Oriental Medicine has yet to organize its system. The number of Oriental Medical Doctors are limited, and there is no structured means of communication to expand the professional knowledge of these specialists to the public. Accordingly, an effective and structured knowledge system is in great need. In the process of building an Oriental Medicine database, prescription[方劑] and medicines[本草] are first in line. Consequently, we have taken the first step by organizing various methods to build the database containing information such as the formula, ingredients, composition, handling methods of herbal medicine written in traditional medical publications and herbal texts before the Song(宋) dynasty.

청강 김영훈 진료기록 데이터베이스모형 개발연구 (Research of Database Model of Kim-YoungHun's Medical Chart)

  • Cheong-Gang Medical Chart is 60 years worth of diagnosis records kept by Oriental Medicine Doctor Kim Young Hoon [金永勳, 號 晴崗 1882-1974], who held practice in Seoul's Jong-ro from 1915 till 1974. Kim Young Hoon's eldest son, Kim Ki Su (金琦洙) donated the medical records exceeding a thousand volumes to KyungHee University, and researches are being made presently. The author of Cheong-Gang Medical Chart, Kim Young Hoon, was a medical scholar who studied the essence of the traditional medicine of his time. He was handed down the quintessence of traditional medicine by keeping in touch with the prominent oriental doctors in Seoul at that time, and he constantly applied it to his practice and made records of it. Consequently, his diagnosis charts contain a whole new form of prescriptions, treatment skills, and processes of clinical application that have never been seen before in the texts of Korean Medicine. The writer has written a paper on the present condition of Cheong-Gang Medical Chart, which was published in the Journal of Korean Oriental Medicine in 2004. This manuscript reports the results of the test studies made to develop an efficient database model as a prior step to organizing the medical records into a data bank.

『황제내경(黃帝內經)』에 나타난 원방보사(員方補瀉)에 관한 고찰(考察) (A Study on Won Bang(員方) Supplementation and Draining Method in the 『Huangdineijing』)

  • Objectives : This study aims to grasp the principle of the Won Bang supplementation and draining method as mentioned in the 『Lingshu·Guanneng』 and the 『Suwen·Bazhengshenminlun』, which have contrasting properties. Methods : The texts in each chapter were analyzed to understand the supplementation and draining principle that matches the meanings of round[won,員] and angular[bang, 方]. Especially in the case of the chapter 「Bazhengshenminlun」, a hypothesis was drawn upon the relationship between the abstract explanation of the round[won,員] and angular[bang, 方] and the actual manipulation technique. This hypothesis was tested against other texts and annotations for further discussion. Results & Conclusions : The expressions 'bang[angular, 方]' and 'won[round,員]' refer to the same meanings in both chapters, as 'to be upright' and 'to be smooth,' respectively. The difference between the two chapters is that in the 『Lingshu·Guanneng』 the standard for Won Bang is the needler's movements, while in the 『Suwen·Bazhengshenminlun』, it is the patient's breathing. Moreover, while in the former the subjects of supplementation and draining are clearly divided into healthy qi and exterior pathogenic qi, in the latter the subject of manipulation is the deficiency and excessiveness caused by deviation of the healthy qi, thus making the subject of both supplementation and draining healthy qi. Based on these findings, we can conclude that the supplementation and draining of needling is divided into two methods; separating the healthy and pathogenic qi and manipulating the deficiency and excessiveness of healthy qi within the body.

"신간명의잡저(新刊明醫雜著)" 간행의 의사학적 의의 (The Historical Significance of the Publication of ShinKanMyungYiJapZhe (新刊明醫雜著))

  • This study aims to take a look at the ShinKanMyungYiJapZhe(新刊明醫雜著)'s publication process, influence on later generations, and its historical significance. A publication of the Ming dynasty, MyungYiJapZhe(明醫雜著) was republished by ChoSung(趙晟), a Chosun Confucian doctor, in 1551. In the process of its republication, the clinical value of this text was proven, and further explanations on ambiguous topics were also added. . A Japanese book based on the Chosun ShinKanMyungYiJapZhe(新刊明醫雜著) was discovered in the process of this research, and an analysis of this book also took place. Through the above research it could be shown that the Chosun republication of Chinese medical texts was an important means of medical information exchange and that through such processes new medical knowledge could be created.

심소장(心小腸) 표리(表裏) 관계의 임상 적용에 대한 고찰 -소변이상 관련 처방을 중심으로- (A Study on the Clinical Application of the Exterior-Interior Relationship Between the Heart and Small Intestine -Focusing on Prescriptions for Urine Disorders-)

  • Objectives : The aim of this paper is to examine texts that deal with the clinical application of the exterior-interior relationship between the Heart and Small Intestine. Methods : Texts that apply the Heart and Small Intestine relationship to treating urine disorders were selected and analyzed. Results : The relevance of the Heart-SI relationship to urine disorders was first discussed in the 『Zhubingyuanhoulun(諸病源候論)』, and the theory was finally applied to clinical treatment of urine disorders in the 『Waitaimiyao(外臺秘要)』. Text analysis revealed that Daochisan of the 『Yujiweiyi(玉機微義)』, Daochisan of the 『Yizongjinjian(醫宗金鑑)』, HupoDaochitang of the 『Yichunshengyi(醫醇賸義)』, and Daochiyinjiaweifang of the 『Xuezhenglun(血證論)』 were of Daochisan affiliation, while those that were not of this affiliation were Gandihuangwan of the 『Waitaimiyao(外臺秘要)』, Xijiaotang of the 『Shengjizonglu(聖濟總錄)』 and 『Pujifang(普濟方)』, Rushensan and Xijiaodihuangtang of the 『Qixiaoliangfang(奇效良方)』, and Liangxinlishuitang of the 『Bianzhenglu(辨證錄)』, indicating that the formulas used for treatment were mostly affiliated with Daochisan. When clinically applying the exterior-interior relationship of the Heart and SI to urine disorders, the phenomenon can be most closely matched to the biomedical concept of Overactive Bladder. Discussion : Based on the finding that the formula following the exterior-interior relationship of the Heart and SI was first mentioned in 『Waitaimiyao(外臺秘要)』 published in 752, and was continuously mentioned in the 『Xuezhenglun(血證論)』 which was published in 1884, it is highly probable that the exterior-interior relationship theory of the Heart and SI and its clinical application closely influenced each other.

청강 진료기록을 통해 살펴본 가미보정산의 기원 및 명칭 변화 (The origins and transformations to the name of Augmented Widen the Core Powder (加味普正散) as seen in the Dr. Kim Young Hoon's Medical Records)

  • This paper analyzes information on Augmented Widen the Core Powder (加味普正散) (AWCP) in the Dr. Kim Young Hoon's medical record database, Cheongang Medical Records. AWCP is a prescription that is not found in the existing medical texts, yet is referred to in Dr. Kim Young Hoon's medical record database. By examining these records, this study shows that the original prescription for AWCP was Decoction for Rectifying the Qi with Cyperus and Kudzu (香葛正氣飮) and that this prescription was deeply related to Rectify the Qi Powder Worth More Than Gold (不換金正氣散) as well as Cyperus and Kudzu Decoction (香葛飮). In addition, AWCP had several names over a long period of time, and its name changed from the Powder of Relieving Lives and Rectifying the Qi (普救正氣散) to AWCP. In particular, this study shows that the term 'gami' (加味 augmented) indicates a change in prescription name, not a change in the composition of the prescription.

간지(干支)의 의의(意義) 및 설문해자(說文解字)의 십간(十干)에 관한 연구(硏究) (A Study on a meaning of Heavenly stems and Earthly branches and Seolmunhaeja's Ten heavenly stems.)

  • Objective : Heavenly stems and Earthly branches is a tool used for understanding the virtue of Yin Yang and Five elements. Korean medicine understands the changes in Wuyun through the Ten Heavenly stems, and understand the changes of Liuqi through the Twelve Earthly branches. An accurate understanding of the definitions of Heavenly Stems and Earthly branches and the concept of each of the 10 stems is of vital importance. Method : The paper first reviews the origin, history, and significance of the Heavenly stems and Earthly branches before studying the definitions of the stems and branches as laid out in the works of Yu Onseo, Lee Samun, and Han Dongseok. The paper then reviewed the concept of the each of the ten stems through researching the texts of Seoulmunhaeja, the annotations of the four great Seolmuns, and the texts of Jeongyeokwonui. Result & Conclusion : Heavenly stems and Earthly branches have been in use since more than 6,000 years ago. The central numbers in the changes of Heaven and Earth are five and six. Each number functions with duality, yin and yang, meaning there are ten Heavenly numbers ($5{\times}2=10$) and 12 Earthly numbers ($6{\times}2=12$) which oversees all of the cosmic changes. Stems become the body and signifies water. Branches become the use and signifies divided fire. The meanings of the letters Gab Eul Byeong Jeong Mu Gi Gyeong Sin Im Gye originate from the one year life of a tree which grows, bears fruits, processes Yang qi, and awaits for the next spring. The reason a tree is used is because there is nothing better in studying in detail the changes of a living being through a year.

정년퇴임 교원(윤창열)의 학술 및 봉사 업적 (Achievements of a Retiring Faculty, Yoon Chang Yeol)

  • OObjectives : This paper aims to document the journal's history by researching academic achievements of a retiring faculty member and his contribution to the academy. Methods : To expand the boundaries of medical humanities and sociology within the Colleges of Korean Medicine, the methodology for taking oral history was followed in investigating the faculty member's scholarly and academic achievements, which will contribute to the preservation of the academy's history for years to come. Results : Yoon taught subjects such as Medical Chinese, Medical Classics, Classical Chinese, Reading Classical Texts, Medical Philosophy, Medical I-Ching at the University, while vigorously studying basic Korean Medical fields such as the Huangdineijing and the Five Circuits and Six Qi Theory. He also pushed for international communication and implemented the overseas studying program for students and faculty. At the academy, he stabilized the organization's finances, strengthened the journal's status, and built the foundation for the standardization of Medical Classics education by publishing a common textbook. Conclusions : Yoon's various achievements were made possible by other members of the organization, which was in reciprocation to Yoon's effort to bring the members together to form cooperative relationships.
