Transmesenteric Hernia (경장간막 탈장)

  • Transmesenteric hernia, a type of internal hernias, is a rare cause of intestinal obstruction. This intraperitoneal hernia has no sac and is formed by protrusion of a loop of bowel through an aperture in the mesentery. Incarceration leads to intestinal obstruction and subsequently, strangulation and gangrene of varing lengths of intestine. This is a case report of 4-year-old girl with transmesenteric herniation of the terminal ileum through a defect in its own mesentery. Strangulation of the affected bowel necessitates resection and primary anastomosis with repair of mesenteric defect. The postoperative course was uneventful. Acute intestinal obstruction in the absence of an external hernia and with no history of a previous surgical procedure suggests the possibility of an internal hernia, especially if the patient has a history of chronic intermittent abdominal distress.

  • In order to investigate experimentally the clinical effects. of $Tae{\breve{u}}min$ Chungpaesagan-Tang (太陰人 淸肺瀉肝湯) that was prescribed to cure the $Kansuy{\breve{o}}lriy{\breve{o}}lby{\breve{o}}ng$ (肝受熱裏熱病) of $Tae{\breve{u}}min$, the author experimented various activities of mixed extract from $Tae{\breve{u}}min$ Chungpaesagan-Tang by the methods prescribed in the experimental part. The results of the studies were summarized as follows; 1. In the acetic acid method experiment and the pressing hind paw method experiment, analgesic effects were noted. 2. The prolongation of the duration of hypnosis induced by Pen to barbital-sodium was noted. 3. Spontaneous movement of the isolated mice ileum was markedly suppressed. and Contraction to induced by acetylcholine, Barium chloride was Inhibited. 4. Antipyretic action by Thyphoid vaccine were noted in mice. 5. Significant anti-edemic effects were recognized on the edema induced by yeast. 6. Hypotensive action due to vaso-dilatating of normal rats and rabbits were noted.

Presence of Acetylcholine-like Substance(s) in Sesamum indicum

  • Alcoholic extract of seeds of Sesamum indicum (SI, 1-30mg/kg) caused hypotensive action in anesthetized rats. Heart rate was also decreased at slightly higher doses (10-30 mg/kg). Pretreatment with atropine (2 mg/kg) abolished these cardiovascular responses. In isolated spontaneously beating atria from guinea-pigs. SI caused decrease in force and rate of atrial contractions. In isolated guinea-pig ileum and rat uterus, SI (100-1000 ug/ml) produced contractile responses. All these actions of SI were abolished in the presence of atropine ($1\mu$M). These results indicate that alcoholic extract of seeds of Seamum indicum contains acetylcholine-like constituent(s) which explains some of the folkloric uses of plant.

  • Scolopendra corpus has been used as anticonvulsants and antispasmodics in the oriental medicine. It was previously shown that water extract of Scolopendra corpus had an inhibitory action on ileum in mouse. To investigate the anticonvulsant and antispasmodic activity of Scolopendra corpus, pharmacological studies have been carried out with the water extract of Scolopendra subspinipes mutilans $L.\;K_{OCH}$, which is widely distributed in Korea. 1. The extract depressed convulsions induced by picrotoxin and strychnine. 2. Sedative and antipyretic analgesic action were observed. 3. In mouse and rabbit, tone of intestinal smooth muscle was suppressed with the treatment of the extract and intestinal contraction induced by $BaCl_2$ was also inhibited, suggesting that the extract has a papaverine-like effect. Whereas, in guinea pig, intestinal and tracheal smooth muscle were stimulated, and the effect was antagonized by pre-and after-treatment of diphenhydramine, suggesting that the extract has a histamine-like effect. 4.Flow rate was increased when hind-limb of Toad was perfused with saline containing the extract, but returned to normal within 10 min. Hypotensive effect was observed in rabbit and the effect was abolished by vagotomy.

  • Experimental studies were implemented to evaluate clinical efficacy of 'Daeshiho-Tang' and to validate Oriental medicine description of the recipe which has been widely used in metabolic diseases and febrile diseases. Following results were obtained; 1. Suppressive action was shown on convulsion due to cerebrocortical causes but no such action was noted either myelic or diencephatic causes in mice. 2. Analgesic and antipyretic actions were recognized in mice, rats and rabbits. 3. Antispasmodic actions were recognized on contraction of the ileum induced by $BaCl_2$ and histamine in mice and guinea-pigs. 4. Transport accelerating action of the small intestine was gained in mice.

  • PEG-hemoglobin SB1 (SB1), which is a hemoglobin-based oxygen carrier, is intended to use as a safe blood substitute against brain ischemia and stroke. The general pharmacological profiles of SB1 were studied. The doses given were 0, 5, 10, 20 ml/kg and drugs were administered intravenously. The animals used for this study were mouse, rat and guinea pig. SB1 showed no effects on general behavior, motor coordination, spontaneous locomotor activity, hexobarbital sleeping time, anticonvulsant activity, analgesic activity, blood pressure and heart rate, left ventricular peak systolic pressure, left ventricular end diastolic pressure, left ventricular developing pressure, double product, heart rate, coronary flow rate, smooth muscle contraction using guinea pig ileum, gastrointestinal transport, gastric secretion, urinary volume and electrolyte excretion at all doses tested except the decrease of body temperature. These findings demonstrated that SB1 possesses no general pharmacological effects at all doses tested.

Synchronous ileal inflammatory fibroid polyp and Meckel's diverticulum found during laparoscopic surgery for adult intussusception

  • We present a rare case of synchronous ileal inflammatory fibroid polyp and Meckel's diverticulum detected during laparoscopic surgery for adult intussusception. A 48-year-old woman presented with sudden onset of severe abdominal pain. Abdominal computed tomography revealed a segment of ileocecal intussusception. Thus, laparoscopic exploration was performed, which revealed an ileal mass with an outpouching closed luminal structure in the distal ileum. Two abnormal structures were resected via mini-laparotomy, and the patient was discharged without postoperative complications. Histopathological examination confirmed an ileal inflammatory fibroid polyp and Meckel's diverticulum with ectopic pancreatic tissue.

  • In order to investigate the effects of Kamikwakjeongtang by using the experimental animals. the action on gastrointestinal smooth muscles. the action of gastric juice secretion, the action of antiulcer. the transport ability of intestinal contents. the action of anticatharsis and the actions on the central nervous system were studied. The results were as follow: 1. Spontaneous motility of the isolated ileum of mice was suppressed and antiacetylcho-line chloride action was recognized. 2. Anti-action on barium chloride of fundus-strip of white rat was recognized. 3. Supression effects on gastric juice secretion. free acidity was recognized. 4. Preventive effect on the ulcer induced by pylorus-ligated was recognized. 5. Supression effects of large intestinal transport ability was recognized. 6. Anti-carthartic action was shown but was not recognized. 7. Analgesic effect by the acetic-acid method and prolonged effect of the total sleep time by pentobarbital-Na were recognized. According to the above results, effects based on oriental medical reference were consistent with the actual experimental effects.

  • A ginseng preparation (GP) consisting of ginseng ex., lycium fructus ex., four kinds of vitamines and caffein was evaluated for acute toxicity and general pharmacology. Average lethal doses $(LD_{50})$ of GP in male mice were 2,988mg/kg (i.p.) and more than 3g/kg (p.o.). In dosage of 300 and 900mg/kg (p.o.) showed no analgesic activity in both tests of the writhing method induced by acetic acid and of tail pressure method and no effect on the pentetrazole-induced convulsion. However, it appeared to have a hypothermic action only in dose of 900mg/kg. The duration of hypnosis induced by hexobarbital sodium in mice was shortened by GP, being probably due to caffein. GP Produced no marked contraction of isolated ileum and uterus in high concentrations of up to $1{\pm}10^{-3}g/ml$. These results suggested that GP did not show any considerable central nervous depressant activity and exhibited very weak actue toxicity in mice.

Acute Oral Toxicity Test in Japanese Quail (일본 메추리를 이용한 급성 경구독성시험법의 확립)

  • The acute oral LD5O toxicity values of isazofos, pyraclofos, diazinon and methomyl were determined for Japanese quail based on OECD guideline. The $LD_{50}$ of isazofos, pyraclofos and diazinon was 16.26 mg/kg, and 7.11mg/kg body weight In female respectively. And the $LD_{50}$ of each chemical in male was 21.44, 35.64, 8.28 mg/kg body weight respectively. Diazinon was the most susceptible compounds to Japanese quail in both sexes. The $LD_{50}$ of methomyl was 21.24 mg/kg body weights in female, and 28.28 mg/kg body weight in male respectively. Diazinon, isazofos and methomyl were more toxic In the female than male. The symptoms of poisoning were similar in quails administrated with each chemicals. The clinical sign in Japanese quail were ataxia, salivation, diarrhea, ruffled feather and convulsion at dead point. There were severe hemorrhage and catarrhal inflammation from duodenum to ileum In all compounds. In Japanese quail treated with organophosphorus and carbamate compounds, brain acetylcholinesterase was inhibited by 88-96. The recovery was not observed after 5 h in sublethal dose.
