파골세포모양 거대세포를 동반한 갑상선 미분화 암종의 세침흡인 세포학적 소견 - 1예 보고 - (Fine Needle Aspiration Cytology of Anaplastic Carcinoma of the Thyroid with Osteoclast-like Giant Cells - A Case Report -)

  • Multinucleated giant cells of osteoclast-like appearance can be seen in a type of anaplastic carcinoma of thyroid and only a few case reports for fine needle aspiration cytologic findings are found in the literatures. Recently, we experienced a case of anaplastic carcinoma of thyroid with many osteoclast-like giant cells in a 72-year-old woman. The cytologic features and immunohistochemical results are described with special emphasis on differential diagnosis.

흉선의 신경내분비 종양 (Thymic Neuroendocrine Tumor)

  • 신경내분비 종양은 매우 드문 흉선 종양으로, Kultschizky 세포에서 유래하는 것으로 흉선종과는 다른 것으로 생각된다. 흥선 신경내분비 종양의 병리학적 진단은 광학 현미경 검사, 면역조직화학 염색 및 전자현미경의 소견을 바탕으로 한다. 흥선 유암종의 약 50%에서 내분비 질환을 가지며, 재발과 흉곽외 전이가 특징적이다. 초발 종양이나 재발의 경우에 외과적 절제가 가장 효과적이라고 생각되나, 항암치료나 방사선 치료의 역할은 분명하지 않다. 최근 외과적 절제가 시행된 흉선 신경내분비 종양을 경험하였기에 여기에 보고한다.

  • A case of chordoid meningioma occurring in a 61-year-old woman who did not have a Castleman's disease is presented. The patient had suffered from headache and motor dysphasia. Laboratory findings are normal. The tumor, located in the left frontal region and associated with peritumoral edema, was totally resected. Surgical specimen revealed a solid mass with irregular surface that measured $3.5{\times}4.5{\times}4cm$. Immunohistochemical staining revealed that the tumor cells expressed epithelial membrane antigen[EMA] focally, but not S-100 protein and glial fibrillary acid protein [GFAP], and the Ki-67 proliferative index of the tumor was 9%. The neoplasm was diagnosed chordoid meningioma of the World Health Organization[WHO] grade II. After total resection, her preoperative headache and dysphasia were disappeared.

Feline Gastrointestinal Eosinophilic Sclerosing Fibroplasia in a Bengal Cat

  • A 2-year-old neutered male Bengal cat presented with a 6-month history of weight loss and chronic vomiting. An abdominal ultrasound revealed increased thickness of the pylorus and ascending duodenum with concurrent enlargement of the mesenteric lymph nodes. Histologically, branching and anastomosing trabecular pattern of dense collagen was the characteristic feature, and large populations of spindle cells were also observed. These large spindle-shaped cells were positive for smooth muscle actin and vimentin on immunohistochemical examination. Based on these findings, the cat was diagnosed with feline gastrointestinal eosinophilic slcerosing fibroplasia (FGESF), and immunosuppressive therapy was initiated immediately. Unfortunately, the cat's condition deteriorated despite treatment; the cat died 56 days after initiation of therapy. This is the first report of FGESF in South Korea.

Rapid-Growing Juvenile Xanthogranuloma on the Scalp in 18-Month-Old Girl

  • Juvenile xanthogranuloma (JXG) is an uncommon histiocytic cutaneous lesion. An 18-month-old girl visited our clinic due to rapid growing orange-yellowish lesion on scalp. Enlarging time from 1 mm to 12 mm was just 8 weeks. We excised the tumor and adjacent normal tissue. Histopathological study showed numerous eosinophils and Touton giant cells within the lesion. Immunohistochemical study revealed positive immunoreactivity for CD68 in most areas. No recurrence was seen during 12 months after resection. We report a case with rapidly growing JXG on scalp with peculiar histopathologic findings.

하악에 발생한 횡문근육종 (Alveolar rhabdomyosarcoma involving the mandibular ramus and its surrounding tissues)

  • Rhabdomyosarcoma, when it occurs in the head and neck, is primarily found in children. Alveolar rhabdomyosarcoma is rarely seen in the oral lesion, comparing to the embryonal and the pleomorphic variants. This is a report of a case of alveolar rhabdomyosarcoma in the mandible in a ten-year old girl who complained of a non-painful swelling on the right cheek. The right lower 1st molar was mobile. Her radiographs revealed an extensive radiolucency with somewhat irregular border on the right mandibular ramus. The right mandibular 1st and 2nd molars lost their lamina dura and were floating. CT images revealed smooth-outlined soft tissue mass occupying the pterygomandibular space, the infratemporal space, and the masseteric muscle with thinning and perforation of the right mandibular angle and ramus. Histopathological and immunohistochemical findings established the final diagnosis of alveolar rhabdomyosarcoma.

Immunohistochemical Expression of Caspase 3 and Its Relationship with p53 Expression in Endometrial Cancers

  • Apoptosis is an important significance in the pathogenesis of cancer. Caspase 3 and p53 have been identified as important members of the apoptosis related proteins. This study was performed to define roles of caspase 3 expression and its relationship with p53 expression in endometrial cancers by immunohistochemistry. Immunoreactivity for caspase 3 was found in 13 (65.0%) out of 20 endometrial hyperplasia cases and 8 (36.4%) out of 22 endometrial cancers. Seven (87.5%) of the 8 cases with a positive caspase 3 immunoreactivity showed a positive p53 expression in 22 endometrial cancers. There were no significant associations between caspase 3 and p53 expressions. These findings suggest that caspase 3 expression might be associated with carcinogenesis of endometrial cancers. Further studies are needed to define the relationship between caspase 3 and p53 and apoptosis for examining the mechanisms of tissue-specific apoptosis related protein.

Periarticular histiocytic sarcoma of a thoracic limb in a Rottweiler

  • An 8-year-old, castrated, male Rottweiler was referred for evaluation of chronic right thoracic limb lameness and a progressively growing mass surrounding the right elbow joint. On admission, the dog's general health was good, without abnormalities detected on physical examination. The dog was diagnosed with periarticular histiocytic sarcoma. Although draining lymph nodes and lung metastases were suspected, palliative amputation was performed. Localized histiocytic sarcomas, with destructive lesions involving multiple bones of a joint and periarticular soft-tissue masses, are uncommon in dogs. This case report presents clinical findings, imaging characteristics, and histopathologic and immunohistochemical features of a periarticular joint histiocytic sarcoma.

흉막에 발생한 고립성 섬유종 - 외과적 치험 1례 - (Solitary fibrous tumor of Pleura - 1 case report -)

  • Solitary fibous tumor of pleura is a rare and slow vowing neoplasm. The histogenesis of this tumor is thought as mesenchymal origin by immunohistochemical and ultrastructural bases. This case report is of a 69-year-old female who presented severe dyspnea and orthopnea with long-term history. Preoperative chest X-ray an chest CT revealed huge and hemithorax-occupying heterogenous mass and cardiomegaly. She underwent surgical resection and a 22 X 18 X 10 cm sized visceral pleural based mass was evacuated. Histologic findings of tumor showed patternless proliferation in a fibrocollagenous and well vasculized stroma. Immnohistochemical staining revealed strong positive in CD34, that are compatible with solitary fibrous tumor of pleura. The patient was discharged without any complications and no evidence of recurrence was found after 2 years.

Disseminated non-Hodgkin's lymphoma presenting as bilateral salivary gland enlargement: a case report

  • Non-Hodgkin's lymphoma (NHL) constitutes a group of malignancies those arises from cellular components of lymphoid or extranodal tissues. The head and neck is the most common area for the presentation of these lymphoproliferative disorders. Primary involvement of salivary glands is uncommon. This report described a case of a 73-year-old female patient who presented with involvement of both nodal and extranodal sites, with predominant involvement of salivary glands. The tumor staging worked up along with imaging, histopathological, and immunohistochemical findings were discussed. Computed tomographic images showed the involvement of Waldeyer's ring, larynx, orbit, and spleen. This report described imaging and prognostic tumor markers in diagnosing, treatment planning, and prognosis.
