Meta-analysis of factors predicting resistance to intravenous immunoglobulin treatment in patients with Kawasaki disease

  • Purpose: Studies have been conducted to identify predictive factors of resistance to intravenous immunoglobulin (IVIG) for Kawasaki disease (KD). However, the results are conflicting. This study aimed to identify laboratory factors predictive of resistance to high-dose IVIG for KD by performing meta-analysis of available studies using statistical techniques. Methods: All relevant scientific publications from 2006 to 2014 were identified through PubMed searches. For studies in English on KD and IVIG resistance, predictive factors were included. A meta-analysis was performed that calculated the effect size of various laboratory parameters as predictive factors for IVIG-resistant KD. Results: Twelve studies comprising 2,745 patients were included. Meta-analysis demonstrated significant effect sizes for several laboratory parameters: polymorphonuclear leukocytes (PMNs) 0.698 (95% confidence interval [CI], 0.469-0.926), C-reactive protein (CRP) 0.375 (95% CI, 0.086-0.663), pro-brain natriuretic peptide (pro-BNP) 0.561 (95% CI, 0.261-0.861), total bilirubin 0.859 (95% CI, 0.582-1.136), alanine aminotransferase (AST) 0.503 (95% CI, 0.313-0.693), aspartate aminotransferase (ALT) 0.436 (95% CI, 0.275-0.597), albumin 0.427 (95% CI, -0.657 to -0.198), and sodium 0.604 (95% CI, -0.839 to -0.370). Particularly, total bilirubin, PMN, sodium, pro-BNP, and AST, in descending numerical order, demonstrated more than a medium effect size. Conclusion: Based on the results of this study, laboratory predictive factors for IVIG-resistant KD included higher total bilirubin, PMN, pro-BNP, AST, ALT, and CRP, and lower sodium and albumin. The presence of several of these predictive factors should alert clinicians to the increased likelihood that the patient may not respond adequately to initial IVIG therapy.

Inhibition of Eosinophil Infiltration and Humoral Immune Reaction by Ketotifen in BALB/c Mice Infected with Echinostoma hortense

  • Eosinophils play an essential role in allergy reaction after parasite infection. To examine the immune reaction induced by eosinophils, we investigated the allergy reaction in BALB/c mice infected with Echinostoma hortense's metacercariae, as well as the effect of ketotifen, an anti-allergy drug, on eosinophil immune reaction in the villi of host intestine. The worm recovery rate was higher in ketotifen-treated mice than in untreated mice and the worms in ketotifen-treated mice survived longer than those in untreated mice. The antibody titer in the serum of ketotifen-treated mice was very low. Especially, Echinostoma hortense infection strongly increased serum IgE level and eosinophil infiltration into the villi of the mouse intestine. Ketotifen treatment suppressed eosinophil infiltration into the infected areas and inhibited IL-4 production. The reduced IL-4 production may be related with the reduction of IgE, IgG1 and IgG2 production. In conclusion, ketotifen inhibited eosinophil infiltration functioning in the allergy reaction induced by parasite infection and the expression of immunoglobulins and cytokines.

Changing Patterns of Serum and Bile Antibodies in Re-infected Rats with Clonorchis sinensis

  • Rats develop strong resistance to re-infection and super-infection by Clonorchis sinensis. The present study investigated the antibodies present in the sera and bile juice of rats that were primary infected and re-infected with C. sinensis. The serum level of specific IgG antibodies, which were elevated 2 wk of the primary infection, peaked at 4 wk and subsequently remained unchanged even during re-infection. The total IgE level in serum increased slowly from 388 ng / ml to 3,426 ng / ml beginning 2 wk after the primary infection, and remained high up to 8 wk but dropped to a normal level (259 ng / ml) after treatment. In resistant re-infected rats, the serum IgE level increased rapidly and peaked within 1 wk, whereas no increase was observed in immunosuppressed rats. The serum level of specific IgA antibodies was elevated beginning 1 wk after infection, and decreased 4 wk after treatment. The total bile IgA level unchanged during the primary infection but increased in treated and re-infected rats. The elevated levels of serum IgE and bile IgA indicate that these immunoglobulins may be correlated with the development of resistance to re-infection by C. sinensis in rats.

  • 사구체에 면역글로불린은 음성이면서 C3 만 침착되는 경우, 다른 원인이 없는 경우 C3GN을 의심하여야 한다. 광학현미경으로는 세포의 증식이 있는 경우와 없는 경우가 있지만, 전자현미경으로는 EDD가 내피세포 밑으로 혹은 메산지움 등에 침착함을 확인하여야 한다. 대체보체경로의 조절이상이 원인임으로 혈청 C3 치를 비롯하여 CFH, CFI, CFB 등을 측정하여야 하며, C3 nephritic factor, anti-factor H 자가항체 등을 조사하는 것이 필요하다. 보체 조절이상을 일으키는 유전자 돌연변이를 조사해 보아야 한다. 예후는 안정적이지만, 치료는 대증적이다. 이론적으로 보체의 활성화를 저지하는 약으로 가능할 것으로 생각되며, C5 항체인 eculizumab으로 치유된 보고도 있다.

면역이적법에 의한 간흡충 항원분획과 감염자의 항체반응 양상 (Immunoblot patterns of clonorchiasis)

  • 우리 나라에서 가장 감염자가 많은 기생충인 간흡충의 현 감염에 특이한 항원 분획을 찾고자 면역이적법을 이용하여 감염자의 혈청을 검색하였다. 실험적으로 토끼에서 얻은 간흡충 성충의 조항원에 시스테인계 단백질분해효소 억제제인 I-64를 첨가하였을 때 큰 분자량의 분획을 가장 잘 보존하여 200-14 kDa의 범위에 20개 이상의 분획을 관찰하였다. 이 조항원을 이용하여 경북 상주군 주민의 혈청을 면역이적법으로 검사하였다 대변검사, 피내반응검사, 효소면역법검사(ELISA)를 이 용하여 검사한 73명 중 충란양성자 49명의 혈청 내에 특이 IgG와 IgE 항체가 생성되었고, IgM과 IgA 항체는 특이하게 반응하지 않았다. 여러 항원 분획 중에서 43, 34, 28-25 kDa 항원이 현 감 염에 특이한 분획이고. 항체와 반응한 94, 80, 72, 68, 64, 62, 52, 47, and 40 kDa 분획은 특이하지 않은 분획임을 확인하였다. 추후에 각 항원과 이에 대한 혈청반응에 대하여 특성을 더 연구 할 필요가 있으며, 특히 치료후 추적검사를 통하여 혈청내 항체의 소실을 구명하여야 할 것이다.

발육단계별로 정제한 폐흡충 시스테인계열 단백분해효소의 IgG 분해양상 (Activities of different cysteine proteases of Pcrogonimn westermani in cleaving human IgG)

  • 폐흡충 피낭유충 4주, 7주 및 16주 충체에서 분자량 15, 17, 27, 28 및 53 kDa인 시스테인 단백질 분해효소를 정제하고 각 효소가 IgG를 분해하는 양상을 비교하였고. 탈낭시킨 폐흡충 피낭 유충을 IgG 용액에 배양하여 유충이 분비하는 27 및 28 kDa 단백질분해효소가 IgG을 분해하는 양상을 관찰하였다 1시간 배양하였을 때 IgG heavy chain은 Fab로 분해되었으며 10시간 반응시킨 결과 전체 IgG 분자가 분해되었다. 피낭유충에서 정제한 27 및 28 kDa 효소, 4주, 7주(성장충) 충체 및 16주 성충에서 정제한 15, 17, 27, 28, 53 kDa 효소와 IgG를 반응시킨 결과 모 든 효소가 IgG를 hinge region에서 분해하였다. IgG 분해 양상은 피낭유충에서 가장 강하고, 성충으로 성장하면서 각 효소의 IgG 분해능이 점차 감소하는 양상을 보였다.

Studies on Vitamin Mineral Interactions in Relation to Passive Transfer of Immunoglobulins in Buffalo Calves

  • Status of blood minerals and their absorption by neonate calves as influenced by fat soluble vitamins supplementation in their respective mothers, mineral supplementation in calves themselves has been evaluated. The objective was to know the impact of antioxidant vitamin supplementation to advance pregnant buffaloes, on enhanced acquired immunity during first few hours after birth, in relation to weight gain in buffalo calves. Advance pregnant buffaloes (n = 30) consisting of average body weight of $550{\pm}15$ kg and of 4-6 parity were fed on 25 kg green (green Jawar-Sorghum bicolor), 2-3 kg wheat straw and 3-4 kg concentrate mixture individually per day. Intramuscular injections of vitamin triplex A $D_3$ E consisting of -2,500,000 IU of vit A -Palmitate; 2,500,000 IU of vitamin $D_3$ and 1,000 IU of vit E (dl-alpha tocopherol acetate) were given per dose, a month prior to parturition, twice at 15 days interval to 15 dams. Rest of the 15 pregnant buffaloes served as negative controls. Secretion of immune proteins, immunoglobulin (Ig) enhanced by 80% in colostrum. The blood serum levels of Zn, Cu, Ca, Mg were measured from birth to 90 days in calves. A significant (p<0.05) difference between the blood serum Zn levels of calves born to vitamin supplemented and non-supplemented dams was measured and a positive correlation between blood serum Zn levels and injections of vitamins was identified. Association of Zn and Cu with passive immunity status has been identified in these calves. A significant positive correlation between Zn and Cu was also identified which showed a change under the impact of vitamin supplementation in buffaloes. The study signifies the role of micronutrients supplementation in dams prior to parturition, in calf immunity development. The study indicates significant mineral - vitamins interactions during this process.

Association of Toll-like receptor 2-positive monocytes with coronary artery lesions and treatment nonresponse in Kawasaki disease

  • Purpose: Activation of Toll-like receptor 2 (TLR2) present on circulating monocytes in patients with Kawasaki disease (KD) can lead to the production of proinflammatory cytokines and interleukin-10 (IL-10). We aimed to determine the association of the frequency of circulating TLR2+/ CD14+ monocytes (FTLR2%) with the outcomes of KD, as well as to compare FTLR2% to the usefulness of sIL-10. Methods: The FTLR2% in patients with KD was measured by flow cytometry. Serum levels of IL-10 (sIL-10) were determined in 31 patients with KD before the initial treatment with intravenous immunoglobulin (IVIG) and in 21 febrile controls by using enzyme-linked immunosorbent assay. Patients were classified as having coronary artery lesions (CALs) based on the maximal internal diameters of the proximal right coronary artery and proximal left anterior descending coronary artery one month after the initial diagnosis. Results: We found that FTLR2% greater than 92.62% predicted CALs with 80% sensitivity and 68.4% specificity, whereas FTLR2% more than 94.61% predicted IVIG resistance with 66.7% sensitivity and 71.4% specificity. Moreover, sIL-10 more than 15.52 pg/mL predicted CALs and IVIG resistance with 40% and 66.7% sensitivity, respectively, and 73.7% and 76.2% specificity, respectively. Conclusion: We showed that measuring FTLR2% before the initial treatment could be useful in predicting CAL development with better sensitivity than sIL-10 and with results comparable to sIL-10 results for the prediction of IVIG resistance in patients with KD. However, further studies are necessary to validate FTLR2% as a marker of prognosis and severity of KD.

Characterization of Carp (Cyprinus carpio L.) Immunoglobulin Structure

  • Serum immunoglobulins (Igs) from Israeli carp were purified using affinity chromatography. Fish were immunized with purified mouse IgG, and the specific fish antibodies were purified from the immune serum on a mouse IgG-immobilized agarose gel. Rabbit anti-Israeli carp Igs (R $\alpha$ I. carp Igs) antibodies were produced following hyperimmunization with mouse IgG specific carp antibodies. SDS-PAGE analysis under reducing condition showed that Israeli carp Igs were composed of two $\mu$-like heavy chains with about 82 and 50 kd, respectively, and one light chain with about 25 kd. On immunoblotting analysis, however, R $\alpha$ I. carp Igs failed to react with the light chain. When both protein A and protein G-purified normal carp Ig were compared with mouse IgG-specific Israeli carp Ig, no significant structural differences among them were observed. To investigate if there is any homology between other fish Ig molecules, cross-reactivity of R $\alpha$ I. carp Igs against Ig molecules from 6 different fish sera and mouse control serum was checked on immunoblotting analysis. As a result, R $\alpha$ I. carp Igs responded to Israeli carp, common carp, and tilapia Ig molecules. In flow cytometry study, however, R $\alpha$ I. carp Igs appeared to recognize 42.0%, 35.8% and <5% of Israeli carp, common carp and tilapia $Ig^+$ head kidney cells, respectively. The result suggests the heterogeneity between receptor Igs on B-like lymphocytes and soluble Igs in serum. It is crucial to obtain pure fish Igs to produce reagent antibodies as tools for the study on their specific immune responses.

치근단 병소에서 면역글로불린의 분포에 관한 연구 (A QUANTITATIVE ANALYSIS OF THE IMMUNOGLOBULIN CONTAINING CELLS IN PERIAPICAL LESIONS OF THE HUMAN TEETH)

  • Periapical lesions develop as a result of immunopathologic response to irritants from infected root canal systems. Removal of these irritants from the root canal system and sealing the root canal space may induce he31ing of the periapical lesions. 83 periapical lesions diagnosed as periapical abscess, periapical granuloma, chronic nonspecific inflammation, fibrosis and periapical Cyst were evaluated for the distribution of immunoglobulin containing cells. The influence of the state of root canal treatment on the distribution of immunoglobulin containing cells has evaluated. All lesions were divided into a group with no treatment, a group with canal enlargement, a group filled with gutta percha, and a group filled with Vitapex(calcium hydroxide). The distribution of immunoglobulin-containing cells according to the presence of pain and fistula was also evaluated. The following results were obtained. 1. Statistically significant difference in the distribution of immunoglobulin-containing cells among periapical abscess, periapical granuloma, chronic nonspecific inflammation/fibrosis and periapical cyst were found.(Kruskal-Wallis analysis, P<0.05) The number of immunoglobulin-containing cells in fibrosis was remarkably lower than that of periapical abscess, granuloma and cyst. 2. IgM and IgA containing cells were predominantly observed in periapical abscesses and periapical cysts, respectively. 3. All periapical lesions showed a large number of IgG containing cells followed by IgM, IgA and IgE containing cells. 4. There was a decrease in all Ig-containing cells in the group with canal filling compared to groups without treatment or with enlargement. That is, there is a decrease in Ig-containing cells as treatment progresses. 5. No significant correlation existed between the presence of pain and fistula and the distribution of immunoglobulin containing cells in periapical lesions.(t-test) Results appear to support that immune response are actively involved in the development and progress in periapical lesions. The fact that distribution of immunoglobulins differ according to the state of endodontic treatment suggests that root canal treatment may alter the humoral immune response of the periapical lesions.
