The Effects of Short Term Schroth Exercise on the Cobb Angle, Angle of Trunk Rotation, Cosmetic Appearance, and Quality of Life in Idiopathic Scoliosis (단기간 슈로스(Schroth) 운동이 특발성 척추측만증 환자의 척추측만각도(Cobb Angle), 몸통회전도, 외형 그리고 삶의 질에 미치는 영향)

  • PURPOSE: The purpose of this study was to describe the effects of short term Schroth exercise on the Cobb angle, angle of trunk rotation (ATR), cosmetic appearance, and quality of life (QOL) in idiopathic scoliosis patients. METHODS: Five subjects with idiopathic scoliosis, (female, 3; male, 2) curvature type: thoracic, 2; lumbar, 3 underwent short term Schroth exercise for seven days. The exercise was performed for 3 h long sessions per day. The Cobb angle and QOL were measured before and after the intervention. ATR and cosmetic appearance were measured once. Sessions consisted of one baseline, seven intervention and three follow-up phases. The sessions were conducted with a one day interval. RESULTS: After the intervention, the Cobb angle was found to be significantly improve (p <.05), while the QOL did not differ (p>.05). The ATR showed decreased trends in the intervention phase. Data points during the intervention and follow-up phases showed a decrease in comparison with data points at the baseline, indicating that Schroth exercise might be effective in reducing the ATR and that these effects can be maintained after the intervention. Cosmetic appearance did not showed changed trends during the Schroth exercise intervention phase. CONCLUSION: Our results suggest that short term Schroth exercise may be valuable in improving the Cobb angle and ATR in patients with idiopathic scoliosis.

Predictive Clinical Factors for the Treatment Response and Relapse Rate in Childhood Idiopathic Nephrotic Syndrome (소아 일차성 신증후군의 치료반응과 재발빈도에 관련된 인자)

  • Purpose : This study was aimed to determine the predictive risk factors for the treatment response and relapse rate in children diagnosed with idiopathic nephrotic syndrome. Methods : We analyzed the medical records of children who were diagnosed and treated for childhood idiopathic nephrotic syndrome from November 1991 to May 2005. Variables selected in this study were age at onset, sex, laboratory data, concomitant bacterial infections, days to remission, and interval to first relapse. Results : There were 46 males and 11 females, giving a male:female ratio of 4.2:1. The age($mean{\pm}SD$) of patients was $5.8{\pm}4.1$ years old. Of all patients who were initially given corticosteroids, complete remission(CR) was observed in 54(94.7%). Of the 54 patients who showed CR with initial treatment, 40(70.2%) showed CR within 2 weeks and 14(24.6%) showed CR after 2 weeks. The levels of serum IgG were lower in the latter group who showed CR after 2 weeks(P=0.036). Of the 54 patients who showed CR with initial treatment, 47(82.5%) relapsed. Of these patients, 35.1% were frequent relapsers and 43.9% were infrequent relapsers. There was no significant correlation between the frequency of relapse and the following variables : sex, days to remission, and laboratory data. However, age at onset and interval to first relapse had a negative correlation with the frequency of relapse(Pearson's coefficient=-0.337, -0.433, P<0.012, P<0.01). Conclusion : The age at onset and the interval to first relapse were found to be predictive clinical parameters for the relapse rate, while the levels of serum IgG at initial presentation were a predictive laboratory factor for treatment response in childhood idiopathic nephrotic syndrome.

  • Idiopathic renal hypouricemia is a disorder characterized by impaired urate handling in the renal tubules. This disease usually produces no symptoms, but hematuria, uric acid nephrolithiasis or acute renal failure may develop. A defect in the SLC22A12 gene, which encodes the human urate transporter, is the known major cause of this disorder. We describe a 10-month-old boy with idiopathic renal hypouricemia. He was diagnosed with transient pseudohypoaldosteronism at admission, but hypouricemia was accidentally found through follow-up study. By gene analysis, his diagnosis was confirmed to idiopathic renal hypouricemia. In addition, we report a mutation in the human urate transporter 1 (hURAT1) gene identified in his family.

A Clinical Study on Adolescent Idiopathic Scoliosis using DITI (DITI를 활용한 청소년기 특발성 척추측만증에 대한 임상적 고찰)

  • Objectives : This study was performed to investigate the correlation between Cobb's angle and digital infrared thermographic imaging(DITI) on adolescent idiopathic scoliosis. Methods : A total of 56 patients with adolescent idiopathic scoliosis were classified into four groups according to their Cobb's angle. Four groups were made for each increment of $5^{\circ}$: group $5-10^{\circ}$, group $11-15^{\circ}$, group $16-20^{\circ}$, and group $21-25^{\circ}$. Next we checked thermal differences of 6 acupoints(GB21, BL13, BL15, BL20, BL23, BL26) on DITI and analyzed correlation by SPSS ver. 20.0.0 for windows. Results : 1. The average patient age was approximately 13 years old. 2. The majority of patients complained of pain on both sides. 3. Significant correlation was found between group $11-15^{\circ}$ and group $16-20^{\circ}$, group $16-20^{\circ}$ and group $21-25^{\circ}$ on Simsu(BL15) by Duncan test. Conclusion : Using DITI, Simsu(BL15) is expected to be a valid indicator for the diagnosis and treatment of adolescent idiopathic scoliosis.

A Case Report of Polyarticular Type Juvenile Idiopathic Arthritis(JIA) Patient Ineffective Response to DMARDs and NSAIDs (항류마티스제(DMARDs)와 비스테로이드성 소염진통제(NSAIDs)에 반응하지 않는 다수관절형 소아기 류마티스 관절염 환아 증례 보고)

  • Objectives This study is to examine case that has an significant result on treating Polyarticular Type Juvenile Idiopathic Arthritis(JIA) the patient who did not response to DMARDs and NSAIDs with oriental medicine. JIA is the most common form of persistent arthritis in children before age 16. The symptoms of JIA is pain and edema of joints, fever, lethargy, reduced physical activity, and poor appetite. But the symptoms vary. Methods The patient felt pain and had limitation of mobility on his joints, but the DMARDs and NSAIDs were not effective. Therefore, he was treated with oriental medicine and withdrew from DMARDs and NSAIDs. The patient took DMARDs and NSAIDs from May 2007 to November 2008(He did not took DMARDs and NSAIDs from December 2007 to June 2008), and took oriental medicine from November 2008 until now. Results The patient's pain and limited mobility on his joints, edema, morning stiffness, fatigue was gone, and could maintain the condition. He doesn't have DMARDs or any other western medicine anymore. Conclusions This study shows that a Polyarticular Type Juvenile Idiopathic Arthritis patient who had ineffective response to DMARDs and NSAIDs can reach to the clinical remission by oriental medicine treatment. His morning stiffness, fatigue, pain of joints, edema was gone and ESR was in normal range with Oriental medicine treatment.

  • The most common causes of severe sleepiness beginning or progressively worsening in adults are SLEEP APNEA and RELATED BREATHING DISORDERS DURING SLEEP. Idiopathic hypersomnia is excessive sleeping without obvious cause. Idiopathic hypersomnia diagnosis can be explained as follows: One could be chronically sleepy due to either something wrong with sleep that makes it non-refreshing, or a problem with the brain mechanisms which normally should keep one alert whether caused by primary problems within the brain or its chemistry, or by other factors (such as sedating medications or thyroid problems). A 71-year-old male who had suffered from excessive sleeping was admitted to our department for oriental treatment on 7th of July, 2003. He was diagnosed as an idiopathic hypersomnia for excessive sleeping without obvious cause. Initial treatment modalities with administration of "SoonHwanGi1HoBang(循環器1號方)" were not effective. However, after administration of "YukMiGiHwangTang(六味地黃湯)" desirable effects were seen.

A Review of the Domestic Trends of Korean Traditional Medicine for Idiopathic Scoliosis (특발성 척추측만증의 한의학적 치료에 대한 국내 연구 동향: 국내 학술지를 중심으로)

  • Objectives This study aimed to analyze the trends of Korean traditional medicine treatment for idiopathic scoliosis. Methods We searched clinical studies used Korean traditional medicine for idiopathic scoliosis in 4 Korean online databases 'Koreanstudies Information Service System', 'Earticle', 'Oriental Medicine Advanced Searching Integrated System', 'Research Information Sharing Service'. We selected 18 studies and analyzed their characteristics according to author, publication year, number/sex/age of patient, treatment period, intervention, outcome measure and result. Results Eighteen case report studies were selected by criteria and randomized controlled trials were not found. The most frequently used therapy was Chuna therapy. Cobb's angle was most commonly used outcome measure. Conclusions In this study, we reviewed studies used Korean traditional medicine treatments for idiopathic scoliosis. However, evidence is limited because all selected studies were case reports. This study suggests that further clinical studies are needed to provide higher evidence about Korean traditional medicine treatment for idiopathic scoliosis.

  • Transient peripheral eosinophilia occurs in several disorders, such as allergic diseases, cancer, and parasitic infections. However, in most cases, their presence is not accompanied by tissue destruction or organ dysfunction. In certain disease states, eosinophils can accumulate in any organ in the body and cause tissue destruction as a result of the eosinophil infiltration or the toxic effects of the degranulated proinflammatory products. Idiopathic hypereosinophilic syndrome is a rare disorder characterized by persistent eosinophilia of an unknown origin, usually associated with a dysfunction of organs such as the heart, lung, skin, and nervous system. Idiopathic hypereosinophilic syndrome usually has an indolent course over a period of several months. However, in some cases, they have grave symptoms if vital organs such as heart and lung are infiltrated. Here we report a case of idiopathic hypereosinophilic syndrome presenting acute pulmonary edema involving the heart, bone marrow, and lung with a review of the relevant literatures.

Effects of Korean Medicine on Patients with Idiopathic Parkinson’s Disease: A Retrospective Study (특발성 파킨슨병 환자에 대한 한의치료의 효과 : 후향적 연구)

  • Objectives: The purpose of this study was to investigate the effects of Korean medicine on patients with idiopathic Parkinson’s disease.Methods: The charts of 47 patients diagnosed with idiopathic Parkinson’s disease in the Department of Internal Korean medicine, Stroke and Neurological Disorders Center, Kyung Hee University Hospital, Gangdong between August 2012 and July 2016 were reviewed. The Unified Parkinson’s disease rating scale (UPDRS) was administered before and after treatment with Korean medicine.Results: Thee average UPDRS Ⅱ+Ⅲ, UPDRS Ⅱ, and UPDRS Ⅲ of the 47 patients decreased significantly from 22.26±15.15, 10.19±7.53, and 12.06±8.35, respectively, pretreatment to 16.96±13.63, 7.47±6.44, and 9.49±7.73, respectively, post-treatment. The average postural instability and gait difficulty (PIGD), tremor, and bradykinesia also significantly improved post-treatment.Conclusions: These results provide evidence that Korean medicine can improve the activities of daily living and motor function of patients with idiopathic Parkinson’s disease. In particular, Korean medicine may be effective for the treatment of PIGD, tremor, and bradykinesia.

The impact of two doses of coenzyme Q10 on semen parameters and antioxidant status in men with idiopathic oligoasthenoteratozoospermia

  • Objective: Oxidative stress contributes to male infertility, and antioxidants have been recommended for treating idiopathic oligoasthenoteratozoospermia (OAT). There is, however, a lack of agreement on the type, dosing, and use of individual antioxidants or combinations thereof. The purpose of this study was to compare the effects of two doses of coenzyme Q10 (CoQ10) on semen parameters and antioxidant status in men with idiopathic OAT. Methods: In this prospective study, patients with idiopathic OAT received 200 mg/day (n = 35) or 400 mg/day (n = 30) of CoQ10 orally for 3 months. All patients underwent semen analysis according to the fifth editions of the World Health Organization criteria. Total antioxidant capacity (TAC), catalase (CAT) activity, and superoxide dismutase (SOD) activity were measured both before and after treatment. Results: Treatment with CoQ10 (200 mg/day or 400 mg/day) resulted in a significant increase in sperm concentration from baseline ($8.22{\pm}6.88$ to $12.53{\pm}8.11million/mL$, p= 0.019; $7.58{\pm}5.41$ to $12.33{\pm}6.1million/mL$, p= 0.002, respectively), progressive motility ($16.54%{\pm}9.26%$ to $22.58%{\pm}10.15%$, p=0.011; $14.22%{\pm}12.85%$ to $26.1%{\pm}14.52%$, p= 0.001, respectively), and total motility ($25.68%{\pm}6.41%$ to $29.96%{\pm}8.09%$, p= 0.016; $23.46%{\pm}12.59%$ to $34.82%{\pm}14.17%$, p= 0.001, respectively). CoQ10 therapy also increased TAC (p= 0.009, p= 0.001, respectively), SOD activity (p= 0.004, p= 0.001, respectively), and CAT activity (p= 0.039, p= 0.024, respectively). Furthermore, antioxidant measures correlated significantly with seminal fluid parameters (r = 0.36-0.76). Conclusion: CoQ10 supplementation improved semen parameters and antioxidant status in men with idiopathic OAT, with a greater improvement shown in men who took 400 mg/day than in those who took 200 mg/day.
