Study on Depressive Syndrome in Hyungsang Medicine (울증의 형상의학적 고찰)

  • From a view point of Hyungsang medicine a study is made on depressive syndromes through 'Donguibogam(東醫寶鑑)' ,'Clinical lectures by Mr. Jeesan(芝山先生 臨床學特講)' and other literature. Depressed energy(?) originally meant the disorder of vital energy in the circulation of nature with the lapse of time. In later, it has been changed to mean the disease of man, especially related to the mind. Depressive syndromes come from the stagnation of the Ki or the abnormal circulation(rising, falling, coming and going), which happens when man cannot adjust oneself to the circumstances due to the disharmony between internal and external conditions. Depressive syndrome easily attacks the following types of persons; Dam type person with excessive Ki and deficient Hyung(形), manly woman, womanly man, Bird type person who shows a great variety of emotion, Turtle type person declined to melancholy, Ki type person apt to be depressive, and Shin type person with upward gush of the fire. It also attacks the persons who have smudgy face with the scar between eyebrows, those whose pulses are mixture of two different kinds and those whose pulses extend on two ranges in Jeesan's diagram. Depressive syndromes recorded in 'Donguibogam' are globus hystericus, chest congestion, gastric discomfort, cough and asthma caused by depressed Ki, abdominal mass, edema, and tympanites. The most effective prescriptions for depressive syndromes are Eajintang(二陳湯), Guibitang(歸脾湯), Hyangsosan(香蘇散), and Gamisachiltang(加味四七湯).

  • This thesis makes a review of symptoms and cause of the numbness in the literature. clinical case are studied and classified according to the Hyungsang of the patients. The followings are the conclusions: In case of Dam type persons, numbness is mainly caused by Fire with blood deficiency and so cured by tonifying the Jung and blood. In case of Bang Kwang type persons. it is generally caused by the deficiency of Ki and damp-phlegm and treated by reinforcing Ki and removing dampness. Persons of Jung and Hyal types are apt to be afflicted with numbness of deficiency symptoms which can be cured by tonifying the Jung and blood. Those of Ki and Shin types are inclined to get numb from the congestion and stagnancy and so treated by promotion the flow of Ki, dispelling stagnancy, resolving phlegm and promoting digestion. Numbness is common to the old people and women. The deficiency of blood and primordial energy is a cause of old people's numbness. Women get numb due to the pent-up feelings and emotional disturbance. The function disharmony of five Jang and six Bu makes the body lose the control of heat and cold or deficiency and excessiveness, which can be a cause of numbness. Especially the deficiency of the spleen results in numbness. When the special parts of the body become numb treatment must be decided after due consideration on which of the Jang and Bu or meridians is related to that affected area. In case of person with six meridian types. unbalance of Ki and blood leads the six atmospheric influences(wind, cold, heat, dampness, dryness and fire) get into the body through the most developed meridian and brings in numbness.

Examination of the Applications of Semen Persicae and Flos Carthami in the herbal Perscriptions Appeared in the book of ${\ulcorner}$Donguibogam${\lrcorner}$ according to Hyungsang Medicine (("동의보감"에서 도인 홍화 (桃仁 紅花)를 사용한 처방에 대한 형상의학적 고찰)

  • Following conclusions were drawn from the examination of the usage of semen persicae and fols carthami from the view of Hyungsang Medicine. Semen persicae is used to vitalize the blood flow and eliminate the blood stagnation of the whole body in ${\ulcorner}$Donguibogam${\lrcorner}$, and therefor it is used for the blood flow of the heptal meridian and sometimes the intestinal meridian. Flos carthami is applied to vitalize the blood flow, remove obstructions in meridians, eliminate the blood stagnations, and stop the pain in ${\ulcorner}$Donguibogam${\lrcorner}$, therefore used in such symptoms of blood flow of the heptal meridian as amenorrhea, spasm of muscles, and coldness of the genitals. Also is applied in those cases of exhaustion, perspiration, and in digestive remedies which belong to the symptoms of cardiac blood flow. And it is used for dryness of the skin in which case the blood should be developed and malaria for the elimination of the blood stagnation and production of new blood. The combination of semen persicae and flos carthami is applied to eliminate the blood stagnation and stimulate menstrual discharge in uterus, as well as in the cases of lumbago and stomachache due to blood stagnation, blood stasis, mass in the abdomen, and abdominal distention. This combination is also used for the eyes, ears, numbness, and paralysis of hands and feet for the purpose of making the flow of the meridian smooth, and for diabetes, dryness of the skin, malaria, anal itching and pain for the purpose of eliminating the mass and renewing the tissues. The combination is diversly used in the book of ${\ulcorner}$Special lectures on clinical cases${\lrcorner}$ for the aftereffects of traffic accident, constipation, diabetes, dryness of the skin, paralysis of hands and feet, numbness of finger of hand and foot, and especially used frequently with prescription of Yijintang and Samultang combined in the cases of numbness. The prescription is used in Hyungsang medicine when the uterus, the external appearance(axis), hands and feet, heart, and the stomach is not good for the vitalization of the flow of the 12 meridians. It was though difficult to find extinguishing differences between the two herbs in the clinical cases applied in the book of ${\ulcorner}$Special lectures on clinical cases${\lrcorner}$.

A Study on the Characteristics of Facial Shape in Adult Women by Sasang Constitution Using Hyungsang Classification (형상분류를 이용한 성인여성의 체질별 안면형태 특징에 관한 연구)

  • Objectives This study was aimed to analyze characteristics of facial shapes in adult women by sasang constitution using hyungsang classification. Methods Using a digital camera, we took a picture of 1,011 women who participated in clinical study on menstrual pain and acquired their 3D facial images with a face-only scanner. They filled out SSCQ-P(sasang constitution questionnaire for patient) for the diagnosis of sasang constitution. Based on the above photographs and 3D images, one of the hyungsang medicine specialist diagnosed according to five diagnostic criteria. The sasang constitution was diagnosed by referring to questionnaires and photographs. Frequency analysis was performed using the statistical analysis system version 9.4 and chi-square test was performed for validity evaluation. Results In taeeumin, the wide face shape(n=261, 74.36%) was much more than the narrow shape(n=90, 25.64%) and the convex face profile(n=164, 85.86%) was much more than the concave profile(n=27, 14.14%). Regardless of sasang constitution, angular face shape(n=501, 50%) was the most, followed by oval shape(n=317, 31.64%). Subjects with big ears(n=291, 29.19%) were the most, while big eyes(n=104, 10.43%) were the least. Subjects with eyes and nose tip upward(n=615, 78.05%) were the most, while eyes and nose tip downward(n=22, 2.79%) were the least. Conclusions Most Korean adult women have angular face, such as square or diamond, with slanted eyes and upturned nose. Taeeumin women have wide facial shape and convex profile.

Treatment of Skin Diseases in Hyungsang Medicine (피부병의 형상의학적 치료)

  • By treating skin diseases with Hyungsang medicine and reviewing the clinical cases of them through classifying skin diseases into general care and into Hyungsang medical care, the writer came to the conclusion as followings. The skin is a house of Kyungnak, and it' s a place which Ki comes in and out, which human body controls conditions of his physiological function and which doctors can make a diagnosis from seeing it. The skin is subject to lung and large intestine in the point of organ, but in the point of movement, to spleen and stomach. Skin diseases come from unbalance of Ki and Hyul, mostly from fever in blood and also from Dam, Wind, Dampness and Dryness because of including fire. Generally skin diseases are treated according to classifying into distinctive symptoms, region, cause or time being occurred, and concomitant symptoms. After being divided into man and woman, age and youth, fatness and leanness, Jung Ki Shin Hyul type, visceral figures, Yukkyung shapes on Hyungsang medicine method, the remedy of skin diseases which is combined common meditation with Hyungsang medical method is efficacious. In distinguishing into man and woman, age and youth, man gets sick mainly by drinking liquor and exhausting Jung, woman is taken ill generally by indigestion and Damwha, especially by drinking liquor. Young child gets ill mostly from inherent weakness, but in many cases he is occurred by disturbed digestion, so he must be taken care of ingesting food. The aged becomes ill from weakening, so he must be treated according to reasons of diseases. To fatness and leanness, skin diseases are showed symptoms by weakened Yang, insufficient Yin or excess fever on the theory of upper and lower parts, by unbalance of Ki and Hyul on the theory of left and right, by Dryness and Dampness on the theory of front and rear and by insufficiency of vitality and combined fever on the theory of inside and outside. Therefore doctors must cure the symptoms according to the theory of upper and lower, left and right, front and rear and inside and outside. In type of Jung Ki Shin Hyul, the balances between Jung-Hyul and Shin-Ki are important points. Concretely Jung type comes to easily shortage of Jung or gathering abnormally Dampness and Dam, Ki type is easy to make a mass with Ki or to be deficient of Ki, Shin type comes to press seven emotions togather or to be insufficient of Yin and fever inside, and Hyul type is inclined to be deficient of Hyul and to become feverish or to be deficient of Ki. Therefore on the ground of the above reasons skin diseases come into existence. Inclusively in Jung-Hyul type , Ki must be moved on the base of full Jung-Hyul, so Wukmijiwhangtang or Samultang is prescribed basically. In Shin-Ki type , Jung-Hyul is saved on the base of moving Ki, so Hyangsosan or Yijintang is the proper prescription. Considering the visceral figures, doctors judge for diagnosis and remedy by putting together the elements, that is, great and small sizes of a set of features (eye, ear, mouth, nose), color and temper. The yukkyung types are classified according to many or few of Ki-Hyul, and then skin diseases appear by being inclined toward one side between Ki and hyul, or among the six atmospheric elements(wind, cold, heat, dampness, dryness, and fire). Especially because Yangmyung type is full of much Ki and Hyul, and also has much fever in stomach, he gets skin symptoms frequently. So his prescription is based on taking off fever in stomach, and also he must be careful about regimen certainly.

Apoplexy and Hyungsang Medicine (중풍의 형상의학적 고찰)

  • The followings are the conclusions drawn from the clinical cases of apoplexy; The main cause of apoplexy is the deficiency of both Jung and Ki. dam typed persons are stricken with paralyses because of the insufficiency of the kidney water. On the other hand, bangkwang typed persons' apoplexy comes from the deficiency of Ki and dump-phlegm. The prevention of apoplexy is very important so that porpe medical care should be taken at the appearance of premonitory symptoms like vertigo, dim sight tinnitus, stiff neck, numbness and others. It appears very reasonable both clinically and pathologically that Li dongyuen classified the apoplexy in to three groups : the first group is apoplexy involving meridians ; the second. involving Bu ; the third, involving Jang. The accurate diagnosis of apoplexy regulates com prehension consideration of four factors configuration color, pulse and symptoms and distinction from the similar diseases. Apoplexy is the up wand floating of Yaug in deficiency due to the deficiency of genuine Yim. In its early stages it should be treated by eliminating the excess in the upper pant. In the lower pant becomes the fundamental treatment.

A View on Hyung-Ki in Donguibogam (『동의보감(東醫寶鑑)』의 '형기론(形氣論)'에 대한 소고)

  • The narration of "Donguibogam" is focused on humans whilst medical books before it focused on diseases. This is shown from the first subtitle of External Body(身形門) chapter, [The Origin of Hyung-Ki(形氣之始)]. The contents of [The Origin of Hyung-Ki] explains that the human body is composed of 'Hyung(形)' and 'Ki(氣)' This perspective is shown throughout "Donguibogam". First, in the aspect of construction, the viewpoint is shown from the JipRye(集例). It divides the body into inside and outside, which is a result of Hyung-Ki perspective. This continues in the table of contents. Naegyeongpyeon(內景) and Oehyeongpyeon(外形) describes the inside and outside of the body, in other words 'Hyung-Ki'. Japbyeongpyeon(雜病) describes complex illnesses with mixed insides and outsides. Tangaekpyeon(湯液) and Chimgupyeon(鍼灸) can be understood as division of treatment methods into inside and outside. When we look at the contents, the human body is created on the basis of the essence and vital energy of the world. Cheon-Ki becomes the 'Ki' that forms the functions of the body, while Ji-Ki becomes the 'Hyung' that constructs the bodily structure. It is considered that 'Hyung' is composed of SaDae(the body essence, vital energy, mentality and blood), and 'Ki' is composed of OSang(the five Jang organs and six Bu organs). 'Hyung' and 'Ki' show various appearances according to physiology and pathology. 'Hyung' is especially shown by obesity or thinness, 'Ki' by color. The obese may have Ki deficiency, coldness, dampness or phlegm. The thin could have blood deficiency, heat, fire or dryness. The color could show the pathology of the five Jang organs by the five colors, but it can simply be divided into black and white, where black means Eum deficiency, and white means Yang deficiency. It is said that the distinctive feature of traditional Korean medicine is that it is a constitutional medicine. In this perspective, further study on 'Hyung-Ki' is of value. If "Donguibogam" was the foundation of the Sasang Constitutional Medicine(四象醫學), study on 'Hyung-Ki' means searching for the origin of the Sasang Constitutional Medicine. Also, the study on Sasang Constitutional Medicine and Hyungsang medicine(形象醫學) will show the modern image of "Donguibogam". Hence, interchange study between "Donguibogam", Sasang Constitutional Medicine and Hyungsang medicine through the term 'Hyung-Ki' could hopefully lead to academic progression.

Comparative Study on Frequent Disease Patterns and Prescriptions by Three Societies of Korean Medicine for Gastrointestinal Disease (소화기 질환을 대상으로 한 3개 한의학파의 다빈도 병증 및 처방 비교 연구)

  • Objectives The aim of this study is to analyze the therapeutic direction and characteristics of gastrointestinal diseases by three societies of Korean medicine by comparing the frequent disease patterns and prescriptions of them. Methods The experts of the three societies of Sasang constitutional medicine, Hyungsang medicine and Sanghan-GeumGwe medicine discussed and confirmed a list of clinical data to make use for diagnosis. Then, clinical data on 200 patients suffering from gastrointestinal symptoms were collected. The each experts diagnosed appropriate disease patterns and decided prescriptions according to the method of their affiliated society. Results and Conclusions The three different societies' experts showed the similarity in the sense of categorizing disease patterns focusing on the specific physiology and pathology of the patients, while the each societies have different diagnosis systems. Treating chronic gastrointestinal patients, they treated the mental or psychological symptoms of the patients as well as their physical problems. Based on this study, it is necessary to develop a systematic and precise research model to objectively and statistically analyze the correlation of disease patterns and prescriptions by different societies.

Study of Hands and Feet Hyperhidrosis according to Hyungsang Remedy (수족다한증(手足多汗症)의 형상의학적 고찰)

  • We have come to a conclusion below after studying about the cause of limb hyperhidrosis through references, putting the result into practice clinically from Hyungsang remedy point of view, examining the effect of treatment. The cause of limb, palm and sole hyperhidrosis. The cause of limb hyperhidrosis is stomach heat(Yangmyung heat), weakness and coldness of stomach, moisture and heat of spleen and stomach, energy insufficiency of spleen and stomach, weakness of Yin of spleen and stomach, heat by weakness Yin meridian, unspread energy of liver, severe heat of heart, weakness of heart, shortage of blood, the functional disorder of kidney meridian and heart meridian(disposition of water ascension and fire descent), heat of blood(the functional disorder of heart) etc. The cause of palm hyperhidrosis is blood insufficiency of heart and liver or the functional disease of heart and stomach. The cause of sole hyperhidrosis is the functional disease of heart and kidney or the functional disease of kidney. Types classified by the cause of limb hyperhidrosis. The case of stomach heat, moisture and heat of spleen and stomach is found a lot in Yangmyeong and Fish Type. The case of weakness and coldness of stomach or insufficiency energy of spleen and stomach is found in Bangkwang and Ki Type, the shape of disorder of transfer and transformation of spleen and stomach. The case of severe heat of heart, the functional disorder of kidney meridian and heart meridian(disposition of water ascension and fire descent), heat of blood(the functional disorder of heart), heat by weakness of meridian of Yin is found a lot in Dam, Bird and Shin Type. Limb sweat caused by unspread energy of liver is found a lot in Horse Type. Palm sweat is found a lot in Bird or Yangmyeong Type. Sole sweat is found a lot in Fish or Taeum Type. Jeokbokryung-tang was good especially for type limb hyperhidrosis falling under the shape of the small intestines heat which conforms to the person having a short perpendicular furrow of the upper lip or lifted upper lip. Palmul-tang was good for type limb hyperhidrosis tending to having leaking liquid and humor because of the disorder of transfer of Jung, Ki, Shin, Hyul. Dossipyungwi-san and Hyangsapyungwi-san were good for type limb hyperhidrosis caused by stomach heat by food accumulation and Ki depression.

Treatment of Vertigo in Hyungsang Medicine (현훈(眩暈)의 형상의학적(形象醫學的) 치료(治療))

  • Vertigo is one of the subjective symptoms which appears commonly in so many diseases that it is often neglected. But it needs active medical care because it can be a forewarning of serious illness like cerebral apoplexy. This thesis makes a study of vertigo in view of Hyungsang medicine. The followings are the conclusion drawn in diagnosis and treatment of Vertigo: Excess in the upper and deficiency in the lower part(上實下虛) is a main cause of male vertigo. Deficiency in the upper and excess in the lower part(下虛上實) is a general cause of female vertigo. Vertigo is also caused by the insufficiency of the kidney water(腎水) or reservoir of marrow(髓海). persons of Jung pe(精科) with large check bones , those of Fish type(魚類) with Dig mouth and thick lips ; and those with large heads are mainly attacked by this cause. A fleshy person's vertigo comes from the deficiency of Ki and damp phlegm(氣虛濕症) so it is cured by reinforcing Ki, removing dampness and dispelling phlegm. A slim person's vertigo comes from the deficiency of blood and dryness heat(血虛有火) so it is treated by tonifying the blood, cleaning heat away the heat. In case of womanly shaped male and manly shaped female, the vertigo is due to the phlegm fire. Wind-heat(風熱) can cause vertigo generally 施 the persons of following types : wind type(風人), running animal type(走類),bird type(烏類), and Yangmyung meridian type (陽明形). Vertigo also comes from consumption. The deficiency of Ki can be a cause of vertigo in case of male; persons with pale complexion; and those with pronounced noses. The deficiency of blood can be a reason of vertigo in the persons of Hyul type(血科) and those with pale eyelids and lips. The hypofunction of the splean and stomach brings on the deficiency of Ki and blood, which can give rise to the vertigo for those with large nose and mouth. The old people's vertigo comes from the hypofunction of Ki, blood, Yin and Yang.
